Engineers investigate a bridge collapse by preserving the scene, reconstructing the order of failure, examining and testing components, checking design and loads, and tracing inspection and maintenance decisions. The aim is not simply to identify what broke, but to establish how the failure began, how it spread, and whether design, construction, upkeep, oversight, or other safeguards could have prevented it. Conclusions depend on evidence and the stage of the investigation: an early factual update is not the same as a final cause determination.
What investigators are trying to establish
A collapse is an event with a sequence, not just a pile of damaged material. Investigators work to identify the initiating failure, determine how forces and damage propagated through the structure, and establish which conditions contributed. They also examine the safety system around the bridge: design review, construction decisions, inspections, repairs, load ratings, owner oversight, and any response to visible distress.

These questions are related but not interchangeable. A corroded connection may be the component that fractured; a missed repair recommendation may help explain why it remained in service; a weak inspection process may be a broader contributor. A report can distinguish the initiating event, probable cause, contributing factors, and severity factors. A single failed part does not necessarily explain the entire collapse.
There is no universal test sequence that fits every bridge. The investigation plan depends on the material, bridge type, observed damage, environmental conditions, and competing explanations. Investigators use physical evidence, records, calculations, interviews, and sometimes laboratory testing or engineering models to test those explanations rather than treating any one test as a cause by itself.

How an investigation proceeds
The following sequence is a practical synthesis of documented investigations, not a claim that every authority uses an identical checklist. Some tasks overlap or change order as new evidence emerges.
1. Secure and document the scene
Investigators record the collapse geometry, the parts of the structure still standing, the debris field, environmental conditions, and the condition of nearby approaches and foundations. Photographs, measurements, video, and records can preserve the scene as it existed before recovery or cleanup changes it. They also document how evidence is identified, moved, and stored so that later analysis can be tied to the original location and condition.
Where recovery is necessary, investigators identify and examine components rather than treating debris removal as the end of the inquiry. In its January 26, 2023 update on Pittsburgh’s Fern Hollow Bridge, the National Transportation Safety Board (NTSB) described recovery and examination of bridge components and review of bus-camera footage. The update also said the public factual docket contained 2,137 pages. That page count describes the docket’s size, not the investigation’s quality or completeness. 2. Reconstruct the event sequence
Investigators compare video, witness accounts, operational records, component positions, and damage patterns to work out what failed first and what happened next. A recording may establish timing or show movement, but its angle and coverage may not reveal the internal cause. Physical evidence and calculations are needed to interpret what the recording shows.
At Fern Hollow, bus-camera video showed deck separation and the west end already off the abutment. That helped investigators assess the sequence; it did not, on its own, establish why the bridge failed. The key distinction is between documenting an event and explaining its mechanism.
3. Examine and test recovered components
Examiners look for fracture surfaces, corrosion and material loss, deformation, connection damage, weld condition, and concrete deterioration. They select tests to resolve specific questions—for example, whether a component’s material or fracture characteristics fit a proposed failure mechanism. Depending on the structure and evidence, this can involve mechanical, chemical, metallurgical, or non-destructive evaluation. No single test is mandatory in every case, and a test result matters only in context with the component’s location, loading, condition, and the rest of the evidence.

For Fern Hollow, the NTSB reported three-dimensional examination and metallurgical testing of bridge-leg portions, and planned mechanical and chemical testing. In its investigation of the Florida International University (FIU) pedestrian bridge collapse, the NTSB documented forensic examination and destructive testing of steel and concrete samples. Those methods helped evaluate evidence; they were not standalone explanations of either collapse. The FIU final report is available as the NTSB pedestrian bridge collapse report.
4. Check design, capacity, and loads
Investigators compare calculations and drawings with the bridge as built and as used. They may examine material specifications, construction stages, modifications, traffic and other loads, and how forces were distributed through members and connections. If the observed failure does not fit the design assumptions, that mismatch can point to a calculation, detail, construction, or loading issue.
The FIU final report attributed the collapse to errors in load and capacity calculations at a main-span truss member 11/12 node and its deck connection. In its guidance on gusset plates, the Federal Highway Administration (FHWA) summarizes the NTSB’s finding that design error left inadequate load capacity on the I-35W Mississippi River bridge under increased bridge weight, traffic, and concentrated construction loads. These findings are case-specific; neither means that every collapse involving a connection or a construction project has the same cause. FHWA guidance on inspection of gusset plates

5. Review inspection, maintenance, and oversight
Investigators review inspection reports, load ratings, repair recommendations, work orders, and decisions by the bridge owner or responsible agencies. They ask what condition had been observed, whether it was recognized and recorded, whether a repair or restriction was recommended, and what happened afterward. Records can reveal whether an issue was visible or documented before the collapse and whether follow-up matched its seriousness.
At Fern Hollow, the final NTSB findings connected corrosion and section loss in a fracture-critical tie plate with maintenance and repair recommendations that were not acted on. The Board also identified problems involving inspections, identification of fracture-critical members, load-rating calculations, and state oversight. A later NTSB release said three inaccurate inputs in the load rating concerned treatment of holes and section loss, the effective-length factor used to estimate buckling capacity, and the amount of asphalt wearing surface. The Board reported that a correct load rating would have required closure and issued 11 new recommendations in that case; those counts and findings do not describe bridges generally. For U.S. highway bridges on public roads, the National Bridge Inspection Standards (NBIS) provide the safety-inspection framework. FHWA says the standards were most recently updated in 2022. The existence of an inspection framework does not, by itself, establish that an individual bridge was inspected adequately or that identified problems were addressed; investigators examine the actual records and decisions. 6. Compare explanations and report findings
Investigators test whether each plausible explanation is consistent with the timeline, physical evidence, calculations, and records. Evidence may support one mechanism, rule another out, or leave uncertainty. The final report explains the agency’s adopted analysis, findings, probable cause where applicable, and safety recommendations. Recommendations address problems found in the investigation; they are not a guarantee that every similar risk has been eliminated.
Investigation materials are published in stages. Early updates or factual dockets may contain substantial evidence without an adopted cause conclusion. In its January 2023 Fern Hollow update, the NTSB expressly cautioned: “The docket contains only factual information collected by NTSB investigators; no conclusions about the probable cause of the collapse should be drawn from the information within the docket.” For settled conclusions, readers should use the responsible body’s final findings rather than treating an interim document as a verdict.

Case studies show why the investigation has to fit the structure and its evidence. A failure involving a steel connection calls for different evidence than one involving a riverbed foundation or a design calculation. The examples below summarize the findings attributed to the investigating authorities; they should not be generalized into a ranking of common causes.

| Fern Hollow Bridge, Pittsburgh | The NTSB identified failure of the southwest leg’s transverse tie plate, a fracture-critical member, due to corrosion and section loss. | Maintenance recommendations, inspection and load-rating shortcomings, oversight, recovered components, laboratory examination, and video helped explain the failure and its progression. |
| Schoharie Creek Bridge | The NTSB found that pier failure and erosion beneath spread footings followed inadequate maintenance of riprap around the piers. | The investigation also identified ambiguous construction plans and specifications, an inadequate inspection program, and weak oversight. This case shows why foundation and water conditions matter alongside the visible superstructure. |
| Mianus River Bridge | The NTSB found corrosion-induced forces displaced hangers in a pin-and-hanger assembly. | Inspection and maintenance deficiencies contributed, making the connection system and upkeep part of the investigation. |
| FIU pedestrian bridge | The NTSB attributed the collapse to load and capacity calculation errors at a main-span truss node and its deck connection. | Inadequate independent peer review and failure to recognize the seriousness of cracking and stop work or close the road contributed. |
| I-35W Mississippi River bridge | FHWA’s guidance summarizes the NTSB finding of inadequate gusset-plate capacity due to design error under increased bridge weight, traffic, and concentrated construction loads. | FHWA also notes that documented corrosion at other gusset plates had been overlooked or underestimated in visual inspection. The agency reports 13 deaths and 145 injuries in the collapse. |
The figures for I-35W are reported by the Federal Highway Administration on its undated gusset-plate guidance page (accessed 2026); they describe the consequences of that 2007 collapse, not a general bridge-collapse rate. How to read an investigation update or final report
Look first at the document type and date, then at the exact language used for the conclusion. An update may report what investigators have recovered, tested, or learned without deciding the cause. A factual docket is a collection of evidence and records, not necessarily a finished analysis. A final report is where the responsible body presents its adopted findings and cause determination.
Quick Recap
- Check the stage: Is this an interim update, a factual docket, or a final report?
- Separate evidence from interpretation: A video, test result, or inspection record is evidence; the agency’s analysis explains what it supports.
- Distinguish cause from contributors: Read whether the report identifies an initiating failure, probable cause, and contributing factors as separate findings.
- Keep scope attached to numbers: Counts such as Fern Hollow’s 11 recommendations apply to that investigation, not all bridges or investigations.
- Follow the cited record: FHWA’s bridge inspection resource index includes the Bridge Inspector’s Reference Manual (2022 NBIS version, March 2023), an Underwater Bridge Inspection Reference Manual (September 2024), and guidance on nonredundant steel tension members and non-destructive evaluation. FHWA also says its Bridges and Structures office provides technical guidance and undertakes special assignments such as failure investigation. These are professional resources, not instructions for the public to conduct field inspections. FHWA bridge inspection resources
