For adults with nonspecific lower-back pain, the best-supported general firmness choice is medium-firm—not the hardest mattress you can find. That is a broad evidence-based starting point, not a guarantee of relief or a doctor-endorsed list of mattress brands. The right bed also depends on your body size, sleep position, pressure-relief needs, and comfort.
What mattress firmness is best for back pain?
Medium-firm is the most defensible general recommendation for adults with nonspecific low-back pain or morning stiffness. In a 2026 review of 47 studies, the evidence most consistently favored medium-firm surfaces over very firm or very soft ones. The authors concluded: “Medium-firm mattresses represent the most defensible general recommendation for adults with nonspecific low back pain or morning stiffness.” Caggiari et al., 2026
That recommendation is a starting point, not a universal prescription. The review found varied study designs, materials, outcomes, and participants, so its authors could not combine results in a meta-analysis. They also identify body habitus, age, sex-related anthropometry, sleep position, and thermal comfort as relevant considerations. The review rates its evidence Level III (Oxford 2011).
“Medium-firm” is not a standardized feel across retail brands. A label can help you compare mattresses within a particular brand, but it does not ensure two brands’ beds feel alike. Look for a surface that supports your body without creating uncomfortable pressure, and assess it in your usual sleeping position.

What does the clinical evidence actually show?
A randomized trial favored medium-firm over firm for some outcomes
A 2003 randomized, double-blind, multicentre trial enrolled 313 adults with chronic nonspecific low-back pain who had pain while lying in bed and on rising, without referred pain. Participants used either a firm mattress or a medium-firm mattress, with outcomes assessed after 90 days. Firmness was measured on the European Committee for Standardisation H(s) scale, where 1.0 was firmest and 10.0 softest: the comparison was H(s)=2.3 versus H(s)=5.6.
At 90 days, the medium-firm group had better odds of improvement in pain in bed (OR 2.36, 95% CI 1.13–4.93), pain on rising (OR 1.93, 95% CI 0.97–3.86), and disability (OR 2.10, 95% CI 1.24–3.56). These are study-specific results for that population and comparison—not a prediction that any mattress sold today will produce the same outcome. Kovacs et al., The Lancet, 2003

What the findings do not establish
The evidence concerns nonspecific low-back pain; it does not show that a mattress treats every cause of back pain, prevents injury, or replaces diagnosis and care. An earlier AHRQ/NCBI evidence review likewise noted a lack of evidence for specific mattress types as treatment and summarized the 2003 trial as favoring medium-firm over firm for some outcomes. How to choose a mattress for lower-back pain
- Start with a medium-firm feel. Treat it as an evidence-based direction for nonspecific low-back pain, not as a required label or a promise of symptom relief.
- Match support and pressure relief to your body. Consider whether the mattress feels supportive in your usual position while avoiding pressure that makes it uncomfortable to lie down.
- Account for sleep position and body size. The same mattress can feel different to different people; the 2026 review specifically identifies body habitus and sleep position among the factors to consider.
- Check thermal comfort and movement. Whether you sleep hot, share the bed, or need to move easily on the surface may affect which model feels comfortable in practice.
- Compare the actual purchase terms. Before ordering, verify the current price for your size and configuration, delivery, trial conditions, any required break-in period, return fees, and warranty. These details can change.
Which mattresses are worth comparing in 2026?
There is no established physician-endorsed brand consensus in the reviewed sources. Consumer testing guides can help you build a shortlist, but an editorial award is not a clinical endorsement and does not prove that a model cures back pain.
