Call your local emergency number after a significant fall from a ladder or roof, especially if the person is unconscious, confused, having trouble breathing, bleeding heavily, vomiting, seizing, weak or numb, or may have a head, neck, or back injury. Keep them still, do not ask them to stand or walk, and follow the dispatcher’s instructions. A person can be awake and still have a serious internal, head, or spinal injury.
First, make the scene safe
Do not rush beneath an unstable roof edge, damaged ladder, falling materials, exposed wiring, or other active hazard. Stop work, keep other people back, and call for help from a safe position. Only approach when you can do so without becoming a second casualty. This scene check is practical safety advice; the medical steps below follow NHS, MedlinePlus (U.S. National Library of Medicine), and emergency-service guidance.

What to do in the first minutes
1. Call the local emergency number
A fall from height deserves urgent assessment when it is significant or when any concerning symptom is present. Tell the dispatcher that the person fell from a ladder or roof, the approximate height, the landing surface, whether the head was struck, and what you can see now. Give the exact location, including site access details if this is a construction job. Use the number for the country where you are; do not assume another country’s number applies.

| Location | Number and guidance in the cited material | Important qualification |
|---|---|---|
| United Kingdom | Call 999 for a fall from height or other life-threatening emergency, according to South Central Ambulance Service. | NHS head-injury guidance says a fall of more than 1 metre or five stairs after a head impact is a reason to call 999. That height rule is UK-specific and is not a universal safety threshold. |
| United States | Call 911, or the local emergency number, for serious head-injury symptoms, breathing problems, or other emergencies, according to MedlinePlus. | Follow the dispatcher’s directions and use local protocols if you are elsewhere. |
If you are uncertain, describe the fall and symptoms rather than trying to diagnose the injury yourself. The dispatcher can decide the response level.

2. Assume a spinal injury until it is ruled out
A fall from a ladder or roof is an event that can injure the spine. Do not pull the person up, sit them in a chair, straighten their neck, or ask them to walk “to see if they are okay.” Keep the head and neck as still and aligned with the spine as you can without forcing them into a painful position.
The NHS recovery-position guidance states: “If you think a person may have a spinal injury, do not attempt to move them until the emergency services reach you.” Severe neck or back pain, inability to move the neck, weakness, numbness, paralysis, or loss of bladder or bowel control make the concern even stronger. These signs are reasons to tell the dispatcher immediately.
3. Check responsiveness, airway, and breathing
Speak to the person and observe whether they respond normally. Check whether the airway is open and whether they are breathing normally. MedlinePlus advises checking airway, breathing, and circulation and beginning rescue breathing or CPR if needed. Follow the emergency dispatcher’s instructions and provide CPR or rescue breathing only if it is appropriate and you are trained to do so.

If the person is unconscious but breathing, treat the situation as a possible spinal injury. Stabilize the head in line with the spine and continue watching breathing. Do not place an apparently breathing person in a routine recovery position unless the airway requires it and the dispatcher directs you, because rolling can move an injured spine.
4. Move them only to protect the airway or escape immediate danger
The general rule is to leave the person where they are. There are two critical exceptions: you may need to move them away from an immediate hazard, or you may need to turn them to protect the airway if vomiting threatens breathing.
MedlinePlus advises rolling the head, neck, and body as one unit while stabilizing the head and neck when vomiting threatens the airway. This is a controlled airway maneuver, not permission to reposition the person for comfort or to test whether they can stand. Explain to the dispatcher what is happening and ask for step-by-step instructions.

5. Control serious bleeding carefully
For many open wounds, press firmly with a clean cloth or dressing while waiting for help. A bleeding head wound requires a specific caution: MedlinePlus advises direct pressure unless a skull fracture is suspected. If a depression or other sign of skull fracture is suspected, do not press directly on the wound and do not remove embedded debris; cover the area with sterile gauze or another clean dressing and wait for professional instructions.
Do these 3 things before closing this tab:
1Repair Windows errors before they cause bigger problems2Scan for outdated or missing drivers - takes under a minute3Clear out junk files and repair common Windows errorsDo not pull out glass, metal, wood, or other objects stuck in the body. Apply pressure around, not on, an embedded object if the dispatcher instructs you, and keep watching breathing and alertness.
6. Stay with the person and monitor changes
Keep observing breathing, responsiveness, speech, movement, and bleeding until help arrives. Report any change immediately. Do not leave the person alone to fetch equipment or drive them yourself unless emergency professionals tell you to do so. Being awake, talking, or having no obvious wound does not rule out a serious injury.

Signs of a serious head, neck, or back injury
Tell emergency services about any of the following. Several are particularly important after a head impact, but a high-energy fall can injure the spine or internal organs even without a visible head wound.
- Any loss of consciousness, even if brief, or failure to wake normally after being knocked out.
- Very unusual sleepiness, increasing difficulty staying awake, confusion, abnormal behavior, or abnormal speech.
- A seizure.
- Repeated vomiting (MedlinePlus identifies vomiting more than once as an immediate-help warning sign).
- A severe or worsening headache, stiff neck, or severe neck or back pain.
- New numbness, weakness, paralysis, inability to move an arm or leg, balance problems, or trouble speaking.
- Unequal pupils or other changes in vision or responsiveness.
- Heavy bleeding, breathing difficulty, or signs that the person is deteriorating.
- Signs of a skull injury, such as a visible depression, an open wound with bone concern, or clear fluid or blood from the nose or ears after a head impact.
- Loss of bladder or bowel control after the fall.
NHS head-injury guidance uses a fall of more than 1 metre or five stairs after a head impact as a UK reason to call 999. That criterion should not reassure someone who fell a shorter distance: symptoms, the landing, age, medications, and the force of the impact also matter. Outside the UK, use your local emergency service’s criteria.
What not to do after a ladder or roof fall
- Do not make the person walk. Testing movement can worsen an unstable spinal or limb injury.
- Do not move them simply to make them comfortable. Leave them still unless there is immediate danger or an airway emergency.
- Do not twist or straighten the neck. Support the head in its existing alignment without forcing it.
- Do not press directly on a suspected skull fracture. Cover it and follow dispatcher instructions instead.
- Do not remove an embedded object. Stabilize it only as directed by emergency personnel.
- Do not leave the person alone. Continuous observation can reveal a change that needs urgent action.
- Do not use the wrong emergency number. UK guidance refers to 999; U.S. guidance refers to 911 or the local emergency number. Use the number where the incident is occurring.
If the person appears only mildly hurt
A fall can produce a concussion, spinal injury, fracture, or internal injury without dramatic external bleeding. Do not rely on the person’s confidence that they are fine, particularly if they hit their head, take medicines that increase bleeding risk, or cannot clearly describe the event. Call the local emergency service when the fall was significant or when any warning sign is present, and let clinicians determine whether examination or imaging is needed.

While waiting, keep the person still and supervised. Avoid giving food, drink, alcohol, or medication unless emergency professionals advise it; swallowing may be unsafe if consciousness changes, and clinicians may need an accurate assessment. If the person deteriorates, update the dispatcher immediately.
After emergency care: document and report the incident
For a workplace fall
Once the injured person is in professional care, preserve the scene as required by your employer and local law, record the time and approximate height of the fall, and note witnesses, weather, surface conditions, equipment position, and the actions taken. Do not restart work on an unsafe ladder, roof, scaffold, or access route until it has been assessed and made safe.
NIOSH reported in 2019 that falls are the leading cause of construction-worker deaths on the job. Its prevention material emphasizes planning for falls from roofs, ladders, and scaffolds, not merely reacting after an injury.

For a worker suspended in fall-arrest equipment
Do not improvise a high-risk rescue. NIOSH’s 2024 bulletin on falls from elevation says a fall-protection plan should include a rescue strategy and equipment ready for rescue, including for a worker suspended in a personal fall-arrest system. Activate the site’s trained rescue plan and emergency response. A suspended worker may need rapid, coordinated rescue, and untrained rescuers can create another fall.
Prepare before the next fall
- Keep a stocked first-aid kit with clean gauze or dressings where crews can reach it. It is a preparedness item, not a substitute for emergency services or medical evaluation.
- Post the correct emergency number and the site’s full address, gate, floor, and access instructions.
- Train workers to recognize head and spinal warning signs, call for help, protect the airway, and avoid unnecessary movement.
- Maintain a written fall-protection and rescue plan, with suitable equipment and named, trained responders.
- Inspect ladders, roofs, edges, access points, and surfaces before work; correct hazards rather than relying on a worker’s balance or experience.
The most useful preparation is a plan that lets someone call quickly, describe the location accurately, keep an injured person still, and summon trained rescue without creating a second emergency.
