Choose a hospital parcel only after proving that every essential user can reach it, that utilities can deliver the required capacity with continuity, and that stormwater, flooding and wastewater can be managed without creating risk for the facility or its neighbours. A road or utility main at the boundary is only a starting point.
The decision should combine transport mapping, written utility confirmation, topographic and hydrologic studies, wastewater planning, resilience analysis and a realistic estimate of connection and site-development costs. Federal, state and international guidance provide useful questions, but only local approvals and parcel-level engineering can establish whether a particular site is suitable.

Start with a site brief tied to the hospital’s services
Before comparing land, define the facility that must operate there. An emergency hospital, outpatient centre and long-term-care campus have different traffic, utility and resilience demands. List the departments, bed count, operating hours, imaging and laboratory equipment, kitchen and laundry functions, oxygen and medical-gas systems, parking, loading, waste handling and planned expansion. Engineers can then turn the service plan into design flows, peak demand, fire-protection requirements, vehicle movements and backup needs.

1Fix the driver behind crashes, sound loss and screen glitches2Repair Windows errors before they cause bigger problems3Scan for outdated or missing drivers - takes under a minuteKeep the brief location-specific. The U.S. Department of Health and Human Services (HHS) planning guidance identifies access, physical features, utilities and site-development cost as core selection criteria, but it does not set one universal travel-time, road-capacity, slope, flood-elevation or drainage number. Those values must come from the applicable jurisdiction and the hospital’s design.

Test access for every person, vehicle and operating condition
“Access” means more than a public road touching the property. HHS calls for vehicular capacity, service access for trucks and semi-trailers, pedestrian connections and public-transport access in urban areas. New York’s health-facility rule illustrates the level of detail a jurisdiction may require: paved roads and walks to public and service entrances, including loading docks; a clearly marked emergency-department entrance; and routes that do not conflict with other vehicles or pedestrians. New York requirements are not automatically applicable elsewhere.
Map separate movements
Draw a route map for each user group and identify where movements cross:
- Ambulances approaching, queuing, unloading and leaving the emergency department.
- Patients arriving by car, taxi, wheelchair transport or public bus.
- Staff commuting at shift changes and reaching staff parking.
- Visitors walking from parking, transit stops and nearby streets.
- Delivery vehicles, including semi-trailers, fuel trucks and medical-gas suppliers.
- Waste, linen and food-service vehicles using loading and service yards.
- Fire apparatus and other emergency responders requiring unobstructed access.
Check turning radii, gate widths, queue lengths, loading-dock geometry, sight lines, grades, pedestrian crossings and the ability to reverse or pass safely. Confirm that emergency and service traffic can be separated from public drop-off activity. The New York rule’s wording is a useful test: “Each health facility shall be easily accessible to patients or residents, staff and visitors and to service vehicles such as fire protection apparatus.”

Observe peak traffic, school and event periods, winter or monsoon conditions where relevant, and nearby construction. Ask the road authority whether planned works, bridge restrictions or intersection changes could affect the site. A route that works in a dry off-peak inspection may fail when a lane closes or a low point floods. The cited guidance establishes the categories to assess, not a universal ambulance travel-time or road-capacity threshold.
Verify public transport and pedestrian continuity
In urban locations, identify existing stops, schedules, sidewalks, crossings, lighting and accessible paths. Measure the complete journey from the public right-of-way to the entrance, not merely the distance to the parcel boundary. Where sidewalks or transit stops must be extended, include easements, approvals, construction cost and a maintenance responsibility in the feasibility review.

Confirm utility capacity and continuity in writing
Utility proximity does not prove that a hospital can obtain adequate flow, pressure, fault tolerance or connection timing. HHS lists water, storm drainage, sanitary waste disposal, electricity and communications as site criteria. Its water guidance covers domestic and firefighting needs, including adequate firefighting pressure; it expects adequate sanitary sewer in developed areas and access to on-site disposal or the capability to develop on-site sewage treatment in undeveloped areas.
Water supply
Request the provider’s written statement of available flow, static and residual pressure, connection point, water-quality requirements, outage history and the works needed to serve the parcel. Have the design team calculate average and peak domestic use, clinical processes, cooling, kitchens, laundry, irrigation, storage, simultaneous demand and fire-flow requirements. Determine whether mains, tanks, pumps and internal zoning can maintain pressure during a main break or power outage.
The CDC’s Emergency Water Supply Planning Guide advises healthcare facilities to plan for interruptions by considering use, response capability and alternative supplies. Apply that approach during site selection: test the feasibility of storage, a secondary connection, tanker filling, a deep well where legally and hydrogeologically possible, and treatment or disinfection. Do not treat a nearby water main as proof of hospital-scale supply.
Fire protection
Confirm hydrant locations, fire-flow testing, pressure at the required elevation, fire-department access and any requirement for dedicated tanks or pumps. Ask the authority how expansion or simultaneous demand in the service area could change available flow. Record who owns, tests and maintains each component.
Electricity and backup power
Obtain the utility’s available capacity, voltage, fault level, connection route, substation constraints, lead time and outage performance. Map where lines, substations or transformers could be exposed to flood, wildfire, high wind or vehicle impact. Size the resilience concept around the hospital’s actual critical loads: life-safety systems, intensive care, operating rooms, medical gases, refrigeration, communications, water pumps, lifts and essential IT.

Compare one utility feed with dual feeds, on-site generation, fuel storage, battery systems and the space and approvals each option needs. A generator is not a complete resilience plan unless fuel delivery, testing, ventilation, noise, flood elevation and maintenance access are workable.

Communications
Verify carrier availability, diverse fibre routes, radio coverage, internal public-safety communications and service-level commitments. Consider how communications will function when commercial power or one cable route fails. The Philippines Department of Health guide, for example, lists radio communication lines among contingency provisions; that is country-specific guidance, not a universal rule.
Sanitary sewer or on-site treatment
Ask the sewer authority for invert levels, available hydraulic and treatment capacity, connection limits, pretreatment requirements, discharge standards, emergency overflow arrangements and the cost and programme for extension. Confirm that gravity service is possible or budget pumping stations and standby power. If no public sewer is feasible, establish whether the parcel has enough suitable area and soil conditions for on-site treatment, reserve fields, access, permits, operator requirements and long-term maintenance.
Stormwater service
Identify the receiving system, allowable discharge, connection level, detention requirements and ownership of downstream assets. A storm connection is not permission to discharge unlimited runoff; local drainage authorities may require detention, treatment, attenuation or sustainable-drainage features.
Investigate drainage, flooding and off-site water
HHS advises that a site have adequate surface runoff or storm sewers, that the planned facility not be affected by drainage from upstream sites and that it not harm downstream sites. A boundary walk cannot answer those questions. Review the whole catchment and the way future development could alter it.

Commission parcel-level studies
- Topographic survey showing spot levels, ridges, low points, overland flow paths and connection elevations.
- Hydrologic and hydraulic modelling for relevant storm events required by the local authority.
- Floodplain mapping, historical flood records, groundwater information and climate or sea-level allowances where applicable.
- Soil infiltration, contamination and bearing information that affects detention ponds, infiltration systems, foundations and on-site wastewater.
- Downstream capacity and the effects of the hospital’s roofs, roads, parking and grading on neighbouring properties.
NHS resilience guidance treats flood likelihood and neighbouring development as site-selection issues. It promotes grounds and drainage measures, including sustainable drainage, to reduce disruption and stresses that healthcare access and egress must remain functional. Use that as a resilience prompt, then apply current local flood maps, climate data, drainage calculations and approvals.
Keep critical functions above credible hazards
Compare finished-floor levels, plant rooms, electrical rooms, generators, fuel stores, medical-gas compounds, ambulance ramps and access roads with the assessed flood and overland-flow levels. Do not protect the building while leaving the only ambulance route or utility compound vulnerable. Provide safe maintenance and inspection access to channels, swales, tanks, pumps and outfalls.
Manage upstream and downstream responsibilities
Trace water entering from higher neighbouring land and identify culverts, ditches, blocked inlets and informal flow paths. Obtain rights to alter or maintain off-site drainage where necessary. Model the post-development discharge and demonstrate that detention or sustainable-drainage measures will not shift flooding onto a lower parcel. Include inspection, sediment removal, vegetation management and ownership in the operating plan.
Use country guidance carefully
The Philippines Department of Health planning guide lists freedom from flood danger, drainage, topography and soil conditions as location considerations, and mentions generators, deep wells and water pumps among provisions to establish. Treat those as examples of contingency planning. Confirm detailed requirements against an official, current government edition and the law governing the actual project.
Evaluate resilience as an operating requirement
A hospital must remain reachable and functional during the hazards reasonably expected at the site. Create a hazard-and-dependency map covering flood, severe weather, wildfire, seismic events, landslides, heat, power interruption, water outage, communications failure and loss of a key road. For each hazard, record the trigger, duration, affected assets, alternate route or supply, responsible party and recovery time.

Test scenarios rather than listing equipment. For example, ask whether a flooded approach road still leaves a usable ambulance entrance; whether a utility outage can be managed while fuel deliveries are delayed; whether a water interruption leaves enough stored water for clinical and firefighting needs; and whether a failed sewer connection can be isolated without contaminating the site. The answers should shape land selection, not be postponed until detailed design.
Compare parcels on one documented scorecard
Use the same evidence requests and assumptions for every candidate. A simple scorecard prevents a visually attractive but infrastructure-poor parcel from winning by default.
| Decision axis | Evidence to collect | Cost or risk to record |
|---|---|---|
| Access reliability | Emergency, patient, staff, pedestrian, transit, delivery, waste and service routes; conflicts, queues and closures | Road upgrades, easements, intersection works and weather-related downtime |
| Utility adequacy | Provider-confirmed water flow and pressure, fire flow, power, communications, sewer and stormwater capacity | Connection fees, extensions, substations, tanks, pumps and programme risk |
| Continuity and resilience | Alternate routes and supplies, backup power and water, hazard exposure of critical infrastructure | Generator and fuel systems, storage, dual feeds, flood protection and maintenance |
| Drainage and flood | Elevation, runoff, upstream flows, flood mapping and history, downstream discharge and sustainable-drainage feasibility | Grading, detention, outfalls, land take, approvals and residual flood risk |
| Wastewater feasibility | Sewer invert and treatment capacity, connection approvals or on-site treatment area and soil suitability | Pumping, treatment plant, reserve area, permits and operating labour |
| Development constraints | Topography, soil, rights-of-way, protected areas, existing infrastructure and local planning conditions | Earthworks, relocation, environmental mitigation and approval delays |
Assign weights only after the clinical service plan and jurisdictional requirements are known. A trauma hospital may give emergency-route reliability greater weight; a remote campus may place more weight on water, power and wastewater independence. Keep the underlying evidence with each score so the ranking can be audited.
Run a staged due-diligence process before committing
- Define the operating brief. Set departments, capacity, hours, expansion, critical loads, vehicle types and resilience objectives.
- Screen the catchment. Remove parcels with obvious legal-access, protected-area, floodplain or incompatible land-use barriers.
- Map every route. Produce emergency, public, staff, pedestrian, transit, delivery, waste and responder circulation plans and test peak conditions.
- Obtain provider letters. Request capacity, pressure, connection points, standards, lead times, outage information and extension requirements for water, power, communications, sewer and stormwater.
- Commission surveys and studies. Complete boundary and topographic surveys, traffic review, flood and drainage assessment, geotechnical work, environmental screening and wastewater feasibility.
- Develop resilience options. Price alternate access, storage, wells where legal, generators, fuel, dual feeds, pumps, treatment and flood protection.
- Check approvals. Confirm zoning, health-facility rules, road permits, water and sewer consents, discharge limits, environmental requirements, fire access and accessibility obligations with the local authorities.
- Compare lifecycle cost. Include land preparation, road and utility extensions, detention and treatment, maintenance, inspections, energy, staffing, insurance and the cost of disruption.
- Set decision gates. Do not acquire or announce a preferred site until fatal constraints are cleared and unresolved risks have an owner, budget and programme.
Common warning signs and how to respond
- “The main is at the boundary.” Require flow, pressure, fire-flow and outage confirmation; design storage or alternate supply if continuity is inadequate.
- “There has never been a flood here.” Check maps, records, upstream catchments, climate allowances and drainage modelling rather than relying on anecdote.
- “One entrance is enough.” Test simultaneous ambulance, visitor, delivery and fire-apparatus movements and identify an alternate route.
- “A sewer connection is available.” Verify invert, hydraulic and treatment capacity, discharge limits, pumping needs and approvals.
- “A generator solves outages.” Confirm fuel logistics, ventilation, flood protection, testing, maintenance and the loads that must actually be supported.
- “The developer will fix drainage later.” Obtain a funded concept, discharge approval, maintenance responsibility and downstream-impact assessment before accepting the risk.
- “A generic checklist proves suitability.” Use guidance as a prompt only; local codes, provider data and professional investigations control the decision.
Who must sign off the site decision?
The owner’s clinical and facilities leads should approve the operating brief. Transport, civil, drainage, geotechnical, structural, electrical, mechanical, fire-protection, environmental and wastewater specialists should validate their disciplines. Utility providers, road authorities, fire services, planning officials and health regulators must confirm external requirements. Record assumptions, dates, responsible parties and conditions in the decision log so that a low initial land price cannot conceal an unbudgeted infrastructure obligation.
HHS, CDC, NHS, New York and Philippines Department of Health materials serve different jurisdictions and audiences. They help frame questions about access, utilities, emergency water, flooding and resilience; they do not replace local regulatory review or parcel-specific engineering.
