Unlike a typical institutional project, it combines construction with infection prevention, clinical continuity, highly interdependent utilities, space-specific ventilation, and operational readiness.
Healthcare construction has a dual mission
An office, school or civic building is usually judged on whether it is built safely, meets its program and opens on schedule. A healthcare project has those obligations plus a second mission: preserve a functioning care environment while the building is being changed.

Patients may remain in the building, including people who are especially susceptible to airborne infection. Dust, moisture, noise, vibration, pressure changes, temporary power, water interruptions or a blocked corridor can affect diagnosis and treatment. The construction boundary therefore cannot be treated as the only area at risk.
The practical difference is that healthcare work is planned around clinical consequences. The owner, infection-prevention staff, facilities engineers, clinicians, contractors and design team must decide how work will affect patient populations, adjacent rooms, supplies, utilities, emergency routes and service capacity. A successful project is not merely complete; the affected department must be ready to provide care when it reopens.

Construction risk extends beyond the marked work zone
Air moves through ceilings, doors, shafts, return-air paths and pressure relationships. CDC environmental infection-control guidance says a construction risk assessment may need to include adjacent patient-care spaces, supply storage and areas above and below the project. Demolition dust or moisture that escapes a partition can enter a clinical area even when no worker crosses the barrier.
Dust and moisture are clinical hazards
Dust can carry fungal spores and other contaminants. Moisture can support microbial growth in building materials or systems. The risk is not the same in every part of a hospital: a corridor serving a low-risk administrative area is different from a unit caring for severely immunocompromised patients. The facility’s infection-control process must identify the affected population and select controls accordingly.
ICRA is a project decision process
An infection-control risk assessment (ICRA) is completed before repair, demolition, construction or renovation begins. It identifies potential exposure of susceptible patients to dust and moisture and determines the need for containment and other precautions. The exact form, risk matrix and approval path vary by facility policy and applicable jurisdiction; there is no universal barrier specification that fits every project.

Typical decisions include the type and location of barriers, pressure control, worker entry and exit, debris routes, negative-air equipment where required, cleaning frequency, monitoring, temporary relocation and communication with affected departments. The assessment should be revisited when the scope, schedule, patient mix or method changes.
Renovation often happens while the facility stays open
New-build healthcare projects can be isolated from patients, but many of the most difficult jobs are renovations in an operating hospital, clinic or long-term-care facility. The owner may need to maintain emergency access, imaging, surgery, inpatient beds, pharmacy operations, sterile processing, laboratory support and life-safety systems throughout the work.

Phasing is a clinical operating plan
Phasing is more than dividing drawings into bid packages. It establishes which rooms are unavailable, when patients or staff move, which services can be interrupted, how long a shutdown may last and what contingency is available if restoration fails. A phase plan should identify:
- Patient groups and clinical functions affected by each phase.
- Temporary entrances, corridors, elevators, waiting areas and wayfinding.
- Utility shutdowns, bypasses and backup capacity.
- Permitted work hours, noise limits and escalation contacts.
- Emergency, fire-department and ambulance access at every stage.
- Cleaning, inspection and acceptance requirements before reoccupation.
Shutdowns require more than a calendar notice
A planned outage of power, water, medical gas, data, controls or ventilation can affect multiple departments at once. Facilities engineering and clinical leadership should trace the system to its actual points of use, confirm redundancy, define a start-and-stop procedure and establish a rollback plan. A shutdown is not complete when a valve is closed; it is complete when the affected service has been tested and the clinical owner accepts the restored condition.
Healthcare systems are unusually interdependent
ASHRAE describes healthcare facilities as serving a uniquely vulnerable population exposed to elevated health, fire and safety hazards. It also highlights the need to coordinate normal and emergency power, plumbing, medical gases, automatic transport, fire protection and extensive information-technology systems within a limited building envelope.
That interdependence changes construction sequencing. A ceiling replacement can disturb medical-gas alarms, nurse-call devices, fire detection, return-air paths and infection-control barriers at the same time. Relocating a wall can change room pressure, sprinkler coverage, electrical loads and data pathways. The team must verify interfaces rather than treating each trade as independent.
| System | Why a healthcare project must treat it differently | Typical construction concern |
|---|---|---|
| Normal and emergency power | Clinical equipment and life-safety functions may depend on separate sources, transfer equipment and tested capacity. | Temporary power, selective shutdowns, generator or transfer-switch testing, and protection of critical circuits. |
| HVAC and controls | Air changes, pressure, filtration, temperature and humidity vary by clinical space. | Maintaining pressure relationships during demolition, isolating zones, protecting controls and recommissioning after work. |
| Plumbing and water | Water quality, temperature, backflow protection and continuity affect patient care and infection prevention. | Dead legs, outage planning, flushing, disinfection and confirmation that renovated branches are ready for use. |
| Medical gases | Oxygen, medical air, vacuum and other gases support treatment and may have alarmed, redundant distribution. | Permit-controlled shutdowns, identification of users, temporary supplies, brazed-joint procedures and alarm verification. |
| Fire protection | Patients may not evacuate independently, so detection, suppression and compartmentation are central to life safety. | Impairment plans, temporary detection or suppression, fire watches and maintaining rated assemblies. |
| IT, nurse call and transport | Clinical communication, records, monitoring and movement of medicines or specimens can depend on connected systems. | Protection of active cabling, planned cutovers, testing at endpoints and fallback communication methods. |
Ventilation is space-specific, not a single hospital-wide setting
Healthcare ventilation requirements depend on the function and risk of each room. Operating rooms, airborne-infection-isolation (AII) rooms, protective-environment (PE) rooms, procedure spaces, patient rooms, laboratories and support areas can have different air-change, pressure, filtration, temperature and humidity expectations. A value suitable for one area cannot be copied across the facility.

CDC guidance recommends coordinating HVAC work with infection-control staff and restoring intended performance before occupancy. Critical rooms may also require backup power or other resilience provisions. During renovation, the team must protect supply and return paths, verify door operation and pressure relationships, and control how temporary fans or barriers interact with the permanent system.
ASHRAE Standard 170 provides healthcare ventilation guidance and was developed with the Facility Guidelines Institute (FGI) and the American Society for Health Care Engineering. The applicable edition and enforceable requirements depend on the jurisdiction, facility type and adoption status. The design professional and authority having jurisdiction (AHJ) should confirm the governing edition before specifications are issued.
Regulatory requirements come in layers
Healthcare projects may be governed by state or local building and fire codes, licensing rules, adopted FGI provisions, ASHRAE requirements, accessibility law, environmental rules and the facility’s own policies. The controlling document is not automatically the newest national publication.
FGI describes its guidelines as covering minimum recommended programs, space, risk assessment, infection prevention, architectural details and engineering criteria. More than 40 state agencies had incorporated or adopted the cited healthcare design guidance by reference, according to a CDC page; that count is not dated there and should not be presented as a current 2026 total.
Healthcare Design reported in July 2024 that FGI was developing a 2026 edition called the “FGI Facility Code,” with proposed changes including water-age provisions. That report described a public-comment draft, not final text or adoption. Project teams should verify publication and local adoption rather than designing to a proposal.

Commissioning determines whether the space is ready for patients
In ordinary construction, substantial completion may focus on finishes, equipment and code inspections. In healthcare, readiness also depends on measured system performance and clinical acceptance. CDC recommends commissioning HVAC in new healthcare facilities and renovated spaces before occupancy, with particular attention to operating rooms, AII rooms and PE areas.
A healthcare turnover sequence
- Define the clinical requirement. Confirm the room function, patient population, pressure relationship, ventilation criteria, utility connections, equipment and emergency dependencies.
- Complete installation checks. Inspect ductwork, filters, dampers, controls, medical-gas piping, plumbing, electrical work, fire protection and rated construction before concealment or energization.
- Test systems under operating conditions. Verify airflow, pressure relationships, temperature, humidity, alarms, emergency power transfer and control sequences at the points that staff will use.
- Resolve deficiencies. Record the problem, responsible party, corrective action and retest result. Do not rely on a general punch list for a failed pressure or alarm test.
- Clean and release the area. Remove construction materials, perform the facility-required environmental cleaning, restore affected systems and obtain sign-off from engineering, infection prevention and the clinical department.
- Train users and document the handover. Provide operating instructions, emergency procedures, maintenance information, test records, as-builts and warranty contacts before patients return.
CDC states that it has no recommendation for routine microbiologic air sampling before, during or after construction, or before or during occupancy of areas for immunocompromised patients. Sampling decisions should therefore follow the facility’s risk assessment, policy and applicable requirements rather than being presented as a universal CDC mandate.
How healthcare projects differ from other institutional buildings
The comparison below is a planning framework, not a formal code classification. Schools, offices and laboratories can also have demanding safety and technical requirements; healthcare projects differ in how many high-consequence conditions must be managed simultaneously.
| Planning axis | Healthcare facility | Typical office or school | Laboratory or other specialized institution |
|---|---|---|---|
| Occupant vulnerability | Patients may be ill, sedated, immobile or immunocompromised; acuity varies by department. | Occupants are generally able to follow instructions and leave independently, subject to age or disability considerations. | Occupants may be trained staff, students or visitors; risk depends on the process and materials. |
| Occupied construction | Renovation commonly occurs beside active care, with relocations, detours and protected clinical access. | Phasing is important, but temporary closure or remote work may be more feasible. | Work may require process shutdowns, cleanroom controls or hazardous-material isolation. |
| Service continuity | Power, medical gases, water, HVAC, nurse call, records and emergency routes can be essential to immediate care. | Outages disrupt comfort, teaching or business operations but may not interrupt treatment. | Outages can compromise experiments, containment or expensive equipment. |
| Air and pressure control | Room-by-room ventilation and pressure relationships support infection prevention and treatment. | Ventilation is important, but fewer spaces have clinical pressure regimes. | Containment or cleanliness may be process-specific rather than patient-specific. |
| Readiness evidence | Commissioning, infection-control release and clinical sign-off are needed before use. | Code inspection, systems testing and owner acceptance commonly dominate turnover. | Validation may focus on process performance, containment or equipment qualification. |
| Regulatory layering | Licensing, adopted healthcare guidance, building and fire codes, and facility policy overlap. | Building, fire, accessibility and education or workplace rules generally apply. | Building and fire rules are joined by environmental, biosafety or process regulations as applicable. |
A practical planning sequence for an operating healthcare project
- Map care and occupancy. With the owner and facility leaders, list services, patient populations, occupied rooms, support areas, public routes and nearby departments.
- Map affected systems. Trace HVAC zones, electrical feeders, emergency circuits, plumbing branches, medical gases, fire protection, data, nurse call and transport routes above, below and beside the work.
- Approve the infection-control risk assessment. Set containment, pressure control, worker routes, debris handling, cleaning, monitoring, communication and relocation measures before mobilization.
- Build the phase and shutdown plan. Assign dates, outage windows, responsible approvers, temporary services, contingency capacity and a documented restoration test for each interruption.
- Educate the workforce. Train contractors and facility staff on barriers, entry procedures, personal protective equipment, dust control, reporting and escalation. Monitor compliance during the work, not only at orientation.
- Coordinate daily operations. Hold a short coordination process for clinical changes, deliveries, noise, alarms, access, housekeeping and unexpected conditions. Escalate a barrier breach or utility failure immediately.
- Release each phase deliberately. Clean the area, restore systems, complete required testing and obtain written acceptance before moving patients, equipment or supplies back.
Common failure modes and how to recover
A barrier is intact, but dust appears outside it
Stop the activity that is generating or transporting dust, protect nearby patients and supplies, notify infection prevention and facilities, and inspect pressure, door operation, penetrations and return-air paths. Clean affected areas using the facility’s procedure, correct the source and document the restart approval.
A shutdown reveals an unlisted department connection
Do not improvise a live reconnection. Stabilize the affected service, contact the clinical owner and facilities engineer, identify the actual distribution path, install an approved temporary or bypass arrangement if available, and update drawings and the shutdown plan before rescheduling.

Ventilation testing fails at turnover
Keep the room out of clinical service. Check filters, dampers, controls, balancing, door seals, pressure references and adjacent-zone operation; correct the defect and repeat the test under the specified operating mode. A signed punch list is not a substitute for demonstrated performance.
A phase opens without clear ownership
Assign one clinical representative, one facilities representative and one construction lead for release decisions. Provide a single list of outstanding deficiencies, due dates, temporary measures and escalation contacts so staff do not have to infer whether a room is safe to use.
What healthcare construction means for cost and schedule
There is no universal healthcare “infection-control percentage” or standard shutdown allowance that can be applied to every project. Cost and duration depend on the facility’s acuity, occupancy, phasing, temporary utilities, night or weekend work, containment, testing, relocation and the authority’s requirements.
Do these 3 things before closing this tab:
1Scan for outdated or missing drivers - takes under a minute2Clear out junk files and repair common Windows errors3Fix the driver behind crashes, sound loss and screen glitchesEarly investigation generally reduces avoidable disruption: verify existing conditions, identify hidden services, survey airflow and pressure, confirm available emergency capacity and involve departments before the design is fixed. The most expensive change is often a late discovery that a supposedly isolated system serves an active clinical function.
Who must coordinate the work
Healthcare construction requires a broader decision group than a conventional institutional project. The owner’s project manager coordinates scope and priorities; infection-prevention staff define risk controls; facilities engineers understand existing systems and shutdowns; clinical leaders explain patient-care consequences; design professionals translate requirements into details; contractors control means and methods; commissioning specialists verify performance; and the AHJ confirms enforceable code requirements.
That group should have a documented route for approving changes. A field decision that affects airflow, water, power, medical gases, access or patient relocation needs the people responsible for those consequences at the table.
The bottom line
Healthcare construction is different because it changes a building that is also a treatment environment. The defining work is not only erecting walls and installing equipment; it is controlling infection risk, preserving clinical services, coordinating interdependent systems, meeting locally adopted requirements and proving that each renovated space is ready for patients. Start with the facility-specific risk assessment and clinical plan, maintain disciplined phasing and communication, and commission the systems before reoccupation.
