There is no single document checklist for housing assistance after displacement. What you need depends on whether you are applying for initial FEMA disaster assistance, continued FEMA temporary housing assistance, a HUD public housing or Housing Choice Voucher program, or state or local emergency rent help. Start with basic information about your identity, household, housing, income, insurance, and disaster-related damage, then follow the specific agency’s current checklist and any case-specific request.

First, identify which program you are applying to

“Housing assistance” can mean several different programs, and their paperwork is not interchangeable. FEMA handles certain disaster assistance; public housing and Housing Choice Vouchers are administered locally by public housing agencies (PHAs); emergency rent assistance is generally administered by state or local programs. FEMA’s application information is national guidance, while HUD and local programs may have agency-specific forms and requirements.

Type of help Who to contact What to expect about documents
Initial FEMA disaster assistance FEMA, through its disaster-assistance application channels Prepare core application information; FEMA may request proof to verify particular facts or resolve a missing item.
Continued FEMA temporary housing assistance FEMA, using the instructions and form for the case A more specific set of records is required, including a signed current lease and proof of temporary-housing spending.
Public housing or a Housing Choice Voucher Your local PHA The PHA gathers information about income, assets, and household composition; its current checklist controls.
State or local emergency rent help The agency administering the local program Criteria and required records vary by program and location.

The key distinctions are who administers the help, whether this is an initial application or a request to continue aid, whether you rented or owned the affected home, and which facts the agency needs to verify. A document that matters for an owner’s home-repair request may not matter for a renter’s application; a FEMA list does not replace a PHA or local program’s checklist.

For an initial FEMA application, prepare information as well as records

USAGov says applicants need a Social Security number, insurance information, a description of the damage, annual household income, contact information, and bank account information if they want direct deposit. These are application details to have ready; they do not mean every applicant must upload a separate document for every item. FEMA may seek supporting records when it cannot verify eligibility or needs more information. See USAGov’s application guidance and FEMA’s Questions and Answers About FEMA Individual Assistance (page printed August 30, 2025).

  • Identity and household: Have your Social Security number and identifying information available. FEMA examples of identity evidence include a U.S. passport, military ID, motor-vehicle registration, or a Social Security card together with a federal or state ID. A payroll stub showing some or all of the Social Security number is another example in FEMA’s guidance. These are examples, not a checklist every applicant must submit in full.
  • Insurance: Gather policy or coverage details and any claim settlement or denial letter. This can help FEMA understand which costs may be covered by insurance and which remain unmet.
  • Where you lived: A lease or rental agreement, rent receipts, utility bills, or other occupancy records may help show that you lived at the affected address. FEMA may verify some information through government records or an inspection; if it contacts you, ask what fact or document remains unverified.
  • Ownership, when relevant: If you owned the damaged home, records such as a deed or title, mortgage documentation, homeowner’s insurance, property-tax bill or receipt, manufactured-home certificate or title, or purchase contract may help establish ownership. Ownership evidence is particularly relevant to repair assistance for an owned home. Renters should not treat a deed as a universal requirement.
  • Damage: Be ready to describe the disaster-caused damage clearly. If FEMA asks for repair-related bills, estimates, or receipts for an appeal, its guidance says they should include the service provider’s contact information.
  • Income and payment: Have annual household income information available and, if you want direct deposit, your bank account information. Only provide identity and bank details through an official, verified program channel.

Think of this as a preparation list, not a promise that submitting every item will qualify you. FEMA’s supporting-document requests depend on the information it needs to check in your particular application. For example, occupancy records address where you lived, while ownership records relate to owning the damaged property. Insurance letters help clarify insurance coverage; they do not replace the damage description or other application information.

Use a FEMA notice or determination letter to identify missing proof

Supporting documents are often tied to a specific question in your file: for example, whether you lived at the damaged property, owned it, had insurance, or experienced the reported damage. FEMA’s document guidance gives examples rather than requiring every applicant to submit every listed record. Read your determination letter or other notice carefully; it should explain what information FEMA needs and the next step. A missing document can affect processing, so respond with the evidence requested rather than sending unrelated paperwork. FEMA’s examples and case-specific guidance appear in its Individual Assistance questions and answers.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

If a notice says information is missing, use it to focus your response: identify the issue FEMA is asking about, gather the relevant record, and follow the instructions in that notice. If you are unsure what remains unverified, ask the agency which fact or document it needs. Do not assume that a long stack of documents is better than the specific evidence requested.

For continued FEMA temporary housing assistance, assemble the specific records

Continued temporary housing assistance is different from an initial disaster-assistance application. FEMA’s Continued Temporary Housing Assistance Quick Reference Guide, dated March 2024, describes assistance as conditional rather than automatic. It calls for a completed application and supporting documentation. Follow the specific instructions sent with your form or case.

  1. Current lease or rental agreement: Provide a copy signed by both landlord and tenant. The guide calls for the rental-unit location, rent amount, lease duration, and number of occupants.
  2. Proof that prior FEMA rental assistance paid for temporary housing: Keep rent and utility receipts. FEMA’s guide also lists hotel or motel receipts showing the dates, address, and time covered.
  3. Current income documentation for household members age 18 and older: Examples include recent pay stubs, W-2 forms or the most recent tax return, self-employment records, government assistance documentation such as Social Security, or a statement of no income when applicable.
  4. Pre-disaster housing costs, only if you remain responsible for them: Records may include mortgage or lease payments, property taxes, insurance, and essential utilities for the pre-disaster home. This evidence applies only when those costs are still your responsibility.

This is a more defined paperwork set than the initial application list: the lease describes the current temporary rental arrangement, receipts document prior temporary-housing spending, and income records cover adult household members. The pre-disaster expense records are conditional. Include them when you still owe those costs, not simply because you once owned or rented the damaged home.

Keep these records together with the application and any additional items FEMA specifically requests. The guide does not make continued assistance automatic; FEMA evaluates the request under the program’s conditions.

For HUD public housing or a Housing Choice Voucher, ask the local PHA

HUD says applicants apply through a local public housing agency and that documentation can vary by agency. The application gathers information about family income, assets, and family composition. Contact the PHA serving the area where you are applying for its current checklist and local forms. Do not assume that FEMA’s disaster-assistance documents will satisfy HUD or PHA requirements. HUD’s Housing Choice Voucher Tenants page explains the program and local-agency role.

In practice, start by asking the PHA which records and forms it currently wants for those application areas—income, assets, and household makeup. The national HUD guidance does not establish a universal document list for every PHA, so a checklist from another city or another assistance program may not apply to you.

For emergency rent help, check the program in your area

State and local emergency rent programs set their own criteria, so a national list cannot establish exactly what a particular agency will require. Contact the program administrator for its current application and document list. USAGov’s Get emergency rent assistance page directs applicants toward available help. If the displacement followed a disaster, HUD’s Disaster Resources page also points people to FEMA and local assistance channels.

For a local program, confirm both that it serves your situation and which documents it wants before submitting sensitive information. Requirements depend on the program and location; the FEMA or HUD information in this guide should not be read as a substitute for the local administrator’s own directions.

How to prepare and submit records safely

  1. Confirm the program and application stage. Determine whether you are applying for initial FEMA assistance, asking FEMA to continue temporary housing aid, applying through a PHA, or seeking local rent help.
  2. Get the current checklist or notice. Use the program’s official site, application materials, or case-specific letter. For PHA and local aid, ask the administering agency directly because requirements may vary.
  3. Match each record to the fact it proves. For example, a lease or utility bill may show occupancy; a settlement letter may clarify insurance; a signed current lease and rent receipts support a continued FEMA housing request.
  4. Check that the record is readable and complete. Make sure names, addresses, dates, signatures, and relevant contact details are visible. For appeal-related bills, estimates, and receipts, include the service provider’s contact information as FEMA instructs.
  5. Submit through a verified channel and keep a copy. Do not send bank or identity information to someone whose authority you have not verified. Retain copies of what you submit and note any confirmation or follow-up instructions.
  6. Respond to requests with the specific missing evidence. If an agency says it cannot verify a fact, ask which fact remains outstanding and what further documentation it needs rather than assuming every possible document is required.

The goal is to make the relevant information available to the right program, not to collect every record named in every program guide. A determination letter, form instructions, or current local checklist should take priority over a generic list because it explains what is needed for your application.

Where to get help if you cannot find a record

USAGov lists online, telephone, and in-person Disaster Recovery Center routes for disaster-assistance applications. HUD directs disaster survivors to FEMA and HUD-approved housing counseling agencies through its disaster resources page. USAGov also describes Disaster Legal Services for eligible low-income people in federally declared disaster areas, which may assist with certain insurance claims, lost legal documents, home-repair contracts, or landlord problems. Availability depends on eligibility and the disaster area; check the official disaster and local service channels for current access details.

If there is no federal disaster declaration, USAGov notes that state or local agencies and other services may be the route to explore. These sources can help you identify where to apply or which agency to contact; they do not guarantee eligibility or a particular outcome.

Frequently Asked Questions

Do I need to submit every document on this list?

No. The list describes information to prepare and examples of supporting proof, not universal requirements for every applicant. FEMA may request records to verify a particular fact, while PHAs and state or local programs set their own checklists. Follow your program’s application instructions and any case-specific notice.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Do renters need to provide a deed or other ownership proof?

Not as a universal requirement. Ownership records such as a deed, title, or property-tax record may be relevant to repair assistance for an owned home. Renters may instead need occupancy evidence, such as a lease, rent receipts, or utility bills, if the agency asks them to show where they lived.

What documents are needed for continued FEMA temporary housing assistance?

FEMA’s guide calls for a completed application, a current lease signed by landlord and tenant with the unit location, rent, lease duration, and occupant count; proof prior FEMA rental assistance went to temporary housing, such as rent, utility, or qualifying hotel or motel receipts; current income documents for household members age 18 and older; and records of pre-disaster housing costs only if you still owe them.

What if I do not have a federal disaster declaration or cannot find a record?

For disasters without a federal declaration, USAGov points people toward state or local agencies and other services. If a record is missing, read the agency’s notice and ask what fact remains unverified and what documentation it needs. FEMA may verify some information through government records or an inspection, but do not assume that will resolve every missing item.

Will having these documents guarantee housing assistance?

No. Documents help an agency evaluate the information in an application; they do not guarantee eligibility or an award. Assistance is subject to the particular program’s criteria and, for continued FEMA temporary housing assistance, FEMA describes aid as conditional rather than automatic.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

The Bottom Line

Start by identifying the program: initial FEMA assistance, continued FEMA housing aid, a local PHA, or state/local rent help. Prepare the records tied to your situation, but treat examples as examples—not a universal checklist. Follow the program’s current instructions and respond to any notice with the specific evidence it requests.
