Temporary healthcare facility options when a hospital project is delayed

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

When a hospital project slips, the defensible interim choices are a prefabricated or modular healthcare building, a carefully converted existing building, or a temporary tent/mobile treatment system. Select among them by the clinical services and acuity required, how long the space will operate, the site and utilities available, infection-control and accessibility obligations, approval timelines, workforce and equipment, total cost, and how the facility will be removed or reused.

None is a plug-and-play substitute for a licensed hospital department. Before patients enter, the hospital and its authorities having jurisdiction must approve the design, life-safety measures, utilities, clinical operations, staffing, commissioning, and an exit plan.

The three workable interim facility models

1. Prefabricated or modular healthcare buildings

Modular buildings use factory-produced components assembled on a prepared site. They can provide repeatable patient rooms, treatment bays, support spaces and circulation for an interim campus that may operate for months or longer. The World Health Organization’s 2025 guidance on prefabricated modular primary-healthcare facilities identifies rapid deployment, adaptability, scalability and potential cost-effectiveness as advantages while requiring systematic assessment, selection, assembly and performance monitoring.

WHO’s technical report explains why this approach is considered for urgent capacity: conventional construction can take months or years, while some COVID-era modular hospitals reached operational readiness within weeks when designs, procurement and workflows were prepared in advance. That example is an emergency lesson, not a guaranteed schedule. Foundations, ground works, utility connections, permits, equipment, commissioning and staffing can remain on the critical path even when modules are built off-site.

A modular scheme needs structural stability checks, site-specific engineering and, where relevant, seismic design. Factory quality control should continue through transport and installation, followed by inspection and post-installation performance monitoring. Plan for medical gases, HVAC filtration and pressure relationships, nurse-call and information systems, fire detection and suppression, accessible routes, clean and dirty flows, waste handling and secure medication and records areas according to the services provided.

2. Repurposing an existing building

A vacant office, school, hotel, retail building or other suitable structure can be converted when it is close to the population and already has useful power, water, ventilation, loading access and an accessible envelope. WHO’s severe acute respiratory infection facility manual lists repurposed buildings alongside new construction and temporary tents as possible care settings.

Conversion is not simply moving beds into empty rooms. The clinical plan must establish patient and staff circulation, reception and triage, isolation capacity, clean-storage and waste routes, hand-hygiene stations, emergency egress and observation. Engineers must verify floor loading, electrical capacity, ventilation performance, fire protection, accessibility and the feasibility of medical gases or equipment. A building that appears inexpensive can require major upgrades, hazardous-material abatement, acoustic work or landlord negotiations.

Commissioning should test every room and system under realistic operating conditions before opening. Include emergency power, alarms, water quality, temperature and humidity control, communication systems, infection-prevention measures and staff procedures. Existing-building work can start quickly when the property is available, but hidden defects and approval changes can erase the apparent time advantage.

3. Temporary tents and mobile treatment systems

Tents, trailers and other mobile units are suited to short-duration surge, triage, quarantine, respiratory assessment, vaccination or lower-complexity community care where speed and removability matter more than long service life. WHO’s 2024 manual specifically addresses temporary tent solutions for severe acute respiratory infection treatment, quarantine and community-care functions.

They require a full facility design rather than a purchase-and-place assumption. Provide stable flooring, ramps and accessible toilets; weather and wind protection; heating, cooling and ventilation; reliable power with backup; potable water and wastewater arrangements; lighting; fire protection; security; communications; waste segregation; and a controlled patient flow. Determine how beds, oxygen, monitors, medication, cleaning supplies and emergency equipment will be secured and replenished. A tent may be useful for triage while a more robust building houses treatment, or for a defined seasonal surge, but it is rarely appropriate for every hospital service.

How to choose the right interim model

Start with the service package, not the structure. Define the number of patients, peak arrival patterns, acuity, diagnostic and treatment functions, isolation needs, operating hours and the date by which capacity is required. Then score each option against the following factors.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  • Time to safe operation: Include design, procurement, site preparation, permits, installation, equipment delivery, staff training and commissioning, not only fabrication or delivery.
  • Expected duration: A few weeks of triage has different durability and lifecycle requirements from a two-year inpatient unit.
  • Clinical acuity: Critical care, surgery, imaging, dialysis and high-dependency observation demand more robust structure, utilities, infection controls and support than screening or consultation.
  • Capacity and expandability: Decide whether the initial footprint must accommodate later modules, additional utilities or a second shift.
  • Site and logistics: Check land ownership, access for cranes and ambulances, foundations, drainage, parking, deliveries, security and connections to power, water, sewer, data and medical gases.
  • Regulatory exposure: Identify building, fire, accessibility, environmental, historic-preservation, healthcare-licensing and planning requirements in the relevant jurisdiction.
  • Patient and staff safety: Evaluate infection prevention, fire compartments, evacuation, acoustics, privacy, slips and falls, temperature, noise, violence prevention and medication security.
  • Resources: Confirm clinicians, facilities staff, biomedical technicians, equipment, consumables, cleaning, laundry, food, transport and maintenance.
  • Exit strategy: Decide whether the asset will be relocated, reused, leased back, sold, dismantled or disposed of, and budget that work from the start.
Decision factor Modular building Repurposed building Tent or mobile system
Typical role Durable interim campus or staged expansion Care space near an existing patient population Short surge, triage, isolation or community care
Readiness evidence Some emergency examples became operational within weeks; no universal benchmark Not stated; depends on building condition and approvals Not stated; depends on site, weather, utilities and equipment
Expansion Usually designed for repeatable additions Limited by the host building and site Possible with additional units, subject to flow and utility limits
Site work Foundations, access, utilities and installation are required Survey, alterations, fire and accessibility upgrades are required Level surface, anchoring, flooring, utilities, drainage and access are required
Best fit for long operation Generally stronger fit when engineered and maintained Possible if the structure and systems are upgraded Usually limited to defined, shorter-duration functions
Cost benchmark Not stated in the cited guidance Not stated in the cited guidance Not stated in the cited guidance

The table is a planning comparison, not a promise of performance. Local codes, design, climate and clinical requirements can reverse the apparent ranking.

Approvals and readiness steps before opening

Temporary status does not remove legal or clinical obligations. NHS England states that temporary facilities used during construction should meet healthcare standards as far as practicable, with acoustic limitations identified and requirements agreed for the temporary building. Its fire-safety guidance requires temporary patient-care spaces to achieve the same safety levels as permanent healthcare facilities, with fire-safety-adviser input and a new or revised fire-risk assessment before use.

  1. Define the clinical brief. Document services, patient numbers, acuity, hours, isolation categories, diagnostics, medication processes, resuscitation capability, referral destinations and minimum staffing.
  2. Appoint accountable leads. Establish a project manager and named clinical, infection-prevention, facilities, engineering, fire-safety, accessibility, security, procurement and finance leads. Identify the authority having jurisdiction and the person who can stop opening if a condition is unsafe.
  3. Survey the site or host building. Verify title or lease rights, surveys, structure, utilities, flood and environmental constraints, vehicle and ambulance access, loading, parking, neighboring hazards, noise and security. For a conversion, perform a measured survey and hazardous-material review.
  4. Develop the clinical and engineering design. Draw patient, staff, supply, waste and emergency flows. Specify room data, ventilation and pressure relationships, electrical and backup power, water, drainage, medical gases, fire systems, data, acoustics, accessibility and equipment clearances.
  5. Secure permissions. Confirm planning and building permits, fire authority review, healthcare licensing or registration, accessibility compliance, environmental and historic-preservation obligations where applicable, utility approvals, infection-prevention sign-off and any landlord or neighboring-property consents. Requirements vary by federal, state, tribal, territorial and local jurisdiction.
  6. Procure and coordinate. Contracts should cover design responsibility, factory quality, transportation, craning or installation, site works, equipment, supplies, staffing support, maintenance, insurance, mobilization, demobilization and restoration. FEMA’s temporary-expansion playbook treats lease, purchase or construction; equipment and supplies; staffing; wraparound services; clinical care; and mobilization and demobilization as parts of the proposal, not afterthoughts.
  7. Install, test and commission. Complete inspections, fire and alarm tests, generator and emergency-power tests, water and HVAC verification, pressure testing where relevant, medical-gas certification, accessibility checks, equipment calibration, network and nurse-call tests, cleaning validation and infection-control walkthroughs. Record defects, owners and due dates.
  8. Run a go-live exercise. Rehearse registration, triage, deterioration, evacuation, fire, utility failure, security incidents, transfers, death management, waste handling and communications. Train every shift and verify that referral hospitals and ambulance services know the operating plan.
  9. Authorize opening and monitor. Obtain written sign-offs from the responsible clinical executive, facilities lead, infection-prevention lead and fire-safety authority or other required officials. Track incidents, falls, infection indicators, temperature, air pressure, equipment downtime, patient feedback and staff concerns, with a process for corrective action.

Design details that determine safety and usability

Infection prevention and patient flow

Separate arrivals, assessment, treatment, discharge and transfers where possible. Provide hand-hygiene points at entrances and care spaces, clean and dirty utility separation, isolation capability appropriate to the disease risk, adequate air changes and filtration, and a plan for cleaning high-touch surfaces. Avoid crossing waste, linen and food routes with patient flows. If the temporary site cannot provide the required level of isolation or ventilation, narrow its clinical scope and transfer higher-risk patients.

Fire, accessibility and evacuation

Plan compartmentation, detection, alarms, suppression, emergency lighting, exit widths, travel distances, fire doors, evacuation equipment and assembly areas. Ramps, door clearances, toilets, signage, hearing or visual alarms and accessible parking must work for patients, visitors and staff, including people using beds or mobility devices. A tent or mobile unit needs particular attention to anchoring, escape routes and weather-related evacuation.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Utilities, equipment and resilience

Calculate peak electrical demand rather than relying on a standard connection. Provide tested backup power for life-safety and clinical loads, fuel logistics, water pressure and quality, wastewater capacity, heating and cooling under local extremes, oxygen and other gases where required, and redundant communications. Match equipment to the room layout and verify delivery paths, infection-control compatibility, maintenance access and biomedical support.

Acoustics, privacy and security

Temporary construction can transmit noise and vibration, undermining sleep, communication and confidentiality. Identify acoustic limits during design and agree practical requirements, as NHS England recommends. Include visual and acoustic privacy, controlled staff-only zones, duress alarms or other security measures, visitor management, lighting and protection for medicines, records and devices.

Schedule and cost: what to put in the business case

No universal price, capacity or delivery benchmark is established in the cited guidance. Build a project-specific estimate with the authority having jurisdiction and the clinical, facilities, finance and procurement teams. Separate one-time and recurring costs:

  • Design, surveys, permits, legal work and inspections.
  • Lease, purchase or construction of the structure.
  • Ground works, foundations, transport, craneage, installation and utility connections.
  • Clinical equipment, furniture, information technology, security and medical gases.
  • Staff recruitment or overtime, training, cleaning, laundry, food, waste, transport and other wraparound services.
  • Rent, utilities, maintenance, insurance and consumables for the operating period.
  • Mobilization, relocation, demobilization, site restoration, storage, resale or disposal.

Use a schedule that shows design freeze, procurement lead times, site readiness, inspections, equipment delivery, staff onboarding, commissioning and contingency. A module arriving on site does not equal a functioning ward. Long-lead generators, switchgear, air-handling equipment, imaging, oxygen systems, permits or specialist staff can control the opening date.

Continuity planning while the permanent project is delayed

WHO’s 2021 handbook provides procedures and a planning template for minimizing disruption and maintaining services during public-health emergencies. The same discipline applies to a capital-project delay. Define essential services, minimum safe staffing by shift, stock and supplier dependencies, laboratory and pharmacy interfaces, communications, downtime procedures, referral and transfer triggers, and the conditions for reducing or expanding the temporary site.

Set review points tied to the permanent project, demand forecasts, safety data and seasonal risks. Maintain a current map of beds and equipment, contact lists, transport arrangements and mutual-aid agreements. If the temporary site is remote from the main hospital, specify who has clinical authority, how records follow the patient, how results are returned and how deteriorating patients reach definitive care.

Common failure modes and corrective actions

  • Choosing on delivery speed alone: Recheck whether the option can support the intended acuity, ventilation, utilities and evacuation before signing.
  • Underestimating site work: Perform surveys and utility-capacity studies early; make site readiness a milestone separate from fabrication.
  • Opening before commissioning: Use written acceptance tests and a go-live exercise; do not convert an unresolved defect list into a clinical risk.
  • Ignoring workforce and supplies: Confirm rosters, training, oxygen, medications, consumables, cleaning and maintenance for every operating shift.
  • Treating temporary rules as optional: Obtain the same level of fire and patient-safety assurance expected of permanent care space and complete the required risk assessment.
  • Leaving removal undefined: Price demobilization, reinstatement, data and equipment disposition, and responsibilities in the initial contract.
  • Letting scope expand informally: Require clinical and safety review whenever a service, bed count, operating hour or patient group changes.

A practical decision sequence

  1. List the services that must continue, the date capacity is needed and the maximum safe duration of interim operation.
  2. Rule out sites that cannot provide lawful access, fire egress, infection controls, utilities or emergency transport.
  3. Compare a modular building, an existing-building conversion and a tent/mobile system using the same clinical brief and whole-life cost categories.
  4. Obtain preliminary feedback from the building official, fire authority, infection-prevention team, accessibility adviser and healthcare licensing body before committing.
  5. Select the smallest solution that safely meets demand but can expand if the delay or patient volume grows.
  6. Approve a funded plan covering construction, operation, staffing, monitoring and exit, then open only after documented commissioning and sign-off.
