For a leaning, non-self-supporting ladder, use the 4-to-1 setup rule: the base is about 1 foot out for every 4 feet of working length, producing an angle of roughly 75 degrees. A stepladder must be fully opened with its spreaders or locking device engaged; its top step and top cap are not working steps.
This is practical guidance for portable step and extension ladders. US OSHA rules differ between general industry and construction, while UK HSE guidance applies to UK workplaces. Home users should also follow the ladder manufacturer’s instructions and applicable local requirements. None of this replaces a required competent-person inspection or site-specific risk assessment.

Choose a ladder that fits the job
The safest setup starts before the ladder leaves storage. CDC/NIOSH identifies inappropriate ladder selection as a major cause of ladder falls. Select a ladder whose design, length, load rating and condition match the access point and the work you will perform.
Recommended Free Tools
| Decision | What to check | Why it matters |
|---|---|---|
| Ladder type | Use a self-supporting stepladder where the work can be done from an open, stable frame. Use a leaning extension ladder only where a suitable upper support is available. | A ladder must be used in the configuration for which it was designed; a stepladder is not a substitute for a leaning ladder. |
| Working or access height | Measure the height you must reach and account for the ladder’s safe standing limits and any landing access. Do not select a length simply because it is the longest available. | Too short encourages overreaching or standing on prohibited steps; too long can create an unstable or incorrectly positioned setup. |
| Rated capacity | Confirm the label and rated load cover your body weight, clothing, tools and materials. If the capacity label is missing or unreadable, do not use the ladder. | Exceeding the rating can damage the ladder or make it unstable. |
| Ground and support | Identify a firm, level base and a sound upper support. Consider whether doors, vehicles, pedestrians or equipment could contact the ladder. | A correct angle cannot compensate for a weak support or a ladder that can be displaced. |
| Electrical exposure | Where contact with exposed energized equipment is possible on a construction site, use a ladder with nonconductive rails as required by OSHA construction rules. | Rail material is part of the hazard assessment, not an optional accessory. |
| Intended design | Check the manufacturer’s instructions for permitted configurations, height limits, accessories and tasks. | Model-specific limits cannot be inferred from a general rule. |
For a particular ladder recommendation, the job height, task, load and surroundings must be known first. A portable extension ladder may be appropriate for one access point and unsafe for another.

Inspect before climbing
Make the inspection a visual and functional check, not a quick glance. HSE advises a user to check a ladder at the start of the working day and again after a change such as a drop or move. OSHA requires workplace ladders to be inspected before initial use in each work shift and more often as necessary. The following sequence is an accessible pre-use check; it does not replace a manufacturer’s procedure or a legally required competent-person inspection.
1. Examine the stiles and side rails
- Look for bends, twists, cracks, splits, corrosion, dents and other structural damage.
- Check that rails are straight and that no fastener, joint or connection is loose.
- Do not assume a minor-looking bend is harmless; a damaged rail can change how loads are carried.
HSE specifically calls for checking the stiles. OSHA construction rules identify broken or split rails and corroded components among defects that require attention.
2. Check every rung and step
- Confirm that each rung or step is present, secure and free of damage.
- Look for looseness, cracks, excessive wear, contamination, paint, oil, mud or any slippery residue.
- Check that the walking surface is not distorted or compromised.
Broken or missing rungs, cleats or steps are defective conditions under OSHA construction requirements. Clean contamination before use only if the ladder remains structurally sound; cleaning does not repair a damaged component.

- Check each foot or shoe for wear, cracks, missing parts and secure attachment.
- Remove dirt, mud or embedded stones that could create a false contact point.
- Verify that the feet can sit evenly on the intended surface and that any slip-resistant material is present and usable.
HSE specifically directs users to examine the feet. Slip-resistant feet do not replace careful placement or securing, especially on a slippery or contaminated surface.

4. Test stepladder hinges and spreaders
Open the stepladder fully and verify that the hinges move correctly and that the spreaders or locking devices engage and hold. A ladder that will not lock in its open position is not ready for climbing. Never use the top cap or the top step as a step unless the manufacturer expressly identifies a particular standing surface as permitted; ordinary stepladder top caps and top steps are not working steps.
5. Test extension sections and locks
For an extension ladder, inspect the extension hardware, guides and rung locks. Extend and set the sections only as the manufacturer instructs, and confirm that the locks engage fully on both sides before climbing. Do not use an extension ladder if a lock is bent, missing, loose or unable to hold the section.
6. Read the labels and capacity marking
Confirm that warning labels, configuration instructions and the rated capacity are legible. If you cannot establish the ladder’s capacity, identity or permitted use, remove it from service rather than guessing.

When a defect means “do not use”
Do not climb a ladder with a structural or other safety defect. In US general-industry workplaces, OSHA 29 CFR 1910.23(b)(10) states: “Any ladder with structural or other defects is immediately tagged ‘Dangerous: Do Not Use’ or with similar language in accordance with § 1910.145 and removed from service until repaired in accordance with § 1910.22(d), or replaced.” See the full rule at OSHA 29 CFR 1910.23.

Tag the ladder prominently with “Dangerous: Do Not Use” or equivalent wording and physically remove it from the work area so someone else cannot pick it up. Do not improvise a repair with tape, wire, rope, a nailed-on rail or another unapproved fix. Under OSHA construction requirements, a repair must restore the ladder to its original design criteria before it returns to service. If that cannot be demonstrated, replace the ladder.
- Immediate stop-use conditions: cracked, split, bent or corroded rails; broken or missing rungs or steps; loose structural connections; damaged or missing feet; failed spreaders or locks; an extension section that will not lock; unreadable capacity or safety information; or contamination that cannot be removed safely.
- After an incident or move: repeat the inspection after a drop, impact, relocation or other change, even if no damage is obvious.
- After repair: do not return the ladder to work until the repair is verified as restoring the original design and all functions operate correctly.
Set up a leaning extension ladder with the 4-to-1 rule
The 4-to-1 rule applies to a leaning, non-self-supporting ladder, not to a stepladder. OSHA construction rules specify that the horizontal distance from the top support to the foot should be approximately one-quarter of the ladder’s working length. NIOSH describes the resulting angle as about 75 degrees. In plain language, place the base about 1 foot out for every 4 feet of working length.
| Working length | Approximate base distance from support |
|---|---|
| 12 feet | 3 feet |
| 16 feet | 4 feet |
| 20 feet | 5 feet |
| 24 feet | 6 feet |
These are examples of the ratio, not a substitute for the manufacturer’s instructions or a site assessment. “Working length” refers to the ladder length along the rails between the foot and the upper support, not the vertical height of a wall.

1Scan for outdated or missing drivers - takes under a minute2Repair Windows errors before they cause bigger problems3Fix the driver behind crashes, sound loss and screen glitchesImportant exception for certain job-made wooden ladders
OSHA specifies a different proportion for job-made wooden ladders with spliced rails: the horizontal distance is one-eighth of the working length. Do not apply that exception to an ordinary manufactured portable extension ladder.

Position the base and top
- Choose a firm, level surface and a sound upper support. Never level a ladder by placing loose blocks, boxes or other objects under one side.
- Set the base at the 4-to-1 distance, then check that both feet bear evenly and that the rails rest securely against the support.
- Keep the top and bottom areas clear of tools, materials and debris.
- Secure the ladder against displacement. Use the manufacturer-approved method or tie the ladder at a suitable point to a rigid support; where tying is not immediately possible, have another person hold it while the setup is secured.
- Where people, vehicles, doors or workplace operations could strike the ladder, barricade the area or otherwise control the hazard. A ladder in a traffic path needs protection even when its angle is correct.
On slippery surfaces, secure the ladder or use suitable slip-resistant feet, but do not treat the feet as a substitute for placement and securing. OSHA’s construction requirements address stability and protection from displacement; see OSHA 29 CFR 1926.1053.

When a portable ladder is used to access an upper landing in construction, OSHA generally requires the side rails to extend at least 3 feet above the landing. That extension gives the user a handhold while stepping on or off the ladder. If a 3-foot extension is not possible, secure the top to a rigid support and provide a grasping device so the transition can still be made safely.
Keep the landing itself clear and stable. Do not step sideways onto an unprotected surface, use a loose object as a platform, or rely on a ladder that shifts as you transfer your weight.
Configure and position a stepladder
- Place the stepladder on a firm, level surface with all four feet supported.
- Open the front and rear sections fully.
- Engage the spreaders, hinges or locking device and verify that they hold before climbing.
- Keep the top cap and top step clear; use only the steps or standing surfaces identified by the manufacturer.
- Keep the area around the ladder clear and control nearby doors, traffic and equipment that could strike it.
Do not lean a closed stepladder against a wall unless the manufacturer specifically designed and labeled it for that configuration. Do not move, shift or extend any ladder while someone is on it.

Climb and work without defeating the setup
- Face the ladder while ascending and descending.
- Maintain at least one hand grasping the ladder.
- Carry only a load that does not make you lose balance; use a hoist, tool belt or another method for heavier or awkward materials.
- Stay within the ladder’s intended use and rated load.
- Do not overreach to either side. Descend and reposition the ladder instead.
- Keep rungs and steps free of oil, mud, ice, wet paint and other slip hazards.
- Never move, shift or extend a ladder while a person is on it.
These behaviors matter because an otherwise sound ladder can become unstable when a worker reaches beyond the rails, carries an unwieldy load or changes its position while climbing.
Why angle and setup deserve attention
CDC/NIOSH reports that about 40% of ladder-related injury cases involve the ladder sliding out at the base because of an incorrect setup angle. The figure appears on its Ladder Safety page, dated 2024, which cites the 2014 MMWR study “Occupational Ladder Fall Injuries — United States, 2011.” It is an approximate share for the reported ladder-related injury cases in that evidence, not a claim about every ladder incident or every population.
The practical response is to measure the base distance, check the support and feet, and secure the ladder rather than relying on visual judgment alone.
A complete pre-climb decision sequence
- Match: Confirm the ladder type, length, rated capacity and electrical properties fit the task and location.
- Inspect: Check rails, rungs or steps, feet, hinges, spreaders, extension locks, labels and cleanliness.
- Reject: Tag and remove any ladder with a structural, functional or identification defect.
- Place: Use firm, level ground and a sound upper support; never compensate for an uneven surface with loose objects.
- Angle: For a leaning portable ladder, use approximately one foot out for every four feet of working length, unless the specific OSHA job-made wooden-ladder exception applies.
- Secure: Prevent displacement at the top or base and protect the setup from traffic, doors and nearby operations.
- Access: For construction landing access, provide the required 3-foot rail extension or an equivalent secured grasping arrangement.
- Climb: Face the ladder, keep one hand on it, avoid overreaching and do not carry a destabilizing load.
Use official guidance for your jurisdiction
For US general-industry workplaces, review the inspection and removal-from-service requirements in OSHA 29 CFR 1910.23. For construction work, review setup, access, stability, inspection and repair requirements in OSHA 29 CFR 1926.1053. OSHA’s illustrated portable-ladder guidance, including angle and stepladder configuration, is available in the OSHA Ladder Safety eTool.
UK users should consult the HSE’s pre-use ladder check and the requirements that apply to their work. HSE’s timing and emphasis on stiles and feet are UK workplace guidance; they should not be presented as a replacement for local law elsewhere.
CDC/NIOSH also provides a free Ladder Safety resource and app for extension- and step-ladder safety. Use it as an aid alongside the ladder’s instructions and your employer’s procedures, not as permission to overlook a defect or an unsuitable setup.
