How to Keep Hospital Construction Sites Safe While Patients Are Nearby

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Hospital construction can continue beside occupied patient-care areas only when the work is managed as a clinical safety project, not just a building project. The hospital should complete a multidisciplinary pre-construction risk assessment (PCRA) and infection-control risk assessment (ICRA), create a fire-safe, smoke-tight separation, control dust and airflow, preserve emergency routes and critical utilities, coordinate disruptive work with clinical teams, and inspect the controls throughout the job.

Because requirements depend on local building and fire codes, the facility’s patient population, and the project’s intensity, the final plan must be approved by infection prevention, facilities, safety, clinical, security, housekeeping, the contractor, and the authority having jurisdiction.

Why construction beside patients needs a different safety plan

An occupied hospital cannot be treated like an empty commercial building. Patients may be immunocompromised, dependent on uninterrupted power or medical gases, unable to use a detour independently, or especially sensitive to noise and vibration. Construction can also open pathways for dust, moisture and fungal spores; interfere with ventilation; compromise fire protection; obstruct emergency access; and create security risks.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

The hospital therefore has two simultaneous obligations: deliver the construction scope and keep diagnosis, treatment, evacuation and infection-prevention systems functioning. A written plan should connect each construction activity to a patient-care consequence, a control, an accountable person and a response if that control fails.

Start with a PCRA and ICRA before mobilization

Assemble the decision-making team

The assessment should be multidisciplinary. Include facilities and engineering, infection prevention, occupational and construction safety, affected clinical departments, nursing, environmental services or housekeeping, security, fire-safety personnel, the general contractor and relevant subcontractors. Include representatives of any service that could be affected by shutdowns or rerouting, such as information technology, laboratory, pharmacy, imaging and emergency services.

Give the team authority to change the work sequence, require additional controls and stop work when a safety condition is not met. A contractor’s method statement should not override the hospital’s infection-prevention or life-safety requirements.

Assess the actual work area and its neighbors

The PCRA/ICRA should examine, at minimum:

  • Patient vulnerability, including immunocompromised patients and units caring for people at high risk from airborne environmental disease.
  • Air pathways, pressure relationships, supply and return ducts, air-handling units, filtration and the possibility that dust can bypass a barrier through ceilings, doors, shafts or service penetrations.
  • Demolition, drilling, cutting, sanding and other activities that generate dust, moisture or vibration.
  • Existing or suspected hazardous materials, including materials that require a separate regulated survey and abatement plan.
  • Fire detection, suppression, smoke compartments, rated walls, fire doors, egress paths and emergency-service access.
  • Medical gases, electrical distribution, emergency power, water, data, communications and other utilities whose interruption could affect care.
  • Noise, vibration, odors, lighting, public circulation, privacy, security and access to restricted areas.
  • Clean and dirty transport routes for workers, tools, waste and supplies.

Record the baseline conditions, the controls to be used, inspection frequency, escalation contacts and the thresholds that require a pause, relocation or redesign. Reassess whenever the work area, patient mix, season, ventilation arrangement or construction method changes.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Define stop-work triggers

Examples include a breached or unsealed barrier, visible dust outside the containment, loss of required negative pressure, an alarm or fire-protection impairment that is not managed under the hospital’s impairment procedure, an unexpected utility interruption, blocked egress, uncontrolled water intrusion, or work that creates unacceptable noise or vibration during a procedure. The plan should identify who can order a stop and who authorizes restart after corrective action.

Build a true separation system

Use smoke-tight, fire-appropriate partitions

The Joint Commission states: “The Joint Commission standards require that temporary construction partitions be smoke tight and built of noncombustible or limited combustible materials (sheet rock, gypsum board) that will not contribute to the development or spread of fire.” This means ordinary, unrated plastic sheeting is not a substitute for a compliant temporary fire and smoke barrier.

Design the partition as a continuous assembly from the floor or slab to the structural deck or another approved enclosure. Seal joints, corners, penetrations, door frames and service openings. Preserve the required fire-resistance and smoke-control characteristics of walls, doors and dampers. Coordinate any penetration with the hospital’s permit and fire-stopping process, and inspect repairs before they are concealed.

Control every alternate dust path

Dust can travel above a suspended ceiling, through door undercuts, around windows, along utility shafts and through shared return-air paths. Close or seal nearby openings where the risk assessment requires it. Provide a controlled entry with a self-closing door or other approved arrangement, and keep the construction side from becoming a shortcut into patient-care space.

  • One-Handed Loading - Keep one hand free with a 1/4 in. hex chuck that accepts 1 in. bit tips

Do not rely on a barrier’s appearance. A barrier is effective only when its edges, penetrations, pressure relationship and adjacent building systems are working together. Photograph or otherwise record the completed enclosure, then inspect it at the start of each shift or at the risk-based interval specified by the ICRA.

Protect egress and emergency access

Before erecting a partition, map normal and emergency routes with clinical and fire-safety staff. Keep required exits, exit signs, fire doors, hose connections, extinguishers, alarm devices, sprinkler coverage and fire-department access available. If a route or system must be temporarily altered, use the hospital’s approved impairment and interim-life-safety procedures, provide equivalent protection where required, and brief affected staff before the change.

Control dust, airflow, moisture and fungal spores

Set the pressure and airflow strategy

The ICRA should specify how air will move relative to occupied areas. In many projects, the construction zone is maintained at negative pressure to adjacent patient-care space so leakage moves inward rather than outward. The exact arrangement depends on the hospital’s HVAC design, the location of returns and supplies, the containment assembly and local requirements.

Protect or isolate HVAC pathways that could distribute construction contaminants. Do not disconnect, block or redirect a system without engineering review: an improvised change can create positive pressure in the work zone, starve a clinical area of conditioned air, or affect temperature, humidity and pressure relationships elsewhere.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Use engineering controls, not housekeeping alone

CDC guidance calls for impermeable barriers, sealed openings, HVAC monitoring, engineering controls that clean air, daily cleanup and surveillance for airborne environmental disease where appropriate. A portable HEPA air scrubber or negative-air machine can support containment when its airflow, filtration, discharge location, noise, electrical load and maintenance are suitable for the project and approved by hospital engineering and infection prevention.

Place equipment so that it supports the intended pressure direction and does not create a trip hazard or obstruct egress. Verify filters, alarms, duct connections and exhaust paths. A machine that is switched off, has a loaded filter or exhausts into a shared ceiling void is not providing the intended control.

Keep moisture and debris from becoming an infection hazard

Plan for water supply work, wet demolition, leaks, condensation and weather exposure. Stop and isolate a water event immediately; remove wet porous materials and investigate the source before the area is returned to service. Use covered, controlled routes for debris. Keep waste containers closed, remove debris on the schedule set by the risk assessment and clean the construction side daily so material does not accumulate at the containment boundary.

Where the patient population and project risk warrant it, the hospital should use surveillance for airborne environmental disease and define how a suspected construction-related exposure will be evaluated. The assessment should also identify when high-risk patients need relocation or additional isolation.

Protect patients, staff and visitors from the work

Identify patients who need extra protection

Map nearby rooms and units by patient vulnerability rather than treating every occupied area the same. Immunocompromised patients and other high-risk groups may require relocation, isolation, enhanced filtration or a different work sequence. Make the decision with the treating clinical team and infection prevention; construction staff should not make a patient-placement decision independently.

Provide clear communication

Notify affected departments, patients and visitors before work begins. Explain changed routes, expected noise periods, elevator or corridor restrictions and the contact for concerns. Use signs that are readable from the approach direction and update them when routes change. Give clinical staff a direct way to report dust, odors, noise, vibration, temperature changes, access problems or suspected barrier failures.

Control access and movement

Separate construction circulation from patient and clean-supply circulation wherever feasible. Restrict access to the work zone, verify worker identification and keep doors closed. Establish a clean route for materials entering the hospital and a dirty route for waste leaving it. Schedule deliveries and debris removal to avoid medication rounds, procedures and emergency traffic.

Keep clinical operations and emergency systems functional

Protect critical utilities

List every utility serving the work area and adjacent clinical spaces, including normal and emergency electrical power, medical gases, domestic and treated water, drainage, heating, cooling, ventilation, fire alarm, sprinkler, communications and data. For each planned interruption, document the affected rooms, maximum duration, backup source, responsible supervisor, notification list and restoration test.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Use a permit and lockout process for work on energized or pressurized systems. Test temporary connections and emergency power arrangements before a shutdown. A contingency plan should state where patients will be moved, how essential equipment will be powered and who declares the area safe to resume care.

Preserve fire and life safety during impairments

Construction often changes walls, ceilings, detectors, sprinklers and exit routes before the permanent installation is complete. Coordinate impairments with the hospital’s fire-safety program and authority having jurisdiction. Maintain required detection, suppression, compartmentation and alarm notification, or implement the approved interim measures for the duration of the impairment. Never leave an open penetration, removed door or incomplete fire stop untracked.

Maintain emergency-service access

Ambulances, fire responders and internal emergency teams need a reliable route at all times. Keep staging, dumpsters, lifts, stored materials and temporary fencing out of that route. If access must move, mark the alternate route, brief responders and verify it in the field rather than relying only on a drawing.

Manage noise, vibration and sensitive procedures

Drilling, demolition and impact tools can interrupt sleep, communication, examinations and procedures even when dust is fully contained. The project schedule should identify rest periods, medication rounds, operating-room lists, imaging windows, intensive-care activities and other times when noise or vibration is unacceptable.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Use lower-impact methods where practical, isolate vibrating equipment, close doors, maintain communication with the affected clinical area and stop when staff report that conditions are interfering with care. For procedural areas, establish a named clinical contact who can request a delay or shutdown. Advance notice is necessary but not sufficient: conditions in a hospital can change rapidly, so the communication channel must remain active during the work.

Use a phase-by-phase control process

Before mobilization

  1. Complete and approve the PCRA and ICRA with all affected disciplines.
  2. Define the work limits, patient risks, barrier design, pressure strategy, utility plan, fire-safety measures, routes, hours, inspection frequency and stop-work triggers.
  3. Survey for hazardous materials and confirm permits, interim-life-safety measures and authority approvals.
  4. Brief workers and clinical teams on access, hygiene, protective equipment, reporting and emergency procedures.
  5. Document baseline air, pressure, temperature, humidity, noise or other measurements that the risk assessment identifies as important.

At setup and each shift

  1. Inspect the enclosure, seals, doors, penetrations, pressure or airflow indicators and equipment status.
  2. Confirm exits, fire-protection equipment, emergency routes and utility backups remain usable.
  3. Verify that clean and dirty paths are separated and that waste containers and carts are controlled.
  4. Check that planned noisy or disruptive work still fits the clinical schedule.
  5. Record findings, correct defects immediately and escalate any condition that exceeds the approved plan.

During active work

  1. Use wet methods or other dust-suppression practices where appropriate, while controlling moisture.
  2. Keep doors closed, maintain the intended pressure relationship and operate air-cleaning equipment as specified.
  3. Remove debris on the approved schedule and clean the work area daily.
  4. Report and investigate barrier breaches, alarms, water events, utility interruptions, access obstructions and complaints.
  5. Reassess the ICRA when the method, area, patient population or building system changes.

At closeout

  1. Complete construction, fire-stopping, repairs and testing of affected building systems.
  2. Perform terminal cleaning and remove dust, debris and temporary protective materials.
  3. Verify air and barrier conditions, including any pressure, filtration or environmental checks required by the plan.
  4. Obtain infection-prevention, facilities, clinical and safety acceptance before removing containment or returning the area to normal use.
  5. Archive inspection logs, training records, permits, incident reports, corrective actions and final approvals.

Choose controls by risk, not convenience

The PCRA/ICRA should drive the selection and combination of controls. Compare proposed arrangements using the same questions each time:

Decision area What the team should verify
Infection and dust containment Whether the barrier, seals, pressure strategy and cleaning plan prevent migration into patient-care areas.
Fire and smoke control Whether temporary partitions and penetrations meet the required noncombustible or limited-combustible and smoke-tight criteria.
HVAC effect How the arrangement changes pressure, airflow, temperature, humidity and adjacent clinical ventilation.
Egress and emergency access Whether patients, staff and responders retain usable routes throughout the work.
Noise and vibration Whether installation, operation and maintenance will interfere with procedures, sleep or communication.
Installation and removal How quickly the system can be installed, modified and removed without exposing occupied areas.
Monitoring burden What inspections, readings, maintenance, training and documentation the control requires.
Total project cost The full cost for the project duration, including labor, maintenance, cleaning, monitoring and temporary clinical measures.

There is no universal barrier or machine that is correct for every hospital. A solution that works in a vacant shell may be unsuitable beside an intensive-care unit, and a short overnight task may need a different control set from a months-long renovation.

Common failures and how to correct them

“The plastic is taped up, so containment is complete.”

Unrated sheeting may not provide the required fire or smoke performance, and small gaps can connect the work zone to occupied space. Replace or supplement it with an approved smoke-tight, noncombustible or limited-combustible partition, seal penetrations and have the responsible hospital teams inspect it.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

“Negative air is running, so the HVAC problem is solved.”

Negative-air equipment cannot correct an unsealed barrier, a blocked return, an incorrect exhaust location or a failed filter. Verify the whole airflow strategy, monitor the pressure relationship and investigate readings that change unexpectedly.

“Cleaning can wait until the end of the week.”

Accumulated dust and debris increase the chance of migration and make a breach harder to detect. Follow the daily-cleanup requirement, remove waste through the controlled route and clean promptly after any event that releases dust.

“The route is open on the drawing.”

A corridor can be technically open yet unusable because of carts, doors, equipment, poor signs or a construction delivery. Walk the route with clinical and emergency staff, keep it clear and recheck it whenever the site layout changes.

“Staff were notified once.”

Schedules, patient locations and work conditions change. Provide advance notice, repeat it for changed phases, and maintain a live reporting channel so clinical teams can flag unsafe noise, vibration, dust or access conditions.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Make accountability visible

Assign named owners for infection-prevention controls, facilities and HVAC, fire and life safety, utilities, security, housekeeping, contractor supervision and clinical communication. The inspection record should show the date and shift, inspector, barrier and pressure findings, equipment status, route and utility checks, deficiencies, corrective action, escalation and release to continue.

Trend recurring defects rather than closing each one in isolation. Repeated door breaches, pressure alarms, delayed debris removal or complaints about a particular task indicate that the method or schedule needs revision. Contractor compliance records should be retained with the hospital’s project and safety documentation.

When can the hospital remove the barriers?

Removal is a controlled transition, not the final demolition task. The construction scope must be complete; penetrations, fire stopping and affected utilities must be tested; dust and debris must be removed; required air or pressure conditions must be verified; and the receiving clinical area must accept the space. Perform terminal cleaning before turnover, then remove temporary barriers in a way that does not release trapped dust into occupied areas. Archive the monitoring and incident records so the hospital can demonstrate how the risks were managed.

A practical standard for occupied-hospital construction

Safe work beside patients is achieved by layering controls: a multidisciplinary PCRA/ICRA, a continuous smoke-tight and fire-appropriate separation, managed airflow and dust removal, protected utilities and emergency routes, patient-specific precautions, coordinated noise and communication plans, and documented inspection with authority to stop. Treat every change in construction or patient care as a reason to reassess the plan. That discipline allows construction to proceed without making patients, staff or responders absorb risks that the project team could have controlled.
