Make the situation safe first, then record exactly what you saw and report it to the building owner, manager or other person responsible for fire safety. Do not alter a fire door, remove fire-stopping material or attempt a repair. An apparent defect may be cosmetic, unrelated to fire protection, or significant only in combination with other conditions. Competent assessment—not appearance alone—determines the risk and the remedy.
This guide explains a practical evidence and reporting process. The legal roles and inspection intervals described are for England; other UK nations and other countries use different rules and authorities.

1. Deal with immediate danger before gathering evidence
If there is a fire, smoke or an immediate threat
Follow the building’s emergency instructions and contact emergency services. Evacuate or shelter as instructed. Do not delay an emergency call to take photographs, measure a gap or collect documents. Do not enter a smoke-filled area, approach a damaged structural element or prop open a fire door to inspect it.
If the concern is not immediately dangerous
Keep clear of the suspected defect and avoid touching, moving, drilling, painting or cleaning it. A door closer, intumescent seal, fire-stopping product or protective coating can be disturbed by well-intentioned investigation. If the area is unsafe to access, report that fact rather than trying to obtain a closer image.

2. Define what you actually observed
Use “suspected fire-safety defect” until a competent person confirms a defect and its significance. Separate observation from interpretation:
- Observation: “The communal corridor door did not close fully when released at 10:15 on 17 October 2026.”
- Inference: “The door may not provide its intended fire separation.”
- Unverified conclusion to avoid: “This is definitely a 30-minute fire door and is illegal.”
Visual evidence can show a condition that needs attention, but it usually cannot establish a door’s original specification, installation quality, fire-resistance period or overall performance. The same caution applies to dark boards, sprayed coatings, damaged cladding and missing-looking material: they may or may not be structural fire protection or another fire-safety feature.
Details to record at the time
- Date and time, including the time zone if records may be shared outside the UK.
- Exact location: building address, block, floor, stair or corridor, room number and a nearby fixed reference.
- The component or area: for example, flat entrance door, riser cupboard, column, beam, ceiling penetration, compartment wall or external wall.
- What was visible, measured or heard, without guessing the cause.
- How you observed it: routine use, an alarm test, maintenance visit, move-in inspection or another circumstance.
- Whether anyone was exposed to an immediate hazard, and whether access was restricted.
- Names or job titles of people notified and the date and time of every response.
Photographs and video
If it is safe and permitted, take a wide photograph that fixes the location, followed by closer images showing the condition. Include a scale only if placing it does not require touching or disturbing the feature. Keep the original files, not just compressed copies in a messaging app, and note when and how they were taken. Do not publish images that reveal residents’ personal information or security details.

Send the report to the building owner, landlord, managing agent, facilities manager or other person who controls the relevant premises. For a suspected faulty fire door, London Fire Brigade’s public guidance directs people to report it to the building owner and says: “Never carry out any repairs or work on your fire door.”
In premises covered by England’s Fire Safety Order, the legally responsible role is the Responsible Person. Depending on the building and arrangements, that may be an owner, employer, landlord, managing agent or another person with control. A resident does not become the Responsible Person merely by noticing a problem.

What to include in the initial report
- State that you are reporting a suspected fire-safety defect.
- Give the exact location and component.
- Describe only the condition you observed, with date and time.
- Attach safe, relevant photographs and identify any witnesses.
- Explain any immediate access or life-safety concern.
- Ask for confirmation that the report was received, the name or role of the person who will assess it, and any interim instructions.
Use the building’s emergency or out-of-hours number when the normal office channel could leave people exposed. Keep a copy of the message, attachments and delivery confirmation. If someone reports the issue verbally, make a dated note of what was said and send a short written confirmation.
4. Obtain a competent assessment
The Building Safety Regulator’s guidance on damage to structural fire protection advises first establishing whether the damaged material is structural fire protection at all. Records may identify its design and purpose; where they do not, a competent person may be needed. It is not always possible to identify fire-protection material by looking at it.
What “competent” means in practice
The responsible building party should select a person or organisation with suitable knowledge, training, experience and independence for the specific feature. A fire-door inspection, a compartmentation survey, a structural-fire-protection assessment and an external-wall fire-risk review are different tasks. Ask what competence is relevant to the item, what evidence will be inspected, and what the assessor will provide in writing.
A visual walk-through may be appropriate for an initial triage, but it is not automatically a technical examination. For example, a routine check can identify that a door does not close or that a seal is visibly damaged; it cannot by itself prove the door’s tested configuration or fire performance.
Information that can help the assessor
- Approved drawings, fire strategy documents and fire-risk assessments.
- Schedules identifying fire doors, fire-stopping systems, compartment walls or structural protection.
- Product, installation and inspection records.
- Photographs from before the damage or alteration.
- Records of refurbishments, drilling, cable work, leaks, impact or unauthorised work.
- Previous defect reports, temporary controls and completed repairs.
5. Control risk while the decision is being made
The responsible party should consider whether a temporary control is needed before the assessment or repair is complete. The appropriate measure depends on the finding, the people affected and the building’s fire strategy. Possible controls include restricting access to an area, preventing use of a nearby car park, arranging additional patrols or providing an alternative route. The Building Safety Regulator gives restricting use of a car park near a damaged column as an example of a short-term measure; it is not a universal instruction.

Only the responsible building party should decide and communicate controls that affect evacuation, access or building operations. Record who made the decision, when it starts, what condition triggers review and when it will be checked. Do not wedge a fire door open, remove a closer or cover a suspected defect to make the area look tidy.

What a basic check can identify
Current England guidance for relevant blocks describes simple checks of operation and visible condition. A person carrying out a routine check can look for:
- Whether the door closes fully when released.
- Visible damage to the door leaf, frame, glazing, seals or hardware.
- Obvious gaps around the door or frame.
- Signs of alteration, wedging, drilling or other interference.
Record the result, location and date. If a door repeatedly fails to close, note the circumstances—such as a latch that does not engage or a closer that stops short—without attempting to adjust it.
Inspection intervals in England’s relevant blocks
For communal fire doors in blocks covered by the Fire Safety (England) Regulations 2022, checks are required at least every three months. Flat entrance doors must be checked using best endeavours at intervals no longer than 12 months. These are statutory routine-check intervals for the specified premises and doors; they are not a guarantee that every building has the same duty.
What a visual check cannot prove
The absence of a certification mark or a visible smoke seal does not, by itself, establish that an existing door is unfit for purpose. Government guidance says adequacy is considered through the fire-risk assessment. Likewise, a door should not be replaced solely because it does not meet a current new-build standard. A competent contractor should decide whether adjustment, repair, replacement or another measure is necessary.

Do not remove panels, seals or hardware to search for a label. Do not sand, plane, repaint, fit a new closer or install seals yourself. London Fire Brigade specifically warns the public not to carry out repairs or other work on a fire door.
7. Investigating damage to structural fire protection or compartmentation
Damage may result from ordinary wear, a specific incident or deliberate damage, or planned or unauthorised work. It may involve protection applied to a column or beam, a compartment wall or floor, service penetrations, a riser, or another element intended to resist fire spread.
Describe the condition without naming the material
Write “a section of board or coating is missing from the column” rather than “the fireproofing has failed” unless a competent person has established that fact. Note the approximate size and location, exposed substrate, cracks, impact marks, water damage, nearby cables or pipes and any recent work. Do not scrape the surface or remove debris to identify it.
What the responsible party should establish
- Whether the affected material is actually structural fire protection or another construction layer.
- What element it protects and what fire performance the design requires.
- Whether the condition affects compartmentation, structural stability, escape or fire-service operations.
- Whether the cause is continuing, such as a leak, construction activity or unauthorised alteration.
- What urgent or short-term controls are proportionate.
- What repair method and completion evidence are required.
Repairs should be arranged by the responsible party and completed to the required standard by competent people. Keep records of the assessment, decision, products or methods used, installer, inspection, photographs and sign-off. A closed work order without evidence of the completed condition is not a complete investigation record.
8. External walls and high-rise residential buildings
For higher-risk residential buildings in England, fire-safety information shared with the fire and rescue service includes the external walls and their materials, known defects, assessed risk and mitigating steps. Identifying a cladding or insulation material alone may not explain the fire risk; specialist knowledge may be needed to assess the complete wall build-up, detailing and condition.

The higher-risk building regime applies to buildings at least 18 metres high or at least seven storeys high and containing at least two residential units, under the cited England regime. Not every block or building is in this category. The Accountable Person manages building-safety risks within that regime, while the Responsible Person’s Fire Safety Order duties remain a separate role. A report should therefore identify the building type and send the concern to the appropriate contacts rather than assuming one authority handles every issue.
9. Escalate when the concern is ignored or inadequately controlled
First check whether the owner or manager has acknowledged the report, appointed an assessor and put proportionate controls in place. Provide new information, such as worsening damage or a failed temporary measure, through the same documented channel.
In England, fire and rescue authorities enforce the Fire Safety Order. They may inspect records and take enforcement action, but a complaint does not guarantee a particular outcome or timetable. The Building Safety Regulator has responsibilities within the higher-risk-building regime. The correct route depends on jurisdiction, building type, defect and urgency.

Prepare an escalation pack
- A dated description of the observation and every subsequent change.
- Location photographs and a simple plan or marked-up image.
- Copies of reports, replies, call notes and missed response deadlines.
- Any assessment, temporary-control decision and repair evidence.
- A clear statement of current risk, access restrictions and people potentially affected.
- The building’s owner, managing agent, Responsible Person and, where relevant, Accountable Person contact details.
Do not overstate the case by assigning a fire rating, declaring a breach or alleging a cause that has not been established. Precise records are more useful to an authority than an unsupported conclusion.
10. Keep a durable investigation record
The Building Safety Regulator recommends a system for reporting and investigating possible building-safety issues. A simple register can be maintained electronically or on paper, provided entries are dated and protected from accidental alteration.

| Suspected damage to a column, beam or compartment barrier | Prevent disturbance, document and request identification of the construction element | Responsible building party and a competent specialist |
| Possible external-wall defect in a higher-risk residential building | Record location, materials if known and visible damage; notify the relevant building-safety contacts | Accountable Person and competent fire/building-safety specialists, alongside Fire Safety Order responsibilities |
| No meaningful response | Preserve the reporting trail and use the applicable regulator or fire-and-rescue escalation route | The authority with jurisdiction for that building and issue |
12. Common mistakes that weaken a report
- Repairing first: changing a door or removing material can hide the original condition and create a new defect.
- Calling a material by guesswork: a coating or board may not be fire protection.
- Reporting only verbally: without a dated record, the response trail is difficult to verify.
- Using a close-up without location context: an assessor cannot reliably identify the affected area.
- Treating routine checks as certification: a door that passes a simple closing check has not thereby been proven to meet its design specification.
- Assuming current new-build standards apply retrospectively: adequacy of an existing fire door is considered through the fire-risk assessment.
- Escalating to the wrong regime: England’s Fire Safety Order, higher-risk-building arrangements and other jurisdictions have different roles and thresholds.
- Closing the case on a promise: retain evidence of the assessment, control, repair and completion, not just an assurance that work was arranged.
