How to Compare Medical Office Lease Costs Beyond the Base Rent

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

The cheapest medical-office lease is not the one with the lowest advertised rent. Compare each proposal on the same rentable-square-foot basis and across the entire initial term, then add pass-throughs, utilities, facility services, escalations, build-out, opening cash, and exit obligations. Finally, confirm that the premises can legally and physically support your clinical use. A lower rate can become the more expensive choice when the suite needs major HVAC, plumbing, electrical, accessibility, waste-handling, or restoration work.

Why base rent is an incomplete number

Base rent is the landlord’s charge for occupying the premises. It is only one line in the occupancy budget. Depending on the lease, the practice may also pay property taxes, building insurance, common-area maintenance (CAM), utilities, janitorial service, waste removal, parking-area maintenance, grounds care, shared-area utilities, HVAC maintenance or replacement, and interior repairs. A lease labeled “gross” can still pass through particular costs, while a lease labeled “NNN” can define those costs narrowly or broadly. The labels are starting points, not a substitute for reading the allocation clauses.

Lucien W. Roberts III, M.H.A., FACMPE, author of a Physicians Practice leasing article, puts the purpose plainly: “Understanding these costs will help practices negotiate better leases.” That understanding comes from converting every proposal into the same cash-flow model, not from comparing headline rates.

Start with a common area and pricing basis

Separate usable and rentable square feet

Record the suite’s usable square feet, the quoted rentable square feet, and the rentable-to-usable factor. Rent is often charged on rentable area, which includes the tenant’s usable space plus a share of corridors, lobbies, restrooms, mechanical rooms, or other common areas. If one proposal quotes usable area and another quotes rentable area, their rates are not comparable until you normalize them.

For example, the New Hampshire Small Business Development Center illustrates a 1,500-square-foot usable suite with a 15% common-area factor. The billable area becomes 1,725 rentable square feet (1,500 × 1.15). At the same quoted rate, the larger billable area produces a higher monthly payment than a quote based only on 1,500 square feet.

Normalize the quote’s units

Write down whether the rate is annual or monthly, whether it is quoted per square foot or for the whole suite, and whether it applies to usable or rentable area. Convert all proposals to a single format, such as annual dollars and monthly dollars on rentable square feet. Confirm whether parking, storage, signage, or ancillary rooms are included in the stated area or billed separately.

Use the same assumptions for every proposal

Do not let one proposal use a landlord estimate for operating expenses while another uses a fixed contractual amount without labeling the difference. Identify which figures are fixed, which are estimates, and which are reconciled to actual costs. Keep the same opening date, term length, escalation assumptions, operating hours, and clinical scope when comparing sites.

Build a complete occupancy-cost ledger

Create one row for every cost category. Classify each row as recurring, one-time, refundable, or variable. The following structure prevents commonly missed items.

Cost category What to capture Questions to ask
Base rent Starting annual and monthly rent, area basis, commencement date, rent-free periods, and scheduled steps or index adjustments. Is the quoted rate annual or monthly? Does rent begin at possession, opening, or a stated commencement date?
Taxes, insurance, and CAM Current estimate, prior-year actuals if available, pro-rata share, exclusions, caps, management fees, and reconciliation timing. How is the tenant share calculated? Are vacant areas included in the denominator? Can records be audited?
Utilities Electricity, water, sewer, gas, internet, telephone, and any separately metered or submetered charges. Are utilities included, allocated by formula, or billed directly by the provider? What happens during extended or after-hours use?
Facility services Janitorial work, shared-area utilities, grounds, parking-area maintenance, garbage, and medical or hazardous-waste removal. Which services are included in CAM and which are direct tenant contracts? Are clinical waste volumes or pickup frequencies limited?
HVAC and interior maintenance Routine service, repairs, replacement reserves, filters, controls, and interior-system maintenance. Who pays for major equipment failure or replacement? Are after-hours run times billed separately?
Escalations Base-rent steps, index formulas, expense-growth assumptions, and caps or floors. Is an increase fixed, tied to an index, or based on actual expenses? Is there a notice and reconciliation process?
Tenant improvements Allowance, landlord-delivered work, practice-funded work, eligible costs, approval rules, retainage, and reimbursement date. Does the allowance cover design, permits, technology, medical waste rooms, shielding, and commissioning, or only construction?
Opening cash Deposit, prepaid rent, legal and architecture fees, permits, cabling, signage, moving, equipment installation, and working capital during build-out. Which amounts are refundable? When must each payment be made relative to reimbursement?
Exit costs Restoration, removal of improvements, disposal, final cleaning, repairs, moving, downtime, and rent through the surrender date. Which improvements become the landlord’s property? Must specialized rooms, wiring, shielding, or signage be removed?

Use a full-term, cash-flow comparison

Step 1: Capture the premises and term

  1. Record usable and rentable square feet, the common-area factor, suite number, permitted use, possession date, rent commencement date, initial term, options, and notice deadlines.
  2. Record the quoted rate and convert it to annual and monthly amounts on the same rentable-area basis.
  3. List every space included in the deal: exam rooms, procedure rooms, waiting areas, storage, staff areas, parking, and any separately leased rooms.

Step 2: Calculate first-year recurring cash

  1. Enter the monthly base rent.
  2. Add the landlord’s current estimate for taxes, insurance, CAM, and other pass-throughs as separate lines.
  3. Add utilities, parking, janitorial, medical waste, internet, and after-hours HVAC that the practice will pay directly or through a surcharge.
  4. Mark each line as fixed, estimated, or subject to reconciliation. Do not combine a landlord estimate with a guaranteed amount in one total without showing the distinction.

Step 3: Model every change during the initial term

Enter each scheduled base-rent step in the month it occurs. Then model the lease’s method for adjusting taxes, insurance, CAM, utilities, and other operating expenses. A fixed annual increase, a consumer-price index formula, and a reconciliation to actual expenses produce different results. Show the assumptions next to the calculation so the output is a planning model rather than an interpretation of ambiguous lease language.

  • One-Handed Loading - Keep one hand free with a 1/4 in. hex chuck that accepts 1 in. bit tips

Step 4: Add concessions and build-out economics

  1. List free-rent months and the exact conditions for receiving them.
  2. List the tenant-improvement allowance and the work the landlord will deliver.
  3. Separate eligible project costs from costs the allowance excludes, such as design, permit fees, technology, equipment, specialty finishes, or overruns.
  4. Record when invoices must be submitted, what inspections or lien waivers are required, and when reimbursement is expected.
  5. Show the practice’s temporary funding need while paying contractors before reimbursement.

Step 5: Add opening and closing cash

Include deposits, prepaid rent, legal review, architecture and engineering, permits, cabling, signage, moving, equipment installation, and working capital during downtime. Keep refundable deposits separate from consumed costs, but show both in the peak cash requirement. At the other end, include restoration, removal, final cleaning, repairs, moving, and any rent or operating expenses during a planned move.

Step 6: Report three decision numbers

  • First-year monthly cash outlay: the amount the practice must pay in a normal operating month after recurring charges are added.
  • Peak pre-opening cash requirement: the largest cumulative funding need before the practice receives reimbursements and begins collecting revenue.
  • Total and effective cost over the initial term: all recurring and one-time costs, less documented concessions, shown with the escalation and pass-through assumptions.

Read NNN, CAM, and operating-expense clauses line by line

Triple-net language commonly signals that the tenant pays amounts beyond base rent, often taxes, insurance, and CAM. The actual lease controls the scope. A medical-practice leasing checklist identifies water and sewer, electricity, internet and phone, property taxes, insurance, CAM, HVAC routine maintenance and repair or replacement, parking-area maintenance, shared-area utilities, grounds, garbage, hazardous-waste removal, and interior maintenance as items that may sit outside base rent.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Verify the tenant-share formula

Find the definition of the tenant’s pro-rata share. Confirm whether the denominator uses rentable or usable area, how separately leased areas are counted, how vacant space is treated, and whether the landlord can remeasure the building. A small change in the denominator can change every pass-through year.

Identify exclusions and management fees

Check whether the operating-expense definition excludes leasing commissions, landlord financing, costs of correcting code violations that predate the lease, capital improvements that benefit the building rather than the tenant, and tenant-specific work. PracticePath presents exclusions for leasing commissions and tenant-specific work, along with caps and audit rights, as negotiation ideas—not universal rules. Ask whether management fees are included, capped, or calculated on a separate basis.

Understand estimates, reconciliations, and audit rights

Determine when the landlord sends an annual statement, how estimated payments are reconciled to actual expenses, and whether the tenant can inspect supporting records. Request an annual CAM statement showing the pro-rata distribution. Ask about deadlines for disputing a reconciliation, credits for overpayments, and payment timing for a shortfall.

Negotiate control where possible

Caps, exclusions, notice requirements, and documentation rights can reduce uncertainty, but tax, insurance, utility, and other categories may be treated differently in the contract. Have counsel test each proposed cap against the expense definition and the practice’s likely operating pattern.

Model escalations instead of assuming the first-year rate lasts

A proposal can look inexpensive because its first-year base rent is low while its later steps or pass-through exposure are high. Build a month-by-month or year-by-year schedule for the entire initial term. For each period, show base rent, each pass-through category, direct utilities, facility services, and any concession.

Use the exact contractual formula where it is known. If the lease allows an index adjustment, model a range of outcomes and label it as a scenario. If operating expenses are estimated, show a low, planning, and high case rather than presenting one estimate as a promise. Keep capital repairs, taxes, insurance, and utilities separate when the lease treats them differently.

Renewal options deserve their own schedule. Record the option exercise deadline, the rent-setting method, any market-reset language, expected improvements, and whether a new allowance is available. A low initial term can be followed by a costly renewal if the option price or required work is unclear.

Value tenant improvements and free rent by both economics and timing

Tenant-improvement allowances are not unlimited cash

An allowance reduces the practice’s net project cost only for eligible work and only when the landlord actually funds it. The lease may require approved plans, competitive bids, permits, inspections, lien waivers, certificates of occupancy, or paid invoices before reimbursement. Design fees, medical equipment, cabling, shielding, specialty plumbing, and overruns may fall outside the allowance.

Free rent changes timing, not necessarily project cost

Free rent lowers scheduled occupancy payments during specified months, but it may not cover construction-period rent, utilities, insurance, service contracts, or financing costs. Record the concession in the month it applies and check whether it is lost after a default or if the lease commencement date changes.

Calculate the cash bridge

Make a separate schedule for deposits, contractor draws, professional fees, permits, equipment delivery, and reimbursement receipts. This reveals the maximum amount the practice must finance even when the allowance makes the long-term economics acceptable. A proposal with a larger allowance can still create a larger cash crunch if reimbursement is delayed.

Test whether the premises can support the clinical use

Medical suitability is part of lease cost. A low-rent suite that cannot support the care model may require expensive changes, prolonged approvals, or a move before the term ends.

Confirm legal and regulatory permissions

  • Verify that the lease’s permitted-use clause covers the intended specialty, procedures, imaging, laboratory activity, and ancillary services.
  • Confirm zoning, medical-use permissions, certificate-of-occupancy requirements, signage rules, and local licensing conditions with the relevant authorities.
  • Check ADA and life-safety requirements, including accessible routes, restrooms, exits, fire protection, and any required upgrades.

Inspect building systems

  • Evaluate HVAC age, capacity, controls, filtration, operating hours, and replacement responsibility.
  • Confirm electrical service, emergency power where required, panel capacity, data pathways, and equipment loads.
  • Verify plumbing capacity, hot-water service, drains, medical-gas needs if applicable, and locations for sinks and treatment rooms.
  • Identify water intrusion, roof history, indoor-air concerns, and responsibility for correcting existing conditions.

Check patient and staff operations

  • Assess patient access, accessible parking, staff parking, ambulance or delivery access, elevator capacity, and wayfinding.
  • Confirm after-hours HVAC and security arrangements, including the surcharge for evenings, weekends, or extended procedure schedules.
  • Plan for clinical finishes, cleanable surfaces, biohazard and medical-waste storage, sharps handling, and regular pickup routes.
  • Determine whether radiation shielding, specialized ventilation, or infection-control measures are needed for the specialty.

Commercial healthcare guidance and MedCBO’s planning tool identify these items as screening prompts. MedCBO expressly treats its result as an economic estimate, not a lease interpretation. Verify jurisdiction-specific requirements with qualified architects, engineers, contractors, attorneys, and authorities having jurisdiction.

Review clauses that create costs later

HVAC and major-system responsibility

Distinguish routine filters and service from compressor, rooftop-unit, controls, and full replacement obligations. A clause that makes the tenant responsible for replacement can outweigh a modest base-rent discount.

Assignment and sublease

Check whether the landlord’s consent is required, whether consent can be withheld unreasonably, and whether a transfer triggers fees, recapture, personal guarantees, or continuing liability. Flexibility has economic value if the practice merges, relocates, or changes specialty.

Casualty, interruption, and rent abatement

Read the rules for fire, flood, utility failure, and other casualty. Determine when rent abates, who pays for temporary space, what happens to tenant improvements, and when either party may terminate. A lease without workable abatement can leave the practice paying while unable to treat patients.

Renewal and surrender

Record notice deadlines and the rent formula for each option. At surrender, determine whether improvements become the landlord’s property, may remain, or must be removed. Restoration can include partitions, specialty plumbing, wiring, shielding, signage, and damaged finishes. Federal facilities guidance from HHS illustrates how lease clauses can allocate improvement financing and restoration; that federal policy does not govern a private physician’s lease.

Compare proposals on six decision axes

Axis What to compare Why it can change the decision
First-year all-in occupancy Base rent plus pass-throughs, utilities, services, and required recurring charges. Shows the cash burden when the practice opens, rather than the advertised rate.
Initial-term cost All recurring and one-time costs under disclosed escalation and expense scenarios, less concessions. Reveals whether a low starting rate remains economical over the commitment.
Opening cash Deposits, prepayments, build-out funding, professional fees, and reimbursement timing. Tests whether the practice can fund the launch without a damaging liquidity gap.
Expense uncertainty Caps, exclusions, estimate-to-actual reconciliation, documentation, and audit rights. Measures how much of the future bill the practice can predict or control.
Clinical fit Permitted use, systems capacity, accessibility, parking, waste handling, and required construction. A space that cannot support care is not a bargain at any rent.
Flexibility and exit Renewal pricing, assignment, sublease, casualty rights, restoration, and downtime. Protects the practice if its size, ownership, specialty, or location needs change.

Questions to send with the letter of intent or lease draft

  • What exact area is being billed, and what common-area factor produced it?
  • Which taxes, insurance, CAM, utilities, and services are included in base rent?
  • Can the landlord provide prior-year operating-expense statements and the current budget?
  • How is the tenant’s pro-rata share calculated, and how are vacancies treated?
  • Which costs are excluded, capped, or subject to audit?
  • Who pays for HVAC maintenance, emergency repair, replacement, and after-hours operation?
  • What work is included in the tenant-improvement allowance, and what documentation triggers payment?
  • When does rent commence, and what happens if permits or construction delay opening?
  • Are deposits refundable, and which pre-opening costs are reimbursable?
  • What is the permitted medical use, and who bears the cost of zoning, accessibility, life-safety, or certificate-of-occupancy work?
  • What are the renewal rent formula, option deadlines, transfer rules, casualty rights, and surrender obligations?

How to interpret published cost figures

A 2024 United Health Realty Trust annual report reported weighted-average rental rates on its medical-office lease renewals at approximately 3% above rents on the expired leases. The same report reported weighted-average tenant-improvement costs of approximately $7 per square foot for new or renewed medical-office leases. Those are one company’s portfolio results, not a national benchmark, quote, or forecast for a particular practice. The report also notes that rates, improvement costs, and concessions vary with property, occupancy, building age, local conditions, proximity to hospitals, and competing supply.

There is no independent national figure that establishes a typical all-in medical-office lease cost. Commercial guides can help identify categories and questions, but they do not replace the proposed lease, a site inspection, or local professional advice.

Make the final choice

Select the proposal that best supports the practice’s care model and cash capacity after the full-term model is complete. A lower base rent should lose its apparent advantage if it carries uncapped, poorly documented pass-throughs, requires major unfunded infrastructure work, delays reimbursement, or creates expensive restoration obligations. Conversely, a higher rate can be economical when it includes usable area, reliable building systems, meaningful landlord work, predictable expenses, and flexibility at renewal or exit.

Have a commercial real-estate attorney review the allocation language, and use qualified design and construction professionals to verify medical suitability, code requirements, and a realistic build-out budget before signing. Lease law, taxes, accessibility, occupancy approvals, and medical-use permissions depend on the jurisdiction and the property; the executed lease and applicable local requirements control.
