Future-ready hospitals are designed to keep treating patients when earthquakes, floods, landslides, heat, drought or utility failures disrupt normal operations. Peru’s experience shows that resilience is not a single structural specification: it is a service-continuity strategy that combines hazard-aware siting, locally appropriate building systems, dependable utilities, maintainable equipment and coordinated health networks.

The most useful lesson is to design for the actual place. A hospital on Lima’s coast cannot use the same assumptions as a health center in Cusco’s highlands or Amazonas’s rainforest. Peru’s policy and pilot work provide a practical framework for making those distinctions without abandoning common safety objectives.

What a resilient hospital must achieve

The Pan American Health Organization (PAHO) defines a safe hospital as “a facility whose services remain accessible and functioning at maximum capacity and within the same infrastructure immediately following a natural disaster.” That definition sets a higher bar than preventing a building collapse. Patients must still reach the facility, essential departments must operate, staff must be able to work, and power, water, communications and medical gases must remain available or be restored quickly.

Structural strength remains fundamental, especially in a country exposed to major earthquakes. But a structurally intact building can still fail as a hospital if access roads flood, oxygen systems lose power, a plant room is inundated, information systems go offline or damaged equipment cannot be repaired. Design reviews should therefore ask how long essential services can continue, at what capacity and with which dependencies, rather than stopping at a code-compliance checklist.

Why Peru is a useful case study

Peru combines a highly active seismic setting with sharply different climates and landscapes. The World Bank’s Peru Integrated Health Networks appraisal identifies the Nazca–South American plate boundary, El Niño-related extreme precipitation, flooding and drought among the risks that affect health infrastructure. Landslides and slope instability add another layer of exposure in mountainous areas, while coastal facilities face tsunami and drainage concerns.

El Niño Costero demonstrated the consequence of ignoring local climate. The World Bank reported that first-level health infrastructure had not been designed for the heavy rainfall and floods that occurred. The lesson is not that every facility needs the same flood defenses; it is that rainfall intensity, drainage, catchment behavior and access routes must be part of the initial site and building decisions.

Peru also has an institutional foundation for systematic assessment. A 2021 World Bank account said PAHO’s Hospital Safety Index had been used to evaluate 91% of the country’s hospitals and institutes, and that more than 90% of Ministry of Health hospitals had been assessed. Peru’s national safe-hospitals policy, published by the Ministry of Health in 2010, sought facilities that remain stable, accessible and operational at maximum installed capacity after a destructive event. The policy was updated and approved in 2017, requiring health facilities to integrate risk-reduction procedures.

The same World Bank account reported US$24 million allocated for planned retrofits at five Lima hospitals, with completion expected by 2022. That was a historical expectation, not confirmation that all work was completed. It also recorded growth in Peru’s broader disaster-risk and emergency-response budget program from US$19 million in 2011 to US$400 million in 2019; those figures are national disaster-risk context, not a hospital-construction budget.

Three regional prototypes, three different design problems

The Inter-American Development Bank (IDB) described a method applied to three public projects representing Peru’s principal geographic conditions. The projects were adaptations of already viable investments, using Peruvian public-investment procedures and health-sector rules. The purpose was to add resilience and environmental-performance criteria without changing each project’s investment logic or administrative status.

Prototype Setting Design implication
Huaycán Hospital Urban coast of Lima Coordinate hospital functions with a dense urban network, access routes, utilities and heat-management needs.
Ccatcca Health Center Highlands of Cusco Address seismic and slope exposure, cold conditions, difficult logistics and the reliability of local infrastructure.
Putuyakat Health Center Rainforest of Amazonas Plan for intense rainfall, flooding, humidity, drainage, transport constraints and maintainable building systems.

The contrast is important. “Resilient hospital” is not a universal building type. The envelope, roof drainage, shading, ventilation strategy, equipment location, road access and maintenance plan should respond to the site’s hazards and operating realities. The IDB work also considered the Huaycán integrated health network, not just the hospital parcel, examining service continuity, interdependencies, patient and supply flows and response capacity.

A practical three-stage planning method

The IDB account organized the work into three stages. A project team can use the same sequence for a new hospital, a major extension or a retrofit.

  1. Assess context and exposure

Map seismicity, tsunami potential, slope stability, rainfall, flood paths, drought, heat and other hazards at the proposed site. Review historical events, projected climate conditions, watershed behavior and access-road vulnerability. Establish where critical rooms, generators, fuel, water storage, medical-gas equipment, data systems and waste facilities would sit in relation to those hazards.

Do not treat a regional label as a risk assessment. Two coastal sites can have different flood elevations and drainage constraints; two highland sites can differ in slope, road access and water availability. Require evidence at the parcel and catchment scale before fixing the site plan.

  2. Assess effects on care and the environment

Translate each hazard into an operational consequence. A flood may isolate ambulance access, disable a transformer, contaminate water or interrupt outpatient services. An earthquake may damage partitions, ceilings, medical equipment, elevators and communications even when the primary frame remains stable. Heat can raise cooling loads and threaten temperature-sensitive medicines; drought can reduce water available for dialysis, sterilization and sanitation.

At this stage, model patient, staff, supply, waste and referral flows. Identify single points of failure and dependencies between departments and external networks. Evaluate energy, water and emissions performance alongside continuity, because an efficient system that cannot be maintained locally may be less resilient than a simpler one with dependable spare parts and trained operators.

  3. Prioritize adaptation and environmental measures

Rank interventions by their effect on essential services, feasibility, lifecycle cost, maintenance burden and compatibility with the project’s budget and approvals. Protect the functions whose failure would stop emergency, inpatient, surgical, diagnostic, pharmacy, sterilization or critical-care services first.

Record assumptions, owners, inspection intervals and trigger levels for every measure. A flood barrier without a maintenance responsibility is a design intention, not an operating safeguard. The final package should state what must work immediately, what can operate at reduced capacity and how services will be relocated if a zone is temporarily closed.

Design choices that support continuity

Site, access and ground conditions

  • Place buildings outside mapped floodways where feasible, and verify finished-floor levels, site drainage and emergency access under extreme rainfall rather than average storms.
  • Use geotechnical and seismic studies to address liquefaction, slope failure, settlement and ground-motion demands. In steep terrain, design retaining and drainage systems as part of the health facility, not as separate civil works.
  • Provide more than one practical route for patients, staff, supplies and emergency vehicles when the network allows. Check bridges, culverts and urban bottlenecks that could fail beyond the property boundary.
  • Keep critical plant, electrical switchgear, communications rooms, medical-gas equipment and fuel stores out of predictable flood and impact zones, with protected service access for inspection.

Structure and nonstructural safety

Use the applicable Peruvian seismic and building requirements, but extend the review to ceilings, façades, partitions, equipment anchorage, pipes, cable trays, elevators and storage. Nonstructural damage can close an operating department even when the frame is safe. Hospitals should also have protected movement routes and spaces where patients can be triaged or sheltered if internal areas are temporarily unavailable.

Power, water and information systems

Continuity planning should identify the minimum electrical, water, wastewater, communications and medical-gas loads required for each essential service. Provide appropriately protected backup capacity, test transfer arrangements under load and keep fuel, batteries and replacement parts accessible. The World Bank appraisal proposed distributed data-center locations to reduce the vulnerability of health information systems; the broader principle is to avoid one unprotected location becoming a system-wide failure.

Water strategies should include resilient sources, storage, treatment, pressure management and safe wastewater handling. Drought planning matters even where the immediate project brief focuses on floods or earthquakes. The operating team needs procedures for rationing nonessential uses without compromising sterilization, hygiene or clinical care.

Climate-responsive and lower-impact building systems

Peru-specific project documents identify environmental measures that can reinforce resilience when they fit the site and operating model. They include passive design, external shading, natural ventilation where clinically appropriate, solar control, green spaces, permeable paving, stormwater management, efficient HVAC, insulated glazing, LED lighting and controls, water-saving fixtures and improved healthcare-waste management.

These are options, not a universal specification. Natural ventilation may be unsuitable for isolation rooms or areas requiring controlled air pressure. Permeable paving can reduce runoff but must be compatible with soil, contamination controls and accessibility. Green space can lower urban heat but needs irrigation, maintenance and a plan for keeping roots, debris and pests away from critical infrastructure. Efficient HVAC saves energy only if filters, controls and servicing can be maintained.

Design the network, not only the building

A hospital’s ability to function depends on facilities around it. The IDB’s Huaycán work examined an integrated health network, which is especially relevant where primary centers, referral hospitals, laboratories, pharmacies, ambulance services and public-health teams share responsibilities.

Network planning should document which facility receives patients when another is closed, how records and laboratory samples move, where medicines and oxygen are sourced, and which roads or communications links are essential. Distributed services can reduce dependence on one building, but only if referral rules, transport capacity, staffing and information exchange are prepared in advance. A network map should include municipal utilities and emergency agencies, because failures outside the health estate can determine whether a technically sound hospital remains reachable.

Operations and maintenance are part of the design

PAHO’s Smart Hospitals resources include preventive-maintenance and contingency-planning materials. Their practical implication is that resilience must be budgeted and assigned after handover. Specify inspection intervals for roofs, drains, pumps, generators, batteries, fire systems, medical gases, anchors, shutters and flood barriers. Keep an inventory of critical spares and identify qualified local technicians.

Run drills that test the real sequence of events: loss of utility power, blocked ambulance access, a flooded plant room, a damaged communications link or a temporary evacuation. Measure restoration time and service capacity, not merely whether staff attended. Update plans after construction changes, equipment replacement, new hazards or lessons from an incident.

How to compare competing hospital proposals

When reviewing two sites or design schemes, use a common scorecard rather than selecting the lowest capital cost. At minimum, compare:

  • Hazard and site: seismic, slope, rainfall, flood, drought, heat, tsunami and drainage exposure.
  • Continuity of care: access, essential-service capacity, protected utilities and time to restore operations.
  • Local climate fit: envelope performance, shading, ventilation and solar control suited to the setting.
  • Environmental and operating performance: energy, HVAC, lighting, water, stormwater, green space and waste systems.
  • Network and delivery: dependencies, referral flows, response capacity, governance and coordination.
  • Lifecycle readiness: maintenance staffing, spare parts, training, inspection access and operating funds.

Require each proposal to state what evidence supports its assumptions and which measures are optional, mandatory or contingent on further site studies. A design that performs well on paper but cannot be maintained, reached or staffed should not receive a resilience premium.

What Peru’s experience does—and does not—prove

Peru demonstrates that national assessment, policy and project-level adaptation can be connected. The 91% Hospital Safety Index assessment reported by the World Bank shows the scale possible when a country adopts a common framework. The IDB prototypes show how resilience and environmental criteria can be inserted into existing public-investment projects across very different regions.

They are not, however, a randomized test proving that one façade system, drainage detail or energy technology reduces mortality, interruption or lifecycle cost. The prototype account reports a design method and lessons, not completed outcome measurements. Teams should use the approach to structure decisions, then verify performance through engineering analysis, commissioning, drills, maintenance records and post-event reviews.

A decision checklist for the next project stage

  1. Define essential services and the capacity each must retain after a specified hazard.
  2. Complete site-specific hazard, climate, geotechnical and access assessments.
  3. Map clinical, utility, information, supply and referral dependencies.
  4. Set protected locations and backup requirements for power, water, communications, medical gases and data.
  5. Test passive and active environmental measures against infection control, water supply and maintenance realities.
  6. Assign ownership, inspection intervals, training and funding for every resilience measure.
  7. Commission systems under realistic loads and run multidisciplinary continuity drills.
  8. Review the design and operating plan after incidents, near misses, expansions or changes in climate data.

Frequently Asked Questions

Are Peru’s three prototypes building codes?

No. Huaycán Hospital, Ccatcca Health Center and Putuyakat Health Center were prototype adaptations of public-investment projects. They illustrate a planning method; applicable Peruvian laws, regulations, engineering standards and site studies still govern a project.

Does a resilient hospital need every green technology listed in project documents?

No. Passive design, shading, permeable paving, green space, efficient HVAC, LED lighting and water-saving systems must be selected for the site, clinical function, water conditions, maintenance capacity and operating budget.

How should resilience be measured after opening?

Track service availability and restoration time during drills and real incidents, along with utility performance, equipment failures, maintenance completion, access interruptions and the capacity retained by essential departments.
