Hospitals manage construction safely by treating every project as a patient-safety and continuity-of-care problem, not simply a building job. Before work starts, a multidisciplinary team completes a project-specific infection-control risk assessment (ICRA), maps nearby patients and building systems, and sets controls for dust, moisture, airflow, utilities, noise, routes, communication, monitoring and final commissioning. The precautions change with the work, the people nearby and the systems at risk; a generic barrier checklist cannot substitute for a site assessment.

Why hospital construction requires a different safety plan

Renovating a hospital can affect people who are unusually vulnerable to airborne particles, moisture damage, noise, vibration and service interruptions. Demolition dust may move through return-air paths, shafts, corridors or pressure differences into occupied care areas. A small water leak can create a moisture and mold problem. A planned utility outage can interrupt medical gases, data, hot or cold water, mechanical services or clinical equipment. Noise and detours can also interfere with treatment, rest, wayfinding and emergency operations.

That is why the construction room is only one part of the safety picture. The affected footprint may include patient rooms, operating or procedure areas, supply storage, spaces above or below the work, elevators, shafts and engineering systems. Controls must be matched to the actual building and to the patients who may be exposed.

Complete an ICRA before mobilization

Assemble the right decision-makers

The Centers for Disease Control and Prevention (CDC) recommends a multidisciplinary team that includes infection-control personnel to coordinate demolition, construction and renovation. Depending on the project, the team may also need facilities and engineering staff, clinical leaders, environmental services, occupational or worker-safety specialists, contractors, architects and representatives from affected departments.

The team should agree on who can approve the work, who monitors controls each day, who receives complaints and who can stop the job. Infection-control involvement is not a paperwork step added after design; it is part of deciding whether the proposed sequence, containment and ventilation plan are acceptable.

Define the work and its full footprint

An ICRA evaluates the type and extent of work, its duration, potential exposure to dust and moisture, susceptible patients and the containment required. The review should look beyond the immediate room. Trace air pathways, doors, corridors, elevators, shafts, supply routes and spaces directly above or below the project. Identify whether a nearby unit houses immunocompromised patients or has special ventilation requirements.

The CDC describes the purpose clearly: “An infection-control risk assessment (ICRA) conducted before initiating repairs, demolition, construction, or renovation activities can identify potential exposures of susceptible patients to dust and moisture and determine the need for dust and moisture containment measures.”

Classify the activity and patient risk

ASHE’s 2022 ICRA 2.0 tool provides a planning structure that combines the activity type with the risk group of patients in surrounding areas. The activity spectrum runs from non-invasive inspection to major demolition. The patient side of the matrix distinguishes how much protection is needed for the people near the work. The resulting precaution class must then be adapted to the facility, location and sequence of the actual project.

Planning factor Questions the team should answer Why it changes controls
Activity Is the work an inspection, limited repair, renovation or major demolition? More intrusive work generally creates greater dust, vibration, noise and utility risks.
Patient risk Which patients are adjacent, and are any in high-risk or protective-environment care? Susceptible patients may require stronger containment, schedule changes or relocation.
Surrounding spaces What is above, below or beside the work, and how are those areas connected? Air, water, people and materials can cross boundaries that are not obvious on a floor plan.
Building systems Could the project affect HVAC, pressure, medical gases, water, data or mechanical services? A construction control can fail if an engineering system is changed without coordination.
Operations What routes, elevators, entrances, clinical schedules and supply movements are affected? Detours and outages can create operational and emergency risks even when dust is contained.

Use permits and change control

A construction permit should document the scope, location, dates, contractor and approving personnel. The project needs a formal stop-and-review rule: if the scope changes, the work moves to another area, or toxic or biological substances are discovered, workers stop and obtain additional approval. A changed project can require a new ICRA or a higher precaution class. Treating a permit as fixed after unexpected conditions appear is a common way for controls to become inadequate.

Contain dust and moisture at the source

Build and inspect a continuous barrier

For interior work, barriers should separate the construction zone from occupied hospital space and remain sealed at walls, ceilings, floors, doors and penetrations. The ICRA should specify how workers enter, how materials leave and how the barrier will be inspected. A barrier is not effective if doors are propped open, joints are unsealed or debris is carried through a patient corridor.

Hospitals commonly combine physical separation with controlled airflow. The construction area may be maintained under negative airflow relative to adjacent occupied areas, with the pressure relationship checked and documented daily. Return-air paths should be managed so dust does not enter the hospital air-distribution system. Where feasible, air is exhausted from the work zone; if recirculation cannot be avoided, filtration and other engineering measures must be selected by the facilities and infection-control teams.

Control worker and material movement

Plan a dedicated worker route, staging area and debris route where the building allows it. Protect floors and doorways, keep carts covered when appropriate, and prevent workers from moving directly from dusty work into clinical areas without the required clothing, cleaning or route controls. Staff and visitors should receive advisories when alternate entrances, elevators or corridors are necessary.

Daily cleanup is part of containment, not an optional final task. Remove debris in a controlled manner, keep the construction route clean and use wet methods or other approved techniques when they reduce airborne dust. CDC guidance describes HEPA-filtered vacuuming for carpeted areas. A HEPA vacuum is a supporting tool; it cannot replace the ICRA, sealed barriers, pressure control and ventilation management.

Prevent moisture and water-system problems

Water damage can follow pipe alterations, leaks, sprinkler work, roof exposure or failures in temporary systems. Protect occupied areas from leaks, isolate work carefully and establish a response plan before work begins. The plan should identify who shuts off water, who inspects hidden spaces and how wet materials are removed and dried.

Coordinate HVAC, pressure and vulnerable-patient protection

Ventilation is a patient-safety control, not merely a comfort feature. Airflow and pressure relationships influence where dust travels and whether a construction zone can contaminate adjacent care areas. Hospitals must use the engineering conditions required for the specific room and applicable standards; there is no single pressure target or portable-filter setup that is correct for every space.

Plan shutdowns and restarts

Coordinate patient-care HVAC maintenance with infection-control staff. CDC recommendations say not to shut down patient-care HVAC except for specified maintenance, repair, emergency-backup testing or new-construction purposes, and to avoid shutting down an entire HVAC system at one time where possible, especially in acute-care facilities.

If a shutdown or temporary ventilation arrangement is unavoidable, schedule it with the affected clinical units, protect patients during the outage and verify conditions before work proceeds. For protective-environment rooms and other special-care spaces, coordinate system startup, allow time for air to clear after startup and confirm the required air balance before patients return. Relocation of immunocompromised patients may be necessary when the assessment shows that engineering controls cannot provide adequate protection.

Control noise, vibration, routes and utilities

Schedule disruptive work around care

ASHE’s ICRA 2.0 planning prompts teams to consider noise and vibration in surrounding units. Schedule the loudest periods with adjacent clinical leaders, choose lower-noise techniques where practical and vacate or relocate an area when the assessment shows that disruption cannot be controlled. Do not claim a universal hospital noise limit: applicable thresholds and acceptable work hours depend on the facility, jurisdiction, patient population and project.

Protect critical services

Map medical gases, water, mechanical systems, electrical services and data before drilling, cutting or isolating anything. Schedule outages and temporary systems with the departments that depend on them. A utility plan should state the affected service, isolation point, start and stop time, responsible person, clinical contingency and test required before normal service resumes.

Keep routes usable and understandable

Construction routes must preserve emergency access, accessible travel and clear separation between patients, visitors, workers and debris. Post signs before a detour starts, keep exits and fire-protection equipment available, and update maps when the sequence changes. Elevators and shafts deserve special attention because they can connect floors and carry both people and airborne contaminants.

Communicate and train everyone who can affect the controls

Hospitals need clear communication channels for dust control, indoor air quality, noise, vibration and complaints. Assign a person or team to receive reports, investigate them promptly and document the response. A complaint can reveal a failed seal, a route problem or an unplanned pressure change before routine inspection does.

Educate construction workers and healthcare staff who care for high-risk patients. Training should cover the approved routes, barrier rules, entry and exit procedures, cleaning expectations, emergency contacts and stop-work conditions. CDC recommendations say educational materials should be provided in workers’ languages. Contract documents should make these requirements enforceable rather than relying on informal briefings.

Monitor the project every day

Monitoring verifies that the plan is working under real operating conditions. The daily record can include barrier integrity, negative airflow, pressure or airflow observations, debris handling, route condition, cleaning, HVAC status, water-protection checks, complaints and corrective actions. CDC performance measures call for risk assessment of barriers and daily monitoring and documentation of negative airflow in construction or renovation areas.

Monitoring should be tied to action. If a barrier is breached, stop the activity, isolate the opening, clean affected areas and reassess before restarting. If airflow reverses, protect nearby patients, notify facilities and infection-control staff, correct the system and document the event. If moisture is found, prevent further spread and inspect concealed spaces rather than simply drying the visible surface.

Clean, verify and commission before reopening

  1. Remove tools, materials and debris from the work zone and approved transport route.
  2. Perform the specified construction cleaning, including HEPA-filtered vacuuming where applicable, followed by terminal cleaning of affected areas.
  3. Inspect barriers, adjacent spaces and building systems for dust migration, moisture damage or residue.
  4. Verify HVAC operation, pressure relationships, air balance and other engineering conditions required for the space.
  5. Check water-system integrity and complete any required flushing, testing or remediation.
  6. Have the responsible facilities, infection-control and clinical representatives approve the area before occupancy.

Operating rooms, protective-environment rooms and other specially engineered areas may require documented commissioning against their engineering specifications. A room should not return to service merely because construction is finished or the walls look clean.

When water or wind damage triggers restoration work

Restoration after a leak, flood or wind event requires the same disciplined approach as planned construction, often under more urgent conditions. CDC guidance calls for general construction and water-management ICRAs before restoration begins, multidisciplinary assistance, inspection for mold after return to service and clinical or laboratory surveillance for unusual infection clusters.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Cleanup planning must also address worker hazards, including infectious disease, chemical exposure, radiation, electrical shock and fire. Separate damaged materials from occupied care areas, control dust and moisture during removal, and document the inspection and release process. If unusual infections occur after the event, notify the appropriate clinical, laboratory and infection-control teams rather than treating the problem as a routine maintenance issue.

A practical release checklist for hospital projects

  • The project has a current, location-specific ICRA approved by the responsible multidisciplinary team.
  • The work scope, patient-risk group, adjacent spaces and affected systems are mapped.
  • Barriers, worker routes, debris routes and pressure controls are installed and inspected.
  • HVAC shutdowns, restarts, temporary ventilation and patient relocation decisions are coordinated.
  • Medical gases, water, mechanical services, data and other utilities have outage and contingency plans.
  • Noise, vibration, detours, signage and emergency access have been reviewed with affected units.
  • Workers and clinical staff have been trained, with materials available in workers’ languages.
  • Daily monitoring records include corrective actions and stop-work decisions.
  • Cleaning, debris removal, water checks and ventilation verification are complete.
  • Any scope change or newly discovered hazardous substance has triggered reassessment and approval.

What hospitals should not assume

There is no universal containment setup, noise limit or numeric construction-related infection risk that applies to every hospital. Requirements vary by jurisdiction, facility, project and applicable standards. CDC guidance notes that referenced design standards address zone-specific ventilation parameters, so engineering values must come from the facility’s governing standards and the room’s intended use.

Likewise, a plastic barrier, a portable air cleaner or a consumer cleaning routine cannot independently make a project safe. Effective protection comes from the coordinated system: an early ICRA, engineered airflow and utility controls, disciplined movement and cleaning, trained people, documented monitoring and verified commissioning before patients and staff reoccupy the space.
