How Contractors Control Risks During Excavation, Concrete Work, and Heavy Lifting

A practical guide to planning excavation protection, controlling silica during concrete work, and keeping workers clear of suspended loads.

Contractors control risks in excavation, concrete work, and heavy lifting by planning each task around its actual hazards, choosing controls that meet applicable requirements, and checking conditions again as work changes. That means protecting workers from trench cave-ins and related hazards, controlling respirable crystalline silica during concrete operations, and keeping people clear of suspended loads. The details depend on the site, equipment, task, and jurisdiction; OSHA requirements cited here concern their applicable U.S. scope.

Start with a job-specific plan, not a generic safety checklist

These activities often overlap. A crew may excavate a footing, cut or break concrete, then position materials with a crane or other lifting equipment. Each task has distinct hazards, but the work sequence can connect them: equipment near an excavation can add loads at the edge, water used for dust control can affect a work area, and a lift can move a suspended load over or beside workers and excavation boundaries.

Before work begins, identify the operations, the people and equipment involved, the expected travel and work areas, and the conditions that could change during the shift. Then assign responsibility for inspections, control implementation, worker communication, and stopping or adjusting work when conditions no longer match the plan. The control method must fit the specific operation and applicable OSHA requirements; a broad instruction such as “use PPE” or “be careful around the trench” does not establish that the right hazard has been addressed.

Use a practical planning sequence

  1. Define the task and its boundaries. Identify where excavation, concrete cutting or breaking, and lifting will occur, and when those activities may take place at the same time.
  2. Identify the hazards relevant to each operation. For excavation, consider soil, water, underground installations, nearby structures, surface conditions, equipment, and falling loads. For concrete work, identify the silica-generating task and the applicable exposure-control method. For lifting, identify the actual equipment, load, travel path, site conditions, and people who could enter the load path.
  3. Select controls before exposure begins. Choose an excavation protective system based on site conditions; use a silica-control method that fits the task; and plan lifting, rigging, inspection, and worker exclusion according to the equipment and applicable requirements.
  4. Check implementation and reassess. Inspections and competent-person responsibilities depend on the task and rule. Revisit the plan when conditions change, including after rain or other events that may increase excavation hazards.

Excavation: prevent cave-ins and control the surrounding hazards

The Occupational Safety and Health Administration identifies employee injury from a cave-in as the primary trenching and excavation hazard. A trench can become dangerous quickly, so workers should not enter until the necessary protective measures are in place. Soil evaluation informs the choice among sloping, benching, shoring, or shielding; the appropriate system depends on the soil and the conditions around the excavation rather than a preference for one method.

Evaluate the site before digging and before entry

Determine the estimated locations of underground installations before excavation and contact utility owners as required. Consider surface encumbrances, nearby structures and sidewalks, traffic, heavy equipment, and the possibility of falling loads. Excavated material and equipment must be kept back from excavation edges as required by the applicable standard and site conditions. The plan should account for how adjacent work or equipment could affect the excavation, not only what happens inside it.

A competent person should evaluate the soil, select an appropriate protective system, and ensure that system follows applicable requirements. The excavation should be inspected before work, daily before each shift, as needed during a shift, and after rainstorms or other events that may increase hazards. An inspection is a check of current conditions, not a one-time approval that remains valid regardless of what changes on site.

Provide access and egress as well as cave-in protection

OSHA’s construction eTool says a trench 4 feet or more deep needs a safe means of egress, such as a ladder, stairway, or ramp, positioned so workers are within 25 lateral feet of it. Access should be within the protective system; structural ramps have design and construction conditions. These thresholds describe the eTool guidance, and the applicable rule and site conditions still need to be checked.

A ladder helps a worker get out; it does not protect the worker from a cave-in. Treating egress as a substitute for a protective system leaves the central trench hazard unaddressed. Select and position access as part of the excavation plan, including where workers will be working and whether the access remains usable as the work progresses.

Manage water, air, structures, and loads

Water accumulation and potentially hazardous atmospheres call for active assessment. OSHA guidance calls for inspections where water or other hazardous conditions may be present, atmospheric testing when hazardous conditions are likely, and precautions appropriate to the conditions. Those precautions may include water removal or diversion, ventilation, or respiratory protection when needed; the appropriate measures depend on the actual hazard.

Keep workers out from under loads handled by lifting or digging equipment. Coordinate equipment movement and lifting with excavation boundaries, nearby structures, and simultaneous operations. A competent person’s inspection and the selected protective system do not remove the need to watch for a new hazard, such as changed water conditions or equipment operating near the edge.

Concrete work: control silica exposure for the actual task

Cutting, drilling, grinding, chipping, or breaking concrete and related materials may generate respirable crystalline silica. OSHA’s construction silica standard provides two compliance approaches: fully and properly implement the applicable controls in Table 1, or use the standard’s alternative exposure-control method. Under the alternative method, the employer must ensure exposures do not exceed the permissible exposure limit calculated as an 8-hour time-weighted average.

Choose the control method by operation

Table 1 specifies controls by task. The right approach therefore depends on what workers are doing and the tool and operation involved; a control used for one kind of concrete work should not automatically be assumed to cover another. If using Table 1, the listed method must be implemented fully and properly for the applicable task. If using the alternative method, the employer must meet that method’s exposure-control requirements rather than assume that dust appearing less visible means the exposure is acceptable.

OSHA says applying water to a saw blade when cutting silica-containing stone, rock, concrete, brick, and block substantially reduces dust. Water is an important task control where applicable, but the statement does not mean that water by itself resolves every silica hazard or fulfills every requirement. Match dust controls to the tool and task, and check the requirements for the specific operation.

Plan, train, and document covered work

Covered employers must establish and implement a written exposure-control plan. It describes the silica-generating tasks, controls and work practices, and procedures to restrict access where necessary. Employers must also designate a competent person, train workers, and maintain required records. These are elements of a compliance system: the plan identifies how exposure will be controlled, while training and oversight help workers follow the method selected for the operation.

Where workers are required to wear a respirator for 30 or more days per year, OSHA’s silica guidance says they must be offered medical examinations every three years under the described medical-surveillance requirement. This applies to the stated respirator-use threshold and requirement; it should not be generalized to every worker doing concrete work.

Heavy lifting: plan the equipment, load path, and people exclusion

OSHA’s cranes and derricks construction standard covers specified power-operated equipment used in construction that can hoist, lower, and horizontally move a suspended load, subject to the standard’s scope and exceptions. Attachments can include concrete buckets, augers, and personnel platforms. Whether a particular activity falls under that standard, and which inspection or operating provisions apply, depends on the equipment, task, and applicable rule.

Plan each lift around its real conditions

Before a lift, identify the equipment and attachment, the load, the travel path, worksite conditions, and people who might be exposed. Coordinate the lift with excavation limits and other work so a suspended load does not pass over or place people in a hazardous position. Keep workers clear of suspended loads; at excavations, OSHA’s requirements prohibit employees from being underneath loads handled by lifting or digging equipment.

Do not invent a load limit, rigging configuration, inspection interval, or operator qualification from a general checklist. Verify the relevant OSHA provisions and equipment documentation for the actual lift. Inspection and planning requirements vary with equipment and activity. For steel erection, OSHA’s hoisting and rigging section requires a competent person to visually inspect cranes before each shift and specifies inspection elements; that provision should not be presented as the inspection rule for every lifting operation.

Do not treat personnel hoisting as routine

OSHA generally prohibits using equipment to hoist employees unless the employer demonstrates that conventional access would be more hazardous or is not possible because of structural design or site conditions. The standard includes exceptions and detailed requirements. A personnel platform is not, by itself, permission to hoist workers; applicable conditions and safeguards must be followed before that approach is used.

Coordinate controls where tasks overlap

Overlapping work needs a shared plan, not separate controls that ignore one another. For example, a lift near a trench should be coordinated with excavation boundaries and worker locations; concrete work that produces silica-containing dust needs its task-specific controls in place while workers are doing the operation. The people responsible for each activity should communicate the work sequence, access routes, inspection findings, and any change that could affect another crew.

Concrete cutting or other silica-generating work Applicable Table 1 method or alternative exposure-control method Task and tool match, correct implementation, written plan, competent-person oversight, training, and records where required
Heavy lifting Equipment- and activity-specific lift and rigging controls Applicable standard, equipment documentation, inspection provisions, load path, and worker exclusion

The table is a planning aid, not a replacement for the governing requirements. In each case, verify that the chosen method matches the operation and that the responsible people know what conditions require work to stop or the plan to change.

Reassess when conditions change

Controls are effective only while they fit the work as performed. Reassess when a task changes, equipment or loads move, crews begin simultaneous operations, or conditions around an excavation change. For excavation, OSHA guidance specifically calls for inspections after rainstorms or other events that may increase hazards, as well as before work, daily before each shift, and as needed during the shift. For concrete and lifting, check that the control method and requirements still match the actual task and equipment rather than relying on an assumption carried over from another operation.

When a required control cannot be implemented as planned, or conditions make the existing plan unsuitable, pause the affected work, reassess the hazard, and establish an appropriate control before proceeding. The practical objective is not to complete a checklist; it is to keep the protective system, exposure control, or lift plan aligned with the work and conditions workers face.

Check the applicable jurisdiction and rule before work

The requirements described here are based on U.S. federal OSHA guidance and standards within their scope. State-plan OSHA programs, local requirements, and project-specific conditions may add requirements or differ in application. Supervisors should confirm the rules governing the job, determine which standard applies to the equipment and activity, and use qualified personnel for site-specific decisions that require evaluation of soil, exposure, equipment, or rigging.
