Hospitals are still commissioning wings, replacement facilities and expansions because the need for safe capacity has not disappeared, while public capital programmes and already-approved projects keep money moving. Inflation is changing what gets built and when—through phasing, redesigns and tighter financing—not eliminating every project.
Construction costs have risen faster than many hospitals can absorb
The pressure is measurable. Statistics Canada’s 2026 explainer gives a practical comparison: a hospital wing priced at $100 million in 2020 would cost about $128 million to build in 2024. That 28% increase affects structural work, mechanical and electrical systems, specialist medical infrastructure, labour and contingency allowances.

Operating income has not necessarily kept pace. The American Hospital Association reported that economy-wide inflation rose 12.4% from 2021 to 2023, while Medicare reimbursement for hospital inpatient care increased 5.2% over the same period. A project may therefore be more expensive to deliver at the same time that the hospital’s ability to generate cash from care is under strain.
Hospital expense growth makes the capital decision harder
Construction competes with the cost of running the existing estate. The AHA’s 2024 Costs of Caring analysis, based on median 2023 spending estimates, divided hospital expenses as follows:
#1 Best Overall
- Valued Carpenter Pencil Set: You will get 2 pcs solid carpenter pencils with 26 piece 2.8 mm refills, 1 replaceable sharpener, 1 plastic storage box.The complete carpenter pencils combination allows you to finish your work faster and more easily

- Carpenter Pencils with Sharpener: The sharpener is screwed into the top of the work pencil, which won't get lost either. Built-in pencil sharpener that keep the lead with pointed and smooth to Improves line of sight in fine work
- Stronger Solid Lead: This work pencil is matched with a 2.8 mm thick lead , which is much thicker and stronger during the drawing process of construction work, it will not break or damage easily
- Marks on Various Surfaces: 3 colors solid construction pencil can marks on various surfaces, such as metal, plastic, wood, paper etc. Ideals for woodworkers, contractors, craftsmen, builders, merchants and masons
| Expense category | Share of median hospital expenses | Estimated spending |
|---|---|---|
| Labor | 60% | $839 billion |
| Supplies | 13% | $181 billion |
| Drugs | 8% | $115 billion |
| Other expenses | 19% | $269 billion |
The AHA’s 2025 update put care-team spending at 56% of total costs in 2024. Its March 11, 2026 statement also said hospital expenses for labor, drugs and supplies grew twice as fast as hospital prices in 2025. That combination forces boards to weigh a new building against recruitment, wages, supplies, debt service, maintenance and cash reserves.
Why hospitals proceed instead of waiting for cheaper prices
Clinical demand cannot always be deferred
Emergency departments, inpatient beds, operating rooms and outpatient services have to match local demand and the acuity of patients. If an existing hospital is routinely full, delaying capacity can mean longer waits, cancelled procedures, ambulance diversions or treatment in unsuitable spaces. A cost reduction achieved by waiting is not useful if the service shortfall is already affecting care.
Replacement can be more urgent than expansion
Some projects replace buildings that are inefficient, difficult to maintain or unsafe rather than adding discretionary amenities. Obsolete layouts can increase travel distances for staff, limit infection-control separation, constrain imaging or surgical equipment and consume more energy. Replacing a failing plant, ward or theatre block can reduce operational risk even when it does not create a large number of new beds.

The United Kingdom’s New Hospital Programme illustrates this distinction. The Department of Health and Social Care said the programme prioritises hospitals affected by reinforced autoclaved aerated concrete and continues projects already in construction. A safety-driven replacement has a different decision threshold from a modernization that can be postponed without exposing patients or staff to the same risk.
Approved projects acquire their own momentum
By the time a scheme has an approved business case, a site, design work, procurement activity and construction contracts, the owner may already have committed substantial funds. Stopping can leave the hospital with demolition, professional-fee and termination costs while the old facility remains in service. Continuing, renegotiating or phasing the work may preserve more value than cancelling a project after those commitments are made.
Public capital programmes keep a pipeline open
Government-backed budgets can support construction even when hospital operating margins are tight. In the UK, the New Hospital Programme reported a £13.6 billion health capital budget for 2025–2026. The programme keeps approved schemes in construction and assigns future schemes to waves beginning in 2025–2030, 2030–2035 and 2035–2039. Multi-year allocations do not make every project immune to repricing, but they give selected hospitals a funding route that is less dependent on one year’s operating result.

Public commitments also explain why national activity should not be inferred from one hospital’s delay. A project can be re-costed or moved between phases while the wider programme continues. Conversely, a hospital outside an approved programme may have to postpone work, borrow more, seek philanthropy or reduce scope.

Rising prices usually change the project plan before they end the project. Owners and their advisers commonly test the following responses:
- Phasing: build the urgent clinical block first and defer lower-priority floors, public areas or future shell space.
- Redesign: simplify geometry, standardise rooms or select alternative systems that meet the clinical brief and code requirements at lower cost.
- Scope ranking: protect emergency, surgical, inpatient, infection-control and safety work while postponing discretionary features.
- Procurement changes: retender packages, alter sequencing or negotiate with contractors when market pricing changes between design and award.
- Financing changes: combine approved public capital with borrowing or philanthropic commitments, subject to the hospital’s governing rules and affordability.
- Schedule changes: postpone a project that is not yet committed when the cost of capital, labour availability or supply-chain risk makes the business case fail.
These choices can reduce the initial cheque but may increase disruption, extend inflation exposure or leave the hospital operating in a partially modernised estate. A cheaper first phase is not automatically cheaper over the building’s life if it creates duplicate work or prevents efficient staffing and patient flow.

Evidence that building has not stopped in every market
Activity remains visible in the markets covered by the available public data. The Metropolitan Washington Council of Governments reported that new healthcare-space construction rose 3% from 2024 to 2025. That is a regional measure, not a global forecast, but it shows that higher costs have not halted every healthcare project.
The strongest evidence currently covers the United States, Canada, the United Kingdom and the Washington metropolitan region. It does not establish that every country, health system or hospital is expanding, and there is no single global hospital-construction index that can support that broader claim.

A hospital board, public agency or project partner can separate resilient projects from deferrable ones by testing five questions:
Best Value
- Milwaukee Ink all Fine Point Marker, Black, 4 Per Pack
- 4 per pack Features Clog Resistant Marker Tip Writes through Dusty, Wet and Oily Surfaces Durable Marker Tip for Writing on Concrete, OSB and Rough Surfaces
- Clog resistant tip writes on dusty, wet and oily surfaces and is optimized for rough surfaces such as OSB, cinderblock and concrete
- Hard hat clip- attaches for easy access
- Quick dry time with reduced smearing and marking
- What problem is being solved? Document a safety exposure, replacement need, capacity shortfall, mandated service or elective modernization.
- How certain is the funding? Distinguish an appropriated public allocation or signed commitment from a hoped-for grant, donation or future borrowing.
- How far has the project progressed? A concept in early design has more flexibility than a scheme in procurement or active construction, where cancellation costs can be material.
- What is the inflation response? Show the effect of full build, phased build, redesign, postponement and cancellation on both capital cost and service delivery.
- What happens after opening? Model staffing, throughput, maintenance, energy, debt service and the cost of keeping the old facility running during transition.
This framework keeps the decision focused on total value rather than the headline construction price. A project can be financially difficult yet operationally necessary; another can be affordable but poorly timed if it adds space without a credible staffing and utilisation plan.
Why waiting for prices to fall is not a simple strategy
Construction prices may move unevenly, and there is no guaranteed date when a hospital’s preferred materials, labour and financing will become cheaper. During a delay, the hospital still pays to maintain the old building, may lose the benefit of improved capacity and can face another round of design changes when regulations or clinical requirements evolve. Waiting can be sensible for a discretionary scheme with uncertain funding, but it is a risky default for a safety replacement or a service already operating beyond capacity.
The result is a selective building market: urgent replacements, funded public projects and capacity-constrained services continue, while less urgent schemes are redesigned, phased or deferred. Rising costs therefore reshape the hospital pipeline rather than stopping it outright.
