Exercise Balls for Sitting: What the Evidence Says About Posture and Core Strength

Stability balls are exercise equipment, not proven ergonomic-chair replacements. Here’s what studies say about posture, core activity, discomfort, and fall risk.

Stability exercise balls can be useful for movement and balance exercises, but the available evidence does not support using one as an everyday desk chair to improve posture or build core strength. UCLA Health advises against replacing an ergonomic office chair with a ball, particularly because a ball has no back or lumbar support. If you are choosing equipment, treat an exercise ball as exercise equipment—not as a workstation chair.

Are exercise balls good for sitting?

For brief periods, sitting on a ball may provide a different sitting experience and require some postural adjustment. That does not make it a better general-purpose chair. UCLA Health states, “Exercise balls or ball chairs are not recommended as a replacement for a good ergonomic office chair.” The guidance highlights the lack of back support, especially lumbar support. UCLA Health’s ergonomics guidance recommends an appropriately supportive chair instead.

The evidence also does not establish a meaningful general health or wellness benefit from using a stability ball as a workplace chair. A 2015 review indexed by the National Institute for Occupational Safety and Health concluded that evidence did not show significant health benefits and urged skepticism about recommending ball sitting for workplace wellness. The review authors wrote: “Until studies demonstrate more conclusive benefits, the practice of stability ball sitting should be viewed skeptically as a general workplace recommendation in the interest of health or wellness.” Read the NIOSH-indexed review.

Does sitting on an exercise ball improve posture?

There is a difference between feeling more upright and demonstrating an objective, lasting change in posture. A 2013 workplace evaluation reported improvements in employees’ self-reported perceptions of posture after a stability-ball program. The evaluation began with 193 employees; 159 completed at least one questionnaire after the workplace rotation. These are survey outcomes, not proof of lasting posture correction or stronger muscles. In a 2006 comparison involving 14 participants, researchers measured muscle activity, lumbar posture, and perceived discomfort during one hour of stability-ball and office-chair sitting. Ball sitting increased thoracic erector spinae activity, but participants also reported greater discomfort; the authors found little overall biological change and cautioned that prolonged use may not be advantageous. The small sample limits how broadly the findings can be applied. See the 2006 study.

Can an exercise ball strengthen your core?

Doing exercises on a ball and passively sitting on one are different activities. A 2015 experiment with 16 participants compared muscle activation during four pelvic-rotation exercises performed on a stability ball and a dynamic office chair. Activity was generally comparable between the two surfaces. The experiment addressed specific movements; it does not show that simply sitting on a ball provides a useful core-strengthening workout. A separate 2016 randomized crossover study examined ball sitting for 90 minutes per day in relation to low-back pain. Its authors describe ball sitting as a possible adjunct involving core muscles, while emphasizing that clinicians should rely on other evidence-based treatments. It is not evidence that an exercise ball treats back pain or replaces clinical care. See the 2016 study.

Is an exercise ball better than an office chair?

For desk work, the cited guidance favors a good ergonomic chair over a bare exercise ball because a chair can provide back and lumbar support. When comparing chairs, check whether the backrest supports your lower back, whether the seat and other adjustments fit your body and desk, and whether the chair sits securely on a stable floor. A ball does not offer an equivalent backrest or lumbar support.

Study results vary with what researchers measure. Self-reported posture improved in one workplace program, while a short laboratory comparison found increased discomfort and no broad biological advantage. Neither finding establishes that a ball is superior to a properly fitted office chair for everyday work.

What are the risks of using an exercise ball as a chair?

Reduced support and instability are practical concerns. In a 2022 controlled reaching-task study with 16 participants, perceived fall risk was significantly higher on a stability ball than on a backless office chair, even though participants adopted a wider stance on the ball. This result describes participants’ perceived risk during a specific task; it is not an estimate of how often people are injured in everyday use. See the 2022 study.

Greater discomfort during one-hour ball sitting was also reported in the 2006 study. That finding, combined with the absence of back support, is a reason not to assume that a ball is suitable for long, uninterrupted periods at a workstation.

How to choose an exercise ball for exercise

The evidence supports considering a stability exercise ball for exercise and balance movements—not selecting one as a desk-chair replacement. No named product models, comparative product tests, manufacturer sizing guidance, or verified construction specifications are established here, so there is no evidence-based “top pick” or product ranking to give.

Before buying, use the manufacturer’s information to check:

  • Intended use: confirm that the product is designed for the exercises you plan to do, rather than assuming it is suitable for workstation seating.
  • Size guidance: follow the maker’s sizing instructions for your body and intended movements; do not rely on an unverified universal size chart.
  • Construction and safety details: look for product-specific information about materials, load limits, and safe use. Do not assume a ball has anti-burst construction unless its manufacturer says so.
  • Included accessories and space: check whether a pump or other accessories are included and whether you have enough room to use and store the ball safely.
