Do Zirconia Restorations Need Bonding, or Can They Be Conventionally Cemented?

Zirconia restorations do not automatically need adhesive bonding. A crown or bridge can often be conventionally cemented when the tooth preparation is sufficiently retentive and the zirconia manufacturer permits that method. Adhesive or self-adhesive cementation becomes more relevant when preparation retention is limited, the restoration design calls for it, or the specific product instructions require surface conditioning and a primer.

The correct choice is case-specific. It depends on preparation geometry, restoration design, the exact zirconia system, the cement, and the current instructions for both products. “Regular cement” is not a precise clinical category, and using a cement simply because it is available can produce an incompatible protocol.

What the three cementation terms mean

These terms describe different ways a restoration is retained. They should not be treated as interchangeable labels.

Conventional cementation

Conventional cementation relies mainly on mechanical retention. Friction between the prepared tooth, cement layer and internal surface of the restoration resists displacement. The cement is not expected to create a strong chemical bond to zirconia.

For the IPS e.max ZirCAD Prime system, Ivoclar describes conventional cementation as appropriate when the preparation is retentive. Its technical guide gives a preparation angle of approximately 4–8 degrees and a preparation height of at least 4 mm as examples of retentive geometry. Those figures are manufacturer-specific guidance for that system, not universal thresholds for every zirconia product, tooth or clinical situation.

Ivoclar also states that conventional cementation does not increase the overall strength of the ceramic restoration. Conventional cementation may still be entirely appropriate because its purpose is retention and sealing on a suitable preparation, not strengthening the zirconia itself.

Self-adhesive cementation

Self-adhesive resin cements are designed to adhere to tooth structure without a separate, multi-step bonding agent. Their retention combines some micromechanical interaction with chemical interaction from the cement. The product guide cited above still calls for a retentive preparation when its self-adhesive protocol is used.

Self-adhesive does not mean “works regardless of preparation.” A short, over-tapered or otherwise non-retentive preparation can remain vulnerable to dislodgement even when a self-adhesive resin cement is selected.

Adhesive cementation

Adhesive cementation uses a bonding system to condition the tooth and a suitable primer to promote chemical interaction with the zirconia surface. It is considered when conventional mechanical retention is inadequate or when the restoration and cement instructions specify an adhesive protocol.

Adhesive cementation is technique-sensitive. Tooth isolation, surface cleaning, air abrasion parameters, primer selection, cement handling and curing must all match the instructions for the actual products being used.

How the dentist chooses an approach

No single label—“zirconia,” “resin cement” or “bonding”—settles the decision. The following factors are evaluated together.

Preparation retention and geometry

Retention comes from the shape of the preparation as well as the cement. Parallel or near-parallel opposing walls, adequate axial height and controlled taper generally provide more mechanical resistance to removal. A preparation that is very short, highly tapered or compromised by extensive loss of tooth structure provides less.

The 4–8-degree angle and 4-mm minimum height in the IPS e.max ZirCAD Prime guide illustrate how one manufacturer defines a retentive situation for its conventional and self-adhesive options. They should not be copied automatically to another zirconia brand, a different cement or a different tooth. The dentist must use the current instructions for the exact restoration system.

Restoration type and design

A single crown, a multi-unit fixed bridge, an implant-supported restoration and a thin partial-coverage design do not present identical retention problems. Path of insertion, connector design, available enamel or dentin, occlusal forces and the possibility of moisture contamination all affect the cementation plan.

The product instructions may also distinguish between zirconia generations, translucencies, framework designs or indications. A protocol validated for one material should not be assumed to apply to every high-strength ceramic sold as zirconia.

Tooth and restoration surface conditioning

Conventional cementation generally requires cleaning and fitting checks rather than a chemical bonding sequence. Adhesive protocols add conditioning of the tooth and treatment of the zirconia surface. Air abrasion, cleaning and primer application must be performed with the materials and settings specified by the manufacturer.

Cement and primer compatibility

The cement is part of a system. A primer intended for zirconia may differ from one intended for glass ceramic, metal or tooth structure. Mixing a zirconia primer, bonding agent and cement from unrelated protocols can leave the restoration under-treated or contaminate the bonding surface.

What a conventional cementation appointment involves

The exact sequence varies by product, but a conventional approach generally follows this pattern:

  1. Verify the indication. Confirm that the restoration, preparation geometry and occlusion meet the zirconia and cement manufacturer’s requirements for conventional cementation.
  2. Try in the restoration. Check seating, contacts, margins, shade and occlusion before cement is mixed. Corrective adjustments should be polished according to the zirconia instructions.
  3. Clean the preparation. Remove temporary cement and debris without leaving a contaminant that interferes with seating or cement performance. Maintain moisture control appropriate to the selected cement.
  4. Condition the restoration as instructed. Some systems specify cleaning or alumina air abrasion even when the final cement is conventional. Do not substitute phosphoric acid etching or a glass-ceramic protocol for the zirconia instructions.
  5. Apply the cement and seat immediately. Use the working time, seating pressure and cleanup method in the cement directions. Remove excess at the stage recommended by the manufacturer.
  6. Verify the result. Check the margins, contacts and occlusion after the cement has set. Record the material and cement used so future repairs or recementation can follow a compatible protocol.

For the IPS e.max ZirCAD Prime example, Ivoclar recommends ZirCAD Cement for conventional cementation. Ivoclar describes that product as a resin-modified glass ionomer cement intended for retentive preparations and conventional cementation of high-strength zirconia and metal-based restorations.

What changes with self-adhesive cementation

A self-adhesive protocol still starts with a retentive preparation when the manufacturer requires one. The main difference is that the cement is formulated to interact with tooth structure without a separate tooth-bonding sequence.

  1. Confirm that the preparation meets the cement’s retention requirements.
  2. Try in and clean the zirconia using the specified method.
  3. Control saliva, blood and sulcular fluid; contamination can reduce cement performance.
  4. Dispense the self-adhesive cement according to its working and setting times.
  5. Seat fully, remove excess at the recommended stage and allow the cement to polymerize or set as directed.
  6. Inspect margins and occlusion, then document the cement and zirconia system.

In Ivoclar’s IPS e.max ZirCAD Prime guide, Speedcem Plus is the listed cement for the self-adhesive option. That recommendation applies to the named product system; it is not a blanket endorsement of one cement for all zirconia restorations.

When an adhesive zirconia protocol is used

Adhesive cementation can be considered when the preparation does not provide dependable mechanical retention, provided the restoration and cement instructions support it. It is also used when a particular design or clinical situation benefits from an adhesive approach.

  1. Plan isolation first. Rubber dam isolation or another reliable moisture-control method may be needed, depending on the tooth and margin location.
  2. Evaluate and clean the intaglio surface. Remove try-in contamination using the zirconia manufacturer’s approved cleaner or procedure.
  3. Air abrade only as specified. Ivoclar’s guide gives alumina air-abrasion instructions with defined particle sizes and pressures for its system. Those settings are product-specific. Use the current instructions for the actual zirconia rather than transferring a pressure or particle size from another brand.
  4. Apply the zirconia primer when required. For adhesive cementation of the cited IPS e.max ZirCAD Prime system, Ivoclar specifies Monobond Plus as the restoration-conditioning product.
  5. Condition the tooth. Apply the etchant, adhesive or other tooth-conditioning materials exactly as directed for the selected cement system. The correct sequence depends on whether the cement uses total-etch, self-etch or another protocol.
  6. Seat with the compatible resin cement. Maintain isolation, use the stated working time and remove excess before it becomes difficult to clean.
  7. Polymerize or allow chemical cure as directed. Light access, restoration thickness and cement chemistry affect the setting procedure.
  8. Finish and verify. Remove residual cement, check margins and occlusion, and polish any adjusted zirconia surfaces according to the material instructions.

Ivoclar lists Variolink Esthetic for adhesive cementation in the IPS e.max ZirCAD Prime guide. The example demonstrates the complete-system principle: the restoration, primer and cement instructions are intended to work together.

What the bonding evidence actually shows

Laboratory studies help compare protocols, but they do not provide a universal clinical rule.

2024 network meta-analysis

A 2024 systematic review and network meta-analysis in the Japanese Dental Science Review included 77 in-vitro studies of zirconia–resin bonds. Across the included comparisons, surface-treatment results varied by protocol. Air abrasion and silica coating improved measured bonding in evaluated comparisons, and adhesives containing 10-MDP performed better than other acidic monomers.

The review reported that 91.2% of bonds weakened after artificial aging. That percentage describes bond-strength changes in laboratory specimens. It is not the percentage of crowns that fail, the percentage of patients who lose restorations or a clinical survival rate.

2024 cement meta-analysis

A separate 2024 systematic review and meta-analysis found that self-adhesive and self-etch resin cements, used according to manufacturers’ directions, were suitable for bonding to zirconia in the included studies. In its analyses, total-etch cement showed lower measured bond strength than self-adhesive cement.

Those findings compare test results under the protocols represented in the studies. They do not establish that one cement class is clinically superior for every tooth, preparation, zirconia product or operator.

2021 systematic review

A 2021 systematic review reported substantial variation in surface pretreatments, cement types, aging methods and test methods. Adhesive results were generally stronger when mechanical and chemical pretreatments were combined. The variation explains why published studies do not yield one universal zirconia-bonding recipe.

Ivoclar’s IPS e.max ZirCAD Prime example

The following table summarizes the cited manufacturer guidance for one system. It is an example, not a protocol for every zirconia restoration.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
Cementation approach Ivoclar product named in the guide Preparation or conditioning emphasis
Conventional ZirCAD Cement Retentive preparation; the guide gives approximately 4–8° preparation angle and at least 4 mm preparation height as examples.
Self-adhesive Speedcem Plus Retentive preparation remains recommended; cement provides self-adhesive interaction with tooth structure.
Adhesive Variolink Esthetic Tooth conditioning plus appropriate zirconia treatment; Monobond Plus is specified for restoration conditioning in the guide.

The same guide states: “Conventional cementation does not result in an increase of the (overall) strength of the ceramic restoration.” This is Ivoclar’s product guidance, not a conclusion from an independent clinical trial.

Can a zirconia crown be cemented with “regular cement”?

Sometimes, but only when “regular cement” means a cement approved for that zirconia system and the preparation is retentive enough for the method. A resin-modified glass ionomer such as the ZirCAD Cement described by Ivoclar may be suitable for the manufacturer’s conventional protocol. Another cement with a similar-sounding category may have different indications, surface requirements or limitations.

Do not choose a cement solely because it was used for a metal crown, a glass-ceramic crown or a previous zirconia case. Check the current instructions for the exact zirconia, cement and primer. If the preparation is not retentive, conventional cementation may not provide adequate resistance even when the cement itself is properly mixed.

Common causes of trouble and how they are addressed

  • Premature debonding or dislodgement: Reassess preparation height, taper, path of insertion, occlusion and whether the selected method was allowed by the manufacturer.
  • Contaminated intaglio surface: Clean the zirconia with the approved procedure. Saliva or try-in contamination can interfere with primer or cement interaction.
  • Wrong surface treatment: Do not etch zirconia as though it were glass ceramic. Use the specified air-abrasion, cleaning and primer sequence.
  • Moisture contamination: Improve isolation and follow the cement’s moisture-control requirements. Adhesive resin protocols are particularly sensitive to contamination.
  • Excess cement at the margin: Follow the product’s recommended cleanup stage and inspect interproximal and subgingival areas carefully.
  • Assuming laboratory bond strength equals clinical longevity: Treat in-vitro values as comparative evidence, not as a prediction of an individual restoration’s survival.

A practical decision checklist

Before cementation, the clinician can document these questions:

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  • Is the preparation sufficiently retentive for the proposed conventional or self-adhesive method?
  • What zirconia product and restoration design are being used?
  • Does the current manufacturer guide permit conventional, self-adhesive, adhesive or more than one option?
  • Which tooth-conditioning, air-abrasion, cleaning and primer steps are specified?
  • Are the cement and primer approved for that zirconia system?
  • Can the field be kept clean and dry for the entire procedure?
  • Is the available evidence a laboratory bond-strength result or a clinical outcome?

If the preparation is retentive and the product instructions permit it, conventional cementation can be a sound choice. If retention is deficient, an adhesive protocol may be considered only with compatible materials and strict adherence to the restoration and cement instructions.
