There is no mattress model proven to be best after hip replacement. Official patient guidance recommends a firm mattress, but also makes the bed’s height and safe transfers important. Choose a bed that lets you sit and stand safely, and follow your surgical team’s instructions about sleeping position; those instructions vary by patient and hospital.
What mattress firmness is recommended after hip replacement?
MedlinePlus and the Royal National Orthopaedic Hospital (RNOH) advise using a firm mattress after hip replacement. Neither source defines firmness on a retail rating scale or recommends a particular material, brand or construction. Treat “firm” as a practical starting point—not evidence that extra-firm is better or that foam, springs, hybrid or latex mattresses have a recovery advantage.
No reviewed source establishes a universally best mattress or reports head-to-head mattress trials for people recovering from hip replacement. A mattress should not be represented as preventing dislocation, speeding healing or otherwise changing recovery outcomes.

Check bed height as well as the mattress
The mattress, bed frame and total bed height work together. MedlinePlus’s practical check is to sit at the edge of the bed: your feet should reach the floor while your hips remain higher than your knees. This is a transfer guideline, not a universal measurement in inches; the right setup depends on your body and safe movement.
RNOH notes that bed height depends on the person and discusses adding a mattress to raise a bed that is too low. Before changing the setup, ask your surgeon, physical therapist or occupational therapist to assess whether the adjustment is appropriate and stable for you.

When comparing mattresses, consider whether the resulting bed height supports the transfers you have been taught, whether the mattress feels firm and supportive to you, and whether it is comfortable in positions your care team permits. Guidance: MedlinePlus and RNOH.
What position should you sleep in?
Follow the instructions from your own surgical team rather than applying a general timetable. Precautions depend on the operation, and hospital protocols differ. For example, Penn says patients may be asked to sleep on their backs during the first six weeks. RNOH advises back sleeping for the first month in its protocol, while Cambridge University Hospitals (CUH) specifies two weeks for people following hip precautions. These are institution-specific directions, not interchangeable rules for every patient.

Instructions for returning to side sleeping differ too. RNOH’s guidance says operated-side sleeping may be possible when it is not too tender and cautions against the non-operated side in its protocol. CUH’s leaflet allows operated-side sleeping after its initial period, with a pillow between the legs. Ask your surgeon or physical therapist before changing positions or assuming that another hospital’s timeline applies to you.
Penn advises following your own care instructions, and UChicago Medicine explains that precautions depend on the type of surgery. Can a pillow help with positioning?
Some care instructions use an ordinary pillow between the legs or knees as a positioning aid. UChicago Medicine gives a pillow between the knees when side sleeping as an example precaution; CUH includes a pillow between the legs in its instructions. Use one only if that position is allowed by your own care team. These sources do not establish that a specialty pillow is required.
UChicago Medicine also lists avoiding crossed knees, extreme bending and twisting among examples of precautions. The restrictions relevant to you depend on your operation and instructions: UChicago Medicine; CUH.

A practical way to choose or adjust your bed
- Ask about your precautions. Confirm which sleep positions are allowed and whether you have transfer restrictions.
- Assess the whole bed. With your care team’s guidance, check that sitting at the edge lets your feet reach the floor and keeps your hips higher than your knees.
- Use firmness as a starting point. Look for a firm mattress, as MedlinePlus and RNOH recommend; the guidance does not identify a superior material or consumer firmness rating.
- Recheck after any change. A different mattress or added layer changes the height and feel of the bed. Make sure the setup still supports safe transfers and is compatible with your instructions.
- Get help if transfers feel unsafe. Ask your physical therapist or occupational therapist to review the setup rather than relying on a product description or another patient’s routine.
