The best rural health-center site is one that people can reach reliably and that can safely support the services you intend to provide. Compare candidate parcels on real travel time, population coverage, land and drainage, flood and environmental hazards, dependable water and power, sanitation, communications, emergency transport, and resilience to local disruptions. Then confirm the winning site against your country’s engineering, planning, environmental, and health-facility requirements.
Start with the service you are trying to deliver
Site selection is not separate from service planning. A small outpatient post, a maternity center, a laboratory, and a facility with overnight beds have different access, space, utility, waste, referral, and emergency requirements. Write down the intended service package before comparing land.

Random freezes, missing sound and display glitches usually trace back to one bad driver. Find and replace yours safely.Free scan · under a minuteWindows Errors? Fix Them Before They Spread
Repair common Windows errors and clear accumulated junk for a smoother, more stable PC - no reinstall needed.Free scan · no reinstallDefine the catchment and referral role
Estimate which communities the center should serve, how patients will arrive, and where they will go when they need higher-level care. Include routine visits, maternal and newborn emergencies, injuries, disease outbreaks, medicine deliveries, staff travel, and waste collection. A site that is convenient for one village may be poorly placed for the wider catchment or for transfers to a hospital.
Translate services into site requirements
List the rooms, circulation, parking, ambulance access, storage, staff areas, water demand, electrical loads, wastewater arrangements, communications, and future expansion the service package requires. This prevents a low-cost parcel from becoming an expensive constraint after construction begins.

Where should a rural health clinic be located?
Locate it where the population can reach it in predictable time over the routes they actually use, including during the wettest, hottest, or most disrupted season. Straight-line distance or a simple radius is not enough. The World Health Organization’s district-facility planning guidance explains that catchments are shaped by time boundaries and that difficult terrain takes longer to cross.
Measure travel time, not just kilometres
- Map walking, motorcycle, car, bus, boat, and ambulance routes that residents actually use.
- Record road surface, bridges, steep sections, river crossings, bottlenecks, and places where vehicles must transfer to walking.
- Check travel in both dry and wet seasons, at night, and during likely flood or storm conditions.
- Document available transport, fares, operating hours, fuel access, and whether emergency vehicles can reach the entrance.
- Compare travel time for the most remote and vulnerable communities, not only the settlement closest to the parcel.
Do not present one universal distance or travel-time target. National or subnational standards may set a threshold, but the global guidance does not establish a single rule that applies everywhere.
Connect the site to the local service network
Review links to referral hospitals, laboratories, pharmacies, schools, markets, transport hubs, and community destinations. Nearby institutions can make the center easier to reach and can support outreach, health education, and staff recruitment. The site should also have a clear, all-weather route for referrals and supply deliveries.
Screen the parcel and its surroundings
Once access is plausible, test whether the land itself can safely carry the project. WHO planning guidance identifies size, topography, drainage, soil conditions, utility availability, and natural features or limitations as site-selection considerations.
Parcel size, shape, and expansion
Check whether the parcel accommodates the planned building, setbacks required by local rules, internal roads, ambulance turning, parking, waste areas, water and wastewater systems, storm-water routes, security, staff housing where needed, and construction staging. An irregular or narrow parcel may technically contain a building while leaving no safe circulation or expansion area.
Reserve space for likely growth: additional consultation rooms, maternity or diagnostic services, larger medicine storage, solar equipment, water tanks, or a second building. Confirm ownership, boundaries, access rights, permitted use, and any easements before spending on design.

- One-Handed Loading - Keep one hand free with a 1/4 in. hex chuck that accepts 1 in. bit tips
Topography, drainage, and soil
- Have qualified local professionals survey levels and identify natural drainage paths.
- Check whether the building platform, access road, parking, and waste systems can be constructed without creating erosion or ponding.
- Obtain a geotechnical assessment appropriate to the proposed structure; soil-bearing and foundation requirements are local engineering decisions.
- Look for unstable slopes, subsidence, expansive soils, rock, high groundwater, and evidence of past inundation.
Do not assume that filling a low area solves the problem. It can redirect water toward neighbouring land, increase settlement risk, and raise long-term maintenance costs.
Flood, pollution, and neighbouring land uses
Screen river, coastal, flash-flood, landslide, wildfire, extreme-heat, and other hazards relevant to the district. WHO guidance specifically advises avoiding flood danger and pollution. Check air, water, land, and noise pollution from industry, waste sites, heavy traffic, agriculture, mining, and contaminated land. Also consider nearby institutional, educational, cultural, and commercial centres, which may improve access but can increase congestion or exposure to hazards.
Verify water, sanitation, power, and communications
A utility connection shown on a map is not the same as a service that works when patients need it. Check capacity, continuity, quality, maintenance responsibility, connection cost, and a documented fallback for each essential utility.
Water supply and hygiene
Confirm a safe source, seasonal yield, storage capacity, treatment, pressure, distribution to every clinical and cleaning point, and a plan for testing and maintenance. Include water for hand hygiene, toilets, laundry, sterilization, cleaning, drinking, kitchens, and any laboratory or maternity service. A borehole, piped network, rainwater system, or hauled supply may each be viable in a particular setting, but the source, treatment, storage, operating skills, and recurrent costs must be proven locally.

Electricity and critical loads
List loads that must continue during an outage: lighting, communications, vaccine or medicine refrigeration, oxygen or suction equipment, laboratory devices, water pumps, security, data systems, and ventilation. WHO reports that rural and remote facilities face greater electricity-access challenges and warns that a nominal connection may not provide enough usable energy; generators may also be unavailable or non-operational in some settings.
Where the grid is absent or unreliable, evaluate a complete system rather than buying a generator in isolation. Compare grid service, generator capacity and fuel logistics, solar photovoltaic generation, battery storage, and combinations of these options. WHO identifies decentralized solar and batteries as options that can be deployed without waiting for a grid connection. Include replacement cycles, skilled maintenance, battery safety, spare parts, fuel storage, and who will operate the system.

Communications
Test mobile coverage, internet availability, radio options, and backup power at the exact building location. Communications support referrals, emergency coordination, reporting, teleconsultation where available, supply ordering, and contact with maintenance teams. A signal at the nearest road is not evidence of reliable service inside the facility.
Design for climate and operational continuity
A rural center must remain usable when the conditions that create the greatest health need also disrupt roads, water, power, or supplies. WHO’s 2020 guidance states that climate-resilient and environmentally sustainable health-care facilities contribute to quality and accessibility of care.
Build a local hazard-and-service scenario
For each relevant hazard, record what fails first, how long the interruption could last, and what the center must continue providing. Consider access roads cut by floods, heat that reduces equipment performance, storms that damage distribution lines, drought that reduces water yield, wildfire smoke, landslides, and disease surges. Coordinate this work with water and sanitation, energy, transport, environmental, land-use, and emergency-management authorities; climate resilience is a multisector task.
Check practical mitigations before rejecting or accepting a site
- Raise or relocate critical equipment above likely flood levels established by local assessment.
- Provide covered, accessible routes between the entrance, clinical areas, toilets, and emergency vehicle bay.
- Protect water tanks, pumps, fuel, batteries, and communications equipment from predictable hazards.
- Maintain more than one route for patients, staff, and supplies where feasible.
- Store essential medicines, water-treatment supplies, fuel, and spare parts according to the service continuity plan.
Mitigation must be costed over the facility’s life. A site that needs permanent pumping, imported fuel, major retaining walls, or frequent road repair may be less resilient than a slightly more expensive parcel with natural drainage and dependable access.

Make emergency transport and referrals explicit
Emergency access deserves its own test, not a note under general accessibility. Confirm that an ambulance or other emergency vehicle can reach the entrance in the worst expected season, turn around, park without blocking routine traffic, and leave quickly. Check bridge weight limits, gate widths, gradients, overhead clearances, and the route to the referral facility.
Ask who provides transport, at what times, with what vehicles, and how a transfer is requested when mobile networks fail. Include a communications chain, a receiving-facility agreement where applicable, and a safe handover area protected from weather.
Use a transparent comparison process
When several parcels remain viable, apply the same evidence standard to each. WHO’s primary-health-care measurement framework treats facility distribution, geographic access, water, power, communications, and emergency transport as relevant infrastructure and access indicators.
Suggested comparison worksheet
| Criterion | Evidence to collect | Decision question |
|---|---|---|
| Population access | Seasonal travel times, routes, terrain, transport, and catchment map | Can priority communities reach the planned services reliably? |
| Land suitability | Survey, topography, drainage, soil assessment, parcel boundaries | Can the site safely support construction, circulation, utilities, and growth? |
| Hazard and environment | Flood and climate screening; pollution and neighbouring-use review | Can foreseeable risks be avoided or mitigated at an acceptable life-cycle cost? |
| Utilities | Water quality and yield, sanitation route, power capacity, communications tests | Will essential services operate through normal fluctuations and outages? |
| Emergency and referral access | Vehicle route, turning space, transfer time, communications plan | Can urgent patients, staff, supplies, and waste move safely? |
| Approvals and operations | Ownership, permitted use, access rights, permits, maintenance and staffing plan | Can the project be approved, operated, and maintained locally? |
Set weights openly
The international sources do not prescribe a universal scoring formula or weights. Set them with the health authority, community representatives, clinicians, engineers, emergency services, and future operators. A maternity service may give greater weight to dependable emergency transport and water; a remote diagnostic center may place more weight on power quality and communications. Record evidence, assumptions, unknowns, mitigation costs, and the reason a trade-off was accepted.

Global guidance cannot establish your jurisdiction’s minimum parcel area, soil-bearing threshold, flood elevation, environmental setback, catchment population, travel-time target, zoning rule, procurement process, or approval sequence. Before final selection, obtain written confirmation from the relevant planning, health, environmental, water, electricity, roads, and emergency authorities.
- Confirm land ownership, boundaries, permitted health-care use, access rights, and encumbrances.
- Commission topographic, drainage, geotechnical, flood, environmental, and utility assessments appropriate to the service package.
- Validate the catchment, referral routes, seasonal travel times, and emergency transport arrangements with local providers and communities.
- Prepare concept layouts showing buildings, roads, drainage, utilities, waste areas, expansion, and hazard mitigations.
- Obtain preliminary utility capacities, connection conditions, operating costs, and backup-system requirements in writing.
- Submit the required planning, environmental, construction, water, waste, and health-facility approvals before land commitment becomes irreversible.
- Re-score candidates after technical findings and include the cost and feasibility of every proposed mitigation.
- Document the final decision, unresolved risks, owners for each action, and the review date for assumptions that may change.
Electricity access: why the risk deserves attention
WHO’s 2023 electricity fact sheet says, “Electricity is needed to power the most basic services in health-care facilities, from lighting and communications to clean water supply.” It estimates that close to 1 billion people in low- and lower-middle-income countries are served by facilities without reliable electricity or with no electricity access.
| Region or measure | Reported finding | Qualification |
|---|---|---|
| South Asia | At least 12% of facilities lack any electricity access | WHO 2023 fact sheet; regional estimate |
| Sub-Saharan Africa | At least 15% lack any electricity access | WHO 2023 fact sheet; regional estimate |
| Latin America and the Caribbean | 8% lack electricity access | WHO 2023 fact sheet; regional estimate |
| Sub-Saharan Africa | 40% have reliable electricity | Average based on available survey data |
| Latin America and the Caribbean | 72% have reliable electricity | Average based on available survey data |
These findings use national survey data from 27 low- and lower-middle-income countries collected in survey years 2015–2022. They describe regional conditions, not the performance of a particular parcel. Your feasibility study still needs site-specific measurements and a financially workable operating plan.
Common site-selection mistakes and how to correct them
- Choosing by straight-line distance: replace the radius with seasonal, route-based travel times and terrain analysis.
- Accepting a utility promise: obtain capacity, reliability, connection, maintenance, and backup evidence for the exact parcel.
- Ignoring the rainy season: inspect roads, drainage, water sources, and emergency routes when disruption is most likely.
- Fitting only the first building: reserve circulation, services, waste areas, and expansion land from the beginning.
- Leaving emergency access to the contractor: test vehicle geometry, route constraints, communications, and referral handover during planning.
- Assuming mitigation is automatically cheaper than relocation: price construction, maintenance, energy, fuel, staffing, and replacement over the facility’s life.
- Using global guidance as local law: have the competent authorities and qualified professionals establish the thresholds and approvals that apply.
A practical go/no-go test
Proceed to detailed design only when the candidate site has a credible access route for priority communities, a buildable and expandable parcel, manageable hazards, dependable water and sanitation, an adequate power strategy, working communications, emergency and referral access, and a clear path through local approvals. If any of these depends on an uncosted promise or an unverified assumption, keep the site in due diligence rather than treating it as selected.
