building safety

What information must a golden thread of building safety information include?

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

In England, a golden thread is the managed digital record that explains how a higher-risk building was designed, constructed, handed over and kept safe in use. It is not one document or a fixed-size folder. The required information changes with the building’s lifecycle, the dutyholder responsible for it and the person who needs to use it.

For an occupied higher-risk residential building, the core operational record includes the health and safety file, safety case report, residents’ engagement strategy and mandatory occurrence reporting system. The record also needs design and construction evidence, handover information, fire and structural-safety records, maintenance history, change records and clear arrangements for sharing information with residents, owners, other dutyholders, regulators and fire and rescue authorities.

Which buildings and duties does the golden thread apply to?

The golden-thread duties described here are part of England’s higher-risk building regime. Building Safety Regulator (BSR) guidance identifies a higher-risk building by its height or storey count and its use. The threshold is generally at least 7 storeys or 18 metres, together with either at least 2 residential units, or use as a hospital or care home. Mixed-use and borderline buildings need to be assessed against the detailed legal criteria rather than by height alone.

A higher-risk building containing at least two residential units must be registered with the BSR before residents move in. Registration is a legal condition of occupation, not a substitute for maintaining the golden thread. The BSR page setting out this scope was published on 29 February 2024 and last updated on 18 September 2024; legislation and guidance can change, so the current rules should be checked for a specific project.

The accountable person (AP) is responsible for the parts of a higher-risk building for which they have responsibility. Where there is more than one AP, the principal accountable person (PAP) coordinates the building-level arrangements. During design and construction, the client, principal designer and principal contractor have separate information and cooperation duties.

What the golden thread must achieve

The record must let a competent person answer, at any point in the building’s life, five practical questions:

  • What is the building made of, and how was it intended to perform?
  • What decisions, changes and approvals occurred during design and construction?
  • What safety risks exist now, and what controls manage them?
  • Who is responsible for each control, inspection, repair and decision?
  • How can the right information reach residents, owners, emergency responders and regulators?

For that reason, the golden thread is a controlled information system rather than an archive of disconnected drawings. It must remain accurate, understandable, secure, accessible when needed and transferable without corruption.

Information required during design and construction

Client records and building-control information

The client keeps information describing the proposed building and demonstrating how the work will comply with building regulations. The project record used for building-control approval can include:

  • Drawings, plans, specifications and the site location plan.
  • Declarations or other evidence of dutyholders’ competence.
  • A construction control plan explaining how the work will be managed.
  • A change control plan and change control log.
  • A building-regulations compliance statement.
  • A fire and emergency file.
  • Client authorisation and decisions affecting compliance or safety.
  • A mandatory occurrence reporting plan.
  • A planning statement.
  • A partial-completion strategy where work is completed in stages.

The purpose is to show not only the intended design but also how the team will ensure that the finished building matches it. If a design assumption changes, the record should show what changed, why it changed, who approved it, what risks were reconsidered and which drawings or specifications were superseded.

Principal designer information

The principal designer captures and keeps design information current. They coordinate the design team, communicate relevant changes and provide evidence that design work meets building-regulation requirements. Information should identify critical fire and structural features, interfaces between systems, assumptions about products and construction, and any limits on future alteration or use.

Principal contractor information

The principal contractor uses the client’s record-keeping system and keeps evidence that the work complies with building regulations. The contractor works with the design team to manage changes and records how construction was controlled. Useful evidence includes inspection and test results, installation records, product information, non-conformance reports, corrective actions, photographs where they demonstrate concealed work, and confirmation that safety-critical elements were installed as designed.

Where site conditions require a deviation, the change should be processed through the agreed change-control system rather than recorded only in emails or a supervisor’s notebook. The final record must make clear which information is current at completion.

What must be transferred at completion and handover?

At completion, the client must demonstrate that the relevant information has been given to the PAP or AP for a high-rise residential building. For non-residential buildings or parts, relevant information goes to the responsible person under fire-safety law. The recipient should confirm both receipt and that the information is in a usable format.

A handover package normally includes:

  • The information used for building-control approval, including the final approved design and controlled revisions.
  • Information supplied for the completion-certificate application.
  • The completion certificate.
  • Details of how building information is accessed and controlled.
  • Relevant fire-safety information for the responsible person.
  • Instructions, warranties, commissioning records and information needed for inspection, maintenance and safe operation.

Handover is not complete merely because files have been uploaded. The receiving AP, PAP or responsible person needs to know what each file is, whether it is final, how it relates to the building and how future updates will be made. Missing information should be identified and managed as a recorded gap, with an owner and a plan to resolve it.

The core record for an occupied higher-risk residential building

Health and safety file

The health and safety file brings together information needed to understand and manage building-safety risks. It should describe the building’s construction, relevant materials and systems, safety-critical features, inspection and maintenance requirements, known hazards and the controls in place. Refurbishment records, information from previous owners, inspection reports, maintenance records and the standard to which the building was built should be retained when available.

Safety case report

The safety case report explains how the AP or PAP identifies and manages building-safety risks, including risks of fire spread and structural failure. It should connect the assessment of hazards to the physical features of the building, preventive and protective measures, monitoring arrangements and actions required when conditions change. The report is not a generic fire-risk assessment; it is the building-level explanation of why the risk controls are suitable and how they are kept effective.

Residents’ engagement strategy

The residents’ engagement strategy sets out how residents and owners will be informed and involved in building safety. It should explain what information will be provided, how feedback and complaints are handled, how residents can report a safety concern and how the AP or PAP will respond.

Mandatory occurrence reporting system

The mandatory occurrence reporting system provides a route for reporting safety occurrences that could indicate a serious structural or fire-safety risk. It should identify how an occurrence is recognised, who receives the report, how it is recorded, how urgent risks are escalated and how required notifications are made. Records should show the report, assessment, decisions and resulting corrective action.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Information residents and owners must receive

Residents and owners must receive information relevant to preventing or reducing building-safety risks. The package should include:

  • A summary of the latest fire-risk assessment and safety case report.
  • Ways to prevent or reduce the severity of an incident and how to report a risk.
  • The locations of escape routes, fire doors, alarms and other relevant safety aids.
  • The location of fire and smoke-control equipment and instructions for using it where residents are expected to do so.
  • Evacuation information appropriate to the building’s strategy.
  • The names or roles, responsibilities and contact details of the people and organisations responsible for building safety.
  • The latest residents’ engagement strategy.
  • Information about the complaints system.
  • Details of documents residents may request and how to request them.

Information should be understandable to its intended audience. Technical drawings or a safety case report may need a plain-English summary so that residents can act on the information without interpreting specialist terminology.

Who receives and shares the information?

Golden-thread duties are recipient-specific. The record should identify the responsible person, the part of the building concerned and the information that recipient is entitled or required to receive.

Digital, security and quality requirements

The golden thread must be digital, but a digital format alone is not enough. The system should:

  • Act as the building’s single source of truth, with clear links between documents, assets, risks and decisions.
  • Use version control so users can distinguish current information from superseded copies.
  • Restrict access to authorised users and protect personal data in accordance with data-protection requirements.
  • Keep information accurate, accessible on request, understandable and usable.
  • Record who changed information and the date of each change.
  • Keep copies of documents and preserve an audit trail.
  • Allow safe transfer to another system or dutyholder without corruption.
  • Use simple formats and plain English where possible.

Access controls should not prevent legitimate safety work. Emergency responders and relevant dutyholders need a practical route to the information they are entitled to see, while sensitive personal or security information should not be exposed unnecessarily.

How to build and maintain the record: a practical process

  1. Confirm scope and roles. Determine whether the building meets the higher-risk criteria, identify the client, principal designer, principal contractor, AP and PAP, and map any responsible persons for fire safety.
  2. Create an information register. List each required record, the building area or system it concerns, the owner, recipient, review date, security classification and current status.
  3. Set the controlled system before work starts. Define naming conventions, revision rules, approval states, permissions, retention and backup arrangements.
  4. Capture design decisions and assumptions. Record safety-critical design information, competence evidence, compliance statements, fire and emergency information and planned change controls.
  5. Control construction changes. Log every material change, its reason, risk assessment, approval, construction action and effect on the final record.
  6. Verify installed work. Collect inspection, testing, commissioning and product evidence, and resolve discrepancies before completion.
  7. Prepare and test handover. Deliver the completion and fire-safety information, confirm that the recipient can open and understand it, and record any outstanding gap.
  8. Operate the building through the record. Keep the health and safety file, safety case report, residents’ engagement strategy and mandatory occurrence reporting system current.
  9. Review after change or incident. Update the risk assessment and related documents after refurbishment, a significant occurrence, a change in use, a new defect or a change in responsible person.
  10. Audit transfer and access. Periodically check that records are readable, permissions are correct, backups work and an incoming dutyholder could take over without losing context.

Common failures and how to correct them

Treating the golden thread as an archive

A folder of historic drawings does not show which information is current or how risks are controlled. Add metadata, revision status, owners, links between records and a documented process for updates.

Leaving changes in informal correspondence

Decisions held only in email, messaging applications or personal notebooks are difficult to audit and may be lost when staff change. Move the decision, approval and resulting drawing or specification into the controlled change log.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Handing over files without context

Unlabelled PDFs and duplicate versions force the recipient to reconstruct the building. Provide an index, a current-status statement, a map of safety-critical systems and instructions for access.

Providing residents with technical material only

Residents need actionable information, not an undigested document dump. Give plain-English summaries, clear contacts, equipment locations, evacuation instructions and a usable reporting and complaints route.

Ignoring shared responsibilities

Where several APs or responsible persons operate in one building, gaps often occur at interfaces such as shared alarms, smoke control, structure or evacuation arrangements. Record those interfaces, agree who controls each one and share the relevant information.

Failing to protect or preserve information

Overly broad access can expose personal or security-sensitive data, while weak backups or incompatible exports can destroy continuity. Apply role-based access, test restoration and transfer copies, and retain the audit history.

What is the minimum sensible checklist?

For a quick completeness check, confirm that the record contains the following, with an identified owner and recipient for each item:

  • Building description, plans, systems and safety-critical features.
  • Design, competence, compliance and construction-control evidence.
  • Approved changes and the final as-built position.
  • Completion certificate and handover confirmation.
  • Fire and emergency information.
  • Health and safety file and safety case report.
  • Residents’ engagement strategy and complaints arrangements.
  • Mandatory occurrence reporting arrangements and records.
  • Inspection, maintenance, refurbishment and defect records.
  • Resident, owner, dutyholder and emergency-service information-sharing arrangements.
  • Version control, access permissions, change history, backups and transfer procedures.

This checklist is a management aid, not a replacement for the detailed information schedules in the accountable persons’ regulations or the BSR guidance. The exact records and disclosure duties depend on the building, its use, the dutyholder’s role and the recipient.
