Hospital construction costs spike when several pressures hit the same project: prices for labor, materials or equipment rise; the original estimate is incomplete or based on outdated prices; the clinical scope changes; site and utility problems emerge; or specialised safety, infection-control and seismic requirements add work. A longer schedule can amplify every one of those effects.
There is no defensible universal percentage for each cause. The best available evidence comes from selected U.S. Department of Veterans Affairs projects reviewed by the U.S. Government Accountability Office (GAO), Illinois project filings and England’s New Hospital Programme. Those sources show recurring mechanisms, not a national average that can be applied to every hospital.

Labor, material and equipment inflation
Input prices are the most visible trigger. An estimate prepared before a sharp rise in wages, steel, concrete, mechanical equipment or specialist fixtures can become inadequate even when the design has not changed. An Illinois Harrisburg Medical Center filing identified “material and labor inflation due to the pandemic” as two substantial cost factors and also described post-pandemic construction pressures.
Escalation affects more than the final purchase price. If a project is delayed, the owner may buy equipment later at a higher price, pay contractors for a longer period, and carry financing, temporary facilities or staff costs for additional months. A price allowance that was reasonable on the estimate date may therefore be insufficient by the time a package is bid.

Incomplete or poorly timed estimates
An estimate is only as reliable as its scope, drawings, assumptions and price date. The GAO’s report GAO-10-189 found that selected VA projects experienced increases and delays when initial estimates were not thorough, when scope changed after the estimate was submitted, and when construction-material prices rose unexpectedly.
Common weaknesses include pricing conceptual drawings as if they were complete, omitting enabling work, using old subcontractor quotes, failing to state whether prices are nominal or inflation-adjusted, and carrying allowances that do not reflect the site or clinical standard. An estimate can look precise while still excluding major unknowns.

Hospital programmes often evolve after an early budget is approved. A department may need more rooms, a different circulation route, additional equipment capacity or revised infection-control separation. Changes can affect structure, finishes, utilities, fire protection, information systems and commissioning, not just the room where the change originated.
The timing matters. A change made before procurement may require redesign; a change made after a package is bought can create cancellation charges, rework, remobilisation and schedule delay. The GAO specifically identified significant changes to project scope after the initial estimate as a cause of cost increases and schedule delays.
Site, utility and regulatory conditions
A hospital site can impose costs that are invisible in a high-level feasibility estimate. The Harrisburg filing cited challenging topography, utility relocations and seismic upgrades. Each can require additional surveys, engineering, excavation, temporary services, structural work or authority coordination.
Local availability also matters. The same filing described safety-compliant fixtures that were more expensive and less available locally. A specification that is straightforward in one market may require long-lead procurement, special transport or a broader bidding area in another.

Specialised clinical and infection-control design
Hospitals are not generic commercial buildings. Their layouts and construction methods must support clinical workflows and safe separation of patients, staff and materials. The Champaign project filing named design elements intended to improve efficiency and infection control among factors in cost increases.
Such requirements can add cost through more complex layouts, controlled transitions, specialised finishes, additional coordination and testing. The correct question is not whether a feature is expensive in isolation, but whether the programme has priced the full design, construction and commissioning consequences of the clinical standard.
Schedule slippage and compounded exposure
A late project can become more expensive even without a new scope item. Extended site overheads, repeated inspections, temporary accommodation, storage, security and supervision accumulate. Procurement windows may be missed, forcing substitutions or expedited shipping. Inflation then applies for a longer period, while revenue or service benefits arrive later.
These drivers interact. A design change can delay a bid package; the delay can expose the package to higher material prices; a difficult site can slow installation; and late decisions can force overtime or resequencing. Treating each increase as a separate event can hide the chain that produced the final variance.

These are project- or programme-level observations. They establish useful categories for investigating an overrun, but they do not provide a common dataset from which to calculate a universal labor, materials, design or site percentage.
How an overrun develops through the project lifecycle
1. Feasibility and site selection
At this stage, owners set the service brief, select a site and establish an order-of-magnitude budget. Risk rises when the budget is treated as a commitment before topography, utilities, seismic conditions, access and regulatory requirements are understood. A low early number can also steer decisions that become expensive to reverse.

2. Schematic and clinical planning
Clinical leaders, infection-control specialists and facilities teams turn the brief into rooms, adjacencies and performance requirements. Every unresolved decision becomes an allowance or an assumption. The budget should identify which items are defined, which are provisional and which are excluded, rather than presenting one total without confidence ranges.
3. Design development and estimating
As drawings mature, estimators should reconcile quantities, specifications, construction sequencing and current market quotations. The price basis needs a clear date and location. If the estimate is several months old, the owner should show what has been escalated and what has not. Scope changes should be logged with their cost and schedule effect before approval.

4. Procurement
Long-lead equipment, specialist fixtures and constrained trades can determine the critical path. A package that attracts few bidders may cost more than an early allowance suggested. Owners should test availability, alternates and delivery dates before freezing the procurement plan, especially where the local market cannot supply compliant products.
5. Construction and commissioning
During construction, the cost report must separate committed cost, forecast cost, approved changes, pending changes, contingency drawdown and escalation. A change that appears small in a room can affect multiple trades or require retesting. Commissioning and clinical readiness should remain visible in the forecast rather than being treated as an afterthought.
How to diagnose a hospital budget spike
When the forecast moves, compare the original and revised positions using the same cost structure. The following sequence keeps scope, price and schedule effects from being mixed together:
- Fix the baseline. Record the original estimate date, design revision, price region, currency basis, escalation assumptions, contingency and exclusions.
- Reconcile scope. List every addition, deletion, specification change and room or service revision since the baseline. Price approved and pending changes separately.
- Measure market movement. Compare labor, material and equipment quotations at the baseline date with current quotations. Identify whether the difference is a broad escalation, a supplier-specific increase, a substitution or a shortage premium.
- Isolate site and compliance work. Track excavation, utility relocation, seismic measures, authority requirements, temporary services and safety-compliant products as distinct cost codes.
- Trace schedule effects. Show how design release dates, approvals, procurement lead times and site productivity changed. Quantify extended overheads and any rework or remobilisation.
- Check the remaining risk. After explaining the current variance, reassess the contingency for unresolved design, market and site risks. A budget that merely funds known overruns may still be under-reserved.
This reconciliation produces an evidence-based explanation without pretending that every dollar has one exclusive cause. For example, a late clinical change may be recorded as scope, while its resulting six-month delay also creates escalation and extended overheads; both effects belong in the forecast, but they should not be counted twice.

What does the 29% average overrun figure mean?
The Irish Parliamentary Budget Office reported in 2025 that international health and hospital construction projects have average cost overruns of 29%, attributing the figure to Flyvbjerg and Gardner (2023). That is a secondary citation of an international average, not a prediction for a particular hospital and not a breakdown by labor, materials, design or site conditions.
Use the figure as a warning that large health projects are exposed to substantial delivery risk, not as a contingency instruction. Geography, project type, contracting strategy, estimate maturity, inflation period and the definition of “overrun” can all change the result. Any business case should rely on its own risk register and documented estimate reconciliation.
Can standardisation prevent spikes?
Standard designs can reduce repeated design effort and support more predictable procurement across a programme. The UK National Audit Office said standardisation has potential to lower costs over time, while also stressing careful planning and schedule monitoring. Standardisation is not a substitute for site investigation or clinical validation: a repeated design still needs adaptation for local ground, utilities, seismic conditions, regulations and service requirements.
The practical test is to distinguish true repetition from nominal repetition. If each “standard” project requires extensive redesign, bespoke interfaces or late clinical changes, the expected savings may not materialise. A programme should measure which components are reused, the adaptations made and the cost and schedule consequences of those adaptations.

How owners can reduce the chance of a spike
- Set a dated, transparent baseline. State the price date, location, escalation treatment, inclusions, exclusions and confidence range.
- Advance site knowledge early. Complete topographical, geotechnical and utility investigations before major budget commitments, and identify seismic or authority requirements.
- Freeze clinical decisions in stages. Use formal gateways for the service brief, room data, infection-control strategy and equipment requirements. Do not hide unresolved choices inside a single allowance.
- Use independent estimate reviews. Have a separate team challenge quantities, market quotations, phasing, temporary works and omitted scope before approval.
- Plan procurement around availability. Test compliant local supply, lead times, alternates and storage needs for specialist fixtures and equipment.
- Control changes visibly. Require every proposed change to show construction cost, design cost, schedule effect, operational effect and funding source before authorisation.
- Monitor schedule as a cost variable. Track design release, approvals, procurement and installation milestones, because delay can create both direct overhead and additional escalation.
- Keep contingency risk-based. Allocate reserves to identified design, site, market and schedule risks, then report drawdown and remaining exposure separately.
How much do labor, materials, design changes and site conditions add?
No current, comparable dataset establishes a standard add-on for those categories. The Illinois filings and GAO review demonstrate that each can be material, but they do not support a universal percentage. The only broad figure identified is the 29% international average reported by the Irish Parliamentary Budget Office, and that figure cannot be divided into four standard shares.
For a specific hospital, the defensible answer comes from the project records: compare the original and revised estimate, price each approved change, reconcile labor and material quotations, document site discoveries and calculate the cost of schedule movement. If those records are incomplete, the correct conclusion is that the cause cannot be quantified reliably—not that an industry-average percentage should be substituted.
Bottom line for a hospital project team
Hospital construction costs spike when an immature estimate meets changing scope, volatile inputs, difficult site conditions or specialised clinical requirements, especially when the schedule gives those pressures more time to accumulate. The most useful response is a disciplined variance analysis that separates scope, escalation, site, compliance and schedule effects, then updates the remaining risk before the next commitment.
