What a Bridge Construction Quality-Control Plan Should Include

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

A bridge construction quality-control (QC) plan is the contractor’s project-specific operating manual for preventing, detecting, correcting and documenting nonconforming work. It should connect every inspection and test to the contract drawings, specifications, approved submittals and owner requirements; assign qualified people to perform the work; set sampling and testing frequencies; define records and review; and explain how agency acceptance, verification and independent-assurance activities interact with contractor QC.

There is no universal bridge testing schedule or approval deadline. The contract and the applicable owner specifications control the required tests, frequencies, qualifications, forms and submittal dates. Generic examples can organize a plan, but they cannot replace those requirements.

Start with the contract, not a generic template

The contract is the controlling source for required tests, acceptance limits, sampling frequencies, report timing and plan approval. Before writing the plan, collect the executed contract, special provisions, drawings, governing standards, approved mix designs and material submittals, owner manuals, and any referenced QC or testing specifications.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Record the document title, revision or edition, and the work item to which it applies. Establish a method for handling revisions and conflicts in accordance with the contract’s change-control procedures. A template is useful only as an index; it must be edited until every bridge activity and supplier process that can affect quality is covered.

Define the work covered

State the project limits and list the bridge elements and related work included in the plan. Depending on the contract, that may include earthwork and foundations, reinforcing steel, structural concrete, prefabricated bridge elements, bearings, drainage, approach slabs, coatings, utilities, temporary works and traffic-stage construction. Include subcontractor and supplier operations when their materials, fabrication or records affect the finished bridge.

State the plan’s interfaces

Explain how the contractor’s QC records will be made available to the owner, designer, resident engineer or other acceptance authority. Identify required hold points, witness points, advance notices, submittal reviews and release procedures. The plan should make clear which activities may not proceed until a specified inspection, test or approval is complete.

Assign authority and qualified personnel

Name the QC manager and the inspectors, samplers, testers and specialists responsible for each work area. Include an organization chart and reporting lines. The QC manager needs authority, within the contract, to require correction, withhold a release recommendation, stop an operation or escalate a nonconformance.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Document qualifications

List relevant certifications, training, experience and delegated responsibilities for each role. Qualifications are project-specific. For example, an FHWA sample specification for prefabricated bridge elements requires concrete testing personnel to hold current ACI Field Testing Technician Grade I certification or an equivalent credential, or to work under the direct supervision of a certified technician. That example does not create a universal requirement; the controlling project specification does.

Cover staffing and independence

Show who is present during preparation, startup and production, including night shifts and high-volume placements. Identify alternates and the escalation path when a responsible person is unavailable. If the owner requires independent assurance or verification testing, identify how those personnel remain separate from the contractor’s process-control function.

List materials, suppliers, laboratories and equipment

Create a material and source register for aggregates, cementitious materials, admixtures, reinforcing steel, structural steel, prefabricated units, backfill, geosynthetics and other products used on the bridge. For each item, identify the proposed source, required certifications, acceptance documents, storage conditions and controls for substitutions.

Pre-work submittal checks

Specify the review required before incorporation into the work: mix designs, mill certificates, material certifications, shop or fabrication records, supplier QC plans, batch information and calibration certificates. Define who reviews each submittal, what acceptance criteria apply and how unresolved comments are tracked.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Laboratory and measuring equipment

Name the QC laboratory and describe its approval, accreditation or qualification status under the owner’s requirements. List test equipment, identification numbers, calibration intervals and accuracy checks. Include field devices such as scales, thermometers, air meters, slump cones, density gauges and survey equipment when they are used. A calibration record should show the device, date, standard or reference used, results and next-due date.

Organize inspections by construction phase

For every activity, describe three levels of process control: preparatory inspection, initial or startup inspection, and production inspection. This structure catches problems before large quantities of nonconforming work are placed.

Preparatory inspection

  • Confirm the applicable specification, drawing detail, approved submittal and acceptance criteria.
  • Verify material sources, certificates, storage and protection.
  • Check that equipment, forms, access, weather protection and qualified personnel are available.
  • Confirm survey, staking, line, grade, elevations and tolerances.
  • Review required preliminary tests, trial batches, mock-ups or test sections.
  • Identify hold points and the notice required before work starts.

Initial or startup inspection

  • Review requirements and workmanship expectations with the crew and subcontractors.
  • Inspect the first operation or first lot against the approved method.
  • Verify that sampling locations, test methods and forms are ready.
  • Record deficiencies and correct them before production continues.

Production inspection

Inspect and test at the frequency established in the schedule, adjusted to the production rate and contract triggers. Record conditions that can change quality, such as placement temperature, weather, delivery time, batch identification, lift thickness, compaction effort or curing protection. QC is process control: results must be reviewed early enough to correct a drift before substantial nonconforming material is incorporated.

Build a contract-based sampling and testing schedule

The schedule is the plan’s operational core. Organize it by work item or material and include all information needed for a tester to act without guessing.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
Required field What to state
Characteristic The property or workmanship feature being checked, such as slump, air content, density, alignment or strength.
Method The specified test method, inspection procedure or measurement standard, including the applicable edition.
Sampling Where, when and how the sample is taken, preserved, identified and transported.
Lot or sublot How a quantity is grouped for testing and acceptance, if the specification uses lots or sublots.
Frequency or trigger A quantity, time interval, delivery, placement, change in source or other event that requires a test.
Responsibility The person or laboratory performing the inspection or test and the reviewer.
Record and timing The form or electronic record, report recipient and deadline for submitting results.
Limit and response The acceptance criterion and the action when a result is borderline, failing or missing.

Coordinate the schedule with the planned rate of production and placement. A frequency that is adequate for a small pour may be inadequate during a large continuous placement if the contract defines tests by quantity or lot. Conversely, do not invent extra acceptance requirements that the owner has not specified.

Concrete example

For concrete placement, the schedule may include mix-design review, delivery-ticket checks, slump, air content, yield, temperature, compressive-strength cylinders, curing records and placement observations where required. State the sampling point, test method, number of specimens, frequency by quantity or placement, curing and transport requirements, responsible tester, reporting route and acceptance process. FHWA bridge-construction review material uses slump, air, yield and compressive-strength cylinders as examples; those examples are not a substitute for the project’s required schedule.

Aggregate and earthwork example

Depending on the specification, controls can include material classification, gradation, moisture-density relationships, in-place moisture and density, lift thickness, moisture conditioning and stiffness or proof-rolling observations. Identify the source, lot definition, sampling approach, test frequency, acceptance criteria and corrective action for each property.

Prefabricated bridge element example

Include pre-pour and post-pour inspections, reinforcement and embedment checks, material certifications, batch slips, concrete test results, compressive testing, dimensional checks, curing records and repair or nonconformance documentation. State the required forms, report recipients, timing, hold points and final acceptance records in the controlling specification.

Define review, reporting and nonconformance control

Set a time for reviewing every inspection and test result, and identify the person who performs that review. Reports should be complete, legible and traceable to the material delivery, location, lot, date, equipment and personnel involved.

Use a clear response sequence

  1. Flag the issue. Mark a failing, missing or questionable result and notify the QC manager and affected supervisors.
  2. Protect the work. Identify the lot, placement or area involved; segregate material or stop the operation when the contract requires it.
  3. Investigate. Check sampling technique, test equipment, calculations, materials, production records and related results before deciding whether the result is valid.
  4. Determine disposition. Obtain the contractually required engineering or owner decision for acceptance as-is, repair, rework, removal or additional testing.
  5. Correct and verify. Document the repair or process change, perform retesting or reinspection, and record who released the affected work.
  6. Close the record. Link the nonconformance, corrective action, supporting test data and final disposition in the project record.

Borderline results deserve the same traceability as outright failures. The plan should state who may authorize additional testing, how affected work is identified and how trends are escalated before they become repeated defects.

Explain QC, acceptance, verification and independent assurance

Contractor QC controls the process; agency acceptance determines whether the work meets the contract. Verification or independent-assurance testing provides an additional check on the reliability of the process and test results. These functions can use related data but are not automatically interchangeable.

Function Typical purpose What the plan should clarify
Contractor QC Control production and identify problems early. Who tests, how often, what records are produced and how corrective action is triggered.
Agency acceptance Determine contractual compliance and payment or release status. Whether the agency performs its own tests, observes QC tests or may use contractor data under stated conditions.
Verification testing Check that QC results and materials represent the work. Sample selection, timing, comparison rules and action when results disagree.
Independent assurance Evaluate the testing system, personnel and equipment over time. Responsible organization, comparison records and resolution of unfavorable comparisons.

Use the roles, frequencies and acceptance arrangements in the governing contract. Do not promise that contractor data will support acceptance unless the owner expressly allows it and describes the validation process.

  • Nonconformance, corrective-action, repair and retest forms.
  • Control charts or trend reports when required or useful for process control.
  • Supplier and subcontractor QC plans, with interfaces and reporting deadlines.

Electronic records should preserve revision history, user identity, date and location, and provide a reliable link between a result and the work it represents.

Develop the plan in a repeatable sequence

  1. Map requirements. Extract every QC, acceptance, testing, qualification, record and notice requirement from the contract into a compliance matrix.
  2. Break the work into activities. Use the schedule of work and bridge element breakdown so no subcontractor or temporary stage is omitted.
  3. Assign ownership. Match each inspection, test, review and release to a named role and backup.
  4. Set control points. Mark preparatory, startup, hold and witness points, including the notice period for owner participation.
  5. Draft the test schedule. Add methods, sampling, lots, frequencies, limits, forms and reporting deadlines; reconcile it with production quantities.
  6. Define failure handling. Write the notification, isolation, investigation, disposition, repair, retest and closeout path.
  7. Integrate QA interfaces. Show how agency acceptance, verification and independent-assurance tests will be scheduled and compared.
  8. Attach working documents. Provide checklists, logs, forms, qualifications, laboratory information and calibration records.
  9. Review before submittal. Have construction, testing, safety and project-management personnel check the plan against the latest contract revision.
  10. Maintain the plan. Revise it when the owner changes a requirement, a source changes, a process changes or trend data shows that a control is ineffective.

Project-specific timing and qualifications must be verified

FHWA’s Sample Guide Specifications for Construction of Geosynthetic Reinforced Soil-Integrated Bridge System (GRS-IBS), August 2012, states: “The QC plan shall be project specific.” That sample specification also uses a 45-day advance submission period before related work. The 45-day period applies to that GRS-IBS example only; it is not a general bridge-plan deadline. Your owner, jurisdiction, bridge type and contract may require a different date, review cycle or approval process.

For the same reason, do not assume that a particular certification, laboratory accreditation, test method or frequency applies to every bridge. Confirm current owner manuals and the executed contract before converting an example into a compliance obligation.

Final readiness check

Before submitting the plan, a reviewer should be able to answer “yes” to each question below:

  • Does every bridge work item and affecting supplier process have a defined control?
  • Are governing specifications, drawings and revisions identified?
  • Are responsible people, qualifications, authority and backups named?
  • Are sources, laboratories, equipment and calibration controls documented?
  • Do preparatory, startup and production inspections match the actual sequence of work?
  • Can a tester determine exactly where, how and how often to sample?
  • Are limits, reporting deadlines, lot definitions and record forms clear?
  • Does the nonconformance process protect affected work and document final disposition?
  • Are agency acceptance, verification and independent-assurance roles distinguished from QC?
  • Are hold points, notices and release requirements visible in the schedule?
  • Can the records demonstrate what was tested, where, by whom, with what equipment and with what result?
