Rural Hospital Renovation Checklist: Safety, Accessibility, and Infection Control

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

A safe rural-hospital renovation starts with a written, project-specific plan—not a generic construction checklist. Before design is finalized, identify whether the facility is a hospital or Critical Access Hospital (CAH), define the work and affected services, complete an infection-control risk assessment (ICRA), map accessibility and egress, and confirm the code editions and approvals required by the state, locality, and authority having jurisdiction (AHJ). Keep infection prevention, life safety, accessibility, and care continuity as coordinated workstreams with named owners.

Use the checklist below to assign decisions before construction, control hazards while work is underway, and document the evidence required before a space returns to service. Your architect, engineers, infection-prevention lead, facilities team, state survey agency, and AHJ must resolve requirements for the specific building and project.

1. Establish the project facts before design is fixed

Write a one-page project brief that every designer, contractor, clinical leader, and inspector can use. Rural status does not remove federal, state, local, accessibility, infection-control, or life-safety obligations; it can, however, make temporary relocation, staffing, and redundancy decisions more consequential.

Confirm the facility and services

  • Record the facility classification: general hospital, CAH, or another designation; note owner and operator type.
  • List services in and next to the work, such as emergency care, surgery, obstetrics, imaging, dialysis, pharmacy, laboratory, behavioral health, dietary, and sterile processing.
  • Identify patient groups with increased susceptibility to airborne or waterborne hazards, including intensive-care patients, neonates, oncology patients, transplant recipients, and people with severe immunosuppression.
  • Mark inpatient rooms, treatment spaces, supply storage, medication areas, clean and dirty utility rooms, staff routes, public entrances, ambulance access, and emergency egress that could be affected.
  • State whether patients will remain in adjacent areas, whether services must operate continuously, and what backup location exists if a unit or route is closed.

Set boundaries, schedule, and decision authority

Define the physical limits, expected duration, demolition and utility shutdowns, dust- or moisture-producing tasks, occupied areas, and planned phasing. Name one decision owner for each control. A useful assignment matrix is:

2. Complete the infection-control risk assessment (ICRA)

CMS survey guidance says hospital policies must address construction, renovation, maintenance, demolition, and repair, including an ICRA before a project begins to define the scope and need for barrier measures. CDC guidance likewise recommends an ICRA before repairs, demolition, construction, or renovation. Treat it as a live risk-management document, not a form completed once for the permit file.

“An infection control risk assessment (ICRA) to define the scope of the project and need for barrier measures is performed before a project gets underway.” — Centers for Medicare & Medicaid Services, Survey and Certification Letter 13-03.

What the ICRA should examine

  • Work activity: demolition, ceiling access, drilling, cutting, excavation, plumbing, roofing, fireproofing, painting, and any task that creates dust, vibration, odor, moisture, or aerosols.
  • Location: the work room, spaces above and below it, adjacent patient-care areas, corridors, air-handling spaces, and supply storage.
  • Patient vulnerability: who is present now, who may be admitted during the schedule, and which patients should be moved or protected.
  • Building systems: supply and return air, pressure relationships, filters, water risers, domestic hot and cold water, drainage, medical gases, fire alarm, sprinklers, power, and emergency systems.
  • Operations: staffing, transport routes, emergency response, food and medication delivery, linen and waste movement, and the ability to isolate a failure.

Update the assessment when the scope, sequence, patient mix, barrier arrangement, HVAC condition, or water work changes. A change-control log should show who approved the revision and which new controls were added.

3. Build the multidisciplinary control plan

CDC recommends a multidisciplinary team that includes infection-control personnel. Include infection prevention, facilities or engineering, clinical leaders for affected units, environmental services, safety, project management, design professionals, and contractor representatives. Add information technology, pharmacy, sterile processing, laboratory, or emergency-management staff when their systems or routes are involved.

Put duties into contracts and work plans

Contract documents and preconstruction meeting minutes should state the required controls, training, inspection frequency, reporting route, correction deadline, and stop-work authority. CDC recommends infection-control adherence agreements in contracts and timely correction of lapses. Require the contractor to submit a work plan that matches the approved ICRA rather than selecting controls informally in the field.

Train everyone who enters the work zone

Orient contractors, temporary labor, hospital staff, and vendors who work in or near susceptible patient-care areas. Cover airborne infection risks, entry and exit routes, protective clothing, hand hygiene, waste and debris handling, reporting of barrier damage, and what to do during an alarm, spill, water event, or pressure failure. Keep attendance and competency records.

4. Design containment and protect occupied care

Containment is an operating control. CDC describes physical barriers capable of containing smoke and dust, along with site preparation, communication, adherence monitoring, HVAC monitoring, cleaning, and debris removal. The ICRA and design team—not a product label—determine the appropriate system.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Compare containment options against the actual risk

Question Why it affects the choice
How much dust, moisture, odor, or vibration will the work create? Higher-generating tasks require stronger isolation, sequencing, and monitoring.
How susceptible are nearby patients? Critical or immunocompromised populations may require relocation or additional separation.
Can ventilation and water systems be isolated and protected? Shared systems can spread contaminants beyond the visible work area.
Will barriers obstruct egress, transport, or emergency access? The selected arrangement must preserve safe routes or provide an approved alternative.
How will integrity be inspected and cleaned? A system that cannot be checked, repaired, and removed without releasing dust is unsuitable for the sequence.
What is the duration and phasing? Long projects need durable, maintainable controls and documented handoffs.

A temporary dust-barrier zipper-door kit may be one supply used in a project-defined barrier, but purchasing a kit does not make a work zone safe or compliant. Specify the complete assembly, access protocol, inspection method, and removal sequence in the approved plan.

Keep patients and services safe during work

  • Publish a schedule showing noisy work, utility shutdowns, corridor closures, deliveries, and high-risk tasks.
  • Notify staff, patients, visitors, transport teams, and emergency responders about changed routes and services.
  • Relocate patients when susceptibility, dust or water-aerosol potential, project duration, or available controls make continued occupancy inappropriate.
  • Protect medication, sterile supplies, food, laboratory specimens, and clean equipment from construction dust and moisture.
  • Use covered, controlled debris removal routes and coordinate timing so waste does not pass through vulnerable care areas.

5. Monitor air, HVAC, water, and housekeeping

Air and HVAC

Protect air-handling equipment from construction impacts. The project plan should identify affected air handlers, supply and return paths, filters, pressure relationships, temperature, and humidity. Monitor the conditions relevant to the ICRA and document readings or inspections at the frequency established by the team. CDC recommends restoring appropriate air-change, humidity, and pressure conditions and commissioning relevant renovated spaces before occupancy.

Do not insert a universal pressure value, filtration class, or air-change rate into a generic checklist. Those values depend on the space, adopted standards, existing system, and design. The engineer and infection-prevention team should define acceptance criteria and the response when a reading is outside them.

Water systems

Construction can disturb sediment or compromise water-system integrity. Protect open piping and equipment, control stagnant sections, coordinate shutdowns, and document who authorizes reconnection. CDC recommends flushing systems after work to clear sediment and coordinating water-management steps with infection prevention and facilities staff. If the project affects hot-water generation, storage, or recirculation, include the facility water-management program and any required testing before use.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Daily cleaning and surveillance

Inspect barriers, adjacent surfaces, entrances, floors, ceilings, and equipment daily. Perform routine and terminal cleaning appropriate to the area, and remove covered debris without tearing containment. CDC recommendations also support adherence monitoring, HVAC monitoring, and clinical or targeted environmental-disease surveillance when the ICRA and patient population warrant it. Record findings, corrective action, responsible person, and closure date.

6. Review accessibility in every altered area and route

The 2010 ADA Standards apply to altered portions of existing facilities. Additions are treated under new-construction requirements, and additional obligations can arise when work affects a primary-function area. Have the design team determine the precise scoping and technical requirements for the facility and entity; do not assume that a rural location or an existing condition automatically grants an exception.

Check more than the renovated room

  • Trace the accessible route from public arrival and parking or drop-off through entrances, registration, waiting, treatment, toilets, elevators, and exits affected by the work.
  • Review door clearances, thresholds, maneuvering space, ramps, lifts, handrails, signage, alarms, and accessible toilet rooms in altered portions.
  • Assess common-use and public-use elements, service counters, patient rooms, exam rooms, shower rooms, and equipment clearances as applicable to the project.
  • Provide an equivalent, signposted route when construction closes the usual route, and keep it free of carts, cords, debris, and barriers.
  • Document whether the project includes an addition or alters a primary-function area so the design team can evaluate related-route obligations.

7. Coordinate life safety, permits, and the AHJ

Life safety is a parallel workstream, not a final inspection item. CMS applies Life Safety Code and Health Care Facilities Code survey requirements to hospitals and CAHs. CMS federal certification context does not, by itself, identify the adopted state or local edition, permit sequence, or project-specific waiver.

Resolve code questions early

  1. Ask the AHJ and design professionals which fire, building, accessibility, mechanical, electrical, plumbing, energy, and health-care provisions are adopted for the site.
  2. Confirm the CMS requirements and current survey expectations that apply to the facility classification. CMS’s manual search result identified Revision 238, issued March 20, 2026, effective September 5, 2025; verify the current manual at project start.
  3. Map fire compartments, smoke barriers, rated doors and penetrations, sprinkler and alarm impairments, emergency power, medical-gas work, and temporary egress.
  4. Obtain permits and impairment approvals before affected work, and define fire-watch, notification, and restoration procedures where required by the AHJ.
  5. Track every AHJ comment, inspection, test, waiver, or equivalent-protection condition to a named owner and close-out date.

Never treat a generic life-safety list as a substitute for the project’s design review, permit documents, survey requirements, or AHJ approval.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

8. Prepare a controlled reopening and handoff

Do not return a renovated space to patient use because construction appears complete. Use a documented readiness review with infection prevention, facilities, clinical leadership, safety, environmental services, the contractor, and the project manager.

  1. Complete final cleaning, covered-debris removal, and the planned barrier-removal sequence. Inspect adjacent areas for dust migration.
  2. Verify HVAC operation, pressure relationships, humidity, filters, and commissioning results required for the space. Record date, instrument or test method, reviewer, and disposition of exceptions.
  3. Flush affected water systems as appropriate and complete water-management actions before connecting fixtures or admitting patients.
  4. Walk accessible routes and altered spaces with the design team; remove temporary obstructions and confirm signs, hardware, clearances, and alternate routes.
  5. Close permits, fire and life-safety inspections, utility tests, and AHJ conditions. Retain certificates, test reports, drawings, and approved changes.
  6. Obtain clinical and operational sign-off: equipment installed and tested, emergency supplies stocked, staff trained, waste and linen routes restored, and service continuity confirmed.
  7. List unresolved deficiencies with an owner, due date, interim protection, and approval for any limited use. Do not conceal an open risk in the turnover package.

9. Keep the checklist usable after turnover

Store the approved ICRA and revisions, drawings, permits, training records, inspection logs, HVAC and water results, cleaning records, surveillance findings, incident reports, and AHJ correspondence in one controlled project file. Feed lessons from the renovation into the hospital’s infection-control, emergency-management, maintenance, accessibility, and life-safety programs.

For facilities without internal ICRA expertise, a qualified healthcare-construction infection-prevention or ICRA consultant can help the team develop the assessment, controls, and monitoring plan. The consultant supplements—not replaces—the facility’s infection-prevention lead, licensed design professionals, state survey agency, and AHJ.
