How to Transfer Your VA Health Care to a New Clinic

Your current VA PACT is the first contact when moving VA care to a new clinic. Here are the steps to confirm your facility, contact details, records and continuity of care.

To transfer your VA health care to a new clinic, contact your current Patient Aligned Care Team (PACT). The U.S. Department of Veterans Affairs says your PACT works with a Traveling Veteran Coordinator to help transfer your care and establish an appointment at the new location. Update your address in your VA.gov profile, then confirm scheduling, prescriptions and any pending care with your team.

The practical process depends on what you mean by “transfer.” Moving to another state is a move-related transfer; changing your preferred primary-care facility may also be relevant even if you are not moving far. In either case, your current PACT is the right first contact for coordinating the next steps. National VA guidance does not promise a standard appointment timeline, require a particular transfer form or say that records transfer automatically, so confirm local arrangements with the teams involved.

Start with your current PACT

Tell your current Patient Aligned Care Team that you are moving or want to receive care at another VA facility. Give the team your destination and expected move date, and ask to be connected with the Traveling Veteran Coordinator. The VA’s Health Care Benefits Overview, Volume 1 (2026) identifies this as the route for transferring care, including when you move to another state.

The 2026 guide states: “To transfer your care to a new VA health care facility, contact your Patient Aligned Care Team (PACT). Your PACT will work with the Traveling Veteran Coordinator to assist in transferring your care and establishing an appointment at the new location.” The statement is from the U.S. Department of Veterans Affairs guide; it does not identify an individual speaker. The important practical point is that the national guidance directs you to your current team rather than to a universal transfer hotline or form.

When you contact your PACT, explain whether you are moving, changing the place where you prefer to receive primary care, or both. Share the new location and expected move date so the team can understand which facility you have in mind and help coordinate the transfer. Ask how to reach the Traveling Veteran Coordinator and whether there are local steps you should complete. The national source describes the coordinator’s role as assisting with transfer and appointment establishment, but it does not publish a universal phone number or specify a single process used at every facility.

Do this early enough to discuss care around the move, but do not assume that a particular amount of advance notice is required: the national guidance does not set one. Likewise, it does not guarantee how quickly the destination facility will schedule an appointment. Ask your current PACT what the local process and expected scheduling arrangements are, and confirm the appointment with the destination team when available.

Choose or change your preferred facility

You may choose a convenient VA facility as your preferred primary-care location. If you want to change it, discuss the request with your primary care provider. The provider coordinates the change with the destination facility’s administrative team, according to the VA’s Primary Care: New Patient Guide and the 2026 benefits overview.

This facility choice is useful to distinguish from simply updating your address. An address change updates the contact details in your VA.gov profile, but the guidance also describes a preferred-facility choice and a request coordinated by your primary care provider. In other words, do not rely on changing your profile address alone to make a clinic your preferred primary-care location. Tell your provider which facility you want and ask them to coordinate the request.

VA assigns a primary-care team and provider based on the main location you choose. That makes the preferred facility relevant to where your primary-care relationship is organized. If your concern is the assigned doctor rather than the facility, the VA says to raise a request with your team leader or the medical center’s patient advocate. A request about a provider is not the same question as requesting a different preferred facility; explain which change you want so your concern reaches the appropriate person.

For a move to another state, the PACT and Traveling Veteran Coordinator are the stated transfer route. For a change of preferred facility, the primary care provider coordinates with the facility’s administrative team. These can be related steps, but the guidance describes them in different ways. Contact your current PACT first and explain your situation; ask whether your provider should also coordinate a preferred-facility change. If your preferred location cannot provide care you need, VA says it will arrange care based on administrative eligibility and medical necessity. The source does not provide more specific eligibility tests or promise a particular alternative facility, so ask the VA team about your own circumstances.

Update your address and contact details

  1. Sign in to your VA.gov profile and open the address section.
  2. Update both your home and mailing address, then save the changes.
  3. Check directly with any VA department that may maintain separate contact information.

The VA’s Change Your Address in Your VA.gov Profile instructions say a profile address change updates health-care contact information used for prescriptions, appointment reminders, lab and test results, and communications from your medical center. Keeping those details current matters during a transition because the VA may use them to contact you about several parts of your health care.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Updating your profile is useful, but it does not replace contacting your PACT about the move and transfer. The two actions serve different purposes: the profile update changes contact information, while the PACT contact starts the coordination described in the transfer guidance. The VA also notes that some departments maintain separate records. If you deal with another VA department or know that it keeps separate contact details, check directly with it rather than assuming one profile change updates every record across the VA.

Review both the home and mailing address fields before you finish. If they are different, make sure each reflects where you want the relevant correspondence sent. The cited VA address page was updated December 22, 2025; follow its current profile instructions if the screen labels or process have changed. The VA address page does not specify a separate address-update deadline or additional address documents, so do not assume a requirement beyond the published instructions. If you are unsure whether a department has your new details, ask that department directly.

Keep records and ongoing care on track

You can review eligible records through VA’s online tools or request copies from the facility’s Release of Information office. The VA’s Review Medical Records Online page describes online access. For a formal copy, use the instructions in How to Get Your Medical Records From Your VA Health Facility. Online access and a formal copy request are separate ways to access records: use the online tools to review records available to eligible Veterans, or contact the records office for a copy request.

The VA states that requests for records stored in an online system may take up to 30 calendar days, while paper-record requests may take up to 60 calendar days. The VA notes that paper records are usually records created before 1998. These are stated processing windows for copies—not a transfer or appointment guarantee. They are not a promise that the new clinic appointment will occur within either period, and they do not establish how quickly one facility will coordinate your care with another.

If you receive care at a community-based outpatient clinic (CBOC) or an outpatient clinic (OPC), call first to ask whether that clinic holds the records or whether its parent facility’s records office handles the request. The clinic where you receive care may not itself be the office that processes a records request. Checking before submitting a request can help you identify the appropriate office; the national sources do not establish a single records office arrangement for every CBOC or OPC.

Do not assume that records transfer automatically just because you are changing facilities. The national sources describe online access and a way to request copies, but they do not establish an automatic-record-transfer rule. Ask your current PACT or the destination facility what information they can access and whether you should request records. If you need a formal copy, follow the facility’s Release of Information instructions and allow for the VA’s stated processing windows. If you are only trying to review what is available online, check VA’s online records tools instead of treating that as the same thing as a copy request.

Ask your PACT how to avoid a gap in care while the new appointment is being arranged. Confirm medication refills, pending tests, and whom to contact if you need care before the appointment. The transfer guidance says that the PACT and Traveling Veteran Coordinator assist with establishing an appointment, but it does not provide a standard timeline. It also does not give a universal interim-care procedure, refill schedule or contact number. Those are location- and situation-specific questions to raise with your team rather than details to infer from the national transfer description.

If the selected facility cannot provide needed care, the VA says it will arrange care based on administrative eligibility and medical necessity. This is a general statement, not a guarantee of a particular service, provider, location or appointment date. Explain what care you need and ask the VA team how the arrangement applies to you. Keep the question about immediate continuity separate from the longer-term question of where your preferred primary-care location should be.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

What to confirm with the teams

  • That your current PACT knows your destination and expected move date and has connected you with, or explained how to reach, the Traveling Veteran Coordinator.
  • Whether the request is a move-related transfer, a preferred-facility change, or both, and whether your primary care provider needs to coordinate a facility change with administration.
  • That the preferred facility and primary-care arrangements reflect where you want to receive care. If you want a different assigned doctor rather than a different facility, ask about the team leader or medical-center patient advocate route.
  • That your VA.gov profile has the correct home and mailing address, and that any relevant separate departmental records are updated directly.
  • How and when the destination facility expects to schedule your first appointment. There is no national timeline in the guidance, so ask for the local expectation.
  • How to manage prescriptions, pending tests and care needs during the transition, including whom to contact before the first appointment.
  • Where to request records if you need copies, especially if your current clinic is a CBOC or OPC and may not hold the records itself.
  • Whether online access is sufficient for what you need or whether you should request a formal copy from the facility’s Release of Information office.

Keep a note of the questions you still need answered and which VA team you contacted. This is a practical way to track the conversation, not a VA-required form or documentation step. The national guidance does not prescribe a transfer checklist, record organizer or special product. You do not need to buy a particular item to follow the official steps described here.

What the national guidance does not specify

The VA’s national guidance does not provide a universal Traveling Veteran Coordinator phone number, require a particular transfer form, or promise a standard appointment interval. Contact your PACT and destination facility for those local details. The guidance describes assistance with transferring care; it does not establish that records transfer automatically, so ask the facilities about records access and request copies when needed.

The sources also do not say that moving requires a new VA health-care application. Do not treat the transfer steps as evidence that a new application is required, or as proof that no other location-specific steps could apply; ask your PACT about your own situation. The reviewed guidance does not specify transfer costs or recommend a paid transfer service, and no particular physical product is needed for the official steps. The distinction that matters is between the VA’s stated coordination process and local details—such as exact contact information, scheduling and records handling—that your facilities must confirm.

Do I need to contact a Traveling Veteran Coordinator myself?

Start with your current PACT and ask to be connected with the Traveling Veteran Coordinator or to be told how to reach them. The national guidance describes the PACT as the first contact and does not list a universal coordinator phone number.

Can I choose a different VA facility if I am not moving to another state?

VA says you may choose a convenient facility as your preferred primary-care location. Discuss a change with your primary care provider, who coordinates the request with the destination facility’s administrative team.

What if I want a different doctor, not a different clinic?

The VA says to raise a request about an assigned primary-care doctor with your team leader or the medical center’s patient advocate. A provider request is distinct from changing the preferred facility.

Does updating my VA.gov address transfer my care?

No. The VA says a profile address change updates health-care contact information, including prescription and appointment communications, but it does not replace contacting your PACT to coordinate a transfer.

Will my medical records transfer automatically to the new clinic?

The national guidance does not establish that records transfer automatically. Check online access to eligible records, and ask the facilities whether you need to request copies from a Release of Information office.

Call first and ask whether the clinic holds the records or whether the parent VA health facility’s records office handles requests. Arrangements can vary by clinic.

What should I ask about while waiting for the new appointment?

Ask your PACT about medication refills, pending tests and whom to contact if you need care before the appointment. The national source does not provide one universal interim-care procedure.

Do I need a new VA health-care application or a paid transfer service?

The sources do not say that moving requires a new application, and they do not recommend a paid transfer service. Contact your PACT for situation-specific steps; no special physical product is identified as necessary.

The Bottom Line

Begin with your current PACT, share your destination and expected move date, and ask about the Traveling Veteran Coordinator and first appointment. Update both addresses in your VA.gov profile, confirm any separate departmental contact records, and ask about prescriptions, pending tests and interim care. For records, use VA’s online tools or contact the appropriate Release of Information office; do not assume they transfer automatically. Local scheduling and records arrangements vary, and the national guidance sets no appointment deadline.
