If an interchange condition presents an immediate danger, use your local emergency or urgent road-hazard channel first—not a routine web form. For a non-urgent confusing sign, missing guidance, damaged sign, or recurring lane-assignment problem, report it to the agency that owns the road: usually the state department of transportation (DOT) for a state route, or a city or county public-works/highway department for a locally maintained road. Give the agency the route, direction, interchange and ramp, nearest landmark, exact sign or hazard, and the driving decision it affected. Never stop, reverse, or make a sudden lane change to gather details; reach a safe place and report from there.
Choose the right reporting route before you describe the problem
The interchange’s state and road ownership determine the correct contact. There is no single national phone number or form for every new interchange, and a standard online form may not be monitored continuously. Use this decision order:

| Situation | Best starting route | What to verify |
|---|---|---|
| Immediate threat to life or traffic safety | Emergency services or the road authority’s urgent hazard line | Whether the jurisdiction requires 911, has a 24/7 transportation-management center, and whether its web form is monitored. |
| Damaged or missing sign, confusing guidance, or recurring lane problem with no immediate danger | State DOT for a state-maintained route; city or county road department for a local road | Who owns the road, which report category covers signs or maintenance, required location fields, and whether photos are accepted. |
| Checking current conditions or considering a 511 report | Your state’s 511 service, only if its instructions support hazard reporting | Whether 511 only distributes travel information or also accepts hazard reports. |
For example, Indiana’s INDOT4U guidance distinguishes state responsibilities from local roads and explains that submitted concerns are reviewed and assigned for investigation; timing depends on the issue and available resources. California’s transportation FAQs likewise separate maintenance requests, such as broken or missing signs and signals, from traffic or construction inquiries. Those examples show why identifying the owner matters, but they are not a substitute for checking the agency in the interchange’s state or city.
If the condition is dangerous right now
Use emergency services for an immediate threat
Call 911 or the equivalent emergency number when debris, a dark signal, a vehicle-trapping lane pattern, a collapsed or displaced sign, flooding, a wrong-way risk, or another condition creates an immediate danger. North Carolina’s roadway-safety guidance tells motorists to notify local law enforcement immediately when roadways unexpectedly become unsafe. Follow the local dispatcher’s instructions and do not attempt to inspect the hazard yourself.

Check for a dedicated urgent road-hazard channel
Some DOTs operate a separate urgent line or transportation-management center. Delaware DOT lists a 24/7/365 Transportation Management Center contact for urgent reports such as a malfunctioning signal, damaged sign, debris, or an improperly set-up work zone. Texas DOT directs people to call 911 for immediate hazards such as dangerous debris and expressly warns that its online report form is not monitored 24/7. These are jurisdiction-specific examples: verify the local rule before relying on a form, email, or social-media account.
What to say during an urgent call
- State that you are reporting an active roadway hazard and whether traffic is moving through it now.
- Give the highway or route number, travel direction, interchange or exit name/number, ramp or lane, and nearest cross street, mile marker, or visible landmark.
- Describe the observable condition and the immediate conflict: for example, a signal is dark, debris blocks a lane, or a work-zone channelization directs vehicles into an unexpected path.
- Tell the dispatcher where you are calling from only if it is safe to do so; do not use a handheld phone while driving.
For a non-urgent sign or wayfinding problem
Identify who maintains the road
State-maintained interstate, U.S., and state routes generally go to the state DOT’s customer-request system. A city or county road, including a locally controlled ramp or street connection, generally goes to that local public-works or highway department. INDOT4U specifically directs reports for local roads to the appropriate city or county department. If ownership is unclear, check the interchange’s official project or road-agency website, the route signage, or the jurisdiction’s general transportation contact; explain that ownership is uncertain so the recipient can redirect the request.

Choose a category such as damaged sign, missing sign, signal, pavement marking, work-zone setup, or traffic operation when the form offers one. A confusing sign does not necessarily mean the sign is damaged: select the closest wayfinding, signing, traffic-control, or general maintenance category and explain the decision problem in the narrative. California’s FAQs illustrate the distinction between maintenance requests and separate traffic or construction inquiries.
Do not treat a single complaint as an automatic fix
Agencies normally review a report, assign it for investigation, and decide what action is warranted. INDOT states that response timing depends on the issue and available resources. Do not promise yourself or another driver a specific repair date, replacement, or sign change.

Describe the decision point, not just the feeling of confusion
Explain what a driver sees in sequence
Write the approach in order: what appears first, what the advance sign says, where the lane arrows or pavement markings change, and how much distance remains before the split or gore. Identify the destination you intended and the destination each lane appeared to serve. A report such as “the interchange is confusing” gives an investigator little to test; a report such as “westbound traffic sees two arrows for the same destination, then the right lane becomes exit-only after the last advance sign” identifies a specific signing and marking sequence.
Separate observation from diagnosis
State facts you personally observed instead of declaring that the sign violates the Manual on Uniform Traffic Control Devices or another standard. Whether a sign complies depends on its exact design, placement, current applicable standards, and the agency’s determination. You can say that an arrow appears to point toward a different ramp, that a destination is absent from advance guidance, or that lane markings contradict the panel; let the road authority determine the technical cause.
Explain why the timing matters
The Federal Highway Administration describes interchange navigation as time-sensitive: an unfamiliar driver must read signs, observe pavement markings, and determine a route before reaching the gore point. On a limited-access facility, a wrong choice may be difficult to correct. Mention a short distance between decisions, multiple required lane changes, a late-revealed destination, or a lack of advance guidance when you observed one. These details help distinguish a sign-legibility issue from a broader sequence or geometry problem.
Photo, video, and personal safety
Only collect images from a safe stopped location
Attach a photo when the reporting system permits it, but take it only after parking legally in a safe place away from moving lanes. Do not stop on a shoulder, ramp gore, merge area, or live lane merely to capture a sign. Never reverse, make a sudden lane change, or cross traffic to recover a missed turn. If you cannot safely photograph the condition, provide a careful written description instead.

Protect the report’s usefulness
- Photograph the sign, signal, marking, or obstruction from a lawful location that shows its relationship to the roadway.
- Include enough approach context for the agency to identify the decision point, but do not put yourself or other road users at risk.
- Record the approximate time because temporary work-zone devices and signal failures can change.
- Do not edit an image in a way that removes arrows, lane lines, destinations, or other evidence.
How 511 fits—and where it does not
511 is primarily a travel-information service, not a universal emergency-reporting channel. Some state systems add a hazard-reporting function. Wisconsin’s 511 phone system, for example, includes an audio option to report a road hazard. Other systems may only provide current conditions or route information. Read the local 511 instructions; if they do not explicitly accept the type of report you have, use the road owner’s customer-request channel or the urgent route described by that jurisdiction. Never use 511 instead of emergency services for an active threat.
Reports involving a new interchange or an active work zone
Identify whether the condition is permanent or temporary
State whether the interchange is newly opened, still under construction, undergoing commissioning, or operating under a temporary traffic-control plan. Note cones, drums, portable signs, temporary signals, lane shifts, or overnight changes. A missing permanent sign and a sign temporarily covered during construction may require different teams, even when they are at the same ramp.
Report the route that drivers are actually using
New interchanges often connect existing roads to a limited-access facility, so include both the mainline route and the local street or ramp name. If a detour or temporary closure sends traffic through an unfamiliar movement, describe the sequence from the first advance sign to the merge. Do not assume that a construction contractor, rather than the public road agency, receives public safety reports; send the concern to the road authority and identify the project or work zone if you know it.
Flag an immediate work-zone setup problem urgently
An improperly set-up work zone, displaced barricade, exposed equipment, or sudden lane conflict can be an active hazard. Use the local urgent channel or emergency services when appropriate, then provide the detailed location and description through the agency’s requested follow-up route if instructed.
What happens after you submit
Save the confirmation number, date, and the exact wording you submitted. Agencies may request clarification, inspect the location, route the issue to a local owner, or determine that a signing, marking, signal, or work-zone adjustment is needed. INDOT explains that reports are reviewed and assigned for investigation, with timing dependent on the issue and resources. Because no universal response time is established, avoid repeated duplicate submissions unless the condition has become more dangerous or the agency asks for additional information.

If the hazard remains active, worsens, or changes from non-urgent to immediate, make a new urgent report rather than waiting for a routine ticket. Describe what has changed and reference the earlier report number if you have one.
Why interchange confusion is worth reporting
FHWA’s research on complex interchanges identifies several contributors to navigation difficulty and driver workload: unfamiliarity, surprise, information overload, multiple lane changes in a short distance, short weaving distances, and insufficient advance guidance. These factors can interact with sign layout, pavement markings, decision timing, and unexpected maneuvers. Your report is more actionable when it identifies which of those observable circumstances occurred at the specific location, without claiming a particular engineering violation.
As FHWA puts it in the foreword to Driver Expectations When Navigating Complex Interchanges (FHWA-HRT-13-048, October 2013), “Clear navigation signage is needed to guide drivers and reduce errors.” Read the agency’s discussion of driver expectations and complex-interchange signing at FHWA’s executive summary and its stakeholder-feedback findings in Chapter 3 of FHWA-HRT-14-069.
