There is no nationally fixed price for an English neighbourhood health centre (NHC). A defensible budget separates one-time development, premises capital, recurring operations and income, then prices the actual building, workforce, service scope and expected use in a local model. A published Community Health Hub comparator gives useful order-of-magnitude context, but it is not an NHS tariff or an approved English project budget.
What a neighbourhood health centre is—and why the cost varies
In England, an NHC is a physical base for a wider neighbourhood health service. The intended model brings general practice together with community services, local-authority provision, civil-society organisations and voluntary-sector partners. It is not a nationally standardised clinic with one room schedule, staffing establishment or price.

Delivery is expected to combine repurposed estate with new facilities. The framework also says neighbourhood services should be funded by actively reallocating existing resources, including a gradual shift from acute care, rather than assuming a universal new funding stream.
That policy direction matters to a cost plan. A centre may be a refurbishment inside an existing health or public building, a major extension, or a digitally enabled purpose-built facility. The catchment population, opening hours, clinical mix, co-location partners, workforce availability and utilisation profile can change both capital and annual costs more than the centre’s name.

What are the start-up and annual operating costs?
Use four linked schedules rather than one headline number.
| Cost schedule | What it covers | Typical evidence needed |
|---|---|---|
| Development and start-up | Feasibility, business case, community-needs work, pathway mapping, design coordination, recruitment, training and backfill, initial furniture and clinical equipment, IT integration and commissioning. | Project brief, staff plan, equipment list, implementation timetable and supplier quotations or approved cost norms. |
| Premises and capital | Land where applicable, construction or refurbishment, building services, accessibility works, professional fees, digital infrastructure, contingency, financing and lifecycle allowances. | Condition surveys, measured area, outline design, statutory requirements, procurement route and financing assumptions. |
| Recurring operations | Clinical and non-clinical staff, occupancy or lease costs, utilities, maintenance, medical supplies, connectivity and IT support, administration, cleaning, security, training and service management. | Workforce establishment, contracts, opening hours, activity forecast, facilities-management plan and replacement cycles. |
| Income and funding | Commissioning allocations, service payments or reimbursements where applicable, local-authority or partner contributions, grants, donations and other expected revenue. | Signed or proposed agreements, payer rules, grant conditions and a schedule distinguishing secured, probable and speculative income. |
Keep cash expenditure separate from opportunity cost. For example, staff time supplied by an existing organisation may not create a new invoice, but it still consumes capacity and should be visible in the model.
Two delivery routes in the 10 Year Capital Plan
Upgrade or repurpose existing estate
The first route is to adapt an underused or poorly configured building. It can support faster co-location, reuse existing utilities and avoid buying land, but the apparent saving can disappear if surveys reveal asbestos, structural defects, inadequate ventilation, accessibility problems or insufficient electrical and data capacity. Model temporary decanting, phased works, clinical disruption and the cost of bringing old systems up to current standards.
Build a digitally enabled facility
The second route is a purpose-built centre where existing estate cannot provide the required scale, condition or configuration. A new building can set an efficient room layout, integrated digital infrastructure and modern energy performance from the start. It also carries land, planning, professional-fee, financing and programme risks, and may take longer before services open.
The capital plan identifies both routes but does not publish one universal cost per centre. Compare them against the same service brief and whole-life period rather than comparing a refurbishment estimate with an unscoped new-build allowance.

How to build a defensible cost model
1. Define the service before pricing the building
Write down the services that will be delivered on site and remotely: for example, general practice, community nursing, rehabilitation, diagnostics, social prescribing, mental-health support or voluntary-sector advice. Set the catchment population, referral rules, opening days, evening and weekend coverage, expected annual contacts and the proportion of activity delivered digitally.

2. Set the physical and workforce brief
Record the proposed area and condition of the building, consultation and treatment rooms, waiting and reception space, storage, staff facilities, accessible toilets, circulation, infection-control requirements and partner accommodation. List each role, whole-time equivalent, grade, employer, shift pattern, recruitment lead time, training requirement and backfill assumption. Workforce scarcity can alter both the opening date and the recurring cost.
3. Build an ingredients schedule
An ingredients-based method identifies every resource and assigns a quantity and price. WHO-CHOICE uses this logic for intervention costing, but its published service-delivery estimates use 2010 base-year inputs; those historical values are methodological references, not current English prices.
For every line, record:
- quantity and unit of measure;
- unit price and price year;
- geography and source;
- payer and whether the value is cash or opportunity cost;
- whether the figure is observed, normatively assumed or forecast;
- replacement, inflation and lifecycle assumptions.
4. Separate one-time and recurring commitments
Recruitment and induction may be start-up costs, while salaries after opening belong in operations. A server installation may be capital, while connectivity and support are recurring. Furniture and a medical examination couch belong on the initial equipment schedule; replacement dates and maintenance belong in the lifecycle and operating schedules. Make these classifications explicit so that a low first-year estimate cannot hide future obligations.
5. Run scenarios, not a false-precision point estimate
Prepare at least base, low and high cases. Vary uncertain inputs such as building condition, construction inflation, staff vacancies, utilisation, energy prices, opening hours, digital uptake and the timing of partner contributions. Present the assumptions beside each scenario and show which variables drive the result.

Comparator figures—and how not to misuse them
A 2025 Milken Institute proposal for Community Health Hubs reports around £2 million in total start-up cost, approximately £200,000 per hub, and about £200,000 in annual operating cost per hub. Its setup inputs include training and backfill, initial equipment, project coordination, IT integration and population assessment; its operating estimate includes site setup, maintenance, staffing, medical supplies and administration.
Those amounts are comparator values for the report’s proposed hub model. They are not an NHS NHC tariff, an approved English budget or a forecast for a particular locality. The report’s service mix, estate, workforce, catchment, currency assumptions and costing boundaries may differ from an English scheme. Treat the figures as a prompt to test scale and completeness, not as a price to copy into a business case. The report presents total and per-hub views at different aggregation levels; do not combine them arithmetically without reconstructing its underlying schedule.
The UK Government’s capital plan identifies around £200 million of public capital for the English NHC approach over the Spending Review period to 2030, alongside public-private partnership funding. That is a programme-level envelope, not a per-centre allocation. The plan says 20% of new builds are to be funded from public capital, with the remainder through public-private partnerships; the cost of those partnerships depends on the national project pipeline and financing structure.
How much does it cost per patient visit?
No current universal English NHC cost-per-visit figure has been established. Calculate it locally only after fixing the service scope, price year, geography and cost boundary.
- Assign direct clinical labour, consumables and other directly attributable costs to each service line.
- Allocate shared staff, reception, estates, IT, cleaning, management and administration using a transparent driver such as clinical hours, rooms used, floor area or activity.
- Decide whether to include capital depreciation, financing, opportunity cost and donated resources; show both an operating view and a full-cost view where useful.
- Divide the selected annual cost by completed visits or contacts, not merely booked appointments, and report the utilisation assumption.
- Test the result against changes in staffing productivity, appointment length, DNA rates, digital contacts and building occupancy.
Capital Link and the National Association of Community Health Centers describe service-line costing approaches that examine these operational drivers. Unit costs are not comparable when one centre includes diagnostics, outreach and partner services while another counts only general-practice appointments.

Should we refurbish an existing building or build a new centre?
| Decision factor | Repurpose or refurbish | Purpose-built facility |
|---|---|---|
| Upfront capital | Often avoids land purchase and some structural work, but hidden defects and compliance upgrades can be material. | Requires land, design, construction and professional fees, with a larger early funding requirement. |
| Speed and disruption | May open sooner, but phased works, decanting and live-clinical-site risks can extend the programme. | Usually longer planning and construction, with less disruption to existing clinical activity once occupied. |
| Capacity and layout | Bound by existing structure, circulation and services; co-location may be partial. | Allows the room schedule, accessibility, infection control and partner accommodation to be designed together. |
| Digital capability | Retrofitted cabling, power, connectivity and security may be expensive or technically constrained. | Digital infrastructure can be integrated into the core design and commissioning plan. |
| Lifecycle and energy | Older plant can mean higher maintenance and energy exposure, even after refurbishment. | Modern systems may reduce operational risk, but maintenance and replacement costs still need long-term allowances. |
| Finance and ownership | May use an existing public asset or lease; confirm ownership, tenure and partner responsibilities. | May involve public capital or a public-private partnership, with financing, indexation and handback terms that affect whole-life cost. |
Use a whole-life appraisal with the same clinical brief, opening date and evaluation period for both options. Include residual value, planned renewals, maintenance and the value of keeping services running during works.
Funding, approvals and local accountability
Integrated care boards (ICBs) are expected to lead locality proposals and identify neighbourhood-health funding through active prioritisation. The capital plan refers to separate NHS England guidance on centre archetypes, funding methodology and approvals; a project should use that guidance when available rather than treating a generic estimate as approval evidence.
Local authorities retain accountability and funding responsibilities for their own services. Partner contributions should therefore be documented by service, amount, duration and decision-maker. National payment flows may change, and prevention or community payment approaches may be tested, but there is not yet a completed universal formula that tells every centre what it will receive.

The Department of Health and Social Care and NHS England state: “These plans should, however, demonstrate that neighbourhood health will be funded by rebalancing existing resources rather than relying on new funding, while recognising that the scale and pace of the shift will be determined locally.” Build a funding plan that identifies the resources being reallocated, the services released or reduced elsewhere, and any transition funding needed while activity moves.
Common modelling failures and their fixes
- Using a national target as a unit price: the number of planned centres and a programme capital envelope do not reveal the cost of one site. Build a local scope and estate option.
- Mixing start-up and annual costs: keep implementation, capital and steady-state operations in separate schedules, then show the cash requirement by year.
- Ignoring backfill and mobilisation: training existing staff can create temporary agency, overtime or capacity costs. Price those explicitly.
- Under-scoping digital and compliance work: include connectivity, cyber controls, clinical systems integration, accessibility, ventilation, fire safety and commissioning tests.
- Counting revenue as guaranteed: label each contribution as secured, conditional or aspirational and model the effect if it is delayed.
- Comparing incomparable unit costs: align service mix, geography, price year, utilisation, capital treatment and allocation rules before drawing conclusions.
- Leaving out lifecycle expenditure: provide renewal dates for building plant, furniture, clinical equipment and IT, not only the opening-day purchase.
A practical approval pack
A decision-ready submission should contain:
- a clearly bounded service and population brief;
- an options appraisal for repurposing and new build;
- condition, access, utilities and digital surveys for the preferred estate;
- a workforce establishment with recruitment, training and backfill costs;
- capital, start-up, operating and income schedules;
- base, low and high scenarios with sensitivity drivers;
- a procurement, planning, clinical-governance and commissioning timetable;
- partner responsibilities, ownership and funding agreements;
- measures for activity, utilisation, quality, outcomes and cost per service line;
- lifecycle, maintenance, replacement and exit assumptions.
For budgeting structure, the US Health Resources and Services Administration’s health-centre manual is a useful illustrative example because it separates projected costs, the federal award share and other expected revenues such as state or local funds, fees and third-party reimbursements. Its requirements apply to the US Federal Health Center Program, not to English NHS centres, so use the structure—not the compliance rules—as a general planning reference.
Bottom line for a construction brief
Price the building and the service together. Start with the local clinical brief, test refurbishment against a digitally enabled new build, and maintain transparent schedules for mobilisation, capital, annual operations and income. Use published comparator amounts only as clearly labelled context. The final English NHC budget must be supported by the ICB’s service assumptions, estate evidence, workforce plan, utilisation forecast, financing route and the current NHS England approval methodology.
