Choose the consultant who can prove comparable hospital experience, put the proposed healthcare-experienced people on your job, explain an independent estimating method, and make assumptions, exclusions, risk and uncertainty visible. A low fee is not evidence of value. Use written minimum requirements and weighted scoring, then verify the highest-scoring team with references and sample deliverables.
Hospital estimates must account for more than the permanent building. Infection-control measures, air-quality controls, utility interruptions, noise, vibration, phasing and access can affect both cost and schedule, especially on a live campus. Your brief and procurement rules should therefore be set before you compare proposals.

What a hospital cost consultant is responsible for
A construction cost consultant—also called a cost engineer or quantity surveyor in some markets—turns the project brief and design information into an independently reasoned view of probable cost. Depending on your appointment, the consultant may also monitor the architect/engineer’s estimate, maintain a cost plan, test options, track changes, support business-case approvals and advise during procurement and construction.
The appointment is most useful when it continues through the project stages. Early estimates guide affordability and scope. Later estimates test whether design development, procurement decisions and changes remain within the approved funding. A consultant appointed only to produce a single headline number may not provide the controls needed to manage a complex hospital project.

U.S. Department of Health and Human Services guidance recommends a highly qualified cost engineer or estimator to help monitor architect/engineer estimates on major capital projects. It describes the desired cost consultant as “fair, impartial, objective, and effective” when evaluating the reasonableness of the design team’s estimate. That is federal guidance for HHS facilities, not a universal contract rule, but it is a useful standard for defining independence and quality. HHS Facilities Manual
Define the assignment before requesting proposals
Consultants cannot be compared fairly if each interprets the assignment differently. Prepare a short scope document that gives every bidder the same information and asks for the same deliverables.
Describe the project and delivery context
- Project type: new hospital, addition, renovation, replacement facility or work across an existing campus.
- Approximate area, departments, clinical functions, bed count if established, site constraints and known enabling works.
- Current design stage, target dates, funding or business-case milestones and anticipated procurement route.
- Whether construction will occur beside occupied clinical areas and which facilities must remain operational.
- Geography, currency, required price base and any known escalation or market constraints.
State what the estimate must include
Require bidders to identify treatment of medical equipment, information technology, commissioning, decanting, temporary services, enabling works, professional fees, permits, owner costs, contingencies and taxes. These items cannot be inferred from a headline construction cost. If an item is outside the appointment, the proposal should say so explicitly and identify who owns the information.
Set stage gates and decisions
List the dates when an estimate, cost plan, option appraisal, funding submission or tender advice is required. Ask how quickly the consultant will respond to design changes and what information is needed for each update. A stage-gate schedule makes capacity and turnaround promises testable rather than aspirational.
Look for genuinely comparable hospital experience
“Healthcare experience” should mean work similar enough to reveal the risks in your project, not a general list of public-sector buildings. Ask for named projects and the people who actually delivered them.
Test comparability
- New build, expansion, refurbishment or phased work on a live campus.
- Clinical complexity, such as diagnostic, surgical, inpatient, emergency or highly serviced areas, where relevant to your brief.
- Project size, location, market conditions and delivery method.
- Experience coordinating with clinical operations, facilities management, infection prevention and design disciplines.
- Evidence of managing utility shutdowns, temporary routes, decanting, noisy or vibration-producing work and constrained access where those conditions apply.
Ask for at least two references who can discuss estimate quality, change control, collaboration and performance after award—not only whether the consultant was pleasant during procurement. Fraser Health describes prequalification as a way to check qualifications, experience and references and to assess a supplier’s awareness of healthcare project requirements. Its business-opportunity material also illustrates the use of weighted criteria rather than automatically selecting the lowest bid. Fraser Health business opportunities

Request useful evidence, not a logo list
For each proposed comparable project, request the consultant’s role, services, project stage, approximate scale, delivery route, named staff and a short description of the estimate outputs. A project list without this context does not show whether the proposed team can perform your assignment.

Large firms often win work with a senior team and deliver it with different staff. Evaluate the named lead and supporting disciplines, not only the organization’s brochure.
- Require a one-page résumé for the project director, lead estimator and any specialist cost managers.
- Record each person’s hospital-project experience, role and availability during every required stage.
- Ask who will prepare the estimate, who will review it and who has authority to approve changes to the cost plan.
- Confirm relevant cost-engineering or quantity-surveying qualifications and membership requirements applicable in your jurisdiction.
- Obtain a written substitution procedure. Replacement staff should meet the same minimum requirements and require your approval.
Ask candidates to explain how they will maintain continuity if the project extends, the design is re-scoped or procurement is delayed. Capacity matters as much as résumé quality: a consultant who cannot attend design coordination meetings or turn around a change analysis when needed will not protect the budget.
Check independence and conflicts of interest
The consultant should be able to challenge the design team’s estimate without a commercial incentive to defend it. Request a declaration of current and potential conflicts, including design, contractor, supplier and development relationships connected with the project.
Ask how the team will independently reconcile quantities, rates, scope and allowances with the brief and design. An independent review should identify omissions and optimistic assumptions respectfully, with an audit trail showing what changed and why. Independence does not mean working in isolation: the consultant must coordinate with the architect, engineers, project manager, facilities representatives, clinicians and contractor while retaining an objective cost opinion.

Minimum contents of a basis-of-estimate report
| Item | What to require |
|---|---|
| Project snapshot | Brief, design issue or drawing list, measurement date and the scope assumed at that stage. |
| Cost structure | Work breakdown or elemental structure that lets you see building, services, site, enabling and other agreed categories. |
| Source data | Rate sources, project comparables, data age, market adjustments and any supplier or contractor quotations. |
| Location and price basis | Geography, currency, base date, inflation or escalation treatment and stated market assumptions. |
| Inclusions and exclusions | Explicit treatment of equipment, IT, commissioning, temporary works, decanting, professional fees, owner costs, taxes and other scope boundaries. |
| Assumptions and tolerances | Design, quantities, productivity, procurement and programme assumptions, plus the tolerance or accuracy range appropriate to the stage. |
| Risk and uncertainty | Base estimate, quantified uncertainty, discrete risk allowances, risk register links, sensitivities and the resulting range or confidence information. |
| Review and sign-off | Internal checking, independent assurance, unresolved issues, decision owners and the consultant’s sign-off. |
| Uncertainty-reduction plan | The information, surveys, design decisions or market tests that would most improve the next estimate. |
Distinguish base cost, uncertainty and risk
Ask the consultant to show the base estimate separately from estimating uncertainty and discrete project risks. For example, incomplete room data may create estimating uncertainty, while a known utility diversion may be a discrete risk. Combining both in an unexplained contingency makes it difficult to decide whether a design decision, survey or mitigation action will reduce the allowance.
The consultant should maintain a dated risk register, identify owners and test sensitivities. Request a range or confidence view appropriate to the evidence available; do not demand false precision from an immature design. Each update should show movement from the previous estimate and explain whether the cause was scope, quantity, rate, escalation, risk or correction.
Test hospital-operational risk capability
Construction beside patient care creates consequences that a generic commercial estimator may overlook. The Joint Commission identifies air quality, infection control, utilities, noise, vibration and other hazards in pre-construction risk assessment for planned or unplanned construction and renovation. It says organizations need a risk-assessment process but does not prescribe one particular method. Its standards FAQ was last updated April 21, 2026. The Joint Commission standards FAQ

- How will infection-prevention requirements and protective measures be translated into cost and programme allowances?
- How will temporary air handling, filtration, monitoring, barriers and pressure relationships be treated where required by the project’s infection-control plan?
- How will planned utility interruptions, temporary services, redundancy and shutdown windows be costed?
- How will noise and vibration limits affect methods, working hours, monitoring and potential rework?
- How will phasing, access, logistics, hoarding, staff and patient routes, security and material storage be reflected?
- How will unforeseen conditions in an existing building be recorded, priced and escalated for a decision?
Do not ask the cost consultant to replace your infection-prevention or facilities specialists. Ask how cost and schedule information will be coordinated with those specialists and how unresolved operational constraints will appear in the estimate’s assumptions and risk register.
Compare proposals with a written weighted method
Set minimum thresholds before proposals arrive. A bidder who lacks comparable hospital experience or cannot name the delivery team should not win by offering a low fee. Then score every compliant proposal against the same evidence.
| Criterion | Evidence to score | Suggested treatment |
|---|---|---|
| Healthcare portfolio | Comparable projects, roles, references and live-campus experience | Use a minimum pass requirement plus a weighted score. |
| People and qualifications | Named staff, relevant credentials, availability and substitution controls | Score the actual team, not the firm’s total headcount. |
| Independence | Conflict declaration, review method and ability to challenge the design estimate | Require a satisfactory response before awarding points. |
| Method and estimate basis | Breakdown, data sources, price basis, scope controls and reconciliation process | Score against a sample basis-of-estimate outline. |
| Risk and uncertainty | Risk register, ranges, sensitivities, allowances and update process | Give substantial weight because hospital unknowns can be consequential. |
| Operational coordination | Infection control, utilities, phasing, access, noise and vibration approach | Assess answers against your actual campus constraints. |
| Deliverables and assurance | Stage reports, options, change tracking, review and leadership communication | Score a sample report and proposed approval workflow. |
| Capacity and working relationship | Response times, meeting attendance, interfaces and continuity | Test with a scenario or interview question. |
| Commercial proposal | Fee basis, reimbursables, boundaries and change control | Score value and transparency; do not rank on fee alone. |
Document the evidence behind each score and keep the same clarification questions for all bidders. Fraser Health’s stated use of weighted criteria and its warning that selection is not necessarily the lowest bid provide a practical precedent; adapt the weights and thresholds to your owner’s governance and procurement rules. Ask for sample deliverables before appointment
A redacted sample can reveal more than a polished presentation. Request a basis-of-estimate report, cost-plan page, risk-and-opportunity register, change log and an executive summary suitable for a funding or steering-group decision. Check whether the documents show assumptions, exclusions, sources, dates, review status and unresolved questions.
Stage-by-stage outputs to specify
- Brief and concept: affordability range, cost drivers, scope gaps, benchmark basis and early risk register.
- Design development: elemental or work-breakdown estimate, option comparisons, updated quantities, escalation and uncertainty treatment.
- Pre-tender: reconciliation to the latest design, procurement assumptions, tender allowances, risk exposure and advice on packaging.
- Tender and award: review of bids or market feedback, normalization of qualifications and explanation of variance from the estimate.
- Construction: change assessments, forecast-at-completion, contingency drawdown, trend reporting and advice on claims or emerging risks within the appointment.
HHS facilities guidance says the architect/engineer should provide an estimate at each scheduled design submission with a narrative explaining the methodology, and recommends a cost consultant to help monitor those estimates on major capital projects. Treat that as an example of a useful stage-gate requirement, not as a contract obligation outside federal HHS work. HHS Facilities Manual
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone.Use benchmarks without creating false confidence
Benchmarks are useful only when definitions match. NHS Healthcare Premises Cost Guides provide department-level building and engineering-services cost-per-square-metre material, but a rate must be checked for edition, scope, location and price basis before it is applied. The available publication page does not establish a single current rate for your project, so do not quote one from a search snippet. NHS Healthcare Premises Cost Guides
Ask the consultant to normalize any comparison for area measurement, department mix, specification, services intensity, site conditions, procurement route, geography, market date, inflation and included or excluded costs. A hospital with a high proportion of theatres or imaging areas cannot be sensibly compared with a general office-like area using one blended rate.
Abu Dhabi Department of Health guidance notes that capital and recurrent costs should both be considered early and that different costing methods and included elements can make healthcare comparisons unreliable. The guidance is jurisdiction-specific, but the discipline is broadly useful: agree definitions before comparing numbers. Department of Health, Abu Dhabi costing guidance

Confirm procurement and approval rules
Selection procedures depend on the owner, funding source and jurisdiction. NHS England’s capital guidance for 2026/27–2029/30 addresses business-case and approval arrangements for NHS trusts and foundation trusts in England, including thresholds and expectations around modern methods of construction. HHS construction guidance addresses U.S. federal facilities. Neither should be copied automatically for a different owner or country. NHS England capital guidance 2026/27–2029/30

Before issuing a request for proposals, confirm applicable rules for competition, conflicts, evaluation records, delegation of authority, public disclosure, insurance, data security, accessibility and contract approval. Put any mandatory forms or thresholds in the procurement documents so bidders price and staff the same obligation.
Interview finalists and check references
Use the same questions for each finalist and ask the proposed lead to answer, not only a business-development representative.
- Show us how you would separate base cost, uncertainty and discrete risk on this project.
- What information would you need in the first 30 days, and what would you refuse to assume without documenting it?
- Describe a hospital estimate that changed materially. What caused the change and how did you report it?
- How have you priced temporary services, utility shutdowns, infection-control measures, noise, vibration or constrained access?
- How will you challenge an optimistic design estimate while keeping the team aligned?
- Who will attend design and clinical coordination meetings, and what is the response time for an urgent change?
- Which proposed staff completed the comparable projects, and can we speak with the owner’s representative?
Ask references whether reports arrived when promised, whether exclusions were clear, whether the consultant identified cost risk early, and whether the named team remained involved. A reference who can discuss only tender price is not enough; you need evidence from design, approvals and delivery stages relevant to your appointment.

Make the fee proposal transparent
Require a fee schedule by stage and role, with assumptions for meeting frequency, site visits, estimate updates, travel, reimbursables, software or data costs and additional services. Define what constitutes a client-driven change, a scope change, a re-baselining event and an emergency request.
Compare the commercial proposals after confirming that scope is equivalent. A very low fee may omit risk analysis, site coordination, independent review or later-stage support. A higher fee may be justified by greater staffing, more frequent updates or live-campus complexity, but only if those services are explicit and measurable.
Include deliverable acceptance criteria, invoice milestones, ownership of working files, confidentiality, insurance, liability limits, conflict updates, records retention and termination or transition assistance. Require prompt notice when an assumption is no longer valid rather than allowing silent erosion of scope.
A practical selection sequence
- Confirm governance: identify the owner, funding route, decision authority, procurement rules and required approvals.
- Write the scope: describe project stage, hospital operations, required services, inclusions, exclusions, outputs and dates.
- Set thresholds and weights: approve minimum healthcare experience, named-team requirements, independence checks and scoring criteria before solicitation.
- Shortlist evidence-based candidates: verify comparable projects, references, qualifications and capacity before inviting a full proposal.
- Issue identical information: provide the same brief, drawings, programme, cost definitions, pricing date and questions to every bidder.
- Evaluate the people and method: score the proposed team, basis of estimate, uncertainty approach, operational-risk response and deliverables.
- Interview and test: use a short scenario based on your project, such as a utility shutdown or phased renovation, and ask for the resulting cost-control workflow.
- Check references: speak with owners who saw the consultant’s work through comparable stages.
- Normalize fees: add omitted services and compare total scope, assumptions and change provisions—not just the fee total.
- Document the award: retain scores, evidence, clarifications, conflicts and approval records, then contract the named team and stage obligations.
Red flags and how to respond
- “Healthcare experience” with no named projects: require project-level evidence and references or fail the minimum threshold.
- A single precise number with no range: ask for assumptions, maturity, uncertainty, risk allowances and sensitivity information.
- Contingency used as a black box: require separate base, uncertainty and discrete-risk treatment.
- Proposal led by staff who will not deliver: make substitutions subject to approval and score actual availability.
- No conflict declaration: pause evaluation until independence is documented.
- Headline rate with unclear inclusions: reconcile equipment, IT, commissioning, temporary works, owner costs and taxes before comparing.
- No live-campus questions: require a response covering infection control, air quality, utilities, noise, vibration, phasing and access.
- Fee is the only differentiator: apply the pre-approved weighted method and record quality and risk evidence.
- Benchmark presented without date or geography: reject the comparison until scope, location, price basis and edition are stated.
Final decision test
Select the consultant whose evidence best matches your hospital’s clinical and delivery conditions, whose named staff can remain engaged, and whose reports make cost, assumptions, uncertainty and decisions auditable. The winning proposal should tell you what the estimate includes, what it does not include, how it will change as the design matures, and how the consultant will work with the people keeping the hospital operational. Confirm the appointment against your owner’s current local rules before award.
