Oncology urgent care is best understood as a connected pathway, not necessarily a new standalone clinic. It can combine virtual symptom triage, an oncology-focused route through the emergency department, flexible rooms near infusion services, rapid palliative support, and links to home care. For a health system, the right configuration depends on patient volume, available clinicians, finances, space and program maturity.
That is the central proposal in GBBN’s September 10, 2020 article in Building Design + Construction. The article presents a design and operations perspective rather than a clinical trial, so its projected savings, outcomes and emergency-department benefits should be treated as planning rationale—not measured results.

Why an oncology urgent-care pathway matters
“Cancer treatment is not a smooth road,” GBBN writes. Chemotherapy, immunotherapy, radiation and related procedures can produce symptoms that appear at night, on weekends or between scheduled visits. A patient may need rapid advice, laboratory testing, medication changes, fluid replacement, pain control or an assessment for infection, yet the outpatient cancer center may be closed or unable to accommodate an unscheduled visit.

A generic urgent-care or emergency setting may not have immediate access to the patient’s treatment plan, recent laboratory results, medication list or oncology team. GBBN’s proposal is to create a pathway that keeps the patient connected to clinicians and services familiar with the cancer history wherever the first assessment occurs.

The pathway can be distributed across an existing health system. A virtual consultation may resolve a minor concern or direct the patient to the right location. A patient who needs an in-person assessment could be fast-tracked through an emergency department (ED) by staff with oncology expertise. Another patient might be seen in a flexible room beside infusion services, with reserved bays for treatment. Palliative-care clinicians, pharmacy, laboratories, imaging, procedures and home-infusion teams can extend the response beyond a single room.
These are options raised by GBBN, not a universal standard or a claim that every configuration improves outcomes. The design question is how to assemble the pieces safely and affordably for a particular community.
Clinical urgency: what patients and designers must recognize
Some treatment-related symptoms cannot wait for the next routine appointment. The National Cancer Institute (NCI) identifies infection during cancer treatment as potentially life-threatening and requiring urgent medical attention. One possible sign is a fever of 100.5°F (38°C) or higher. NCI advises patients to contact their health care team if they have signs of infection and to ask their own team which symptoms require urgent care.

That guidance supplies the clinical context for a responsive pathway, but it does not tell an individual patient whether to use telehealth, an oncology urgent-care room or the ED. Triage instructions must come from the patient’s treating team and local emergency guidance. A facility plan should therefore make escalation easy rather than assume that every symptom can be managed in one setting.
Five configurations a health system can combine
1. Virtual oncology triage
Telehealth can be the front door. An oncology clinician or trained nurse discusses the symptom, checks relevant treatment information, determines whether an examination or testing is needed and directs the patient to an appropriate service. In some cases, the clinician may adjust or prescribe medication within the organization’s rules.
Virtual triage can reduce unnecessary travel and reserve in-person capacity for patients who need vital signs, laboratory work, imaging, a procedure or treatment. It requires reliable access to the oncology record, a clear escalation protocol and a way to arrange same-day in-person care when a video or telephone assessment is insufficient.
2. An oncology pathway inside the emergency department
A health system can create a referral and fast-track route within its existing ED. Patients identified as having an oncology-related concern are directed to clinicians or teams with oncology knowledge, while the ED retains access to resuscitation, imaging, laboratory and specialty services.

This model avoids building a separate emergency department, but it depends on workflow design. Registration, triage, handoff and consultation rules must identify the patient’s cancer treatment promptly. The route should clarify when the patient remains in the oncology pathway and when a general emergency response takes priority.
3. A hybrid unit near infusion services
Near an infusion center, an exam room can support urgent evaluation and reserved infusion bays can accommodate hydration, medication or other treatments ordered by the clinical team. Flexible rooms may alternate between waiting, consultation and infusion use, allowing the area to expand or contract with demand.
Proximity to the established oncology team and treatment record is a major planning advantage. The trade-off is that the space must be designed for unscheduled arrivals without disrupting scheduled infusions, infection-control procedures or medication preparation. Capacity, hours and staffing should be set by local demand rather than by a fixed room count copied from another institution.
4. Integrated palliative care
GBBN emphasizes rapid pain management and access to palliative expertise as part of urgent oncology care. Palliative clinicians can help address pain, nausea, breathlessness, anxiety and goals-of-care questions while the acute issue is being assessed. Their involvement is not limited to end-of-life care; it can support symptom control at any stage of serious illness.

Design implications include consult rooms that protect privacy, rapid access to prescribing and medication services, and a direct connection to the patient’s oncology team. If a patient’s needs exceed the urgent visit, the pathway should connect to inpatient, hospice or home-based services.
5. Links to home care and home infusion
Some patients can continue treatment or symptom management at home when a home-care or home-infusion team is available. A connected urgent pathway can arrange that transition instead of sending every patient back to the hospital. The handoff must include the treatment plan, medication orders, monitoring requirements, escalation contact and a clear route back to in-person care.
Home links also change the building brief: the facility needs coordination space and staff workflows, not only treatment bays. Pharmacy, delivery, nursing and remote-monitoring arrangements may be as important as the size of the waiting room.

Choosing the model: a practical comparison
GBBN does not provide comparative performance data for these approaches. The table below turns the article’s planning axes into a decision aid; it describes likely operational requirements, not proven superiority.

Designing the pathway around local conditions
Before selecting a room type, assess the conditions GBBN identifies: patient volume, finances, physical constraints and program maturity. A system with low volume and limited capital may begin with virtual triage and a formal ED referral route. A mature cancer program with high infusion demand may justify dedicated urgent rooms and reserved bays. Neither arrangement is inherently correct without local operating data.
Start with a shared, multipurpose footprint
Where a dedicated urgent-care center is not practical, GBBN suggests shared multipurpose space and gradual expansion. Rooms can be planned for more than one function, provided infection prevention, privacy, oxygen, monitoring, medication and accessibility requirements are met for each use. Furniture and equipment should be movable only when doing so does not compromise safety or cleaning protocols.
Flexible space also protects the investment during periods of lower urgent volume. A consultation room may support survivorship visits at one time of day and urgent assessment at another, while reserved infusion capacity can be activated under defined rules.
Plan the critical adjacencies
The article specifically points to pharmacy, laboratories, imaging and procedure teams. Map the distance, hours and handoff for each service before drawing the plan. A nearby laboratory is useful only if urgent specimens can be processed within the pathway’s operating hours. Imaging access needs a comparable review, including transport, scheduling and contrast or procedure support.

1Repair Windows errors before they cause bigger problems2Scan for outdated or missing drivers - takes under a minute3Clear out junk files and repair common Windows errorsInclude the wider health system’s expertise and services in the operational map. Security, environmental services, information technology, transport, interpreter services and social work can determine whether a theoretically fast pathway works in practice.
Screen before assigning an urgent visit
GBBN recommends screening patients before assigning an urgent-care visit and navigating those who are better served by telehealth. Screening should capture the treatment regimen, timing of the last dose, current symptoms, relevant vital signs when available, infection concerns, medication issues and the need for immediate emergency response. The screen is a routing tool, not a substitute for clinical judgment.

A phased implementation sequence
GBBN offers a growth sequence that lets a system test workflows before adding specialties:
- Begin with oncology and palliative care. Establish the urgent contact number or digital entry point, clinical ownership, escalation rules, documentation and access to symptom-management expertise.
- Expand to survivorship care. Once the urgent pathway is stable, use compatible rooms, navigation and follow-up processes for survivorship needs without obscuring the emergency escalation route.
- Integrate cardio-oncology or other specialties. Add services when demand, staffing and referral volume justify them, using the same screening, handoff and shared-space principles.
At each stage, define operating hours, staffing coverage, maximum safe capacity, transfer criteria and who is accountable after the patient leaves. Expansion should follow demonstrated workflow readiness and local demand, not a preset construction timetable.
Staffing and governance questions
GBBN frames the planning questions in direct terms: “Should they dedicate space within their program or partner with the ED? How should they staff the program? With what clinicians?” Those questions should be answered together because the physical model cannot compensate for unavailable expertise.
- Clinical coverage: Specify which oncology, palliative, emergency, nursing, pharmacy and laboratory roles are present on site, on call or available through consultation.
- Escalation: Define who can transfer a patient to the ED, admit the patient, call a specialist or activate emergency services.
- Information access: Ensure every entry point can retrieve the treatment plan, allergies, medication list, recent results and contact details for the primary oncology team.
- Coordination: Assign ownership for telehealth scheduling, home-care referrals, follow-up calls and unresolved test results.
- Training: Prepare front-desk, navigation, nursing and environmental-services staff for oncology-specific infection, privacy and handling requirements.
What the proposal does—and does not—prove
GBBN describes a potential “triple-win”: lower system costs, better clinical outcomes and patient satisfaction, more efficient ED and inpatient use, and less infection exposure for immunocompromised patients. The article supplies no statistics, study methods or measured outcomes supporting those projections. They are the authors’ expected benefits and design rationale, not established effects.
Likewise, NCI’s fever threshold is clinical guidance, not a population statistic and not evidence that one building configuration is safer than another. A health system considering the model should establish its own baseline measures and evaluate the pathway after implementation.
Measures for a responsible evaluation
Because the source article is not an outcomes study, operators should decide in advance how they will test whether the pathway works locally. Useful measures include:
- time from first patient contact to clinical assessment;
- time to laboratory, imaging or medication intervention when ordered;
- percentage of screened contacts resolved by telehealth versus routed to in-person care;
- unplanned ED visits or admissions after an urgent-care contact;
- patient-reported ease of reaching the oncology team and understanding the next step;
- staffing utilization, room utilization and impact on scheduled infusion capacity;
- handoff completion for home care, palliative care and follow-up appointments.
These measures should be interpreted with case mix, operating hours and local service availability. A rise in referrals may indicate better access rather than worse clinical performance, while a low ED-transfer rate is not automatically positive if escalation is being delayed.

Common planning pitfalls
Building a clinic before defining the route
A dedicated room does not create continuity by itself. First document how a patient enters, who screens the concern, where testing occurs, who can prescribe, and how the result returns to the oncology team. Then size and locate the space.
Treating every symptom as an urgent-care problem
Some concerns require emergency services or immediate clinician direction. A pathway should make that escalation visible and fast rather than force staff to fit every case into a convenient room type.
Ignoring scheduled-care operations
Hybrid infusion capacity can be valuable, but urgent arrivals must not silently displace planned cancer treatment. Establish reserved capacity, release rules and a method for notifying patients if a scheduled service changes.
Separating palliative care from the urgent workflow
Delaying symptom expertise until after repeated assessments can prolong distress. Include palliative consultation in the initial operating model when the patient population and clinician availability support it.
Assuming home care is universally available
Home infusion and nursing coverage vary by geography, payer and time of day. Confirm the actual service footprint, delivery windows and after-hours escalation before counting home care as capacity.
A construction and facility-planning checklist
- Map virtual, ED, infusion, palliative and home-care entry points on one patient-flow diagram.
- Separate public waiting, clinical assessment, infusion, medication preparation and staff circulation where required by safety and privacy rules.
- Provide adaptable rooms without sacrificing handwashing, monitoring, oxygen, power, accessibility or cleaning requirements.
- Place or connect the pathway to pharmacy, laboratory, imaging and procedure services with documented hours and transport routes.
- Design confidential telehealth and consultation areas with dependable connectivity and acoustic privacy.
- Include space for family communication, interpreters, social work and goals-of-care discussions.
- Plan signage and wayfinding for patients arriving after hours or from the ED.
- Reserve back-of-house space for supplies, waste, medication security and environmental-services workflows.
- Build data capture into registration and the electronic record so response times and handoffs can be evaluated.
- Allow future expansion from oncology and palliative care to survivorship, cardio-oncology or other specialties without rebuilding the entire pathway.
Bottom line for health-system leaders
Oncology urgent care is a service architecture more than a single architectural type. GBBN’s strongest practical recommendation is to connect the places a cancer patient may need—virtual triage, the ED, infusion, palliative care, pharmacy, diagnostics and home services—while scaling the physical footprint to local demand. Start with flexible space and explicit workflows, then expand as staffing, volume and evaluation support the next phase. Keep the clinical team’s triage guidance at the center: the building should make timely escalation and continuity easier, not promise that design alone can produce better outcomes.
