How Much Does It Cost to Build a Community Health Centre? A Practical Cost Guide

There is no dependable worldwide price for a community health centre. Cost depends on country, floor area, clinical services, site conditions, specification, procurement route and what the budget includes. Official examples range from a Hong Kong construction rate of HK$42,941–HK$43,700 per m² at September 2024 prices to US project budgets of nearly US$13 million and US$3,421,309. Those figures are not a universal price range: they describe different buildings, currencies, dates and cost boundaries.

A credible estimate starts with a defined brief and a local healthcare architect or cost consultant. Before comparing numbers, align the gross floor area, service mix, site assumptions, price date and inclusions such as land, professional fees, equipment, taxes and contingency.

What the published figures actually show

The strongest directly comparable evidence is a Hong Kong government unit-cost comparison. The Public Works Subcommittee reported construction costs of HK$42,941 per m² for a proposed project and HK$43,700 per m² for the reference 74MM Community Health Centre cum Social Welfare Facilities at Pak Wo Road. Both figures are expressed at September 2024 prices. The paper warns that projects should not be compared directly until they are adjusted to the same price level.

Other official records provide whole-project budgets, but the retrieved records do not establish enough common detail to convert them into a rate per square metre or a general range.

Location and project Published figure What the figure represents How to use it
Hong Kong proposed project HK$42,941 per m² Construction unit cost at September 2024 prices Useful as a local, date-specific building-rate reference; not a universal rate
Hong Kong reference project, 74MM Community Health Centre cum Social Welfare Facilities at Pak Wo Road HK$43,700 per m² Adjusted construction unit cost at September 2024 prices Compare only after reconciling scope, site and price level
Eastport Health Care, Maine, United States Nearly US$13 million Expected budget for a planned new community health centre; the site was purchased in January 2024 Project-specific budget; the HHS record does not establish a comparable area or cost boundary
Canyonlands Healthcare, Duncan, Arizona, United States US$3,421,309 Total project cost in an HHS TAGGS record Do not treat it as the low end of a national range; size and scope are not shown as comparable with Eastport
Gauteng, South Africa, 2024/25 capital estimates R119,543,000 and R250,000,000 examples Project-cost rows displayed in R’000 in the provincial table Illustrates variation between capital-program entries, not a standard centre price

The Hong Kong paper explains the reason for caution: “As each project has its own uniqueness in terms of site restrictions, scope and nature, scale and other factors, etc., the construction unit cost will vary accordingly.”

Why one centre can cost far more than another

Service programme and floor area

A small primary-care clinic has a different room schedule from a centre combining general outpatient care, a chest clinic, maternal and child health services, dermatology, a social hygiene service headquarters and public vehicle parking. The Hong Kong proposal includes that broader combination. Each service adds rooms, circulation, staff areas, ventilation, plumbing, power, infection-control measures and equipment requirements.

Gross floor area matters as much as the number of consultation rooms. Waiting areas, accessible toilets, storage, staff support, plant rooms, circulation and shared clinical spaces can occupy a substantial part of the building. Ask the designer for a room-by-room schedule and a gross-area definition before applying any rate.

Site, utilities and design constraints

A constrained urban plot, difficult ground, retaining works, contaminated land, weak utility capacity or restricted delivery access can increase costs before the first clinical room is fitted out. The Hong Kong submission identifies site restrictions, project scope, scale and other factors as drivers of unit cost. It also describes a fitness-for-purpose, no-frills approach and design choices intended to improve efficiency; a higher specification or more complex site would not be represented by that assumption.

Geography and price date

Currency conversion alone does not make estimates comparable. Labour, materials, taxes, professional fees, regulations and contractor availability vary by region. Inflation also changes a rate over time. The Hong Kong comparator was specifically adjusted to September 2024 prices and cautions that an unadjusted comparison is inappropriate. A local estimator should apply a recognized local index or market evidence to the same valuation date, then document the index and assumptions.

Cost boundary and procurement

“Cost to build” may mean only the building and building services, or it may mean the entire programme. Define whether the estimate includes:

  • Land purchase, legal work and due diligence
  • Surveys, geotechnical investigations and site clearance
  • Architectural, engineering, project-management and other professional fees
  • Planning, permits, utility connections and statutory charges
  • Enabling works, demolition, roads, drainage, parking and landscaping
  • Structure, envelope, interiors, mechanical, electrical, plumbing, fire and security systems
  • Medical equipment, furniture, information technology and communications
  • Taxes, escalation, contingency, financing, commissioning and relocation

There is no evidence that all published examples use the same boundary. A whole-project budget cannot be compared with a building-only unit rate until the missing categories are reconciled.

How to produce a defensible local estimate

  1. Set the location and valuation date. Record the country, city or province, currency and date at which prices are measured. State whether escalation is included through tender and construction.
  2. Write the clinical brief. List every service, operating hour, patient volume assumption and special requirement. Identify whether parking, community rooms, administration or social-care functions share the building.
  3. Prepare a room and area schedule. Separate net clinical area from circulation, support and plant space, then state the gross internal or gross floor area convention used. Do not multiply an unverified area by a published rate.
  4. Investigate the site. Confirm ownership, surveys, ground conditions, flood or contamination risk, demolition, access for construction traffic, existing utilities and any planning or heritage restrictions.
  5. Choose the cost basis. Use local comparable projects or a current healthcare cost database. If using the Hong Kong figures, treat HK$42,941–HK$43,700 per m² as a September 2024 construction reference, not as a rate for another country or an all-in budget.
  6. Build the elemental estimate. Price substructure, superstructure, envelope, finishes, services, external works and specialist clinical installations separately. This exposes which design decisions are driving cost.
  7. Add non-construction costs explicitly. Show land, design and other fees, permits, equipment, taxes, financing and commissioning as separate lines. If a category is excluded, label it as excluded rather than silently assuming it is covered.
  8. Allow for risk and change. Set contingency against the maturity of the design and known site risks. Keep inflation or escalation separate from contingency so that a later decision-maker can see the difference.
  9. Check procurement and market conditions. A traditional competitive tender, design-and-build contract or construction-manager route can shift fee, risk and contingency requirements. Confirm whether the estimate assumes a single contract or multiple packages.
  10. Validate with a local specialist. Have a healthcare architect, quantity surveyor or construction cost estimator test room areas, code requirements, equipment schedules and current tender evidence. Update the estimate at concept, developed-design and pre-tender stages.

Using the Hong Kong rate without overstating it

A unit rate can illustrate the arithmetic but cannot replace a project brief. For example, applying HK$42,941 per m² to a hypothetical 1,000 m² building produces HK$42,941,000 as a simple construction-rate calculation at September 2024 prices. That result is not a quotation and does not automatically include land, design fees, external works, equipment, taxes, contingency or financing. A different gross-area definition or service mix would change it.

The proposed Hong Kong project and the Pak Wo Road reference project are closer comparisons than the international budgets because both rates are presented in the same currency and price period. Even then, the government paper says each project has unique site and scope conditions. Use the pair to test whether a concept estimate is in the same broad local order of magnitude, then reconcile the designs line by line.

Why the US examples cannot be turned into a national range

Eastport Health Care’s HHS TAGGS abstract records an expected budget of nearly US$13 million for a planned new community health centre after the organization bought a 2.8-acre site in January 2024. The retrieved record does not establish the building area or whether the amount includes land, professional services, equipment, financing or other programme costs.

A separate HHS TAGGS record gives a US$3,421,309 total project cost for Canyonlands Healthcare in Duncan, Arizona. The evidence does not show that the Duncan project matches Eastport in size, clinical programme, site work or cost boundary. Presenting these figures as a US$3.4 million-to-US$13 million price range would therefore be misleading. They are two project-specific data points that require their own briefs and accounting definitions.

South African capital entries show the same limitation

Gauteng’s 2024/25 capital estimates list several primary-health-care community-health-centre rows. The extracted table displays amounts in R’000, including examples with total project costs of R119,543,000 and R250,000,000. Project naming and status are incomplete in the extracted entries, so they cannot establish a typical Gauteng centre cost. They do demonstrate why a capital-programme amount must be read with its project scope, delivery stage and inclusions.

Funding is not the same as construction cost

Grants can help finance an eligible project, but they do not set the market cost of the building and do not guarantee that a project qualifies. The Illinois Capital Development Board describes US$50 million statewide in grant availability for eligible Federally Qualified Health Centers and FQHC Look-alikes. Listed examples include buying and renovating a building for examination rooms and dental care, renovating examination and laboratory space, and roof replacement or signage. The programme is geographically limited; verify current deadlines, match requirements and eligibility before relying on it.

HRSA budget guidance requires a health centre’s total budget to account for projected revenue supporting the Health Center Program project beyond the programme award. The listed sources include fees, premiums, third-party reimbursements or payments, other state, local or federal awards and contracts, private support and other expected support. Those revenue assumptions concern programme funding and sustainability, not a standard construction rate. Keep the capital cost plan and the operating or funding plan as separate documents, then show how the financing sources are expected to cover the approved project budget.

Questions to settle before requesting a price

  • Which country, region and municipality is the site in, and what price date should the estimate use?
  • What is the target gross floor area, and how was it calculated?
  • Which clinical, public-health, social-care, administrative and parking functions are included?
  • Is this a new build, extension, conversion or renovation?
  • Who owns the site, and what surveys, utilities, access constraints or remediation are known?
  • Does the requested figure include land, demolition, external works, fees, equipment, taxes, contingency, escalation and financing?
  • What procurement route and contract packaging are assumed?
  • What design stage is the estimate based on, and what risks remain unresolved?
  • Which local code, accessibility, infection-control, fire and healthcare-planning requirements apply?

Common estimating mistakes and how to correct them

Quoting a global price per square metre

Problem: A rate from one country is presented as if it applies everywhere. Correction: identify the original geography, currency, price date, area convention and scope, then obtain local indexed evidence.

Comparing a grant or project budget with a building rate

Problem: funding availability or a total project announcement is treated as the construction price. Correction: separate funding sources from the cost plan and map every included and excluded category.

Ignoring clinical equipment and commissioning

Problem: the shell is priced, but specialist equipment, testing, staff training, relocation or commissioning is left to the end. Correction: create an equipment and operational-readiness schedule early and assign responsibility for each item.

Using an old estimate without escalation

Problem: a historical rate is carried forward unchanged. Correction: restate it to the target price date with a documented local index, then add a separate allowance for future tender and construction escalation.

Hiding uncertainty in one large contingency

Problem: an unexplained percentage masks design gaps and site risks. Correction: identify risks individually, assign owners and probabilities where appropriate, and reduce allowances as investigations and design progress.

What a useful answer should look like

A responsible early answer is a range or elemental estimate tied to a defined brief, not a headline number detached from assumptions. It should show the location and valuation date, gross area, services, site conditions, procurement route, included cost categories, exclusions, escalation and contingency. Once those inputs are fixed, local market evidence can produce a budget suitable for funding decisions and design development. Without them, the most accurate answer remains that a community health centre can cost very different amounts, and the published figures above should be treated as carefully qualified reference points rather than a universal quote.
