Hospitals can keep heating reliable during a boiler replacement by identifying the services and clinical areas that cannot tolerate interruption, confirming what the remaining plant can safely support, and planning isolation, temporary capacity and recovery around those requirements. Heating is only part of the continuity plan: domestic hot water, steam, ventilation, water safety and clinical operations may all be affected. The right arrangement depends on the hospital’s plant, loads, distribution system and local requirements, so there is no universally suitable temporary boiler size, shutdown duration or temperature threshold.

Start with the services that must remain available

Before deciding how to replace a boiler, establish what it serves and what must continue operating while it is isolated. A hospital boiler house may support several distinct services: space heating, domestic hot water and steam. Each has different users, dependencies and consequences if supply is reduced. A heating plan that overlooks hot water or steam can leave important clinical or support functions exposed even if patient areas remain warm.

ASHRAE’s health-care-facility guidance describes the services remaining boilers should be able to provide when one boiler is unavailable: hot water for clinical, dietary and patient use; steam for sterilization and dietary purposes; and heating for operating, delivery, birthing, labor, recovery, intensive care, nursery and general inpatient rooms. These examples are a prompt for a site’s load and dependency review, not a substitute for checking the hospital’s actual systems and clinical use. (ASHRAE, Chapter 8: Health Care Facilities.)

Build a site-specific critical-load map

Work with facilities engineers and clinical leaders to map which boilers, pumps and circuits serve each area and service. Include the normal arrangement and the arrangement expected with one boiler or circuit unavailable. Identify where a shared component, such as a pump or common main, could undermine otherwise available boiler capacity.

  • Ask clinical and operational teams what service reductions would be unacceptable and what decisions would require escalation.

Do not treat rated plant capacity as proof that the hospital can operate normally with a boiler offline. Engineers need to assess what the remaining equipment and distribution system can deliver under the planned operating arrangement and the loads the hospital must maintain.

Set the outage limits and decision authority

Agree the permitted operating envelope before work begins. It should describe what can stay online, which sections may be isolated, what conditions require work to pause, and who has authority to delay the project, reduce a service or activate a contingency. The exact limits and monitoring signals must be determined for the site; the cited guidance does not establish universal room-temperature thresholds, outage durations or temporary capacity values.

For major boiler houses serving whole-hospital heating or domestic hot water, NHS England’s HTM 00 says sites should consider a more complex, site-specific emergency procedure. It also identifies alarms for major equipment failure, including loss of steam pressure. The publication page identifies the 2014 edition and was updated in 2024. The practical implication is to make the project’s alerting and escalation arrangements explicit rather than relying on staff to infer a problem from a change in conditions.

Decide what will be monitored and who responds

Agree which alarms, pressures, temperatures, plant states or service reports matter to the project, as applicable to the installed system. Assign a named role or team to receive each alert, assess it and notify the people authorized to act. Facilities, clinical operations and contractors should know how a developing problem will be communicated, including outside normal working arrangements if the work could continue then.

Define decision points in advance. For example, the plan should specify who can stop a tie-in, defer the next phase, activate alternative arrangements or ask clinical teams to change activity. The triggers themselves require site-specific engineering and clinical approval; they should not be copied from a generic checklist.

Choose continuity measures for the actual plant

Continuity measures are alternatives to assess, not a standard package every hospital should install. Their adequacy depends on the load to be served, the existing plant and distribution, connection points, fuel and electrical availability, access, clinical needs and the time required to commission and restore service.

Approach What it can contribute What the project team must establish
Remaining or standby boilers ASHRAE describes the remaining boilers as providing hot water, steam and heating for specified hospital uses when one boiler is out for failure or routine maintenance. Whether the installed plant and distribution can meet this hospital’s critical loads in the proposed operating arrangement.
Temporary or portable equipment connections ASHRAE Chapter 9 identifies portable-equipment connections and emergency equipment connections as resilience measures. The services a proposed unit can support, its capacity, connection and isolation needs, fuel and electrical dependencies, access, availability and commissioning requirements.
Staged work with sectional isolation ASHRAE describes isolation valves on system mains and equipment to permit sections to be isolated for work. Whether valves are present, accessible and serviceable, and whether the planned phasing preserves the required services.
Alternative fuel, standby pumps and other resilience provisions NHS HTM 00 identifies alternative fuel capability and duty/standby pumps on all circuits for consideration in major boiler houses. ASHRAE also discusses standby boiler-feed, circulation, condensate-return and fuel-oil pumps. Which provisions address the hospital’s actual single points of failure and are compatible with the plant, fuel arrangements and operating requirements.

Other provisions identified in NHS HTM 00 for consideration at major installations include on-site spares for essential equipment, a permanently available generator connection point, generator access, and alarms for major equipment failure. ASHRAE Chapter 9 also discusses spare parts, local service expertise and load-shifting capability. Evaluate these against the site’s risks and resources; the guidance does not establish that every item is necessary or sufficient for every hospital.

Temporary boiler hire may be an option to investigate, but it is not automatically suitable. The hospital’s competent engineers must establish what the temporary equipment can safely serve and how it will connect to existing systems. A unit’s nominal output alone does not demonstrate that it can provide the required service through the actual connections, pumps, fuel supply and controls.

Plan isolation and phasing around live services

Use the load map and operating envelope to develop the isolation sequence. The objective is to restrict isolation to the equipment and sections needed for each task, where the system’s design and condition permit. A phased plan can help preserve service from parts of the system that remain available, but it does not guarantee continuity: engineers need to verify the effect of each isolation and connection on the complete operating arrangement.

  1. Verify the system and isolation points. Confirm the relevant mains, branches, valves and equipment against the site’s current system information. Check valve location, access and condition before relying on a valve to separate a work area.
  2. Define each work phase. State what is taken out of service, what remains operating, which services may be affected and what checks must pass before the next phase begins.
  3. Coordinate tie-ins with clinical activity. Agree the work sequence and timing with clinical operations and other affected services. The hospital—not the contractor alone—must make decisions about clinical activity and acceptable service risk.
  4. Control changes to the plan. Ensure revised contractor assumptions, limitations or discoveries are understood by the hospital’s facilities and clinical decision-makers before work proceeds.
  5. Verify each transition. Use competent personnel to confirm the required system condition before isolation, connection, commissioning or return to service.

ASHRAE Chapters 8 and 9 describe isolation valves for mains and equipment as a way to isolate sections for work. That is a design and resilience consideration, not evidence that a particular hospital’s valves are suitable for a planned shutdown. If the required isolation cannot be achieved safely, revise the work method and continuity plan rather than assuming the rest of the system will be unaffected.

Treat domestic hot water as a separate safety workstream

A boiler replacement can affect hot-water generation and distribution separately from space heating. Changes to heating plant, storage, circulation or operating arrangements may have water-safety implications that a room-heating review will not capture. NHS HTM 04-01 covers safe hot- and cold-water systems in healthcare premises, and the Health and Safety Executive’s hot- and cold-water guidance addresses Legionella control, with particular relevance to acute hospitals and susceptible patients. CDC guidance also describes healthcare hot-water systems and infection-control context.

Involve the hospital’s water-safety lead and follow the site’s water-safety procedures before altering temperatures, storage, circulation or isolation arrangements. Determine how any planned interruption or changed configuration affects the water system and what controls, checks and authorization are needed. Do not apply a generic temperature setting or assume that a temporary heating solution also resolves domestic hot-water safety.

Coordinate heating with ventilation and infection prevention

Heating and ventilation conditions interact in clinical environments. NHS England’s HTM 03-01 covers specialized ventilation design and validation for new installations and major refurbishments, as well as operational management and performance verification. The site ventilation lead should assess whether the boiler work or any changed heating arrangement could affect required conditions, system operation or operational controls.

Bring infection-prevention colleagues into the review alongside clinical operations, facilities and ventilation teams. Their input can help identify consequences that are not apparent from plant status alone, including effects on hygiene arrangements and the acceptability of operating conditions in particular areas. Keep the distinction clear: maintaining space heat does not by itself establish that ventilation or infection-control requirements are being met.

Prepare communications and clinical fallback actions

Assign responsibilities for routine updates, incident escalation and decisions about affected clinical activity. Include the hospital team, the contractor and any other relevant service leads. Before work starts, confirm that contractor information and caveats are understood by the people who must decide whether the work can proceed.

An NHS England case study published on 21 April 2023 documents two boiler failures at University College London Hospitals NHS Foundation Trust on a January weekday. Reported effects included low temperatures and cold water, delayed theatre starts and patient discharges, discomfort for patients and staff, risks of pre-operative and post-operative hypothermia, and infection-control issues associated with cold water for hygiene. The response included extra blankets, closed blinds to retain heat and delayed theatre starts after environmental risk was considered. The case is an example of possible interconnected effects, not a prediction that every hospital will experience the same outcomes.

The case study’s lessons included stronger communication among services, understanding contractors’ information and caveats, robust escalation and routine maintenance checks. Use those lessons to test whether the project team has practical fallback arrangements, rather than assuming a technical contingency will remove the need for clinical decisions.

  • Set out who contacts clinical teams and what information they need when service conditions change.
  • Agree who can authorize a delay or service reduction and how that decision is escalated.
  • Identify patient-care and operational contingencies relevant to the areas and services on the load map.
  • Make clear how contractors report a changed assumption, failed check or unexpected condition, and who must acknowledge it.
  • Give staff a route to report changing conditions and receive updates as the project moves between phases.

The Health Service Executive’s energy-disruption checklist and guidance also emphasizes contingency planning, backup power, alternative sourcing and staff roles. Its applicability depends on jurisdiction and local arrangements; use it as a planning reference alongside the hospital’s own emergency and energy-continuity procedures.

Commission and restore service deliberately

Plan the return to service before isolation begins. Agree how competent personnel will verify the replacement equipment and affected system, how service will be restored in stages where appropriate, which alarms require a response, and who accepts the handover. The plan should account for the actual system and clinical requirements rather than assume that a completed repair immediately means normal service has resumed.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

The UCLH case study described a staged return after boiler thermal protection, illustrating that recovery may extend beyond the initial repair. For a replacement project, define what must be checked at each restoration stage, how any temporary arrangements will be managed during the transition, and who can confirm that the agreed operating arrangement has been achieved. Record outstanding issues and communicate them to the relevant operational and clinical teams.

What the plan cannot decide generically

No universal temporary heat capacity, replacement duration, room-temperature threshold or phasing schedule can be set from general hospital guidance. Those decisions depend on site loads, climate, plant and distribution design, clinical use, project scope, local codes and healthcare operating requirements. A competent engineering team must develop the technical approach with the hospital’s clinical, water-safety, infection-prevention and ventilation leads, and with reference to the jurisdiction that governs the site.
