There is no officially recommended or tested “best” door lock for dementia. An exterior deadbolt or latch may be one part of a person-specific safety plan, but it must not trap someone inside: the Alzheimer’s Association says to “never lock a person in at home.” Door-opening alarms can alert a caregiver when a door opens, but they do not prevent an exit or replace supervision.
What door security can—and cannot—do
Wandering can happen at any stage of dementia. The Alzheimer’s Association says six in 10 people living with dementia will wander at least once, and many do so repeatedly; the page does not state the estimate’s year. Door security involves two different aims: adding a physical step at an exit, or notifying a caregiver that a door has opened. The first may affect how someone leaves; the second gives a caregiver information to respond. Neither removes the need to plan for the person’s circumstances, supervision, and safe emergency exit.

How to choose a setup for the household
Because the sources do not rank brands or models, choose by fit and safe operation rather than a claim that one product is “best.” Before installing or relying on a device, consider:
- Alert versus physical security: Decide whether the need is to be notified, add a physical step, or combine approaches without compromising exit.
- Caregiver response: For an alarm or sensor, check that the caregiver can hear or receive it where they are and can respond promptly.
- Power and connectivity: Ask whether the device depends on batteries, household power, a network, or a phone, and how its status or failure would be noticed.
- Door and routine fit: Consider compatibility with the existing door, who uses it, and whether the device can be operated reliably in the household’s daily routine.
- Installation and upkeep: Account for installation, testing, battery or other maintenance, and who will keep the system working.
- Emergency exit: Make sure every resident can leave safely in an emergency, including during a power or device failure. Check local requirements with a qualified professional; the guidance cited here does not specify local fire or building codes.
These are practical selection checks, not results from comparative product testing. If considering a high- or low-positioned deadbolt, get individualized advice rather than treating a particular height as universally safe.

Remove hazards and account for fire safety
The Alzheimer’s Association home checklist advises removing interior locks that could let a person lock themself in. It also recommends maintaining smoke and carbon monoxide detectors; professional home-setting guidance calls for smoke alarms and a working fire extinguisher. Door-security choices must not prevent a person from exiting during a fire or other emergency.

Consider other environmental supports
The Association’s wandering guidance also describes night lights, door camouflage or safety covers, visual threshold cues, pressure-sensitive mats, and safe outdoor areas as possible measures. Their suitability depends on the individual and the home. A cue or cover should not confuse the person, obstruct a necessary exit, or create a trip or fall hazard.

Make a response plan before someone goes missing
Prepare practical information in advance: keep a recent close-up photo, tell neighbors how to contact the caregiver, and identify likely destinations and nearby hazards. If the person goes missing, begin searching immediately, checking likely places and hazards. The Alzheimer’s Association advises calling 911 if the person has not been found within 15 minutes. Needs can change with cognition, mobility, the home layout, and the amount of caregiver coverage. The Alzheimer’s Association professional recommendations point to a home assessment by an occupational or physical therapist.
