Confined-Space Entry Permits, Air Testing, and Rescue Plans Explained for U.S. Construction

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Safe confined-space entry is a controlled operation, not simply a worker wearing a gas detector. Before entry, the construction employer must identify the space and task hazards, isolate hazards where required, establish acceptable entry conditions, test the atmosphere with a suitable calibrated direct-reading instrument, verify communications and equipment, and authorize the entry under the applicable OSHA procedure. During the work, conditions must be tested or monitored as necessary, and the entrant, attendant, entry supervisor and rescue service must perform defined roles.

This is a federal U.S. overview for construction work. OSHA’s construction requirements are in 29 CFR 1926 Subpart AA, including the permit-required confined-space program rule at 1926.1204. General industry uses 29 CFR 1910.146, which has different program details. State-plan jurisdictions, maritime work, mining and non-U.S. workplaces may impose additional or different requirements.

Which confined-space rule applies on a construction project?

Start by classifying the work context. Construction activities generally fall under OSHA 29 CFR 1926 Subpart AA, while maintenance or operations performed as general industry may fall under 29 CFR 1910.146. A project should not copy a general-industry permit and assume it is universally compliant. The controlling standard, the employer’s written program, the host employer’s procedures and the actual hazards must all be reconciled.

Decision point Construction work General industry work
Primary federal framework 29 CFR 1926 Subpart AA, including 1926.1204 29 CFR 1910.146
Program and permit approach Use the construction employer’s permit-required confined-space program and project coordination procedures. The employer documents required measures on an entry permit; the entry supervisor signs before entry.
Atmospheric testing order OSHA’s construction requirements use the oxygen, combustible, then toxic sequence. OSHA specifies oxygen, combustible gases and vapors, then toxic gases and vapors.
What determines acceptable conditions The hazards, work methods and limits established by the construction program and applicable OSHA requirements. Conditions identified and documented under the 1910.146 program and permit.
Other jurisdictions State-plan, maritime, mining and non-U.S. requirements may differ and must be checked separately.

On a multi-employer site, the controlling contractor, host employer and subcontractors need a documented method for exchanging hazard information, permit status, isolation details, communications and emergency arrangements. A permit is a control document for a particular space and job, not a generic form that makes every entry safe.

What an entry permit does

An entry permit records the decisions that make a specific entry permissible and gives the work team a common operating picture. For general industry, OSHA 1910.146 identifies the following information for a permit. Construction employers should address the same practical questions in the format required by their 1926 Subpart AA program and site procedures:

  • The space to be entered, the purpose of entry and the planned duration.
  • The names or identification of authorized entrants, attendants and the entry supervisor.
  • Known and potential atmospheric and non-atmospheric hazards.
  • Isolation measures for energy, process connections, inflows, engulfment sources and other hazards.
  • Acceptable entry conditions, including atmospheric limits established for the space and task.
  • Initial and periodic test results, the tester’s identity and the time of each test.
  • The rescue service, the method for summoning it and the communications method.
  • Required personal protective equipment, retrieval equipment, ventilation, access equipment, lighting and testing equipment.
  • Relevant additional information and related permits, such as a hot-work permit.

The entry supervisor authorizes the entry only after the required measures are complete. If conditions change, a hazard appears, a required control fails or the permit no longer describes the work, entrants leave and the space is reassessed under the applicable procedure. For general industry, a canceled permit must be retained for at least one year; construction records should be retained according to the project program, OSHA requirements and employer policy.

Using OSHA’s sample checklist correctly

OSHA Appendix D to 1910.146 shows fields for issue and expiry times, work and space identification, standby personnel, oxygen, flammability and toxic checks, tester signature, source isolation, ventilation and rescue procedures. It is a checklist aid, not a substitute for a site-specific hazard assessment. A construction permit that omits a hazard created by the planned work is incomplete even if every printed box is checked.

Permit workflow: from hazard assessment to closeout

  1. Identify the space and task. Determine whether the space meets the confined-space definition, whether it is permit-required, what work will occur inside and what hazards the work can create. Consider configuration, process connections, energy sources, engulfment, atmospheric contaminants, hot work, coatings, cleaning chemicals and simultaneous activities.
  2. Coordinate employers and workers. Exchange information with the host employer, controlling contractor and subcontractors. Tell affected workers about the space, hazards, controls, permit status and emergency arrangements.
  3. Isolate hazards. Lock out or otherwise control energy, disconnect or blank process lines where required, stop inflows, control engulfment sources and prevent unexpected operation of equipment. Ventilation is an engineering control, not a replacement for isolation when isolation is required.
  4. Define acceptable entry conditions. Set the atmospheric and physical conditions that must exist before entry and the conditions that require evacuation. Base them on the identified contaminants, oxygen hazards, combustible hazards, work process and applicable rule; do not copy a universal gas list or interval.
  5. Select and prepare instruments. Use a calibrated direct-reading instrument capable of detecting the hazards identified. Confirm sensor suitability, calibration status, bump or function checks required by the manufacturer, battery condition, sampling tubing and probe condition.
  6. Test before authorization. Test in the required order, allow each measurement to run for at least the manufacturer’s minimum response time, record actual concentrations, and communicate the results. Entrants or their representatives must be allowed to observe testing under the applicable OSHA program.
  7. Stage people and equipment. Confirm trained entrants, an attendant, an entry supervisor, communications, ventilation, lighting, access and egress equipment, PPE, retrieval equipment where appropriate, first-aid resources and a rescue service that can actually respond.
  8. Authorize the entry. The designated entry supervisor signs or otherwise authorizes the permit after verifying that conditions and equipment match the plan.
  9. Monitor and control the work. Test or monitor as necessary to determine that acceptable conditions continue. The attendant tracks entrants, maintains communications and orders evacuation when required. The supervisor stops or terminates the entry when conditions or controls no longer support it.
  10. Close and review. Remove workers, close the space, cancel the permit, document problems and review the program as required. Correct failures before the next entry rather than carrying them forward.

How should air be tested before and during confined-space entry?

1. Evaluate hazards before choosing sensors

OSHA distinguishes evaluation testing from verification testing. Evaluation testing identifies contaminants and other atmospheric hazards that may exist or arise and establishes procedures and acceptable conditions. Verification testing checks the residual concentrations identified during evaluation against those conditions. The instrument and sensors must match the hazards of the space and the planned work; a basic four-gas configuration is not automatically adequate for every tank, vault, pit or process vessel.

  • One-Handed Loading - Keep one hand free with a 1/4 in. hex chuck that accepts 1 in. bit tips

2. Follow the required sequence

Test oxygen first, combustible gases and vapors second, and toxic gases and vapors third. OSHA Appendix B explains: “A test for oxygen is performed first because most combustible gas meters are oxygen dependent and will not provide reliable readings in an oxygen deficient atmosphere.” Record the actual reading for each parameter, along with the time and tester, rather than writing only “pass.”

3. Allow full instrument response

Each parameter must be measured for at least the instrument manufacturer’s stated minimum response time. Sampling too briefly can produce a falsely reassuring reading. Follow the manufacturer’s instructions for startup, calibration, sampling-line conditioning, sensor limitations, alarm settings and interpretation of a failed or unstable reading.

4. Sample the atmosphere where workers will be

Atmospheres can stratify. When a worker descends into a potentially stratified space, OSHA Appendix B describes sampling approximately four feet (1.22 m) ahead in the direction of travel and to each side. Move slowly enough to account for probe sampling and detector response. Test high, middle and low areas when the space’s geometry or contaminant properties make layering possible; the exact pattern should come from the hazard assessment and instrument capability.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

5. Continue testing during the entry

A pre-entry reading is only a snapshot. Test or monitor as necessary to determine that acceptable conditions remain. The needed frequency depends on the hazards, work process, controls, instrument and applicable program; this evidence does not establish one universal interval or gas list. Continuous monitoring is especially important where conditions can change rapidly, such as during welding, cleaning, coating removal, chemical use or disturbance of residues.

Where isolation is infeasible because a space is large or continuously supplied, OSHA’s general-industry rule requires pre-entry testing as far as feasible and continuous monitoring in the work areas if entry proceeds. Construction employers should apply the corresponding requirements in 1926 Subpart AA and their written program.

6. Act on an abnormal result

If an alarm sounds, a reading exceeds the acceptable condition, the instrument fails, ventilation stops, communications fail or an unrecognized hazard appears, entrants evacuate according to the procedure. The attendant notifies the supervisor and rescue service as required. Re-entry requires investigation, correction, retesting and reauthorization; silencing an alarm is not a corrective action.

Roles that make the permit work

Authorized entrant

The entrant follows the permit, uses the specified PPE and equipment, maintains communication and exits when ordered, when an alarm or prohibited condition occurs, or when the entrant recognizes a dangerous condition.

Entry supervisor

The entry supervisor verifies the permit, isolation, testing, equipment, communications and rescue arrangements; authorizes entry; stops or terminates it when necessary; and ensures the permit is canceled and deficiencies are recorded.

Testing personnel and rescue providers

Testers must be competent to operate the instrument and interpret results. Rescue providers need the capability, equipment, access and practice appropriate to the actual space, not merely a name on a contact list.

What should a confined-space rescue plan include?

A rescue plan must be operational before entry. It identifies how to summon emergency services, how entrants will be rescued, how necessary emergency care will be provided and how unauthorized people will be prevented from entering. OSHA’s rule does not treat an untrained coworker who climbs in after a victim as a rescue strategy.

Choose a service that can perform this rescue

The employer evaluates whether the service can respond in a time appropriate to the hazards, perform the rescue required by the particular space and maintain suitable equipment and proficiency. Evaluate notification and setup time, availability for the entire entry, communications, access routes, vertical or elevated rescue, patient packaging, medical capability and the possibility that the entrant cannot be reached by a straight pull.

Rescuers must be informed of the hazards and given access to the spaces so they can plan and practice. OSHA Appendix F notes that, for certain significant-atmosphere hazards where rescue entry, packaging and retrieval cannot be safely completed in a relatively short time—15 to 20 minutes—employers should consider airline respirators with escape bottles for rescuers and rescue air for the patient. This is guidance for evaluating equipment, not a universal safe-response deadline.

Plan non-entry retrieval, but verify that it helps

OSHA generally calls for retrieval systems or methods to facilitate non-entry rescue when an entrant enters a permit space, unless retrieval would increase overall risk or would not contribute to rescue. A chest or full-body harness and retrieval line may be suitable when attached at a point and routed so the entrant can actually be removed. Geometry, obstructions, openings, entanglement, harness position and patient condition can make a simple vertical line ineffective. The plan must state when retrieval is used, who operates it and what happens if it fails.

Train and practice employee rescuers

When employees are designated to rescue, OSHA requires appropriate PPE and proficiency training, basic first aid and CPR training, and at least one currently certified first-aid/CPR member available. Employees assigned to rescue must conduct a simulated rescue at least once every 12 months using the actual spaces or representative spaces. Practice should test the real access, communications, retrieval equipment, patient packaging, respiratory protection and handoff to medical care.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Prevent a second victim

The attendant and supervisor must be able to summon the designated service quickly and stop unauthorized entry. Barricades, signage, briefing, access control and direct communication with the rescue service support that objective. The historical warning is stark: NIOSH reported in its January 1986 alert that more than 60% of confined-space fatalities occurred among would-be rescuers. A separate NIOSH-hosted analysis of U.S. data from 1980–1988, published in 1994, found an average of 89 work-related confined-space deaths per year, with approximately 23 (25.5%) involving people who died while attempting rescue. These figures describe historical periods and different definitions; they are not a current national fatality rate.

How to compare entry and rescue options

Option to evaluate Questions to answer
Periodic versus continuous monitoring Can the atmosphere change quickly? Is the space isolated? What does the hazard assessment and applicable rule require? Can the instrument alarm be seen or heard by the people who must act?
Instrument and sensor package Does it detect the identified oxygen, combustible and toxic hazards? Is it calibrated and used within manufacturer limits? Can the probe reach the sampling locations?
On-site versus external rescue Can responders arrive, gain access, package the patient and provide the needed respiratory and medical support within a time appropriate to the hazards?
Non-entry retrieval Will a harness, line, winch or other system remove the entrant without snagging or increasing injury? If not, what trained entry rescue method is available?
General industry versus construction procedure Which standard governs this work, and do the permit, coordination, training, testing and rescue provisions match that standard?

Common failure modes and corrections

  • Using a generic permit: Replace it with a space- and task-specific assessment that names actual hazards, controls, test results and rescue arrangements.
  • Testing only at the opening: Sample where the entrant will travel and work, including ahead and to the sides during descent when stratification is possible.
  • Testing in the wrong order: Start with oxygen, then combustible gases and vapors, then toxic gases and vapors.
  • Relying on one pre-entry reading: Establish a monitoring plan and retest or continuously monitor as conditions require.
  • Choosing sensors by habit: Select the instrument for the contaminants and work process identified in the evaluation.
  • Calling a rescue service that cannot access the space: Give rescuers hazard information and access for planning and practice before the entry.
  • Assuming retrieval always works: Check geometry, obstructions and line routing; document when retrieval would increase risk or would not contribute.
  • Allowing an improvised rescue: Train attendants to summon the designated service and prevent unauthorized entry.
  • Leaving permit changes undocumented: Stop work, reassess the space and task, correct the control and reauthorize entry.

  • Correct federal and state-plan rule identified for the work and jurisdiction.
  • Space, task, configuration and all actual or potential hazards assessed.
  • Energy, process, engulfment and inflow hazards isolated or otherwise controlled.
  • Acceptable entry conditions written in measurable terms.
  • Calibrated direct-reading instrument selected for the identified hazards.
