Transportation agencies must reduce risks before failure, warn people fast enough to protect lives, keep a disrupted network moving, and coordinate a safe, funded recovery. The 2024 Francis Scott Key Bridge collapse exposed gaps and challenges across those stages; Pittsburgh’s 2022 Fern Hollow collapse showed why inspection findings and maintenance follow-through are part of emergency readiness too.
What happens when a bridge collapses?
There is no single response sequence that fits every incident, but a capable transportation operation must connect immediate life safety with traffic control, regional coordination, and recovery. Federal Highway Administration (FHWA) guidance treats transportation as a partner in emergency response—not merely the owner of a closed road. It calls for transportation representation at the incident scene, the transportation management center, and the emergency operations center, supported by current plans, joint exercises, interoperable communications, and resources for both response and recovery. FHWA’s Emergency Transportation Operations guidance describes the goal this way: “Transportation is prepared for its role and to deliver its capabilities, which are well understood by all.”
- Reduce risk before failure. Assess structural condition and external hazards, assign responsibility for corrective work, and plan and fund appropriate risk-reduction measures.
- Protect life and control access. Alert bridge operators, responders, workers, and road users; establish incident command; close threatened approaches and organize rescue operations.
- Manage the transportation consequences. Reroute traffic, provide traveler information, coordinate with neighboring agencies, and address freight, port, and other regional effects.
- Recover and restore. Handle debris and damage assessment, determine funding and permitting needs, procure reconstruction work, and plan for a safe return to service and readiness.
These stages overlap. The speed and quality of the first warnings, for example, depend on communications and procedures planned well before an emergency.
How did the Key Bridge collapse test emergency plans?
The Francis Scott Key Bridge in Baltimore collapsed on March 26, 2024, after the container ship Dali struck it. The National Transportation Safety Board (NTSB) found that a loose signal-wire connection at a terminal block, resulting from improper installation of wire-label banding, caused the vessel to lose electrical power. It consequently lost propulsion and steering near the bridge. The NTSB’s investigation also identified bridge vulnerability and warning issues with direct implications for emergency planning. The NTSB investigation and recommendations include findings issued in 2025.

Risk reduction must account for hazards beyond deterioration
The NTSB concluded that the absence of countermeasures to reduce the bridge’s vulnerability to vessel impact contributed to the collapse and loss of life. It called for bridge owners to assess vessel-impact risk and adopt short- and long-term strategies when risk exceeds the specified threshold. In practical terms, emergency readiness should include asking what could disable or damage an asset, how severe the consequences could be, and which protective measures are warranted—not only whether the bridge is in acceptable condition during routine inspection.
Warnings must reach workers as well as drivers
The NTSB found that ineffective and non-immediate communication to notify highway workers to evacuate contributed to loss of life. Its recommendations include a pilot-to-shore emergency contact procedure and consideration of motorist warning systems that could warn drivers and prevent vehicles from entering a threatened bridge. These are recommendations; they do not establish that every named agency has implemented them. Plans need clear evacuation triggers, readily available contact information, and a way to reach people both on the roadway and in work zones before a threat becomes a collapse.

How do agencies warn drivers and workers if a bridge is in danger?
A warning plan is useful only if the right person can send it quickly and the intended recipients can act on it. Agencies can use the NTSB’s Key Bridge recommendations as prompts to check whether their procedures cover the following:
- Who can initiate an alert? Specify how vessel pilots, bridge operators, dispatchers, law enforcement, and other relevant personnel contact shoreside support or emergency response.
- Who must receive it? Include highway workers and contractors, not just drivers and emergency agencies.
- What action follows? Define when to evacuate a work zone, close approaches, stop traffic, or keep people away from the structure.
- How is access controlled? Evaluate warning and traffic-control systems that can keep vehicles from entering a bridge at risk.
- Has the process been practiced? Exercise the notification chain with transportation and public-safety partners, then address failures found in the exercise.
The NTSB recommendations identify possible actions; the evidence does not show a universal system or procedure already operating at every bridge. The relevant test for an agency is whether its own hazard assessment, contacts, triggers, equipment, and partners are established and practiced.

What happens to traffic, freight, and port operations after a major bridge closes?
A closure can redirect vehicles onto other roads and affect freight routes and nearby infrastructure. Plans therefore need preplanned alternate routes around major assets and chokepoints, traffic-management actions, traveler information, and communications between transportation centers and emergency operations. FHWA guidance also calls for identifying personnel and equipment resources, integrating intelligent transportation systems where appropriate, and maintaining interoperable communications with response partners. FHWA’s transportation emergency operations guidance frames these as capabilities to prepare and coordinate, rather than tasks to improvise after an incident.
In Baltimore, the recovery effort extended well beyond closing the bridge. FHWA’s July 10, 2024 testimony describes work with Maryland transportation agencies, the City of Baltimore, the Coast Guard, the Army Corps of Engineers, NOAA, and other partners on traffic, supply-chain impacts, and safely reopening the port. FHWA announced $60 million in initial quick-release Emergency Relief funding on March 28, 2024, after Maryland’s transportation department requested assistance. Its testimony also described coordination on reconstruction procurement, project delivery, environmental review, and permitting. These are dated accounts of the response, not a statement of the project’s present status. USDOT’s July 2024 testimony on the response documents that coordination.

What Fern Hollow shows about maintenance and oversight
The Fern Hollow Bridge collapse in Pittsburgh on January 28, 2022, followed a different pathway from the Key Bridge. It was a deterioration and maintenance failure, not a vessel strike. The NTSB reported that clogged drains allowed water and debris to collect along the bridge legs, contributing to corrosion and section loss in a fracture-critical transverse tie plate. Repeated inspection reports recorded maintenance and repair recommendations that the City of Pittsburgh did not act on. The NTSB also identified poor inspection quality, incomplete identification of fracture-critical members, incorrect load-rating calculations, and insufficient PennDOT oversight of the city’s inspection program. The NTSB’s Fern Hollow investigation details these findings.
The comparison matters because an emergency plan cannot make up for inspection findings that are never translated into action. Bridge owners and oversight bodies need processes that assign maintenance responsibilities, track recommendations to completion, and elevate serious deterioration or risk. The cases point to distinct hazards—vessel impact at the Key Bridge and corrosion and neglected repairs at Fern Hollow—but both show how readiness depends on decisions made before an emergency.

How to assess a transportation agency’s bridge emergency readiness
For a construction or transportation program, the following questions help reveal whether the plan covers the full chain from prevention through recovery:
- Risk reduction: Have structural and external hazards been assessed? Are required countermeasures prioritized, funded, and assigned to an owner?
- Inspection follow-through: Are findings, load-rating work, and repair recommendations accurate, assigned, tracked, and escalated when overdue?
- Life safety: Can people in work zones and on approaching roads be notified immediately? Are evacuation triggers and access-control actions clear?
- Roles and interoperability: Are transportation responsibilities defined at the incident scene, transportation management center, and emergency operations center? Can partners communicate, and do they exercise together?
- Network continuity: Are alternate routes, traffic operations, traveler information, and freight or port impacts included?
- Recovery: Does the plan account for coordination, funding, debris and damage work, permits, procurement, reconstruction, and a safe return to readiness?
FHWA’s August 2009 publication on transportation evacuation preparedness organizes work into preparation and activation, response, and re-entry and return to readiness. It can help frame a timeline, but it is an older workshop publication—not a current bridge-collapse-specific standard. FHWA’s 2009 evacuation-preparedness publication offers that broader planning structure.
What older FHWA planning figures can—and cannot—tell you
An archived FHWA publication dated April 2004 reported historical gaps in surveyed emergency plans. Fewer than 50 percent included details on media coordination, traveler information, and asset protection; 10 percent addressed transportation coordination with the Emergency Operations Center; and fewer than half specified evacuation routes. The publication also said that only two thirds of state plans and one third of municipal plans included transportation contacts. These figures describe plans surveyed at that time, not current national readiness and not bridge-collapse plans specifically. FHWA’s archived April 2004 transportation response publication is the source for the dated findings.
