For adults with chronic, nonspecific lower-back pain, a medium-firm mattress is a better-supported place to start than a very firm one. That is a general starting point—not a universal prescription, a guarantee of pain relief, or a reason to delay appropriate care. The evidence does not identify a best mattress brand or currently available South African model.
What the evidence says about mattress firmness
Medium-firm has the clearest starting-point evidence
A randomized, double-blind, multicentre trial followed 313 adults with chronic nonspecific low-back pain for 90 days. Participants reported backache in bed and on rising, but did not have referred pain. The trial compared a firm mattress, rated Hs 2.3 under the European Committee for Standardisation scale, with a medium-firm mattress, rated Hs 5.6. At 90 days, the medium-firm group had better outcomes for pain in bed and disability; the estimate for pain on rising was less conclusive. The authors concluded: “A mattress of medium firmness improves pain and disability among patients with chronic non-specific low-back pain.” The Lancet trial, 2003.
The result challenges the assumption that an “orthopaedic” or very firm mattress is automatically better for back pain. It applies to the trial’s specific population and comparison; it does not show that a particular modern mattress will produce the same result.
Reviews support the direction, not a one-size-fits-all rule
A 2026 systematic review of 47 studies found that evidence most consistently favored medium-firm mattresses over very firm or very soft surfaces. Differences in mattress materials, outcome measures, follow-up periods, and participant characteristics prevented the reviewers from combining results in a meta-analysis. They also noted that body habitus, age, sex-related anthropometry, preferred sleep position, and thermal comfort can matter when choosing a surface. A 2017 systematic review of 24 controlled-trial articles likewise found evidence that subjectively medium-firm and custom-inflated mattresses may support comfort, sleep quality, and spinal alignment. It found insufficient evidence to determine an appropriate mattress temperature. Sleep Health review, 2017.
How to compare mattresses in South Africa
Use “medium-firm” as a shortlist filter, then compare the actual options against your comfort and practical needs. Firmness labels are subjective, and differences in materials mean that two mattresses described the same way may not feel alike.

- Firmness and assessment: Check the retailer’s own firmness description and, where available, try the mattress in-store or use an at-home comfort assessment.
- Body shape and sleep position: Consider whether the surface feels comfortable for your body habitus and the position in which you usually sleep. The 2026 review identifies these as relevant selection factors, not as a formula that determines a single correct firmness.
- Temperature: Consider whether the mattress feels comfortable to sleep on. The 2017 review found the evidence insufficient to specify an ideal mattress temperature.
- Dimensions: Check that the mattress size suits your bed and sleeping arrangements.
- Purchase terms: Confirm current delivery coverage, trial arrangements, warranty, and return terms directly with the retailer before buying.

Mattress choice may affect comfort and symptoms for some people, but the available evidence does not establish a mattress as a cure for lower-back pain. Nor does it establish that a smart or adaptive mattress should be routinely recommended: the 2026 review describes those systems as promising but says larger, methodologically rigorous trials are needed.
