Can Toothbrushes or Showerheads Make You Sick? What to Know and How to Clean Them

Toothbrushes and showerheads can harbor microbes, but typical household exposure is not proof of infection risk. Here is how to care for brushes and water fixtures using ADA and CDC guidance.

Toothbrushes and showerheads can harbor microbes, but finding bacteria or biofilm on them does not mean they are likely to cause illness. The American Dental Association (ADA) says toothbrush bacteria have not been shown to cause adverse health effects; the Centers for Disease Control and Prevention (CDC) says most healthy people do not get sick from germs in water-pipe slime. Routine care is usually enough for a household: let toothbrushes dry between uses, and flush or clean water fixtures when CDC guidance or visible buildup calls for it.

Can a toothbrush or showerhead make you sick?

Both can harbor microbes, but the evidence does not justify treating ordinary household toothbrushes and showerheads as major infection hazards for most healthy people. The ADA says toothbrushes have been shown to harbor bacteria, but there is no evidence those bacteria cause adverse health effects. CDC says most healthy people do not get sick from germs living in water-pipe slime.

The possible exposure routes differ. A toothbrush contacts the mouth during brushing. Microbes in water-device biofilm can be released when water flows; exposure may occur by breathing mist, through an open wound, or through the nose in particular circumstances. CDC does not provide a household infection rate for showerheads, and the ADA does not provide an infection rate attributable to toothbrush bacteria.

This higher-risk guidance concerns possible exposure to waterborne germs; it does not mean a household showerhead is known to be making someone ill. People responsible for healthcare water systems should follow healthcare-specific guidance rather than applying it as a routine home-cleaning rule.

How to care for a toothbrush

  • Choose a soft-bristled brush and use gentle pressure. ADA guidance says manual and powered toothbrushes can both be effective.
  • After brushing, rinse the brush thoroughly with tap water.
  • Store it upright and uncovered, and let it air-dry between uses.
  • Do not share toothbrushes.
  • Replace a toothbrush about every three to four months, or sooner if its bristles are matted or frayed.

Do not microwave a toothbrush or put it in a dishwasher: high heat may damage it. Routine sanitizing is not established as necessary. The ADA describes one study in which soaking brushes in 3% hydrogen peroxide or Listerine reduced bacterial load by 85%; that finding concerns the amount of bacteria, not proven prevention of illness. The ADA also notes limited data on toothbrush sanitizing. If considering a sanitizer device, look for one cleared by the U.S. Food and Drug Administration.

How to flush and clean a showerhead

Flush after a long period without use

CDC says biofilm organisms may be released when a faucet or showerhead is turned on, and this is more likely after a device has been unused for a while, such as a week or longer. After that kind of break, run the water fully on cold for two minutes, then switch to hot and let it run until it starts to feel hot. CDC also advises flushing each faucet and shower in the home after a drop in water pressure.

Remove visible buildup and maintain filters

Clean a showerhead or faucet aerator when buildup is visible. Depending on the fixture, removal and soaking in a solution such as white vinegar may help remove buildup; follow the manufacturer’s cleaning directions. Maintain and replace showerhead and other water filters according to their manufacturers’ instructions, since germs can grow in filters that are not properly maintained or replaced.

Does a household fixture need disinfecting?

Not routinely just because it may harbor germs. For most household surfaces, CDC says cleaning with soap and water removes most germs in most situations. Household disinfection is generally unnecessary unless someone is sick or has recently visited while sick. If disinfecting is appropriate, clean the surface first and follow the product-label directions.

Healthcare settings have distinct water-system risks and controls. CDC healthcare environmental-control guidance discusses showerhead aerosols and waterborne pathogens, including Legionella-related concerns in higher-risk patient-care settings; it does not generally recommend additional monthly showerhead and aerator cleaning or disinfection in routine patient-care settings. Those specialized considerations are not a reason to apply healthcare water-system procedures to an ordinary home.
