Uneven Leg Length: How to Assess It and Choose a Correction

A difference in leg length should be assessed before choosing a correction. Learn how clinicians measure it and when observation, a shoe lift, or specialist care may be considered.

If your legs look or feel uneven, don’t choose a shoe lift based on appearance alone. A clinician can determine whether there is a measurable structural difference, assess how it affects standing and walking, and consider the cause, symptoms, age, and remaining growth before discussing treatment. Observation or a fitted lift may suit some people; larger discrepancies may warrant specialist discussion of surgery.

What “uneven legs” can mean

A difference in leg length may be structural, involving the bones, or apparent rather than a true bone-length discrepancy. The way someone stands or walks, pelvic position, and other factors can affect how even the legs look. A visible or perceived difference alone does not establish a diagnosis.

The American Academy of Orthopaedic Surgeons (AAOS) notes that more than 50% of people have a subtle leg-length difference, and that mild variations usually do not cause problems. AAOS discusses a difference greater than 2 cm in relation to possible effects on well-being and quality of life; this is not a universal threshold for treatment. AAOS: Lower Limb Length Discrepancy

Research has not established a simple, reliable link between every measured difference and back, hip, or knee problems. A 2003 clinical review describes disagreement about how common clinically significant discrepancies are and uncertainty about their relationship to orthopedic conditions. How a clinician assesses leg length

Assessment may include observing posture and gait, checking pelvic level, and measuring the difference. A tape measurement at home cannot reliably distinguish a structural bone-length discrepancy from other reasons the legs may appear uneven.

Standing assessment and blocks

For children, AAOS describes examining the patient standing barefoot and placing measured blocks under the shorter side until the hips are level. This helps estimate the difference while accounting for how the person stands. Imaging when more precision is needed

If a more precise measurement is needed, imaging may include leg X-rays or a scanogram. AAOS notes that CT may be used in complex cases, but it involves more radiation exposure than X-rays. A clinician decides whether imaging is appropriate. AAOS: Leg Length Discrepancy

What affects the choice of correction

There is no single correction that fits every measured difference. The decision depends on whether the discrepancy is structural, its size and cause, whether it is associated with symptoms or functional difficulty, and whether the person is a child with growth remaining or an adult. A measured difference alone does not dictate treatment.

Observation

For children with minor discrepancies and no angular deformity, AAOS describes observation as an option. A clinician may monitor changes as the child grows rather than correcting a difference that is not causing meaningful problems. AAOS: Leg Length Discrepancy

Shoe lifts

A clinician may consider a lift in or on a shoe for selected cases. Whether it helps depends on the person and the discrepancy; a lift is not a diagnostic tool or a guaranteed fix. The lift approach—an in-shoe insert or an external shoe modification—should be considered in light of the correction needed, comfort, and shoe stability. Do not infer a lift height from an online measurement or a general treatment category.

Surgery

Surgery is a specialist option for selected cases, particularly larger discrepancies or children whose growth can still be taken into account. AAOS describes approaches that may slow growth of the longer limb, shorten it, or lengthen the shorter limb; each has different indications and trade-offs. What is known about lifts and pain relief in adults

A 2018 systematic review of adults with painful musculoskeletal conditions included 10 studies, only one of which was randomized. In the non-randomized studies, 88% ± 3% of 349 participants treated with lifts reported partial or complete pain relief. In the randomized-trial group, all 22 participants treated reported pain relief, with a mean reduction of 27 ± 9 mm on a 150 mm visual analog scale. The review authors rated the overall evidence as low quality, so these results do not predict whether a lift will help a particular person. When to seek an assessment

Consider discussing the difference with a healthcare professional if it is persistent, affects walking or standing, or is accompanied by pain or functional difficulty. Assessment is also important before using a substantial lift or considering a corrective procedure. A clinician can determine whether there is a structural discrepancy and whether observation, a lift, or specialist evaluation is appropriate.
