Nuclear construction in the United States is a layered control system, not a matter of simply “building to a code.” For safety-related work, federal requirements establish the obligations; the licensee’s approved quality-assurance (QA) program turns those obligations into procedures; an applicable construction code supplies technical rules for covered items; and traceability preserves evidence from procurement through final acceptance.
The governing U.S. framework is 10 CFR Part 50, Appendix B. It covers activities that can affect safety-related functions and requires controlled design, purchasing, fabrication, installation, inspection, testing, records, corrective action, and audits. This explanation is U.S.-specific; other countries can use different regulators, codes, classifications, and approval processes.

How the four layers fit together
A useful way to understand nuclear construction is to follow four layers that must work together:

| Layer | What it does | Typical evidence |
|---|---|---|
| Regulation | Sets enforceable obligations for activities affecting safety-related functions. | Applicable federal requirements, license conditions, and approved commitments. |
| Licensee QA program | Defines the organization, procedures, authorities, reviews, inspections, records, and corrective-action processes used to meet those obligations. | QA manual, implementing procedures, audit reports, qualification records, and controlled work documents. |
| Applicable technical code | Provides detailed engineering and construction rules for items and activities within its scope. | Code calculations, material requirements, welding rules, examinations, tests, and certifications. |
| Traceability and records | Connects an item’s identity and history to the requirements used to procure, make, install, inspect, test, and accept it. | Material certificates, heat or serial numbers, inspection reports, test results, nonconformance dispositions, and turnover records. |
These layers are complementary. A code does not replace the regulation or the licensee’s QA program, and a QA standard does not by itself authorize construction. The project’s licensing basis determines which requirements, classifications, code editions, procedures, and records apply.

What 10 CFR Part 50, Appendix B governs
Scope: activities affecting safety-related functions
Appendix B establishes QA requirements for the design, manufacture, construction, and operation of covered structures, systems, and components. Its criteria apply to activities affecting the safety-related functions of those items. The listed activities include design, purchasing, fabrication, handling, storage, erection, installation, inspection, testing, operation, maintenance, repair, refueling, and modification. The practical question is not whether work occurs inside a nuclear facility; it is whether the work can affect a covered item’s safety-related function.
The regulation defines QA as “all those planned and systematic actions necessary to provide adequate confidence that a structure, system, or component will perform satisfactorily in service.” Quality control is part of QA and concerns physical characteristics measured against predetermined requirements. That distinction matters: inspection of a weld is one control, while the broader QA program also controls the design basis, procurement, personnel qualification, records, audits, and corrective action around that weld.
The licensee remains accountable
An applicant or licensee may delegate work that establishes or executes the QA program to contractors or other organizations, but it retains responsibility for the program. Delegation therefore changes who performs a task, not who must demonstrate that the task was controlled.

QA personnel must have sufficient authority and organizational freedom to identify quality problems, initiate or recommend solutions, verify that solutions were implemented, and stop or escalate work when necessary. Appendix B also calls for verification—through checking, inspection, or audit—that safety-affecting work was correctly performed. This independence is intended to prevent schedule or cost pressure from overriding a safety-related decision.
How the QA program turns requirements into work
Design controls and design changes
Design controls translate regulatory requirements and the design basis into specifications, drawings, procedures, and instructions. The project specifies quality standards, controls deviations, manages interfaces between organizations, and verifies design adequacy using appropriate reviews, tests, or checks.

Changes receive controls commensurate with the original design. That includes field changes: a modification made during installation cannot be treated as informal work-around simply because the original drawing is already issued. The change must be evaluated, approved, documented, and incorporated into the information used for inspection, testing, operation, and future maintenance.
Procurement requirements
Procurement documents must include or reference the requirements needed to obtain adequate quality. Depending on the item, they can identify the applicable code, material specification, design conditions, inspection points, testing, documentation, qualification requirements, and controls on sub-suppliers.
Supplier selection and monitoring use established methods. The licensee or its authorized organization evaluates sources, obtains objective evidence of quality, performs source inspections when appropriate, and examines items on receipt. Documentary evidence showing conformity must be available at the site before material or equipment is installed or used, and the evidence must identify the codes, standards, specifications, or other requirements met.
Controlled instructions and interfaces
Work affecting quality is performed under documented instructions, procedures, or drawings that state acceptance criteria. Interfaces are controlled when design, procurement, fabrication, installation, inspection, and testing are divided among organizations. A handoff is not complete until the receiving party has the current requirements, the item identity, the status of inspections and tests, and any open conditions that could affect acceptance.
Construction codes, QA standards, and regulatory authority
Readers often encounter two different standards families. They answer different questions and should not be treated as alternatives.

NRC Regulatory Guide 1.28, Revision 6 identifies NQA-1-2017, NQA-1-2019, and NQA-1-2022 as current design-and-construction guidance, subject to stated exceptions and clarifications. ASME also lists an NQA-1-2026 edition as an available publication. Those are different facts: a publisher can offer a newer edition while the NRC endorsement and a plant’s approved QA program remain tied to particular editions and conditions.
Do not tell a project to adopt the newest edition automatically. Confirm the license, approved QA program, regulatory commitments, contract requirements, and any NRC exceptions or clarifications before selecting an edition. Purchasing a standard is a reference decision, not proof of compliance.
What traceability means in practice
Appendix B Criterion VIII requires measures for identifying and controlling materials, parts, components, and partially fabricated assemblies. Identity can be maintained by a heat number, part number, serial number, or another appropriate method, either marked on the item or carried in records that trace to it. The identity is maintained through fabrication, erection, installation, and use as required.
Traceability is therefore an evidence chain, not merely a barcode or tag. A record set can connect a procurement requirement to a material certificate, receipt inspection, controlled fabrication steps, qualified personnel, inspection and test results, nonconformance dispositions, installation status, and final acceptance.

An illustrative component journey
The following is a generic example, not a reported project experience. Imagine a safety-related pressure-boundary component whose classification and licensing basis require a particular construction code.
- Define requirements. Design documents identify the function, design conditions, applicable code, material specification, acceptance criteria, and required examinations. The procurement package passes those requirements to the supplier.
- Receive and identify material. At receipt, personnel compare the shipment with the purchase order, examine condition, review required certificates, and record the heat, part, or serial number. If the identity is missing or the documentation is incomplete, the item is controlled rather than released for installation.
- Fabricate under qualified controls. Welding, heat treatment, and nondestructive examination use qualified procedures and qualified personnel consistent with the applicable code, specifications, and project requirements. Process records retain the link to the component identity.
- Inspect and test. Independent inspectors verify work against current drawings and procedures. Written test procedures state the design-based requirements and acceptance limits; results identify who performed or observed the work and whether the result was acceptable.
- Resolve discrepancies. A deviation is documented as a nonconformance, evaluated by authorized personnel, and either corrected, accepted under an approved disposition, or rejected. The disposition remains linked to the item and affected records.
- Install and preserve status. Installation records show the component’s location, orientation, status of required inspections and tests, and any hold-point releases. Storage and handling controls protect the item and its identity until use.
- Accept and retain evidence. Final records demonstrate that specified requirements were met, identify outstanding actions if any, and remain retrievable for operation, maintenance, future modification, and regulatory review.
Field controls that protect quality
Special processes
Some results cannot be fully verified by inspecting the finished product alone. Welding, heat treating, and nondestructive testing are examples of special processes. Appendix B requires qualified personnel and qualified procedures, with controls consistent with applicable codes, standards, specifications, and project requirements. Qualification records must remain available as evidence that the process was performed within its approved envelope.
Independent inspection and hold points
Inspections verify work against controlled instructions and are performed by people other than those who performed the inspected activity. When a required hold point exists, the governing documents identify it and work does not proceed past that point without the designated representative’s consent. A witness point may require notification and availability, while a hold point prevents continuation until release; the project procedure must define the distinction.
Testing and measuring equipment
Tests use written procedures with design-based requirements and acceptance limits. Results are documented and evaluated rather than filed without a disposition. Measuring and test equipment is identified, calibrated, and adjusted at specified intervals. If equipment is found out of calibration, the program evaluates the validity of previous measurements and takes appropriate action on affected results.
Purchased items and services
Supplier controls continue after contract award. Source evaluations, objective quality evidence, source inspections where appropriate, and receipt examinations provide separate checks on conformity. The site must have the required documentary evidence before an item is installed or used, so a delivery cannot be treated as acceptable solely because it arrived on schedule.

Nonconformance, corrective action, and audits
Controlling a nonconforming item
Nonconforming items are identified and documented, controlled or segregated as appropriate, and given an authorized disposition. A disposition can require rework, repair, rejection, or acceptance under a documented technical justification when the program permits it. Affected organizations are notified when the condition could influence their work or the item’s safety-related function.
Correcting significant adverse conditions
Significant adverse conditions require determination of cause, corrective action, documentation, and management reporting. Effective corrective action addresses why the condition occurred and checks whether the remedy worked; simply replacing a defective part may not prevent recurrence if the underlying procurement, procedure, training, or oversight problem remains.
Records and audits
Appendix B requires sufficient records to furnish evidence of activities affecting quality. Records are identifiable, retrievable, protected from damage or loss, and retained for the period established by applicable requirements. They should allow a reviewer to reconstruct what was required, who performed or verified the work, what result was obtained, how deficiencies were handled, and why the item was accepted.

A practical review checklist
Before releasing safety-related construction work, a project team can ask:
- Is the item or activity clearly linked to a safety-related function and the applicable licensing-basis requirements?
- Are current design inputs, drawings, procedures, acceptance criteria, and approved changes available at the work location?
- Does the procurement package identify the required code, standards, specifications, inspections, tests, and supplier documentation?
- Can the material or component be identified by heat, part, serial, or equivalent controlled records from receipt through installation?
- Are special processes performed by qualified people using qualified procedures?
- Are inspections independent of the people who performed the inspected activity, and are hold points released by the authorized representative?
- Are test procedures, calibrated equipment, results, and evaluations documented?
- Are nonconformances identified, segregated or controlled, dispositioned, and communicated to affected parties?
- Are records complete, legible, protected, and retrievable for later operation, maintenance, modification, or regulatory review?
- Has the selected code and QA-standard edition been checked against the license, approved QA program, NRC guidance, exceptions, and project commitments?
U.S. boundaries and project-specific decisions
This framework explains the U.S. NRC model, not a universal definition of nuclear construction. Other jurisdictions may assign responsibilities differently or adopt different codes and QA standards. Even within the United States, the precise code edition, component classification, inspection hold points, record-retention period, and supplier controls depend on the plant’s licensing basis and approved program.
The eCFR page for Appendix B was displayed as current through October 8, 2026. NRC guidance and standard availability can change, so project teams should verify the current regulatory text, applicable Regulatory Guide 1.28 position, license commitments, and contract requirements immediately before relying on them.

Bottom line
Nuclear construction quality is demonstrated by a connected chain of obligations and evidence. Regulation defines what must be controlled; the licensee’s QA program assigns authority and methods; an applicable code defines technical construction rules for covered items; and traceability proves that the right material and process produced the accepted installation. If any link—identity, qualification, inspection, testing, nonconformance control, or records—is missing, the project cannot show the same level of confidence that the safety-related function will be performed in service.
