Mining Construction Risk Checklist Before Breaking Ground

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Do not authorize excavation, heavy equipment or site mobilization until a documented hold-point review confirms that permits, responsibilities, ground information, geotechnical controls, excavation protections, equipment plans, environmental measures and emergency arrangements are ready. The checklist below is a practical release gate for mine construction, but it must be adapted to the host country’s law, the mine type, the construction scope and hazards found at the specific site.

Use a staged hold-point gate, not a one-time sign-off

Set a formal “ready to break ground” gate in the project schedule. A competent person releases each stage only after reviewing evidence, recording open actions and confirming who has authority to stop work. Re-run the gate after a design change, new ground information, severe weather, an incident, or a change in contractors or major equipment.

Hold point Release question Evidence to retain
Governance and design Are accountable people, design risks, permits and the construction-phase plan established? Named appointments, design-risk register, approved plans, permits and authority notifications
Ground and services Are ground conditions, historical workings, utilities, contamination and water hazards understood and controlled? Surveys, utility clearances, geotechnical and mine-risk reports, maps and control drawings
Work methods and plant Can excavation, lifting, traffic, access and temporary works be performed with defined protections? Method statements, lift and erection plans, inspection records, equipment certificates and training records
People and environment Are workers, visitors, neighbors and the environment protected before work starts? Inductions, PPE assessments, barriers, traffic plans, environmental controls and communication records
Emergency readiness Can the site detect, communicate, evacuate and medically support a credible incident? Emergency plan, maps, contact lists, drills or briefings, gas-monitoring arrangements and accountability records

Define the evidence required to release each gate, the person who signs it, stop-work authority, change-control rules, incident-reporting routes and the content of daily pre-start briefings. A signature without objective evidence is not a control.

1. Establish governance, design information and legal permissions

Assign accountable duty holders

Name the client, principal designer or equivalent, principal contractor, competent geotechnical lead, safety lead, environmental lead and emergency coordinator. Record their authority, competence, deputies and interfaces with contractors. For a multi-contractor project, the principal contractor should own the construction-phase safety-management plan before construction begins.

Build usable pre-construction information

Compile drawings, surveys, existing reports, land and access constraints, neighboring activities and the design-risk register into a controlled package that crews can use. Site history must cover more than the orebody: investigate asbestos and other contaminants, overhead power lines, buried services, unusual ground, public rights of way, nearby schools, roads or railways, and adjacent industrial operations. HSE notes that gathering health and safety information about the proposed site before work begins is important.

Confirm every required permit and notification

Before mobilization, obtain legally required construction, environmental, water, land-access, blasting, utility and mining-related permits. Check permit conditions for working hours, discharge limits, monitoring, rehabilitation, explosives storage, traffic routes and inspection notices. Notify regulators, utilities, emergency services and affected communities where the law or permit requires it; retain acknowledgements rather than relying on an email in an individual’s inbox.

Control design and scope changes

Freeze the issued-for-construction information for the first work package. For each change, identify new ground loading, excavation, lifting, environmental or public-interface risks, obtain the required designer and geotechnical review, and re-release the affected hold point. Include stop-work criteria in the construction plan so any worker can halt an unsafe activity without waiting for a management meeting.

2. Resolve ground, geotechnical and historical-mine uncertainty

Verify bearing and drainage before positioning equipment

Inspect equipment areas, crane pads, laydown zones and haul roads for slope, compaction, firmness and drainage. OSHA’s 29 CFR 1926.1402(b) states that equipment must not be assembled or used unless ground conditions are firm, drained and graded sufficiently to support the equipment and materials. Where native ground is inadequate, design mats, cribbing, engineered fill or other support; do not treat a visual walk-over as proof of capacity.

Investigate what is below and around the work

Reconcile historical drawings, topographic and utility surveys, boreholes, soil information and field verification. Identify voids, tanks, buried structures, weak layers, groundwater, slope instability, subsidence and unusual ground. Mark uncertainty on the work-area plan and define what discovery—such as an uncharted void, water inflow or unexpected fill—requires evacuation and reassessment.

Address legacy workings and subsidence

If historical mining may have occurred, commission a competent desk-based and, where necessary, intrusive coal- or mine-risk assessment. UK guidance recommends that the assessment identify further investigation, remedial works and mitigation, with sign-off by a chartered engineer or geologist who has at least three years of ground-investigation experience. The assessment should cover shafts, adits, stopes, mine entries, spoil, gas pathways and recorded subsidence, not merely the planned excavation footprint.

Plan movement monitoring and trigger actions

For open-pit or slope work, map the potentially affected area and assess mining-induced movement, consequences for people and the public, infrastructure, property, land and the environment. Establish inspection frequencies, survey or instrument locations, exclusion zones and a trigger-action-response plan. State the measured trigger, who receives the alarm, what immediate action follows, and who can authorize re-entry. Recheck temporary slopes, embankments, fills and support after rain, blasting, dewatering or a change in loading.

3. Control excavation, utilities and hazardous atmospheres

Clear services before digging

Locate overhead lines and underground services from records, utility-owner information and field verification. Positively identify the service at the point of work, mark exclusion zones and issue a permit-to-dig. The permit should name the dig limits, approved method, isolation or protection requirements, competent spotter and expiry or revalidation conditions. Hand expose or otherwise verify services where the method statement requires it; never infer a cable’s position from an old plan alone.

Select excavation protection and safe access

For each cut, document the soil or rock assessment, slope or bench geometry, shoring or shielding, spoil setback, water-control method and access/egress. Assign a competent inspector and list conditions requiring immediate evacuation, such as cracking, sloughing, unexpected water, gas, loss of support or a damaged protective system. Keep ladders, ramps or other exits positioned so workers do not have to climb unstable faces or cross moving plant.

Test and ventilate the atmosphere

Where hazardous or oxygen-deficient conditions exist or could reasonably be expected, test before entry and continue monitoring as the hazard changes. OSHA specifies testing before entry into excavations deeper than 4 feet (1.22 m) in those circumstances. Define the detector’s gases and range from the hazard assessment, bump-test and calibration requirements, alarm set points, sampling locations and response to an alarm. Provide ventilation, rescue and communications suitable for the space; a detector is not a substitute for controlling the source.

4. Make plant, lifting, roads and site layout work together

Design the site for separation and reliable access

Provide access roads, pedestrian routes, turning areas, drainage, lighting and stable equipment platforms before deliveries begin. Separate people from mobile equipment with physical barriers or controlled crossings, set speed and reversing rules, and keep emergency routes clear. OSHA’s steel-erection requirements illustrate the baseline: access roads and a firm, graded, drained work area must be available for the operation.

Plan every significant lift

Coordinate crane setup, ground bearing, outriggers, overhead obstructions, load paths, delivery timing, staging and simultaneous work. Define what makes a lift critical and require a lift plan, qualified operator, appointed lift director, inspected rigging, communications and an exclusion zone. A delivery schedule that prevents trucks, cranes and other trades from competing for the same route is a safety control, not just a logistics preference.

Control mobile equipment

Verify operator competence, pre-use inspections, maintenance, guarding, seat-belt use, visibility aids and reversing controls. Set interaction rules for haul trucks, light vehicles, pedestrians and contractors unfamiliar with mine traffic. MSHA identifies mobile equipment as a core mining-safety topic; apply that discipline to construction fleets as well as production equipment.

Issue a site-specific erection plan where needed

Where site conditions require alternate fall-protection or erection methods, have a qualified person prepare a site-specific plan and keep it available at the worksite. Include sequence, temporary stability, weather limits, connection points, rescue arrangements and interfaces with excavation, cranes and other trades.

5. Protect workers, the public and the environment

Match PPE to the task and hazard

Complete a task-based assessment and issue PPE that fits the exposure and other equipment. Good-practice examples include hard hats, safety glasses, masks where respiratory hazards exist, safety boots and harnesses for defined fall exposures. Specify compatible eye, hearing, respiratory and fall-protection equipment, inspection duties, replacement criteria and training; PPE does not replace elimination, isolation or engineering controls.

Control access and communicate the rules

Install signs, barriers, lighting, controlled entry and visitor induction. Post emergency contacts, muster points, traffic rules, restricted areas and permit conditions in languages workers understand. Account for contractors and visitors through check-in/check-out and shift handover controls. Provide a channel for workers and neighbors to report hazards, complaints and incidents without delay.

Manage dust, noise, vibration, runoff and waste

Control dust from excavation, drilling and stockpiles with water or another suitable method selected for the material and climate. Set controls for noise and vibration near homes, roads, railways and sensitive structures. Protect drains and watercourses from sediment and contaminated runoff; segregate, label and dispose of waste and contaminated materials under the environmental plan and permit. IFC guidance says applicability should be tailored to hazards and risks identified for each project through environmental assessment.

Coordinate community notifications

Notify authorities and affected communities of upcoming works when required. Explain haul routes, blasting or noisy periods, access restrictions, dust controls and the incident-communication route. Record who was notified, when, by which method and how concerns will be answered.

6. Make emergency readiness a release condition

Build a mine-and-construction emergency plan

Maintain current mine and construction maps showing excavations, shafts or workings, utilities, gas-monitoring points, muster areas, access routes and isolation points. List emergency contacts, the incident commander, deputies, radio channels and backup communications. Confirm medical, ambulance and rescue arrangements, travel times, site access for responders and arrangements for casualties from remote work fronts.

Prepare for credible scenarios

Write actions for collapse, subsidence, inundation, fire, gas alarm, utility strike, vehicle collision, lifting failure, severe weather, blasting incident and environmental release where those hazards exist. State when to evacuate, who accounts for people, how an area is isolated and who communicates with authorities, families and the community. Gas monitoring, maps, communications and command functions are central elements in MSHA emergency guidance.

Test accountability and communications

Run a briefing, drill or practical test appropriate to the risk before the first high-risk activity. Confirm that radios work from excavations and remote areas, alarms can be heard, muster boards are current and every person—including visitors—can be reconciled after a shift change. Correct gaps before releasing the work package.

7. Assemble the release package and keep it current

Before breaking ground, retain a signed, accessible package containing:

  • Approved permits, notifications, construction-phase and environmental plans.
  • Named duty holders, competence records, inductions and daily briefing arrangements.
  • Design-risk register, controlled drawings and change records.
  • Survey, utility-clearance and permit-to-dig records.
  • Geotechnical, slope, subsidence, historical-working and contamination assessments.
  • Excavation, temporary-works, lifting, traffic and erection plans.
  • Equipment inspections, maintenance status, certificates, rigging records and operator authorizations.
  • Emergency maps, contacts, medical arrangements, communication tests and drill or briefing records.
  • Photographs showing barriers, drainage, platforms, signage, exclusion zones and other released controls.

Set an owner and due date for every open action. Revalidate the package when ground conditions, weather, design, contractor composition, equipment or work sequence changes. A release is valid only for the conditions and scope it describes.

How to choose competent specialists and monitoring equipment

When comparing geotechnical or mine-risk providers, examine qualifications, mine-type experience, investigation methods, independence of review, monitoring frequency, trigger-action-response design, jurisdictional acceptance and the quality of deliverables. For gas detectors and PPE, compare applicable certification, detection range, environmental rating, calibration and service support, fit and compatibility with the site plan. The lowest purchase price is not a meaningful comparison if calibration, training, emergency response or lifecycle support is missing.

Decision area Questions to document
Hazard coverage Does the method address the actual ground, gas, utility, traffic and environmental hazards?
Competence and sign-off Who is qualified to inspect, approve, calibrate or accept the control in this jurisdiction?
Evidence quality Are assumptions, survey points, test results, drawings, limits and inspection dates traceable?
Schedule integration Can investigation, monitoring, permits and hold points be completed before the planned work front?
Emergency capability Will alarms, communications, rescue and medical arrangements function at the work location?
Lifecycle cost What recurring inspection, calibration, maintenance, training and replacement obligations remain?

The final question is simple: can the team show, at the work face, that the ground is supportable, services are cleared, the method is controlled, people are protected and help can reach them? If any answer depends on an assumption that has not been verified, keep the hold point closed.
