How to Stay Safe Around Sewer Access Points and Confined Spaces

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Do not enter a sewer manhole, wet well, lift station, or connected sewer space unless you are an authorized, trained entrant working under your employer’s written confined-space program. Before entry, the employer must evaluate the space and task, determine whether it is a permit-required confined space, control hazards as far as feasible, test the atmosphere in the required order, provide an attendant and communications, and have a workable rescue plan. Sewer conditions can change without warning: flowing material can engulf a worker, oxygen can fall, toxic or flammable gases can appear, and rain or an emergency release can send a surge through the system.

This guide is based on U.S. federal OSHA requirements and sewer-entry guidance. Your employer’s hazard assessment, written program, state-plan rules, local requirements, and site controls govern the actual job. If those requirements are not in place, stay out.

Is it safe to enter a sewer manhole?

Sometimes entry can be authorized, but a sewer opening is never safe merely because it looks clean, smells normal, or has been entered before. A manhole or connected sewer area may be a permit-required confined space, depending on its configuration and the hazards present. The employer must evaluate the workplace and classify the space before employees enter.

OSHA describes three conditions that make sewer entry unusually unstable:

  • A sewer is usually part of a continuous system and rarely can be completely isolated.
  • Because isolation is incomplete, the atmosphere may become toxic, flammable, explosive, or otherwise lethal suddenly and unpredictably.
  • Material flowing through the system can engulf or drown an entrant.

Those conditions mean that a previous clear test is not a guarantee for the next minute. Entry authorization has to account for the particular space, weather, upstream operations, traffic or industrial activity, and the work being performed.

When the answer is “do not enter”

Do not authorize entry when the required evaluation, controls, trained personnel, communications, monitoring, or rescue capability are missing. Stop and withdraw if monitoring alarms, ventilation fails, communications are lost, water rises, a new odor or visible contaminant appears, a worker develops symptoms, or any condition changes beyond the permit or employer program.

What does the employer have to establish before entry?

OSHA’s permit-space rule requires an employer that permits entry to implement a written program. The program must match the space and task, not simply copy a generic form. It should establish who may enter, how hazards are identified and controlled, how the space is tested, how entry is authorized, how the attendant communicates, and how a rescue will occur.

Evaluate and classify the space

Competent employer personnel should assess the access point and connected space for configuration, entry and exit, atmospheric hazards, engulfment, electrical and mechanical hazards, and the possibility of a changing condition. A sewer manhole may connect to lines or structures that are not visible from the opening. Do not infer that a hazard is absent because it cannot be seen.

Use an entry authorization or permit process

Where the employer’s program requires a permit, the permit should identify the space, authorized entrants, attendant, entry supervisor, hazards, test results, control measures, communications, and rescue arrangements. Follow the employer’s current procedure for issuing, suspending, and closing the authorization. A permit is not permission to continue after conditions become unsafe.

Controls should be selected for the actual geometry and hazards rather than treated as a universal checklist.

What hazards are found around sewer access points?

OSHA’s construction sewer guidance identifies a broad hazard set. The employer’s assessment determines which hazards apply and what controls are needed.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  • Atmospheric hazards: low oxygen, toxic gases or vapors, and flammable or explosive atmospheres.
  • Contaminants: chemicals entering through piping, industrial discharges, or roadway runoff.
  • Engulfment and drowning: sewage, stormwater, or another flowing material can rise or move an entrant.
  • Electrical hazards: energized equipment, damaged wiring, or conductive wet conditions.
  • Falls and falling objects: open access points, slippery surfaces, tools, covers, or debris.
  • Noise and visibility: high noise, darkness, poor visibility, and difficulty hearing instructions or alarms.
  • Communication and egress limits: radio interference, long distances to the exit, ladders, restricted passages, and limited room to maneuver.

Smell is not a gas test. Some dangerous contaminants have little warning odor, and the ability to smell a substance can disappear at hazardous concentrations. Appearance, familiarity, and an uneventful prior entry are not atmospheric controls.

What gases should be tested before entering a manhole?

OSHA’s general-industry permit-space rule specifies this testing sequence:

  1. Oxygen first. An oxygen-deficient or oxygen-enriched atmosphere changes the immediate risk and can affect the interpretation of subsequent readings.
  2. Combustible gases and vapors second. Determine whether flammable conditions could exist.
  3. Toxic gases and vapors third. Test for contaminants identified by the hazard assessment, including known sewer-related or process-related substances.

Use a suitable, calibrated monitor with audible as well as visual alarms. Calibration must follow the manufacturer’s instructions. The monitor should be used in the entrant’s work area and, for sewer-line work, ahead of the entrant’s direction of movement so deterioration can be detected before the worker reaches it.

What do OSHA’s example alarm values mean?

OSHA’s sewer-entry appendix gives these examples:

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
Condition OSHA appendix example Qualification
Oxygen Below 19.5 percent Example alarm condition in the appendix; apply the employer’s program and applicable requirements.
Flammable gas or vapor 10 percent or more of the lower flammable limit Example alarm condition; the monitor and assessment must address the actual hazard.
Hydrogen sulfide 10 ppm Appendix example measured as an 8-hour time-weighted average.
Carbon monoxide 35 ppm Appendix example measured as an 8-hour time-weighted average.

These figures are guidance examples, not a complete list of exposure limits or a substitute for the employer’s hazard assessment. Check the applicable limits, entry conditions, instrument capabilities, and written procedures before setting alarm responses.

Why do sewer workers need a gas monitor?

A monitor provides information that sight and smell cannot. In a continuous sewer system, conditions can change after the initial test because of upstream flow, chemical release, disturbance of settled material, weather, or another event outside the entrant’s control. An alarm gives the entrant and attendant a prompt to stop work, communicate, and leave before the atmosphere or other hazard becomes incapacitating.

A broad-range sensor can help identify a flammable-gas hazard class, while substance-specific sensors can assess known contaminants. Neither approach eliminates the need to consider unexpected contaminants or sudden changes. The selected instrument must suit the workplace, be maintained and calibrated as directed, and be compatible with the employer’s procedures and hazardous-area conditions.

How should a crew prepare for entry?

Preparation is a sequence of decisions, not a last-minute equipment check.

  1. Review the space and task. Identify connected lines, flow paths, chemicals, electrical systems, traffic, access geometry, work duration, and the consequences of a change in conditions.
  2. Check whether isolation is feasible. Use the employer’s engineering and operational controls. Where a continuous sewer cannot be completely isolated, plan for changing atmosphere and flow rather than assuming isolation.
  3. Coordinate outside the crew. Check weather and coordinate, where possible, with local fire or emergency services and operations that could affect the sewer. Identify rain, fire suppression, industrial discharge, transportation incidents, or emergency releases that could send water or contaminants into the space.
  4. Set up the opening. Use barriers, lighting, and safe access and egress controls. Protect workers and the public from the opening and falling objects.
  5. Choose and check monitoring. Confirm the instrument covers the identified hazards, has audible and visual alarms, and is calibrated according to the manufacturer’s instructions. Position it to sample the entrant’s environment and the path ahead when the work requires movement.
  6. Establish communications. Agree on radio, voice, line, or other communication methods, check that they work in the actual space, and define the signal to evacuate.
  7. Assign trained roles. Name the authorized entrants, attendant, entry supervisor, and rescue personnel or service. Each person must understand the duties assigned to that role.
  8. Confirm rescue readiness. Have the planned rescue equipment, trained rescuers, access route, and response communications ready before entry begins.

What should you do if water levels rise while someone is in a sewer?

Tell the entrant to stop work and exit immediately when monitoring, visual observation, communications, or an upstream warning indicates rising water or a possible surge. The attendant should initiate the employer’s evacuation and emergency procedure, notify the entry supervisor, and summon rescue or emergency services as planned. Do not wait for the water to become visibly dangerous.

Sources of sudden flow

  • Rainfall in the drainage area can produce a surge even when conditions at the access point appear manageable.
  • Fire suppression may send large volumes of water into a sewer.
  • An industrial or transportation emergency may release water, chemicals, or other hazardous material.

OSHA construction guidance describes an upstream early-warning system placed far enough from the work to give entrants time to exit. The warning method must be reliable for the site and incorporated into the employer’s plan; a person who is merely nearby and unassigned is not a substitute for a defined warning and evacuation system.

After a surge, flooding, release, or alarm, re-entry requires a fresh evaluation under the employer’s program. Do not simply reset the monitor or send someone back to retrieve tools.

What are the entrant, attendant, and supervisor responsible for?

Authorized entrant

  • Enter only when authorized and trained for the assigned duties.
  • Use the specified PPE, monitoring, lighting, and communications.
  • Keep the monitor in the work area and ahead of movement when directed by the procedure.
  • Report symptoms, alarms, changing conditions, and communication problems immediately.
  • Leave when ordered, when an alarm activates, when a prohibited condition appears, or when the entrant recognizes a danger.

Attendant

  • Remain outside the space for the duration of authorized entry.
  • Maintain an accurate count or other method of knowing which entrants are inside.
  • Communicate with entrants, watch for changing conditions, and order evacuation when required.
  • Summon rescue and emergency help according to the plan.
  • Do not enter to attempt an improvised rescue. An untrained attendant can become a second casualty.

Entry supervisor and employer

The supervisor and employer ensure that the entry conditions, tests, controls, personnel, permit, and rescue arrangements meet the written program. They must stop the entry when conditions are not acceptable and ensure that equipment is available, maintained, and used correctly.

What equipment is needed for confined-space rescue?

Rescue equipment must match the access geometry, hazards, and planned response. Under the permit-space rule, a retrieval system can include a retrieval line, chest or full-body harness, wristlets when appropriate, and a lifting device or anchor. The employer must provide necessary rescue and emergency equipment unless the rescue service supplies it, and must ensure the equipment is properly maintained and used.

  • Clog resistant tip writes on dusty, wet and oily surfaces and is optimized for rough surfaces such as OSB, cinderblock and concrete
  • Hard hat clip- attaches for easy access
  • Quick dry time with reduced smearing and marking

Rescue planning questions

  • Can a non-entry retrieval remove the entrant through the actual opening, ladder, bends, and obstructions?
  • Is the harness and retrieval line compatible with the work and likely rescue forces?
  • Will a lifting device or anchor remain stable and accessible during a flood or other emergency?
  • Can the designated rescue service reach the site quickly enough and handle this particular permit space?
  • What happens if the entrant is injured, unconscious, entangled, submerged, or exposed to a toxic atmosphere?

OSHA warns that untrained rescue attempts can cause multiple deaths. A purchased harness, tripod, or monitor does not make entry safe by itself. Rescuers, entrants, and attendants need training for their duties, and an outside service must be capable of the specific rescue and able to respond in time.

Special equipment for large-bore sewer entry

OSHA’s sewer appendix notes that large-bore operations may require specialized equipment, potentially including an escape self-contained breathing apparatus with at least a 10-minute air supply or another NIOSH-approved self-rescuer, waterproof flashlights, boats or rafts, radios, and rope stand-offs. This is conditional guidance for operations with those access and hazard characteristics, not a universal shopping list. Select equipment through the employer’s assessment and rescue plan.

How should ventilation, PPE, access, and barriers be used?

Ventilation can help control an identified atmospheric hazard, but it is not a reason to skip testing or continuous monitoring where the system cannot be fully isolated. The employer’s procedure should specify the ventilation method, its limits, and what happens if it stops or fails to control conditions.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

PPE should address the hazards identified for the task, such as eye and skin exposure, electrical risk, falls, contamination, or respiratory hazards. PPE selection must not replace elimination, isolation, ventilation, flow control, or other feasible engineering and administrative controls.

Provide safe ingress and egress, adequate lighting, and barriers around the opening. Keep tools and materials from falling into the space or striking workers below. Account for slippery surfaces, ladders, long travel distances, and the possibility that an entrant cannot climb out without assistance.

What are the most common unsafe assumptions?

  • “It smells fine.” Odor is not a reliable test for oxygen, flammability, or toxicity.
  • “The first reading was clear.” Sewer atmospheres and flow can change after the test.
  • “The monitor is turned on, so we are covered.” The instrument must be suitable, calibrated as directed, correctly used, and paired with an alarm response and evacuation plan.
  • “Someone can watch from the truck.” The attendant has defined duties and must remain outside for the entry, maintain communication, and summon help.
  • “A coworker can pull the entrant out.” Rescue requires planned equipment, training, and a capable response; an improvised entry can create another victim.
  • “Rain is still far away.” Upstream weather and operations can affect the space before conditions look dangerous at the opening.
  • “A harness or tripod makes it safe.” Retrieval equipment is effective only when selected for the geometry and integrated into a workable rescue plan.

Pre-entry and entry checklist

Use the employer’s form and procedures; this checklist does not replace them.

  • Space evaluated and classification confirmed.
  • Written program and entry authorization reviewed.
  • Connected flows, chemicals, electrical hazards, traffic, and falling-object risks assessed.
  • Isolation or feasible flow controls established; residual uncertainty addressed.
  • Weather, fire suppression, industrial activity, and emergency-release risks checked.
  • Opening protected with barriers, lighting, and safe access and egress.
  • Atmosphere tested in order: oxygen, combustible gases and vapors, then toxic gases and vapors.
  • Monitor suitable for the hazards, calibrated according to the manufacturer, and set for audible and visual alarms.
  • Monitoring location includes the entrant’s work area and the path ahead when required.
  • Ventilation, PPE, communications, attendant coverage, and evacuation signals confirmed.
  • Retrieval and rescue equipment staged, inspected under the employer’s procedure, and matched to the space.
  • Entrants, attendant, supervisor, and rescuers trained for their duties.
  • Entry stopped and entrants withdrawn for alarms, rising water, lost communication, failed controls, symptoms, or any changed condition.

What is the safest decision when conditions are uncertain?

Do not enter until the uncertainty is resolved by the employer’s competent personnel and written program. If conditions cannot be controlled or monitored reliably, the safe decision is to postpone the work, use a different method, or obtain a capable rescue service and additional controls. Sewer access work is not routine just because the opening is familiar. Authorization, atmospheric control, attendant coverage, and rescue planning come before entry.
