How to Report a Dangerous Road or Bridge Approach to Local Authorities

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

If a road or bridge approach presents an immediate danger, call 911 or your jurisdiction’s emergency number first. Do not wait for a maintenance form: PennDOT says its customer submissions are not monitored outside weekday business hours, and TxDOT warns that its online form is not monitored 24/7. For a non-emergency condition, report it to the agency that maintains that specific road segment—usually a city, county, or state transportation department—and provide the route, travel direction, nearest landmark, exact hazard, and when you observed it.

1. Decide whether the hazard is an emergency

Use the likely consequence and immediacy of the danger, not the inconvenience it causes, to choose a reporting channel. If drivers, pedestrians, workers, or responders could be injured now, use the emergency channel. A routine form is appropriate only when the condition can wait for normal maintenance response without creating an immediate threat.

Call 911 or the local emergency channel now

  • A vehicle, load, fallen tree, washout, or other obstruction is blocking a travel lane.
  • A bridge approach appears to be collapsing, undermined, displaced, or otherwise structurally unsafe.
  • Floodwater, standing water, ice, or debris is preventing safe passage or concealing a major defect.
  • A missing Stop, Yield, or Curve Ahead sign, malfunctioning traffic signal, severe pothole, or sinkhole creates an immediate collision risk.
  • A crash, injury, fire, exposed utility, or other life-threatening event is in progress.

Tell the dispatcher that the report concerns a road or bridge approach, give the direction of travel, and describe whether lanes are blocked or people are in danger. PennDOT’s published wording is explicit: “for emergencies or unsafe conditions you should always call 9-1-1.” TxDOT similarly directs people to 911 for immediate hazards such as dangerous debris. Local instructions can differ: DelDOT describes a transportation management center operating 24/7/365 for certain conditions needing immediate attention, while crashes and comparable emergencies still go to 911. Follow the instructions for the jurisdiction where the hazard is located.

Use a non-emergency report for a dangerous but stable condition

Examples include a worsening but passable bump at the end of a bridge, a damaged guardrail that is not currently struck, faded approach markings, a sign that is present but hard to read, or drainage damage that is not blocking traffic. Submit these through the maintaining agency’s phone line, online service request, or app. State clearly why the condition is hazardous and whether it is getting worse, but do not describe a stable maintenance issue as an active emergency.

2. Identify who owns and maintains the road

The correct recipient is the agency responsible for that segment, not necessarily the agency whose name appears on a nearby sign. A state transportation department may maintain a numbered state highway, while a city maintains the adjoining street and a county maintains another approach. Private roads, toll facilities, and access roads can have separate operators.

Start with the most likely public owner

  • City street: the municipal public works, transportation, or 311 service.
  • County road: the county road department, highway department, or county public works office.
  • State route or state highway: the state department of transportation (DOT) district or roadway-reporting system.
  • Unclear ownership: submit to the agency serving the location and ask it to redirect the request if the segment is outside its jurisdiction.

Road eligibility is often limited. Iowa DOT says its reporting system accepts issues on Iowa DOT-managed roads and directs people to the appropriate local agency for city or county roadways. Caltrans describes its mapped system as covering the California State Highway System and tells users to contact their city or county for locations outside Caltrans property. IDOT likewise says a claim involving a road outside its jurisdiction belongs with the government agency that maintains that roadway.

How to confirm the boundary

  1. Look for a route shield, highway number, mile marker, bridge name, or maintenance sign.
  2. Check the city or county’s public works map or service-request page for the address or intersection.
  3. Use the state DOT’s map only to verify that the segment is on its system; do not assume every road shown on a state map is state-maintained.
  4. If two agencies meet at the approach, describe the exact side of the boundary and report to both when the hazard spans both jurisdictions.

A report sent to the wrong agency can be delayed or closed without repair. Giving the location and asking for a transfer is better than guessing at a statewide phone number.

3. Choose the reporting channel that fits the situation

Agencies commonly offer a phone line, online form, or mobile app. Availability, monitored hours, road eligibility, photo support, and confirmation methods vary by jurisdiction. Use this framework, then verify the local agency’s current instructions.

Channel Best use Important limitations Typical information or follow-up
911 or local emergency dispatch Immediate danger, active obstruction, suspected structural failure, crash, injury, or life threat Not a substitute for a routine maintenance request; the dispatcher may send emergency responders rather than a repair crew Give exact location, direction, lanes affected, and current danger; remain available if safe
Agency emergency line or transportation management center Urgent roadway conditions handled directly by the transportation agency Rules differ; some centers operate continuously, while some agency forms do not Describe the hazard and ask whether a reference number is available
Online service request Non-emergency defects that need a written description, map pin, or attachment May accept only roads maintained by that agency; may not be monitored nights, weekends, or holidays Save the confirmation or reference number and use the agency’s status tool if offered
Mobile app Reports made from a stopped vehicle or by a passenger, often with map location and photos Never use it while driving; app coverage may be limited to a state or local road system Location capture, optional photo, description, and sometimes a tracking number
Non-emergency phone or 311 People who need help identifying the owner or cannot use a web form Hours may be limited; an operator may still redirect state, county, or private-road reports Ask for the receiving department and write down any case number

PennDOT says customer-service submissions are monitored only during stated weekday business hours, while TxDOT warns that its online form is not monitored 24/7. Those examples show why a form confirmation does not mean that someone is watching the report in real time.

4. Gather the details that make the location actionable

“The bridge approach is dangerous” is too vague for a crew to find quickly. Build the report around a precise location and a visible condition. Include only information you can obtain safely.

Location checklist

  • Road name and route number, if posted.
  • City, county, and state.
  • Direction of travel: northbound, southbound, eastbound, westbound, or an approach to a named bridge.
  • Nearest cross street, intersection, address, bridge name, mile marker, or distinctive landmark.
  • Approximate distance and direction from that landmark, such as “about 200 feet west of…”
  • Lane, shoulder, sidewalk, trail, or ramp affected.
  • A map pin or coordinates when the form supports them.

Condition checklist

  • What is wrong: abrupt vertical bump, pavement separation, erosion, sinkhole, missing guardrail, exposed rebar, damaged expansion joint, flooding, debris, missing sign, or signal failure.
  • Where it occurs on the approach: before the bridge, at the deck transition, after the bridge, on a ramp, or beneath the structure.
  • How traffic is affected: lane blocked, vehicles swerving, reduced sight distance, water covering markings, or pedestrians forced into traffic.
  • When you observed it and whether it is worsening, if the form asks for that information.
  • Any immediate warning already in place, such as cones, barricades, or a temporary sign.

Describe observable facts rather than diagnosing the engineering cause. “A 4-inch edge drop across the right lane with loose asphalt” is more useful than “the foundation has failed” unless an authority has made that determination.

Contact details and attachments

Provide a phone number or email when the agency requests it or may need clarification. PennDOT allows a map pin or cross street and accepts an optional photo. Caltrans asks for a map location, direction of travel, nearest cross street, time noticed, and descriptions of the situation and its geographic location. Multnomah County requests a detailed description, a road or address (or distance and direction from a cross street), and a contact number for non-emergency reports.

A photograph can show the defect, lane position, and missing safety equipment, but it is not universally required. Do not create a second hazard to obtain evidence.

5. Submit the report step by step

For an immediate hazard

  1. Move yourself and your vehicle to a safe location if you can do so without entering traffic or worsening the situation.
  2. Call 911 or the local emergency channel. If the jurisdiction identifies a separate transportation management center for roadway emergencies, use it only when the local instructions say it is the proper channel.
  3. State the road, direction, nearest cross street or landmark, affected lane, and exact danger first.
  4. Explain whether the obstruction, water, debris, or structural problem is active and whether anyone is injured.
  5. Follow dispatcher instructions. Do not return to the hazard to take measurements or photographs.

For a non-emergency maintenance report

  1. Open the maintaining agency’s current roadway-concern, hazardous-condition, public-works, or 311 page, or call its non-emergency service line.
  2. Confirm that the form accepts the road category and location. If it does not, use the city or county agency that maintains the segment.
  3. Enter the road, route, direction, cross street or landmark, lane, and map location before writing the description.
  4. Describe the defect, its dimensions or extent if you can estimate safely, the traffic effect, and when you noticed it.
  5. Add a safe photograph only if the agency accepts attachments and the image was obtained without driving, walking into traffic, or stopping in an unsafe place.
  6. Provide contact information if requested, review the submission for location errors, and submit it.
  7. Save the confirmation page, email, or reference number. Note the date and channel used.

6. Report safely—never become part of the hazard

Do not type into a phone, open an app, or take a picture while driving. Iowa DOT’s roadway-reporting guidance permits its app when the reporter is stopped or is a passenger and says never to report while driving. Apply the same rule to every agency, even if an app appears to allow a moving submission.

  • Pull completely out of a travel lane before using a phone, and obey parking and shoulder rules.
  • If you are a passenger, let the driver concentrate while you record the location.
  • Do not walk onto a bridge deck, shoulder, active work zone, or traffic lane for a better image.
  • Do not touch exposed cables, rebar, unstable pavement, barriers, or floodwater.
  • If conditions are changing, leave the area and give responders the best location information you already have.

A blurry photograph from a safe position is preferable to a clear photograph taken in traffic. Agencies can inspect and measure the condition themselves.

7. Keep the confirmation and follow up intelligently

Store the reference number, submission date, agency name, and a copy of your description. PennDOT says its Customer Care Center provides a reference number and a status lookup. If your agency offers a status page, enter the number there rather than opening duplicate tickets.

Follow up when the condition remains dangerous, changes materially, or was routed to the wrong department. Use the agency’s stated follow-up channel and repeat the precise location. If the danger becomes immediate after your original routine report, make a new emergency call; do not rely on an old form submission.

A maintenance report is not a promise of a repair date, closure, inspection result, or compensation. Agencies prioritize hazards according to their own standards, available crews, weather, and jurisdiction.

Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone.Support on Ko-Fi

8. Keep damage claims separate from hazard reports

If a vehicle, trailer, equipment, or other property was damaged, ask the maintaining agency for its separate claims procedure. A report that alerts an agency to a dangerous condition does not automatically open a claim or establish liability. IDOT explains that the agency handling a property-damage claim depends on roadway jurisdiction and that claims use a separate process.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  • Keep photographs, repair estimates, receipts, witness details, and the incident date.
  • Record the exact road segment and any report reference number.
  • Check filing deadlines and required forms with the agency that maintains the road.
  • Do not alter or remove physical evidence when doing so could create another hazard.

For an injury or collision, contact emergency responders and your insurer as appropriate. The road-condition report and any insurance or government claim should remain separate records.

9. A report template you can adapt

Use the following wording in an online form or phone call, replacing the details with facts you can verify:

Road and jurisdiction: [road name/route], [city or county], [state]
Direction: [direction of travel]
Location: [nearest cross street, landmark, bridge name, mile marker, or address]
Affected area: [lane, shoulder, sidewalk, ramp, or both approaches]
Condition: [specific visible defect or obstruction]
Traffic effect: [blocked lane, swerving vehicles, reduced sight distance, flooding, etc.]
Observed: [date/time, if requested]
Immediate danger: [yes/no; explain briefly]
Contact: [phone or email, if requested]

For an emergency call, lead with “immediate danger” and the location; do not wait to complete every optional field. For a routine form, complete the map and roadway fields carefully so the maintenance crew can locate the approach without searching a large bridge corridor.

10. Common reporting mistakes and fixes

Mistake Why it causes delay Fix
Using a state form for a city or county road The system may reject the request or route it slowly Confirm the maintainer and use the municipal or county channel
Writing only “dangerous bridge” Crews cannot identify which approach or defect you mean Add direction, cross street or landmark, lane, and a visible description
Submitting an online form for an active obstruction Forms may not be monitored continuously Call 911 or the locally designated emergency transportation line
Taking a photograph while driving It creates a new collision risk and may violate traffic law Stop safely or have a passenger document it; otherwise omit the photo
Assuming a report is a damage claim Maintenance and compensation follow different procedures Open the agency’s separate claims process and retain evidence
Sending duplicate reports without the case number Duplicates can obscure the original record Use the reference number and the agency’s status or follow-up channel

What a good outcome looks like

The immediate objective is that the right responder can find the hazard and protect road users. An effective report therefore does four things: it uses an emergency channel when people are in immediate danger, identifies the maintaining agency, pinpoints the approach and affected travel path, and supplies a factual description obtained safely. Keep the confirmation for follow-up, and start a separate claim only if property damage or injury requires one.
