Do not wait for a routine online maintenance form. For a non-emergency pothole, damaged barrier, washout or other defect, report it to the agency that owns or maintains the route—usually a state department of transportation for a state highway, or a county or city public-works department for a local road. Give the exact location, what you observed, the direction of travel and whether traffic is currently exposed to danger.
First decide: emergency or routine maintenance?
The urgency of the condition determines the first reporting channel. A defect can be minor in one setting and life-threatening in another, so describe the risk rather than trying to classify the engineering problem yourself.

Use emergency services or law enforcement for an immediate threat
Call from a safe location when traffic is exposed to a serious current hazard, such as a bridge collapse or partial collapse, a sudden washout, large debris in a travel lane, a visibly displaced barrier, a missing section of roadway, flooding that has made the route unsafe, or damage that could cause a crash. Give the road name or route, the nearest cross street or landmark, your direction of travel and a short description of what drivers face. Do not stop in a live lane, enter a closed area or approach unstable structure to obtain a closer photograph.

State guidance illustrates this rule. The North Carolina Department of Transportation says, “In emergencies where roadways unexpectedly become unsafe, immediately notify local law enforcement so that they can enact appropriate public safety measures.” Its Safety & Emergencies page is a state-specific example, not a nationwide telephone instruction.

California’s transportation department gives the same distinction in different words: “This form should NOT be used to report any kind of highway emergency. Report highway emergencies to 911 or the California Highway Patrol immediately.” That warning appears on the Caltrans customer-service request page. Outside California, use your local emergency number and the law-enforcement agency serving the road.
Use a maintenance request for a non-urgent defect
A pothole, cracked pavement, damaged sign, worn guardrail or other condition that is not creating an immediate emergency can usually be sent through the road owner’s maintenance form, telephone line or customer-service system. These channels route a report for evaluation; they are not substitutes for emergency dispatch.
Some systems operate only during posted business hours. Caltrans says requests submitted through its service-request location are sent to the nearest maintenance facility for evaluation and handled Monday through Friday during posted hours. That schedule applies to California’s system and should not be generalized to every state or city.

Who is responsible for the road or bridge?
Responsibility follows ownership and the type of route. Start with the agency that maintains the road, even when another agency owns the structure or right of way.
- State highway: use the state department of transportation’s road-hazard or customer-service channel.
- County road: contact the county public-works, transportation or highway department.
- City street: use the city’s public-works or 311 system where available.
- Federal or tribal route: use the owning federal agency or tribal transportation authority.
Federal bridge-inspection rules recognize exceptions to ordinary state responsibility. The Federal Highway Administration (FHWA) says states are ultimately responsible for inspecting public highway bridges within the state except federally or tribally owned bridges; the federal agency or tribal government that owns a bridge is responsible for its inspection. The FHWA National Bridge Inspection Standards Q&A explains that ownership distinction. Louisiana’s transportation department also states that each bridge owner is responsible for maintaining its structures; its Bridge Maintenance and Inspection page is a state example.

If you cannot determine ownership, submit the report to the state or local agency that offers a public road-hazard channel. Include a precise location and ask the receiving agency to forward it if another owner is responsible. You do not need to find a National Bridge Inventory number before reporting a danger.
What information should you provide?
A useful report lets dispatchers or maintenance staff locate the hazard quickly and understand the exposure without requiring you to diagnose its cause.

- Route or street name and number: include the highway designation, local street name or both.
- Exact location: provide the nearest intersection, exit, milepost, address, bridge name, landmark or a map pin.
- Direction of travel: say eastbound, westbound, northbound, southbound or the relevant lane.
- What you observed: use plain descriptions such as “water has removed pavement,” “concrete appears displaced,” “debris blocks the right lane,” or “guardrail is lying in the shoulder.”
- When you saw it: include the date and approximate time, and say whether the condition is worsening or changing.
- Immediate exposure: explain whether vehicles, pedestrians or workers are currently being forced toward the hazard.
- Safe supporting material: add a photograph or map pin only when the agency accepts it and obtaining it did not require approaching danger.
Report observations, not a structural diagnosis. Do not state that a bridge is “structurally unsound” unless an authority has made that determination. A visible washout, a bent or missing barrier, a deep void, exposed reinforcement, unusual movement or an obstruction is enough for the authority to assess.
How to report a damaged bridge or unsafe road: step by step
- Get clear of the hazard. If you are driving, pull over only where it is legal and safe. Do not walk onto a bridge, enter floodwater or stand in a traffic lane to investigate.
- Choose the urgent channel. For an active threat, call the local emergency number or law enforcement. For a non-urgent condition, continue to the road owner’s maintenance channel.
- Identify the route and owner. Determine whether the location is on a state highway, county road, city street, federal route or tribal route. If uncertain, use the agency with a public road-hazard intake and provide enough detail for referral.
- State the location first. Give the route or street, nearest cross street or landmark, direction of travel and lane or shoulder affected. A map pin is useful when the form supports one.
- Describe the visible condition. Explain what is damaged or obstructed, its approximate size if you can estimate safely, and how traffic is affected. Avoid guessing the engineering cause.
- Explain urgency. Say whether vehicles are currently exposed, whether the route is blocked, and whether the condition appeared suddenly.
- Add safe evidence. Attach a photograph or video only if the reporting system accepts it and you already have it without taking additional risk. Never delay an emergency call to collect media.
- Submit and save the confirmation. Keep the confirmation or reference number, screenshot or email receipt. Use the agency’s listed follow-up route if the hazard remains. There is no national follow-up interval or repair guarantee.
Where to report common road hazards
Potholes and pavement failures
For a routine pothole, broken pavement or shoulder erosion, use the maintaining agency’s maintenance-request form or phone line. State the lane and direction, because a report that says only “on Route 12” may cover many miles. If the pothole has opened suddenly, is deep enough to cause an imminent crash or is combined with flooding, treat it as an emergency and contact law enforcement.
Washouts, flooding and missing roadway
Water can undermine pavement and bridge approaches without leaving a visible crack. A washout, missing shoulder or flooded travel lane that could carry away a vehicle belongs with emergency services or law enforcement first. Do not drive around a barricade or test the depth of moving water. A routine maintenance form is appropriate only after the route is no longer an immediate danger.

Debris, fallen trees and damaged signs
Large debris in a lane, a fallen tree or a sign or signal that creates a current collision risk should be reported urgently. A damaged sign that is still readable and does not expose traffic to immediate danger can go through routine maintenance. Include the lane, direction and closest intersection so crews can find it without searching the entire route.
Bridge barriers, railings and visible structural damage
Report a displaced barrier, exposed reinforcement, a visibly separated joint, a hole in the deck, unusual movement or a damaged approach as an observation. Keep away from the structure and let qualified inspectors decide whether it requires closure, a load restriction, shoring, repair or replacement.
How public reports differ from bridge-inspection records
A hazard report asks an authority to assess a condition and protect road users. It is not the same process as requesting an official inspection report.
The Federal Railroad Administration (FRA) has a FAST Act process for an eligible state or local official to request a public version of a railroad-generated bridge inspection report for a bridge in that official’s jurisdiction. The request requires the official’s identity and capacity, jurisdiction, bridge location and railroad information when known. It is not a general public emergency-reporting route.
Under the FRA process described on its FAST Act Bridge Inspection Report Requests page, last updated February 12, 2025, the railroad has 30 days to provide the requested report after FRA asks for it, and FRA generally sends the report to the requester within 45 days after receiving the request. Those periods apply to the eligible official-record process—not to a public road-hazard report or a repair decision.

What happens after an agency receives a serious finding?
Authorities decide what protective action is warranted after their assessment. FHWA’s bridge-inspection guidance lists possible responses to a critical finding: closing the bridge, restricting a lane or vehicle loads, installing shoring, repairing the defect or replacing the structure. Simply inspecting more often does not, by itself, resolve an unresolved safety issue.
FHWA also describes agency notification duties for specified National Highway System findings. State transportation departments, federal agencies and tribal governments must notify FHWA within 24 hours when a covered finding results in a full or partial closure or involves a nonredundant steel tension member rated serious or worse, and they must provide at least monthly written status reports until the finding is resolved. These are government-to-government duties, not a promise that a member of the public will receive a decision or repair within 24 hours. The details are in the FHWA National Bridge Inspection Standards Q&A, Section 313.

| Situation | First channel | Information to emphasize |
|---|---|---|
| Immediate danger or an unexpectedly unsafe roadway | Local emergency number or law enforcement | Exact route, nearest landmark, direction, blocked lane and current threat to traffic |
| Routine defect on a state highway | State DOT maintenance or customer-service channel | Map location, route, lane, visible condition and time observed |
| Routine defect on a county or city road | County or municipal public works/311 channel | Street address or intersection, direction and description of the defect |
| Unclear ownership | State or local agency with a public hazard intake | Precise location and a request that the report be routed to the responsible owner |
| Request for a railroad bridge inspection record by an eligible official | FRA FAST Act report-request process | Official capacity, jurisdiction, bridge location and railroad details |
Before choosing a non-emergency form, check whether it is staffed around the clock, accepts photos or map pins, allows anonymous submissions and supplies a confirmation number. Features and hours vary by agency; do not assume that a form is monitored overnight.
Examples of clear, useful reports
Example: active bridge approach washout
“Emergency: eastbound Route 40, approximately one-half mile west of the Oak Street exit. The right lane and shoulder have washed away at the bridge approach; vehicles are merging into the left lane. I am stopped safely at the previous turnout. The condition was observed at 7:15 a.m.”
Example: routine pothole
“Northbound County Road 8, outside 2140 County Road 8, about 100 feet south of the Elm Avenue intersection. A pothole approximately two feet across is in the right wheel path. It was present at 4:30 p.m. on October 17. No collision or immediate blockage observed.”

What not to do
- Do not use a routine web form for an active emergency.
- Do not approach a collapsed, flooded or unstable structure to inspect it.
- Do not stop in a traffic lane or direct other drivers while gathering evidence.
- Do not remove debris, straighten a barrier or attempt a repair unless you are authorized and trained to do so.
- Do not delay an urgent call while searching for a bridge identification number.
- Do not assume that a report guarantees closure, inspection or a particular repair date.
- Do not present an observation as a professional structural diagnosis.
California and North Carolina examples, and why they are not universal
California’s Caltrans service-request page asks users to locate a concern on a map and sends the request to the nearest maintenance facility for evaluation during posted Monday-through-Friday hours. It also expressly excludes highway emergencies and directs callers to 911 or the California Highway Patrol. North Carolina’s safety guidance directs people to local law enforcement when a roadway unexpectedly becomes unsafe and distinguishes that route from routine problems.
These pages demonstrate the general pattern—emergency channel first, maintenance channel for routine conditions—but their phone numbers, staffing, forms and agency names apply only within those states. For another jurisdiction, use the local emergency number and the road owner’s current official page.

- Am I in a safe location, away from traffic and the structure?
- Is anyone currently at risk? If yes, have I called emergency services or law enforcement?
- What is the route or street, nearest cross street or landmark, and direction of travel?
- What exactly is visible, and which lane, shoulder, sidewalk or bridge approach is affected?
- When did I observe it, and is it changing?
- Can I provide a map pin or photo without approaching danger?
- Did I save the confirmation or reference number?
Caltrans reports that its system covers 15,133 centerline miles of highway and 13,063 state highway bridges, and that it inspects more than 12,200 local bridges. Those figures describe California’s transportation jurisdiction, not a national total. They illustrate why an exact location and a clear description matter: a maintenance organization may be responsible for thousands of route miles and structures.
The Bottom Line
Call emergency services or law enforcement from a safe place when a damaged bridge or road creates an immediate threat. For a non-emergency, report the visible condition to the agency that owns or maintains the route, with the exact location, direction of travel and traffic risk. Save the confirmation, but do not expect a universal response time or treat an inspection-record request as an emergency report.
