10 CFR Part 50 Appendix B

How to Plan Construction Quality Assurance for a Nuclear or Other Safety-Critical Facility

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Plan construction quality assurance as a graded, documented program before safety-related purchasing or field work begins. For a U.S. nuclear project, use 10 CFR Part 50 Appendix B as the regulatory baseline and the applicable NRC-endorsed ASME NQA-1 edition as the implementation framework. Identify every safety-related structure, system and component (SSC), translate its design-basis requirements into controlled work instructions, qualify suppliers and personnel, verify work with calibrated equipment and independent checks, control nonconformances, and preserve objective records for the facility’s life cycle.

The same logic applies to other safety-critical facilities, but their regulator, safety case, codes and quality standard must be mapped separately rather than assumed to follow U.S. nuclear rules.

What a construction QA plan must accomplish

A construction quality-assurance (QA) plan is the management system that turns safety requirements into repeatable work and defensible evidence. It covers the organizations, processes, people, equipment and records that can affect the ability of safety-related SSCs to perform their design-basis functions.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

For U.S. nuclear work, Appendix B applies to activities affecting safety-related SSCs, including design, purchasing, fabrication, erection, installation, inspection and testing. The applicant must establish the QA program at the earliest practicable time consistent with the project schedule and run it through the plant life cycle. Controls should be graded to the safety significance of the SSC and activity: a high-consequence pressure boundary normally needs more stringent qualification, verification and records than a low-significance support activity.

Establish the governing basis before field work

Define jurisdiction and licensing path

Record the country, regulator, license or authorization path, applicable legislation, design codes, construction standards and contractual commitments. On a U.S. Part 50 project, map the program to Appendix B and identify applicable Part 21 duties for reporting defects or failures to comply. A project outside the United States needs a jurisdiction-specific crosswalk to its regulator and safety case; Appendix B and U.S. practice cannot be applied unchanged.

Select the implementation standard deliberately

ASME describes NQA-1 as quality-assurance requirements and guidance for all phases of a nuclear facility’s life cycle. NRC implementation guidance currently identifies NQA-1:2017, NQA-1:2019 and NQA-1:2022, subject to NRC exceptions and clarifications. Confirm the edition, endorsed provisions, exceptions and organizational commitments in the licensing basis before writing procedures. Do not allow different project teams or suppliers to assume different editions.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Create a requirements hierarchy

Set out a controlled hierarchy showing how laws and license conditions flow into the safety analysis, design basis, codes, specifications, drawings, procurement documents, procedures and field instructions. Define which document prevails when requirements conflict, who may approve a change and how superseded information is removed from use.

Define the safety scope and grade the controls

Build the SSC and activity register

Start with a controlled list of safety-related SSCs and the functions they must perform. For each SSC, record its safety classification or quality-significance category, design-basis function, applicable codes, procurement class, inspection level, acceptance criteria and required records. Include temporary installations, testing configurations and support systems when their failure could affect a safety-related function.

Register field Decision the planner must make Output used in construction
SSC identity and boundaries What equipment, structure, software or support feature is included, and where does responsibility transfer? Tagged drawings, system boundaries and turnover scope
Safety function Which design-basis function must remain available, under what conditions? Design-basis requirements and acceptance criteria
Quality grade What level of supplier qualification, inspection, testing and records is proportionate to risk? Procurement and inspection category
Applicable requirements Which regulations, codes, specifications, drawings and environmental limits apply? Requirements traceability matrix
Verification evidence Which inspections, tests, certificates, measurements and independent reviews are mandatory? Inspection-and-test plan and record index

Set boundaries and interfaces

Identify interfaces between civil, mechanical, electrical, instrumentation, controls, fire protection and commissioning organizations. Assign an owner for each boundary and define the hold point at which one organization accepts work from another. Interface records should identify the drawing revision, test condition, open items and person authorized to release the work.

Write the QA-program description

Organization and independence

Describe the organization in writing, including authority and duties. Give the QA function sufficient independence from production to stop work, reject inadequate results, require corrective action and report unresolved issues to management. Name responsible managers, quality personnel, engineering authorities, procurement staff, inspectors, test personnel, records staff and supplier representatives. Define who can approve a disposition, release a hold point and accept a completed turnover package.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Core program elements

The program description should address:

  • document and configuration control;
  • design control and design-change review;
  • procurement and supplier qualification;
  • inspection and test control;
  • measuring and test equipment;
  • special processes and personnel qualification;
  • identification and control of materials and status;
  • nonconformance, corrective action and trend analysis;
  • audits and management reviews;
  • training and qualification; and
  • records generation, retention, protection and retrieval.

For each element, state the governing procedure, responsible role, required inputs, approval authority, records produced and escalation route. Procedures should be usable by field crews, not merely describe policy.

Translate requirements into work packages

Every safety-related construction activity should have a controlled work package or equivalent set of instructions. Build it from the latest approved design information and procurement requirements, then verify that the package is complete before release.

Use a requirements traceability matrix

Map each regulatory, design-basis, code and procurement requirement to the document and record that demonstrates compliance. A useful matrix identifies the requirement, source revision, affected SSC or activity, responsible organization, inspection or test method, acceptance criterion, record number and closure status. Treat a deviation as a controlled change or nonconformance; never bury it in an informal field note.

Ten-step planning sequence

1. Set the governing basis

Approve the jurisdiction, regulator, license path, codes, standards and QA commitments. For a U.S. Part 50 project, include an Appendix B crosswalk and a review of applicable Part 21 responsibilities.

2. Classify SSCs and activities

Issue the safety-classification and quality-significance list. Link each entry to its design-basis function, procurement category, inspection depth, acceptance criteria and record requirements. Freeze the list under configuration control, with a documented process for additions and reclassification.

3. Approve the QA-program description

Define organization, independence, responsibilities, document control, design control, purchasing, supplier qualification, inspection and testing, equipment calibration, special processes, corrective action, audits, records and training. Obtain management approval before safety-related procurement is released.

4. Convert requirements into work packages

For each activity, identify prerequisites, approved information, materials, environmental conditions, hold or witness points, inspection method, acceptance criteria, personnel qualifications and record forms. Have QA and engineering review the package before issue.

5. Qualify suppliers and control materials

Flow technical and QA requirements into purchase documents. Qualify suppliers using an appropriate combination of evaluation, audit, survey, performance history and source surveillance. Plan receiving inspection, certificate review, material identification, traceability, storage and status marking. Apply dedication or commercial-grade controls where the licensing basis requires them.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

6. Plan inspection and testing

Issue inspection-and-test plans that identify prerequisites, independent verification, calibrated instruments, test boundaries, test conditions, acceptance criteria, data review and release authority. Mark the status of work so crews can distinguish accepted, rejected, held and pending items.

7. Control special processes

List processes whose results cannot be fully verified by later inspection. Typical examples include welding, nondestructive examination (NDE), concrete placement, coatings, lifting, pressure testing and software or digital controls. Qualify the procedure, equipment and personnel before production, define essential variables and retain objective evidence for each production application.

8. Control nonconforming work

Stop, identify or segregate affected work when a requirement is not met. Record the condition, determine the affected scope, obtain a disposition from authorized technical personnel, perform the repair, rework, use-as-is or reject action under controlled instructions, and verify the result. Evaluate whether the condition may require Part 21 or another regulatory report, and trend recurrence.

9. Audit and review performance

Schedule independent audits of the QA program, work instructions, suppliers, open corrective actions and record completeness. Management reviews should examine audit results, significant nonconformances, supplier performance, staffing and training, schedule pressure and turnover readiness. NRC licensing inspections examine whether QA plans, instructions and procedures are actually implemented for safety-related activities.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

10. Close out and retain records

Define turnover packages before construction starts. Specify as-built verification, punch-list closure, inspection and test records, calibration history, nonconformance dispositions, supplier record packages, design changes, training evidence and final indexes. Provide protected storage, retrieval controls and retention for the facility life cycle.

Supplier, procurement and material controls

Flow requirements into purchase documents

Purchase documents should identify the SSC classification, applicable specifications and drawings, codes, inspection and test requirements, qualification obligations, reporting expectations, records to be delivered, change-notification rules and rights of access for audits or source surveillance. State whether substitutions, deviations or process changes require prior approval.

Qualify and monitor suppliers

Use a graded supplier-qualification method. The evaluation should consider the supplier’s QA system, technical capability, personnel qualifications, special-process controls, calibration system, prior performance and ability to maintain traceability. Continue oversight through performance metrics, incoming inspection, source surveillance and audits rather than treating qualification as a one-time approval.

Preserve identity and condition

At receipt, verify identification, quantity, certificates, condition, shelf life, preservation and required documentation. Maintain heat, lot, batch, serial or other traceability as specified. Separate accepted, rejected, quarantined and unverified items physically or through an equally effective status-control system. Protect stored materials from environmental damage and prevent unauthorized substitution.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Inspection, testing and measuring equipment

Design independent verification points

Identify inspections that must be performed by someone independent of the person who performed the work. Mark hold points that prevent continuation without release, witness points where the inspector must be notified, and review points where records are checked after the activity. Define what happens if an inspector misses a hold point or a test result is invalid.

Control measuring and test equipment

Use equipment suitable for the required range, accuracy, resolution and environment. Assign a unique identification, calibration interval, status label and owner. Protect instruments from damage and unauthorized adjustment. Record the instrument used on inspection and test forms so the result can be evaluated against its calibration status.

If equipment is found out of calibration, determine the validity of measurements taken since the last acceptable calibration, identify affected SSCs and records, and document the technical evaluation and corrective action.

Make test records reproducible

A test record should state the item and configuration, prerequisites, instrument identification, test conditions, measured values, acceptance criteria, deviations, personnel, date, independent review and release decision. A pass or fail box without the underlying data is inadequate when the data are needed to demonstrate a design-basis requirement.

Special-process planning

Welding and joining

Control qualified welding procedures, essential variables, consumables, welder or operator qualifications, joint identification, preheat and interpass conditions, inspection and NDE results, repairs and post-weld treatment where applicable. Link each weld record to the drawing, component and material traceability.

Concrete, coatings and embedded items

Define approved mix or coating systems, storage and environmental limits, surface preparation, placement or application methods, curing or drying conditions, testing, repair criteria and records. Verify embedded items and interfaces before concealment; a later visual inspection cannot recreate evidence lost when work is covered.

Lifting, pressure and other irreversible activities

Qualify lifting plans, equipment and operators; verify load paths and pre-use inspections. For pressure or leak tests, define boundaries, calibrated instruments, test medium, temperature, hold time, acceptance criteria and safe release. Identify any activity where a later inspection cannot fully establish whether the original process was correct and apply enhanced controls before production.

Software and digital controls

Where software or digital instrumentation supports a safety-related construction, inspection or test function, include configuration identification, controlled versions, verification, access control, backup and change review in the QA scope. Treat a software change as a configuration change requiring impact evaluation, testing and approval.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Nonconformance, corrective action and regulatory reporting

Contain the condition

When work, material or documentation fails a requirement, stop the affected step, identify the item and prevent inadvertent use. Preserve evidence such as photographs, readings, samples, labels and the applicable document revision.

Evaluate and disposition

Determine the technical and programmatic significance, affected boundaries and whether similar items may be involved. An authorized engineering or design authority should approve repair, rework, replacement, reject or use-as-is dispositions. The disposition must identify revised instructions, inspections and tests needed to prove the result.

Verify closure and prevent recurrence

QA verifies that the disposition was implemented and required retesting or reinspection passed. Corrective-action owners should identify root or apparent causes, assign due dates, check effectiveness and trend recurring conditions. Escalate significant or repetitive issues to management review.

Check reporting duties

Evaluate each potentially reportable defect or failure to comply against Part 21 and any other applicable reporting rule. The QA procedure should specify who makes the determination, what technical review is required, approval authority, notification timing and record retention. Do not treat a nonconformance closeout as a substitute for a regulatory-reporting decision.

Audits, records and lifetime turnover

Audit the work as implemented

Audit plans should sample both the management system and field execution. Review whether current drawings were used, prerequisites were met, hold points were released, instruments were in calibration, personnel were qualified, records were legible and nonconformances were correctly linked. Include suppliers and subcontractors according to their safety significance and performance.

Design the record architecture early

Define a record index and unique identifiers before the first installation. Typical records include approved design and procurement documents, material certificates, traceability logs, personnel qualifications, special-process qualifications, inspection and test data, calibration certificates, audits, training, design changes, nonconformance reports and corrective-action evidence.

Protect authenticity and retrievability

Control revisions, signatures, electronic approvals, access, backups, media migration and disposition of superseded records. A record should show who performed and reviewed the work, when it occurred, which configuration and instruments were involved, the acceptance criteria and the final status. Retention and retrieval arrangements must support the facility life cycle and later regulatory review.

As-built and turnover package

Before system turnover, reconcile installed configuration with approved drawings and design changes, close punch-list items, verify test prerequisites and results, resolve open nonconformances, and assemble the supplier and inspection record package. The release authority should sign a checklist showing that required records are complete, indexed and retrievable.

Choosing an implementation approach

Organizations commonly combine an in-house QA organization, an integrated engineer-procure-construct system, or a prime-contractor system with qualified subcontractors. The label matters less than whether the approach provides the following capabilities.

Decision axis What to test Evidence to request before adoption
Regulatory acceptance Can the system map explicitly to Appendix B or the applicable national requirements? Clause crosswalk, approved procedures and prior regulator commitments
Safety grading Can controls, inspection depth and records scale with SSC importance? Classification method, sample graded work packages and approval rules
Traceability Can it preserve material, weld, inspection, test and configuration links? Traceability matrix, sample traveler and as-built reconciliation
Independence Can verification, audit and release authorities act independently of production? Organization chart, stop-work authority and escalation path
Supplier control Can requirements, audits, source surveillance and dedication controls operate across the supply chain? Purchase-order clauses, supplier list, audit schedule and incoming-inspection plan
Records and turnover Does the approach produce protected, retrievable lifetime records? Record index, retention plan, access controls and sample turnover package
Constructability Are prerequisites, hold points, environmental limits and acceptance criteria usable by field crews? Field work package, briefing record and completed inspection-and-test plan

Adapting the method to other safety-critical facilities

A chemical process plant, aerospace production site, high-hazard laboratory, dam or other safety-critical facility may use a different regulator and terminology. Start with its safety case or hazard analysis, identify systems whose failure has unacceptable consequences, and map those systems to the applicable legal, code and contractual requirements. Then apply the same control pattern: graded scope, written responsibilities, requirement flow-down, qualified suppliers and personnel, controlled special processes, independent verification, calibrated equipment, nonconformance control, audits and durable records.

Document the mapping so an auditor can see which local requirement replaces each nuclear-specific control. Where the local framework is less prescriptive, retain the control when it is needed to demonstrate the safety claim; where it is more prescriptive, follow the stricter requirement.

Common planning failures and recovery actions

Starting QA after procurement

Symptom: safety-related orders or site work begin before classifications, supplier clauses and record requirements are approved.
Recovery: stop affected releases, screen purchases and installed work against the governing basis, issue missing controls, and document a technical evaluation for any work performed without the required oversight.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Requirements trapped in design documents

Symptom: drawings contain acceptance limits that never appear in work packages or inspection forms.
Recovery: build the traceability matrix, revise controlled instructions, brief crews, and repeat or supplement inspections where the original evidence cannot demonstrate compliance.

Independent verification treated as a signature

Symptom: an inspector signs a form without seeing the item, checking prerequisites or reviewing the data.
Recovery: define the verification activity and evidence, retrain personnel, review affected records and perform a documented extent-of-condition evaluation.

Supplier records arrive at the end

Symptom: certificates, calibration evidence or special-process records are missing when turnover is due.
Recovery: place records on the procurement deliverables register, withhold acceptance where justified, perform source or receiving reviews, and resolve gaps before incorporation into the final record package.

Nonconformances closed without recurrence analysis

Symptom: individual repairs are accepted while the same defect repeats across crews or suppliers.
Recovery: trend the conditions, perform a broader cause analysis, expand the affected population, and change the process, training, supplier oversight or design control that allowed recurrence.
