Under 29 CFR 1926.502(d)(21), construction employers must look for wear, damage and other deterioration and remove defective components from service. A PFAS that has arrested a fall must also be taken out of service immediately and may return to employee-protection use only after a competent person determines that it is undamaged and suitable for reuse.
This guide gives employers, competent persons and workers a practical inspection and maintenance process for harnesses, lanyards, connectors, lifelines and related components. It does not replace the instructions for a particular model. Use the manufacturer's inspection, cleaning, storage and service instructions whenever they are more specific than this general workflow.

What a construction PFAS inspection must accomplish
A personal fall-arrest system is one option for construction workers exposed to vertical drops of 6 feet or more, as described in OSHA's construction eTool. The complete system normally includes a body harness, an anchorage and a connecting subsystem such as a lanyard or self-retracting lifeline (SRL). Inspection is not a one-time annual exercise: OSHA's 2003 interpretation says an annual inspection does not replace the inspection required before each use.
The routine check does not have to be performed by a competent person, but workers must be trained by a competent person in the correct inspection procedure. A competent person must make the suitability determination after impact loading and should evaluate any proposed change, substitution or unusual damage.

“Personal fall arrest systems shall be inspected prior to each use for wear, damage and other deterioration, and defective components shall be removed from service.”
—29 CFR 1926.502(d)(21)
An inspection is successful only when the worker can identify each component, examine it methodically, recognize a defect and stop the equipment from being used when its condition or compatibility is uncertain.
Prepare before touching the equipment
Identify the complete system
Separate the equipment into its actual components: harness, lanyard or SRL, connectors, lifeline, anchorage connection and any energy-absorbing element. Record the model or serial information if available and locate the manufacturer's instructions. Inspection criteria, allowable wear, cleaning methods, storage conditions and retirement decisions can differ by model.
Choose a safe inspection setting
Use enough light to see fibers, stitching, labels and hardware. Lay out the equipment so that one component cannot hide a damaged area on another. If dirt or contamination obscures a defect, do not treat the obscured area as acceptable; follow the manufacturer's cleaning instructions or quarantine the item until it can be evaluated.
Confirm the inspection status
Ask whether the equipment has arrested a fall, been exposed to heat, chemicals, welding, sandblasting or unusual loading, or been altered since its last use. Any affirmative answer changes the decision from a normal pre-use check to a removal-from-service evaluation.
Step-by-step pre-use inspection
The sequence below is a practical field process, not a substitute for the manufacturer's procedure. Move from the worker-contacting parts to the connecting hardware, and inspect every surface rather than only the areas that are easy to see.
1. Examine webbing, rope and lifelines
- Run the webbing or rope slowly through your hands while viewing both sides.
- Look for cuts, tears, frayed edges, pulled or broken fibers, abrasion, burns, melting, discoloration, mold, chemical attack and undue stretching.
- Check for stiffness, glazing, flattening, inconsistent diameter or other changes that suggest internal rope deterioration.
- Inspect the full length, including areas hidden under keepers, labels, protective sleeves and hardware.
- Do not accept a component merely because it still bears weight; a significant defect requires removal from service.
2. Check seams, stitching and splices
Inspect every load-bearing seam for broken, pulled, loose or missing stitches. Look closely at stitched terminations, sewn eyes and spliced rope ends. OSHA's Technical Manual recommends starting at one end of a lanyard, slowly rotating it so the entire circumference is viewed, and paying particular attention to spliced ends.

- One-Handed Loading - Keep one hand free with a 1/4 in. hex chuck that accepts 1 in. bit tips
3. Inspect harness body and attachment points
Examine the shoulder, chest, leg and pelvic straps, adjustment points, back pad, labels and the fall-arrest attachment point. Look for loose or damaged mountings, distorted hardware, damaged stitching and any alteration that could affect efficiency. The dorsal fall-arrest attachment point should sit in the center of the wearer's back near shoulder level or above the head when the harness is fitted, as described by OSHA's construction eTool.
4. Test buckles and adjusters as instructed
Inspect buckles, tongues, friction adjusters and keepers for cracks, distortion, corrosion or missing parts. A buckle tongue that does not fit its opening or does not remain engaged is defective. Operate hardware only in the manner specified by the manufacturer; do not force a mechanism, lubricate it with an unapproved product or improvise a repair.
5. Examine hooks, carabiners and connectors
Check connector bodies, gates, noses, locking mechanisms, springs and swivels. OSHA identifies distorted hooks and faulty hook springs as examples of defects. A gate that does not close, lock and remain locked as designed is not ready for use. Look for cracks, sharp edges, corrosion, deformation, damaged mountings and non-functioning parts.
6. Check energy-absorbing components
Inspect the energy absorber for a deployed or partially deployed pack, pulled stitching, torn cover, exposed internal webbing or other signs of activation. A lanyard or SRL that shows evidence of energy absorption must be removed from service while the manufacturer and competent person determine the next step.

Labels should remain present and legible enough to identify warnings, model information and required use limits. Missing or unreadable identification can prevent the required model-specific inspection. Quarantine the item until its identity and instructions can be established.
8. Verify the pieces are approved to work together
Do not assemble components because their connectors appear to fit. Confirm that the harness, lanyard, SRL, anchorage connector and other parts are approved for the intended configuration. A changed system must be fully evaluated by a competent person before use.
What to look for on a lanyard
OSHA's Technical Manual says lanyards should be kept clean and intact and inspected before each use for wear, tear, distortion or signs that an energy-absorbing system has activated. Begin at one end and rotate the lanyard gradually while checking the complete circumference. Inspect both terminations and pay special attention to spliced ends, sewn eyes, connector attachments and the energy-absorber cover.

For a rope lanyard, look for rope wear, internal deterioration, inconsistent diameter, hard or glazed sections and chemical or heat damage. For a web lanyard, inspect each edge and surface for cuts, pulled fibers, abrasion, mold, burns and stretching. A lanyard that has been tied in a knot, altered, shortened or connected in a way not authorized by its instructions should be removed from service pending evaluation.

Defects that require immediate quarantine
OSHA's non-mandatory appendix gives concrete examples of conditions that warrant withdrawal. The following table turns those examples into a field decision.
| Observed condition | Required response |
|---|---|
| Cuts, tears, abrasion, frayed fibers, mold or undue stretching | Remove the affected component from service and have it evaluated or destroyed according to the employer's program and manufacturer instructions. |
| Exposure to fire, acids or other corrosives; discoloration or heat damage | Quarantine immediately. Do not rely on appearance alone to establish remaining strength. |
| Distorted hook, faulty hook spring or a gate that does not function | Remove the connector from service; do not bend, file or repair it in the field. |
| Buckle tongue does not fit or remain engaged | Remove the harness or buckle assembly from service. |
| Loose or damaged mounting, stitching or attachment point | Quarantine the component for competent-person and manufacturer review. |
| Non-functioning part, missing hardware or unreadable identification | Do not use until the part and its suitability are established. |
| Rope wear or suspected internal deterioration | Remove the rope component from service; internal damage may not be visible. |
| Alteration, knot, unauthorized splice or other change | Quarantine and obtain a competent-person determination before any reuse. |
Mark failed equipment clearly as unusable and keep it physically separated from serviceable gear. A tag or marker communicates status but does not repair equipment and does not satisfy the requirement to remove a defective component from service. Destroy equipment when the employer's procedure or manufacturer requires destruction to prevent accidental reuse.
What to do after a fall or other impact
“Personal fall arrest systems and components subjected to impact loading shall be immediately removed from service and shall not be used again for employee protection until inspected and determined by a competent person to be undamaged and suitable for reuse.”
—29 CFR 1926.502(d)(20)
- Stop using every PFAS component that may have been loaded, including connected components whose condition is not known.
- Provide or obtain medical attention and carry out the site's rescue procedure; do not leave a suspended worker waiting for a routine inspection.
- Identify and quarantine the equipment so it cannot be returned to a truck, tool crib or worker by mistake.
- Check the manufacturer's post-fall instructions and provide the equipment to a competent person for a documented suitability determination.
- Replace or destroy components that the manufacturer does not authorize for reuse. A visual glance by the wearer is not enough to restore a loaded system.
If an energy absorber has deployed, treat that as evidence of impact loading. If the history is uncertain, use the more conservative decision and keep the equipment out of service until evaluated.
Cleaning and maintenance without damaging the system
Use the manufacturer's method
OSHA directs employers to obtain supplier instructions covering inspection, use, cleaning and storage. Those instructions govern the choice of detergent, solvent, brush, water temperature, rinsing, drying and any permitted lubrication. This guide does not prescribe a universal cleaner or drying method because materials and coatings vary.
Control environmental exposure
Evaluate seasonal weather and the work processes around the equipment. Welding, chemical cleaning and sandblasting can damage fibers, coatings and hardware. Protect PFAS from those operations where practicable, and establish a cleaning and maintenance program when the work environment makes ordinary wipe-downs inadequate.

Do not disguise damage
Cleaning must not be used to make a questionable component look acceptable. If contamination cannot be removed without an unapproved chemical or aggressive process, quarantine the item and obtain a manufacturer or competent-person decision.
Compatibility and system configuration
OSHA states that, ideally, a PFAS is designed, tested and supplied as a complete system. In practice, employers sometimes replace individual lanyards, connectors, lifelines, deceleration devices, body belts or harnesses as they wear at different rates. That practice does not make arbitrary combinations safe.
Do these 3 things before closing this tab:
1Clear out junk files and repair common Windows errors2Scan for outdated or missing drivers - takes under a minute3Repair Windows errors before they cause bigger problems“Ideally, a personal fall arrest system is designed, tested, and supplied as a complete system.”
Before changing a component, verify the manufacturer's approved combinations and the complete system's fall-clearance, load, attachment and environmental requirements. If the proposed substitution is not expressly approved, a competent person must fully evaluate or test it before use. Do not connect a lanyard between a harness and an SRL unless the manufacturer allows that configuration; the extra length can add free-fall distance for which the system was not designed.
- Never select components solely because their connectors look similar.
- Do not improvise with knots, sharp-edge tie-offs or unapproved adapters.
- Check that the anchorage connector, connecting subsystem and harness attachment are intended to function together.
- Reassess the complete system when the task, anchor location, work position or exposure changes.
Storage and service-life decisions
Store equipment using the manufacturer's specified method and protect it from the environmental conditions identified in the instructions. Keep serviceable equipment separated from quarantined equipment and prevent crushing, cutting, contamination or unauthorized modification during transport and storage.
Do not assign a universal retirement date from a generic chart. Service life depends on the model, use, exposure, inspection history and the manufacturer's instructions. Retire equipment when the manufacturer requires it, when a competent person determines that it is no longer suitable, after an unapproved alteration or whenever a significant defect cannot be resolved.

—29 CFR 1926.502(d)(20)
Before workers rely on a PFAS, evaluate who will perform the rescue, how the site will be reached, and what ladders, rescue equipment or descent-capable devices are available. Equipment alone is not a site-specific rescue plan. Account for access, communication, weather, the worker's condition and the time needed to reach a suspended person.
Include rescue readiness in the job briefing whenever the anchor, work area or crew changes. A system that can arrest a fall but cannot be followed by prompt rescue is not a complete work plan.
Harness, lanyard and SRL choices are task decisions
There is no universally preferable PFAS component. Select and inspect equipment against the task and the complete system design.
| Decision dimension | Questions to answer |
|---|---|
| Task and movement | Will the worker climb, move laterally, work from a platform or remain in one position? Is the selected subsystem approved for that movement? |
| Fall-clearance needs | Does the configuration provide the clearance required by the manufacturer and site assessment, including any additional length from connectors or adapters? |
| Compatibility | Are harness, connector, lanyard, SRL and anchorage parts approved together or evaluated by a competent person? |
| Environment | Could weather, welding, chemicals, sandblasting, edges or other operations damage the materials? |
| Fit and attachment | Is the harness adjusted correctly, with the fall-arrest attachment centered on the back near shoulder level or above the head? |
| Inspection and maintenance | What defects, cleaning steps, storage method and retirement criteria does the manufacturer specify? |
Training, quarantine and documentation
Train workers to recognize the actual defects listed in the manufacturer's instructions and OSHA's examples, not merely to look for a general “inspection sticker.” Demonstrate how to inspect the full circumference of a lanyard, operate connectors, identify an activated energy absorber and separate failed equipment.
Use a consistent quarantine process: mark the item unusable, isolate it from serviceable stock, record why it was removed and route it to the person authorized to decide repair, return, replacement or destruction. Keep the inspection and post-incident records required by the employer's program and any applicable manufacturer instructions. A record cannot make defective equipment safe, but it prevents a known failure from being forgotten or issued to another worker.
Troubleshooting common inspection decisions
The worker cannot identify the model
Stop use until the equipment can be identified and its instructions obtained. Do not substitute a generic cleaning or retirement rule.
The equipment is dirty but appears intact
Follow the specified cleaning method. If dirt or chemical residue hides fibers or hardware, quarantine the component rather than approving it on appearance.
