How to Compare Senior Living Communities Before You Move

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

The safest way to compare senior living communities is to use the same written checklist for every candidate: confirm the level of care, inspect everyday life, verify staffing and oversight, calculate the complete cost, read the contract, and test whether the location and availability work for your family. Visit several communities and treat online ratings as one piece of evidence—not a substitute for observing care and asking direct questions.

Start by defining the care and lifestyle fit

“Senior living” covers different settings, including independent living, assisted living, memory care, continuing care communities and skilled nursing facilities. They do not provide the same services, operate under the same license or face the same oversight. A community that is comfortable for an independent resident may be unable to provide daily nursing or advanced dementia care later.

Write down current and likely future needs

Before calling communities, list what help is needed now and what may be needed if health changes. Include:

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  • Bathing, dressing, toileting, eating, transferring and medication management.
  • Physical, occupational or speech therapy.
  • Regular nursing assessments, injections, wound care or other clinical tasks.
  • Memory-care supervision, behavioral support or secured spaces.
  • Hospice or palliative services, if they may become relevant.
  • Mobility equipment, fall risk, oxygen or other accessibility requirements.

If you are unsure which level of care is appropriate, ask a health care provider for an assessment. A social worker, hospital discharge planner or local aging specialist can also explain care-setting differences. The National Institute on Aging recommends starting with the person’s care needs and preferences before selecting facilities; see NIA’s choosing-a-care-facility guidance.

Record the resident’s preferences

Fit is more than clinical capability. Note preferred meal times and diets, religious or cultural needs, language, smoking policy, pets, room type, quiet hours, social activities, outdoor space and whether family can visit easily. Include transportation needs for medical appointments, shopping and worship. A community that meets a checklist of services but conflicts with daily routines may not be a sustainable choice.

Build a shortlist you can actually investigate

Use care fit, preferences, location and affordability to create a short list. Calling first prevents wasted visits and reveals how staff handle questions.

Call each community with the same script

  1. Describe the resident’s current assistance, diagnoses, mobility and likely near-term changes.
  2. Ask whether the community is licensed and equipped for that level of care and any required specialty service.
  3. Ask what services are delivered by the community’s employees versus outside agencies.
  4. Ask whether an appropriate room is available now, the expected move-in date and how a waiting list works.
  5. Request a current base-rate sheet, fee schedule and sample contract in writing.
  6. Ask what happens if care needs increase, and whether the resident can remain in place.

Keep the answers for every candidate in identical notes. If a representative will not provide written prices or gives vague answers about care changes, record that as a follow-up issue rather than relying on a verbal promise.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Use more than one visit

Visit several communities, preferably at different times of day. A scheduled tour can look different from an evening, weekend or mealtime visit. Ask to see the resident’s likely room, dining area, activity spaces, bathrooms, outdoor areas and any secured memory-care unit. Observe whether residents appear engaged, clean and comfortable; whether hallways smell strongly of waste; and whether staff acknowledge residents and visitors.

Compare care, services and daily operations

Ask every candidate the questions below. The answers should describe what happens in practice, not just what is listed in a brochure.

Care scope and changing needs

  • Which activities of daily living are included in the base service?
  • How are needs assessed before move-in and reassessed afterward?
  • Who creates and updates the service plan?
  • What triggers an added-care charge?
  • Can residents remain if they need two-person transfers, higher medication support, behavioral supervision or hospice?
  • If the community cannot meet a new need, how much notice is given and what transition help is offered?

Specialty programs

Ask whether the community has memory care, rehabilitation, skilled nursing, hospice coordination or other specialized services relevant to the resident. Confirm whether the service is on site, provided by a contracted organization or available only through a separate unit. Ask about staff training for dementia, falls, infection prevention and emergency response.

Staff availability and interaction

Ask who is on duty overnight, how residents summon help, and how the community covers illness, turnover and vacations. Request typical staffing by shift and the roles of nurses, aides, medication technicians, therapists and activity staff. A community may not publish a staff-to-resident ratio; you can still ask how long call lights usually take, how assignments are made and how families are notified about incidents or hospital transfers.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Meals, activities and transportation

  • Are menus posted in advance, and can the kitchen handle allergies, therapeutic diets and cultural preferences?
  • Can residents eat in their rooms, and are snacks available between meals?
  • How are activities selected, and are there options for different abilities and interests?
  • What transportation is included, to which destinations and at what charge?
  • Are outdoor areas accessible, shaded and available without an appointment?
  • How are religious services, clubs, visits from children and community outings handled?

Accessibility and family access

Check door widths, elevators, grab bars, shower design, floor surfaces, lighting, ramps and emergency call devices. Ask about visiting hours, overnight guests, parking, public transit and virtual visits. Measure whether the family can realistically travel there for routine visits and emergencies; a farther community may be unsuitable even if its building is attractive.

Use quality and oversight information correctly

What federal Care Compare covers

Medicare’s federal comparison tools are designed for Medicare-certified nursing homes. They are not a universal rating system for assisted living, independent living or every memory-care program. Use Medicare’s nursing-home selection guidance to find and compare certified facilities, then verify information during a visit.

For a Medicare-certified nursing home, record the separate Care Compare dimensions:

Dimension What to examine How to use it
Health inspections Inspection findings and cited deficiencies Look for patterns, severity, correction history and whether problems are recent.
Staffing Reported nurse and nursing-assistant staffing measures Use as a starting signal; ask how staffing works on the shifts you observed.
Quality measures Clinical and resident-outcome measures included in the system Check which measures apply and over what period they were calculated.
Overall rating The CMS star scale from 1 to 5 Read the underlying inspection, staffing and quality ratings instead of relying on one number.

CMS states: “No rating system can address all of the important considerations that go into a decision about which nursing home may be best for a particular person.” The CMS Five-Star Quality Rating System explanation also notes that ratings cannot capture every factor, such as specialty-care fit or how easily family can visit. Do not interpret an assisted-living community’s lack of a federal nursing-home star rating as a poor rating; the tool simply does not cover that setting.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Check state licensing and inspection records

Ask the community for its license type and the state agency responsible for oversight. Request the latest inspection reports, complaints or enforcement information available for that specific setting. Requirements differ by state and by care type. For nursing-home certification and compliance information, consult CMS’s nursing-home resources; for assisted living and other settings, use the relevant state regulator.

Compare the complete financial picture

Compare written totals, not advertised starting rates. Exact prices and contract terms vary by community, room and care level.

Separate the base rate from add-on charges

Ask the billing office to mark each item as included, optional, usage-based or triggered by a change in need. Common questions include:

  • Does the base rate include meals, utilities, housekeeping, laundry, activities, transportation and internet?
  • Are medication management, bathing, escorts, incontinence supplies, transfers or two-person assistance extra?
  • Are there one-time entrance, community, assessment, move-in or room-preparation fees?
  • What are charges for guest meals, salon services, parking, storage, higher-speed internet and additional transportation?
  • How are rent and care fees increased, and how much notice is provided?
  • Are fees different for a private room, shared room, studio or apartment?

Identify possible payment sources

Ask which expenses can be paid from personal funds, long-term-care insurance, veterans’ benefits, Medicaid or other programs, and which documents the community requires. Eligibility and coverage depend on the person, service and state; obtain a written explanation from the program or a qualified adviser rather than assuming a benefit will pay the bill.

Understand Medicare’s boundary

Medicare says most nursing-home care is custodial. Original Medicare does not cover custodial care when it is the only care needed. In qualifying circumstances, Medicare may cover short-term skilled nursing care after an illness or injury when the program’s conditions are met. It should not be treated as payment for long-term residential custodial care. Review Medicare’s nursing-home coverage information and ask the facility’s billing staff to explain any skilled-care claim in writing.

Read the contract before committing

Request the complete agreement, all fee schedules, resident handbook, discharge policy and any continuing-care disclosure before paying a deposit. Read it away from the sales meeting and mark every term you do not understand.

Clauses that deserve close questions

  • Initial term, renewal and notice periods.
  • Refundability of deposits and entrance fees if plans change.
  • Rules for annual increases and care-level reassessments.
  • Conditions that require a move to another unit or discharge from the community.
  • Responsibility for medical equipment, repairs, transportation and outside caregivers.
  • Medication, pharmacy, therapy and laboratory billing arrangements.
  • Room-change rights, roommate policies and priority for higher-care units.
  • Complaint, arbitration, grievance and emergency-contact procedures.
  • What happens after nonpayment, hospitalization or a temporary absence.

Ask the administrator to explain unclear language in writing. A trusted family member, elder-law attorney, social worker or long-term-care counselor can review provisions that affect finances or discharge rights. Do not sign a document whose fees or care obligations remain ambiguous.

Check availability and plan for a move

Ask whether the exact room type and care unit are available, not merely whether the community has a vacancy somewhere on campus. If there is a waiting list, request its placement rules, expected timing, deposit requirements and refund terms. Ask whether a temporary room can become permanent and whether moving between levels of care changes the contract or price.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Before moving, obtain a written medication list, physician orders, therapy records, emergency contacts and advance-directive information. Confirm who coordinates the move, what furniture fits, when the service assessment occurs and how the family receives updates during the first weeks.

A side-by-side comparison worksheet

Use one row per community and fill it from written documents, observations and follow-up calls. For nursing homes, add the federal ratings separately; do not use those fields to score other settings.

Comparison area Questions to record
Care level and specialty services What needs are met now? What memory, therapy, nursing, hospice or rehabilitation services are available, and who provides them?
Staff and response Who is present on each shift? How are call lights, emergencies, medication and family notifications handled?
Daily life What are the meal, activity, transportation, outdoor-access and visiting arrangements?
Accessibility Do the room, bathroom, entrances and common areas work for current mobility and likely equipment?
Oversight What license applies? What do state inspection and complaint records show? For certified nursing homes, what are the Care Compare inspection, staffing, quality-measure and overall ratings?
Availability Is the desired room and care unit open? If not, what are the waiting-list rules and timing?
Base rate What housing, meals, utilities, housekeeping, activities and transportation are included?
Additional charges Which services trigger fees, and how can charges change when needs increase?
Contract What are the term, deposit, increase, transfer, discharge, refund and dispute provisions?
Location How long will routine and emergency family trips take? Is parking or public transit practical?

Red flags that warrant a pause

  • Staff cannot explain who provides care overnight or how emergencies are covered.
  • The community refuses to provide a contract, fee schedule or inspection information.
  • Observed staffing, cleanliness or resident treatment differs sharply from the tour explanation.
  • Sales staff promise that a resident can remain indefinitely without a written policy.
  • Prices are presented only as a low starting rate with no explanation of care-level charges.
  • Residents and families are discouraged from asking questions or visiting at ordinary times.
  • There is pressure to pay a deposit before the family has reviewed terms or confirmed care fit.

When professional help is worthwhile

Bring in a clinician when the required care level is uncertain, a social worker when discharge or support planning is complicated, and an elder-law or benefits specialist when contracts, Medicaid, veterans’ benefits or asset decisions are involved. The NIA also points families to the Eldercare Locator, local Area Agencies on Aging or human services departments, health professionals and community organizations. These resources can help identify local oversight contacts and care options; they do not replace your own visit and contract review.

A practical decision sequence

  1. Define the person. Write current care tasks, likely changes, health constraints, preferences and family-travel needs.
  2. Filter candidates. Remove communities that cannot provide the required care, location or budget range.
  3. Call consistently. Ask about care scope, staffing, availability and written costs using the same questions.
  4. Visit multiple sites. Observe residents, staff, meals, activities, accessibility and the likely room at different times.
  5. Verify oversight. Check the applicable state license and inspection information; use CMS ratings only for Medicare-certified nursing homes.
  6. Calculate the full cost. Add care, medication, transportation, supplies, deposits, increases and likely future needs.
  7. Review documents. Read the contract, fee schedule and discharge rules with an adviser when needed.
  8. Confirm availability. Get the room, move-in date, waiting-list position and refund terms in writing.
  9. Choose on evidence. Select the community that best matches care reliability, daily life, transparent costs, enforceable terms and family access—not the one with the most polished tour.
