How Sewer Crews Use Tripods, Winches, and Harnesses for Confined-Space Rescue

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

A sewer crew can use a non-entry retrieval system to remove an incapacitated entrant without sending a second worker into the permit space. In the common vertical arrangement, a full-body or chest harness connects to an individual retrieval line, the line runs through an overhead support such as a personnel-rated tripod, and a self-supporting winch outside the opening lifts the entrant. The arrangement only works when the opening, line path, attachment point, and entrant’s profile allow a clear extraction.

Tripods, winches, and harnesses are equipment choices within a written rescue plan—not a universal solution for every sewer. U.S. requirements differ by work context: general industry is covered primarily by OSHA 29 CFR 1910.146, while construction work has a separate rule, 29 CFR 1926.1211. The space’s geometry, hazards, isolation options, communications, and available rescue service determine whether non-entry retrieval is appropriate.

What a non-entry retrieval system does

OSHA defines a retrieval system as a retrieval line connected to a chest or full-body harness, with wristlets where appropriate, and a lifting device or anchor. The line’s outside end must connect to a mechanical device or fixed point outside the permit space so rescue can begin when needed. The purpose is to move an entrant from outside the hazard rather than create another victim by sending an unprotected rescuer inside.

For a vertical permit space more than 5 feet deep, OSHA’s general-industry standard requires a mechanical retrieval device to be available, subject to exceptions when retrieval equipment would increase overall risk or would not contribute to rescue. A tripod and winch are one example of that device. A different anchor or mechanical arrangement may be more suitable when the access is offset, the opening is irregular, or internal obstructions prevent a straight lift.

How does a confined-space rescue tripod work?

The overhead support

A tripod is positioned over the sewer opening with its feet set on stable ground and its head centered as closely as practical over the intended extraction path. The support supplies an overhead lifting point and keeps the line from rubbing across the rim. It must be selected, configured, inspected, and used for personnel retrieval; a general-purpose material-handling frame is not automatically suitable for people.

The personnel-rated winch

The winch is mounted to or routed through the tripod in the configuration specified by its manufacturer and the employer’s rescue plan. An operator remains outside the space and turns the crank or operates the approved mechanism to take up the line. The winch must be designed and rated for personnel retrieval, not merely for pulling tools, debris, or equipment. Brakes, connectors, line, and attachment hardware must be compatible as a system.

The harness and individual line

The entrant wears a chest or full-body harness connected to an individual retrieval line. Under 29 CFR 1910.146, the normal attachment is at the center of the back near shoulder level or above the head. The employer may establish another attachment point only when it presents a sufficiently small profile for removal through the opening. The line then runs to the winch or another suitable mechanical device outside the space.

The lift sequence

  1. Alarm and stop work: The attendant summons the designated rescue service, tells the entrant to stop if communication remains possible, and prevents unauthorized entry.
  2. Clear the extraction path: The winch operator checks that the line is free of slack, sharp edges, pipes, ladder components, and other snag points.
  3. Take up line smoothly: The operator applies controlled tension and raises the entrant in accordance with the rescue plan. Sudden jerks can worsen injuries or cause the line to catch.
  4. Guide at the opening: Rescuers outside manage the entrant’s passage over the rim without placing themselves in the opening or entering the sewer unless they are trained, equipped, and specifically assigned to entry rescue.
  5. Transfer to medical care: Once clear, trained personnel provide first aid and CPR within their qualifications while emergency medical services take over.

How do you rescue someone from a sewer?

There is no single maneuver that makes every sewer rescue safe. The crew follows the permit, the preplanned rescue method, and the instructions of the designated rescue service. A practical sequence is:

1. Confirm the entry and rescue plan before anyone goes in

The permit program identifies the sewer’s hazards, acceptable entry conditions, isolation measures, atmospheric testing, communications, equipment, and the method for summoning rescue. The plan should state who operates the winch, who controls the attendant function, how the entrant will be packaged or guided, and where the entrant will be handed to medical responders.

  • One-Handed Loading - Keep one hand free with a 1/4 in. hex chuck that accepts 1 in. bit tips

2. Inspect and stage the retrieval equipment

Set the tripod or other support before entry, verify that its feet cannot shift, and confirm that the winch, line, connectors, and harness are compatible and in serviceable condition. Test communication equipment and make sure the line reaches the planned working depth without excess slack. The entrant should understand the alarm signal and the limits of the system.

3. Test the atmosphere in the required order

For atmospheric hazards, OSHA specifies testing oxygen first, followed by combustible gases and vapors, then toxic gases and vapors. In a sewer that is part of a continuous system, complete isolation may be infeasible. The employer must test as far as feasible before entry and continuously monitor conditions where authorized entrants are working when entry proceeds.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

4. Initiate non-entry retrieval when conditions require it

If the entrant becomes unresponsive, loses the ability to self-rescue, or an emergency condition develops, the attendant activates the planned response and summons rescue. The attendant should not improvise an entry into the sewer. OSHA’s rulemaking discussion explains that non-entry rescue exposes rescuers to less danger, and the permit-space record has included deaths of would-be rescuers.

5. Stop if the system cannot remove the entrant safely

A line can snag on internal structures, and an entrant’s shoulders, tools, clothing, or body position may not pass through the opening. If pulling increases the hazard or cannot produce a controlled extraction, the team follows the alternate rescue method and waits for trained entry rescuers when required. The retrieval system is intended to facilitate rescue, not guarantee success in every geometry.

Do sewer workers wear harnesses in confined spaces?

Entrants using a retrieval system under the general-industry rule wear a chest or full-body harness unless the employer can justify an alternative. The harness must connect to the retrieval line at a point that permits removal through the opening. A back attachment near the shoulders or an above-head attachment is the usual arrangement because it helps keep the entrant’s profile aligned with a vertical opening.

Wristlets are not a routine substitute. OSHA allows them only when the employer demonstrates that a chest or full-body harness is infeasible or creates a greater hazard and that wristlets are the safest and most effective alternative. The justification belongs in the employer’s hazard analysis and rescue plan.

Each entrant in a permit-required space is generally to have an individual retrieval line. OSHA’s interpretation allows sequential arrangements only when entrants can be removed individually and the arrangement does not impede removal. A single shared line should not be treated as automatically compliant.

When is a winch used for confined-space rescue?

A winch is used when the rescue plan requires controlled mechanical lifting from outside the space. In general industry, a mechanical device must be available for a vertical permit space deeper than 5 feet, subject to the retrieval exceptions in 29 CFR 1910.146. The device is especially useful when the entrant is unconscious, too weak to climb, or too heavy for a safe manual haul.

A winch is not automatically the right tool for a long horizontal sewer run, an offset manhole, or a space where bends and obstructions prevent a straight line. In those conditions, pulling harder can trap the entrant or damage the lifeline. The plan may instead require a different anchor, a guided system, an entry rescue team, or a determination that non-entry retrieval would increase overall risk.

Tripod and winch versus other retrieval arrangements

Planning question Tripod with personnel winch Fixed anchor or another mechanical arrangement
Access geometry Best suited to a relatively vertical opening with room to center an overhead support. May suit an opening with a permanent structural anchor or an offset line path, if the design controls edge and snag hazards.
Opening and entrant profile Requires a harness attachment and lift path that let the entrant pass through the opening. Must meet the same small-profile and removal criteria; a different anchor does not solve an undersized opening.
Line path Tripod head can keep the line clear of the rim, but internal obstructions can still catch it. Anchors may create rubbing, side loading, or deflection unless engineered and protected.
Equipment purpose Use a tripod and winch designed and rated for personnel retrieval. Use a fixed point or device that is designed for personnel retrieval, not an unverified pipe, railing, or material hoist.
Rescue-plan fit Works only if the plan covers setup, operation, communications, and transfer to care. Must be evaluated against the same space-specific hazards and response time.
Effect on risk Can enable non-entry rescue, but may be ineffective where the entrant cannot pass the opening. May reduce setup problems or may increase risk through side pulls, edge contact, or unstable anchorage.

Why sewer work changes the planning

OSHA specifically uses a sewer as an example of a continuous system that may be too large or interconnected to isolate. Gates, plugs, or other isolation methods may not control every connected flow or atmospheric change. When isolation is infeasible and entry is allowed, the employer must test as far as feasible before entry and continuously monitor conditions in the areas where authorized entrants work.

The permit should identify changing flow, engulfment potential, toxic gases, combustible atmospheres, oxygen deficiency, biological contamination, traffic or surface hazards, and any internal structures that could obstruct retrieval. Monitoring results and the time of each test should be recorded as required by the employer’s program. A tripod does not control any of these hazards; it only addresses one part of removing a person.

Rescue-service readiness and training

The employer must evaluate prospective rescue services for timely response, proficiency with the relevant spaces and equipment, and access to the spaces for planning and practice. Appendix F to 29 CFR 1910.146 provides non-mandatory evaluation criteria that can help organize that assessment.

When an employer designates its own rescue personnel, it must provide the necessary personal protective equipment and training for assigned duties. The rescue team or service must have current first-aid and CPR capability available. Employees affected by the permit program must practice simulated rescues at least once every 12 months. Practice should use the actual access points, communications, retrieval equipment, and obstacles expected at the sewer rather than a generic classroom demonstration.

Rescuers also need a defined handoff: who controls the winch, who monitors the entrant, who calls emergency services, where medical personnel can reach the patient, and how contaminated equipment will be isolated or cleaned after the event. These details prevent a successful lift from becoming a second emergency at the surface.

Construction-specific requirement

For construction work, OSHA 29 CFR 1926.1211 requires non-entry rescue unless retrieval equipment would increase overall risk or would not contribute to rescue. When the employer selects non-entry rescue, it must also confirm that emergency assistance will be available if the method fails. A construction employer should therefore document both why the chosen retrieval method is expected to work and what backup will respond if the entrant cannot be removed.

The general-industry rule and construction rule are related but not interchangeable. The controlling standard depends on the work and employer context, so a construction contractor should coordinate its permit program with the controlling OSHA requirements, the host utility’s procedures, and the designated rescue service.

Common failure modes and how crews address them

The harness catches at the opening

Recheck the attachment point and the entrant’s profile during planning. OSHA’s small-profile requirement exists because a harness connection that looks secure can still prevent passage. If the person cannot clear the opening, stop pulling and use the alternate rescue method.

The line snags inside the sewer

Map the line path, inspect for ladders, bends, valves, debris, and branch connections, and account for the entrant’s travel distance. A pre-entry pull test cannot prove that every emergency position will be free, so the rescue plan must state when non-entry retrieval is no longer effective.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

The tripod shifts or the winch cannot take load

Stop the lift, keep personnel out of the fall zone, and follow the equipment manufacturer’s emergency procedure. Reinspect footing, alignment, connectors, and rated capacity before entry begins; do not improvise by tying the winch to an unverified object.

Atmospheric conditions change during retrieval

Continue monitoring while the entrant is in the space and during the rescue. The attendant communicates readings and alarms to the rescue team, which may need respiratory protection, ventilation, or an entry-rescue capability rather than a simple surface haul.

The attendant is tempted to enter

The attendant’s role is to monitor, communicate, summon rescue, and perform only duties that do not expose the attendant to the hazard. An attendant should not make an improvised entry. Entry rescuers require their own training, equipment, and authorization.

What crews should verify before entry

  • The applicable OSHA standard—29 CFR 1910.146 for general industry or 29 CFR 1926.1211 for construction—has been identified.
  • The permit names hazards, acceptable conditions, isolation limits, atmospheric-testing order, monitoring frequency, communications, and rescue contacts.
  • The tripod, winch, line, harness, connectors, and anchor are designed and rated for personnel retrieval and are inspected before use.
  • Every entrant has an individual retrieval line unless a documented sequential arrangement permits individual removal without impeding anyone.
  • The attachment point and opening have been evaluated for a small enough profile and a clear extraction path.
  • The rescue service can reach the sewer, knows its layout and hazards, and has practiced with the equipment.
  • First-aid and CPR capability, emergency medical access, and backup assistance are available.
  • The team has a non-entry method and a defined alternative when retrieval would increase risk or cannot contribute to rescue.
