Hospital Renovation vs. New Construction: A Practical Framework for Choosing

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Neither hospital renovation nor new construction is automatically cheaper, faster, or safer. The defensible choice starts with the hospital’s future care model: define the services, workflows, support spaces, technology, safety requirements, and capacity you will need, then test renovation and a new build against that same program.

Use a functional program, condition and site assessments, safety and infection-control risk assessments, adopted code requirements, continuity-of-care plans, schedule analysis, and a whole-project financial model. If the existing building can support the program with acceptable clinical, safety, operational, schedule, and financial risk, renovation is viable. If it cannot, compare a new facility using the same criteria.

Why the decision is not simply “old building versus new building”

A hospital project is a clinical operating decision expressed through a building. The right question is not which delivery option sounds less expensive. It is whether each option can support the care model the organization intends to operate over the facility’s useful life.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Renovation may preserve a useful location, existing utilities, and familiar adjacencies, but hidden conditions, tight floor plates, infection-control measures, and phased work can make a limited-looking project complex. New construction can provide a clean layout and purpose-built infrastructure, but it introduces land, entitlement, financing, commissioning, relocation, and transition risks.

Facility Guidelines Institute (FGI) application guidance calls for a functional program and safety risk assessment during planning and design, with clinicians, infection-preventionists, and other care providers involved. Apply that discipline to both options rather than treating risk assessment as a renovation-only exercise.

FGI application guidance explains the planning approach.

Define the future care model before evaluating buildings

Write the functional program

The functional program translates strategy into requirements that can be tested. It should describe the services to be delivered, patient volumes and acuity, clinical workflows, staffing patterns, equipment, technology, support functions, and relationships between departments.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  • Clinical services to open, expand, consolidate, or retire.
  • Projected patient volumes, length of stay, acuity, and peak demand.
  • Patient, staff, supply, waste, and emergency movement patterns.
  • Rooms and support spaces, including clean and soiled work areas, medication preparation, storage, staff support, and family spaces.
  • Imaging, laboratory, pharmacy, sterile processing, food service, loading, and facility-support requirements.
  • Digital infrastructure, medical gases, power resilience, equipment clearances, and future flexibility.
  • Accessibility, behavioral-health safety, bariatric capacity, privacy, and patient-experience goals.

Do not let an existing room count define the program. First establish what care must be delivered; only then ask whether the current asset can accommodate it.

Include the people who operate and protect the hospital

Clinical leaders, frontline staff, facilities and engineering teams, infection-prevention staff, environmental services, emergency management, information technology, security, and finance each see constraints that a design team may miss. Involve them while alternatives can still change, not after a preferred concept has been selected.

Document assumptions and unresolved questions. A signed-off program becomes the common baseline for comparing renovation scope with a new-build scope.

What renovation offers—and where it can fail

Potential strengths

  • Location and access: The hospital can remain near its established referral base, transit connections, workforce, and community partners.
  • Existing infrastructure: Portions of the structure, utility distribution, central plant, parking, and site systems may be reusable.
  • Incremental delivery: Work can be phased around clinical priorities instead of waiting for one complete replacement facility.
  • Continuity of identity and operations: Some departments can remain in place while others are upgraded, if safe separations and temporary capacity are available.

Typical renovation constraints

  • Structural and dimensional limits: Existing column grids, floor-to-floor heights, shaft locations, floor loading, and ceiling plenums can restrict room sizes, equipment, and routing.
  • Legacy utilities: Electrical capacity, emergency power, medical gases, HVAC zoning, plumbing, and information-technology pathways may need replacement well beyond the visible work area.
  • Unknown conditions: As-built drawings can be incomplete, and concealed damage, hazardous materials, water intrusion, or undocumented modifications can change scope after demolition begins.
  • Operational interfaces: Construction must coexist with patients, staff, deliveries, emergencies, and sensitive procedures. A small work zone can affect an entire air-handling or circulation system.
  • Compromised adjacencies: A renovation may improve rooms without solving distances between departments, vertical transportation bottlenecks, or the separation of clean and dirty flows.

Renovation is not automatically a lower-risk alternative. Its feasibility depends on whether the existing building and site can deliver the functional program while maintaining safe operations during and after construction.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

What new construction offers—and what it adds

Potential strengths

  • Purpose-built planning: The organization can arrange departments, circulation, support spaces, and infrastructure around the future care model rather than inherited geometry.
  • Integrated systems: Structure, envelope, mechanical systems, power, communications, medical gases, and technology can be coordinated from the outset.
  • Operational separation: A new building can reduce construction exposure to active patient-care areas and may allow the existing hospital to operate until transition.
  • Long-term adaptability: Planned expansion zones, standardized room modules, and accessible utility routes can make future change easier.

New-build risks to test

  • Site and entitlement: Zoning, traffic, utilities, geotechnical conditions, stormwater, environmental review, and community approvals can control the schedule.
  • Capital and financing: Land, enabling works, new parking and roads, temporary facilities, financing costs, and owner-furnished equipment belong in the project case.
  • Transition: Moving patients, equipment, records, staff, and services requires rehearsals, commissioning, regulatory approvals, and contingency capacity.
  • Distance from existing services: A remote or separate site can complicate emergency access, workforce patterns, academic relationships, and shared support functions.
  • New operating footprint: More space can also mean more area to maintain, staff, secure, heat, cool, and eventually renew.

A new building is a better answer only when its additional capability and risk profile outperform a credible renovation scenario for the same program.

Compare both options against the same criteria

Prepare parallel scenarios. Do not compare a fully scoped new hospital with an underdeveloped renovation concept.

Where a value is not established for a specific hospital, it is not stated in the cited sources and must be developed through local investigations and estimates.

A step-by-step selection process

1. Approve the future-state brief

Set the planning horizon, service scope, capacity assumptions, resilience goals, and non-negotiable clinical outcomes. Record what is inside and outside the project so neither option receives an unfair advantage.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

2. Test the existing building and site

Commission a multidisciplinary condition assessment. Review structure, envelope, roofs, vertical transportation, fire and life safety, HVAC, plumbing, electrical distribution, emergency power, medical gases, communications, security, hazardous materials, water systems, and maintainability. Verify drawings in the field where decisions depend on them.

For a new site, perform equivalent due diligence: title and zoning, geotechnical and environmental studies, utilities, access and traffic, flood and stormwater requirements, constructability, and expansion potential.

3. Perform safety and infection-control risk assessments

FGI guidance treats safety risk assessment as part of planning and design. Evaluate patient safety, staff safety, emergency access, fire protection, falls, security, utility failures, and other hazards for both scenarios.

For work near care areas, complete an infection-control risk assessment with infection-prevention staff and the affected departments. Define barriers, pressure relationships, air filtration, negative-air arrangements where appropriate, dust control, worker routes, material handling, water protection, cleaning, monitoring, and response triggers.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

4. Confirm the governing code and authority

FGI describes its codes as baseline consensus minimum requirements for healthcare planning, design, and construction. Its editions page states that 2026 FGI Codes were scheduled for release in fall 2026. The 2026 FGI Code for Planning and Design of Hospitals product description says it incorporates the complete 2025 edition of ANSI/ASHRAE/ASHE Standard 170.

Those publication details do not by themselves determine what governs a project. Ask the authority having jurisdiction which edition, amendments, transition rules, and effective dates apply where the hospital is located. Confirm requirements for renovation, change of use, phased permits, accessibility, fire protection, emergency power, ventilation, water systems, and medical gases before fixing the scope.

5. Develop equivalent concept designs

Draw enough detail to test adjacencies, circulation, room modules, equipment clearances, shafts, loading, logistics, temporary conditions, and future expansion. A renovation concept should show every enabling project and temporary move. A new-build concept should show site infrastructure and transition facilities.

6. Build the continuity-of-care plan

Map how patients, staff, supplies, waste, emergency vehicles, and visitors move during each phase. Identify services that cannot tolerate interruption, such as surgery, intensive care, imaging, laboratory, pharmacy, sterile processing, oxygen supply, emergency power, network connectivity, and fire alarm systems.

Define shutdown permits, notification periods, backup systems, infection-control inspections, emergency stop-work authority, temporary signage, alternate routes, and recovery procedures. If the plan depends on swing space, prove that the space is available, licensed, staffed, equipped, and connected before relying on it.

7. Compare schedule and financial consequences

Model design, approvals, procurement, construction, commissioning, training, licensing, and occupancy—not just the time until substantial completion. For renovation, include surveys, selective demolition, discovery allowances, phased turnover, night or weekend work, temporary utilities, and lost clinical capacity. For new construction, include land and site work, utility extensions, financing, equipment installation, move planning, and dual operations.

Use scenario analysis rather than a single optimistic estimate. Test escalation, interest rates, discovered conditions, schedule delays, temporary capacity, and changes in clinical demand.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

8. Make a documented decision and preserve an exit

Set threshold conditions before selecting an option. Examples include inability to achieve required infection-control separation, insufficient emergency power, unacceptable patient-flow distances, lack of a safe construction route, or a site that cannot obtain approvals. If a threshold is breached, return to the alternative rather than progressively adding exceptions.

Infection prevention is a design and construction requirement

The CDC recommends multidisciplinary coordination that includes infection-control staff for construction and renovation. It also recommends education about airborne risks when work could affect immunocompromised patients. See the CDC Summary of Recommendations.

Apply that coordination from programming through occupancy. The project team should identify:

  • Patients and procedures with elevated sensitivity to airborne particles, waterborne organisms, noise, vibration, or service interruptions.
  • Construction barriers, pressure control, air-flow paths, filtration, monitoring locations, and alarm responses.
  • Dedicated worker entrances, elevators, corridors, staging areas, waste routes, and cleaning protocols.
  • Protection of water systems during shutdowns, tie-ins, stagnation, flushing, and recommissioning.
  • Inspection and documentation before an area is returned to clinical use.
  • Education for contractors, staff, and leaders, with a clear person authorized to stop work.

A new building reduces exposure to active clinical areas during construction but does not remove infection-prevention obligations. Commissioning, water management, ventilation verification, cleaning, and staff readiness still determine whether the finished space is safe to occupy.

How to handle cost evidence without false precision

There is no cited, directly comparable statistic that makes renovation or new construction the universal cost winner. Project cost depends on scope, condition, location, labor market, phasing, temporary capacity, escalation, financing, and the clinical consequences of delay.

CMS’s Cost Reports page provides hospital cost-report data through HCRIS. The page reports a dataset update dated June 30, 2026, released July 14, 2026. HCRIS describes provider-reported costs; it is not a renovation-versus-new-build estimate for a particular project.

Cost categories to include

Category Renovation New construction
Direct building work Alterations, selective demolition, structural reinforcement, envelope and interior work. Building, structure, envelope, interiors, and site infrastructure.
Systems and equipment Replacement or extension of HVAC, power, medical gases, plumbing, information technology, and clinical equipment. New central systems, distribution, controls, medical gases, technology, and equipment.
Temporary operations Swing space, decanting, temporary utilities, barriers, protection, logistics, and out-of-hours work. Existing-campus operations during construction, then move, dual operations, and temporary transition facilities.
Risk allowances Unknown conditions, hazardous materials, undocumented utilities, and phased discoveries. Site, geotechnical, entitlement, utility, market, and design-development risks.
Long-term cost Maintenance of retained systems and the timing of deferred replacement. New operating footprint, staffing, utilities, maintenance, and eventual lifecycle renewal.

For every line, identify who supplied the estimate, the date, escalation basis, scope exclusions, contingency, and whether the amount is one-time or recurring. Present a range and sensitivity cases when uncertainty is material.

Schedule, phasing, and continuity trade-offs

Renovation schedule questions

  • Can surveys and hazardous-material investigations be completed before design is fixed?
  • Which departments can move, and for how long?
  • Can shutdowns occur without compromising emergency power, medical gases, ventilation, fire protection, water, or network services?
  • Will phased permits or partial occupancy approvals add review time?
  • Does the infection-control plan permit the proposed sequence?

New-construction schedule questions

  • What approvals, land actions, utility relocations, and site works precede building construction?
  • When will long-lead equipment, generators, switchgear, air-handling systems, elevators, and clinical technology be ordered?
  • How long are commissioning, licensing, staff training, mock moves, and patient relocation expected to take?
  • What happens if the new facility is complete before some services are ready, or vice versa?

Use an integrated schedule owned jointly by the owner, designers, contractors, clinicians, infection-prevention staff, commissioning team, and move coordinator. A construction completion date is not an opening date until systems, staff, licenses, supplies, technology, and emergency procedures are ready.

Decision signals: when each path deserves priority

Renovation deserves serious priority when

  • The functional program fits the existing structural and utility framework without unsafe compromises.
  • Required services can be phased with credible temporary capacity and infection-control separation.
  • The site’s access, parking, logistics, and support functions remain workable.
  • Condition investigations are sufficiently complete to price risk honestly.
  • The total project case remains competitive after temporary operations and lifecycle costs are included.

New construction deserves serious priority when

  • The existing building cannot provide required adjacencies, room sizes, circulation, or infrastructure capacity.
  • Phased work would expose patients to unacceptable risk or require prolonged loss of essential services.
  • Code, accessibility, fire-safety, ventilation, or resilience upgrades would trigger disproportionate reconstruction.
  • The existing site cannot support logistics, expansion, emergency access, or safe separation of construction and care.
  • A new facility materially improves the future care model and remains feasible after land, transition, financing, and operating costs.

These are decision signals, not universal rules. Each must be confirmed through the project’s program, assessments, risk reviews, schedule, and financial model.

Common failure modes and corrections

Choosing by headline construction cost

Failure: Comparing a renovation contractor’s visible work with a new building’s shell cost.

Correction: Add enabling work, temporary capacity, professional services, equipment, financing, escalation, commissioning, move costs, and lifecycle implications to both scenarios.

Designing around the existing plan before defining care

Failure: Treating available rooms as the program.

Correction: Approve the functional program first, then test workflows and adjacencies with clinicians.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Leaving infection prevention until construction documents

Failure: Discovering that barriers, pressure relationships, routes, or water controls are unworkable after phasing is committed.

Professional references for the planning team

FGI’s 2026 FGI Handbook for Planning and Design of Hospitals is described by its publisher as a digital professional resource combining the hospital code with explanatory material, commentary, examples, graphics, and planning resources. Verify the edition and applicability to the project’s jurisdiction before relying on it.

Use the FGI application guidance, the governing FGI code edition, applicable standards adopted by the authority having jurisdiction, and CDC infection-control recommendations together with project-specific professional advice. None substitutes for a local condition assessment, clinical program, risk assessment, or permitting determination.

A practical decision sequence

  1. Approve the future clinical and operational program.
  2. Engage clinicians, infection-prevention staff, facilities, finance, emergency management, and other affected users.
  3. Investigate the existing building and candidate new site to a level that supports comparable decisions.
  4. Confirm the code edition, amendments, and effective dates with the authority having jurisdiction.
  5. Complete safety and infection-control risk assessments for renovation and new construction.
  6. Develop equivalent concept designs, phasing plans, continuity measures, schedules, and cost models.
  7. Stress-test unknown conditions, delays, escalation, temporary capacity, and demand changes.
  8. Select the option that meets the program with acceptable clinical, safety, operational, schedule, financial, and lifecycle risk.
  9. Document threshold conditions and a governance process for changing course if new evidence invalidates the preferred option.
