Elevator Maintenance Warning Signs: 3 Changes You Should Report

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

It is time to involve building management or a qualified elevator-maintenance provider when an elevator’s operation changes noticeably—such as new vibration or noise, unreliable doors, inaccurate floor leveling, or repeated service interruptions. Those observations are warning signs, not diagnoses. Report them promptly, do not force doors or interfere with safety devices, and do not use a car that building personnel have taken out of service.

What a maintenance warning sign actually tells you

An elevator is a system of mechanical, electrical and safety components. A rider can notice that the system is behaving differently, but the same symptom can have several possible causes, and only a qualified elevator professional can determine which component, adjustment or condition is involved. For that reason, a warning sign should trigger a report and a controlled response—not an attempt to identify the failed part.

“Maintenance” also covers more than fixing a breakdown. A provider may need to inspect, test, adjust, clean, lubricate, replace or document equipment under the building’s service arrangement and the requirements adopted by the local authority. The applicable schedule depends on the elevator type, the building’s jurisdiction and any special use of the property.

The three changes below are practical cues for building owners, property managers and users. The official standards discussed later do not publish a consumer checklist that links each sound or door behavior to a specific fault. Treat these observations as reasons to notify the responsible people.

1. The elevator starts operating differently

Changes riders may notice

  • A new or noticeably louder sound during a trip, stopping sequence or door cycle.
  • Vibration, shaking or a ride that feels less smooth than it did previously.
  • Jerky acceleration, deceleration or movement between floors.
  • Repeated interruptions, resets or cancellations of service.

A change can be intermittent. One quiet trip does not establish that the condition has disappeared, and one unusual sound does not establish which component is involved. Record what happened in ordinary terms: the time, floor, direction of travel, whether the car was loaded, what the ride felt or sounded like, and whether the condition repeated. Do not remove panels, enter a machine room, test switches or ask riders to recreate a problem.

How to respond

Use the building’s reporting channel as soon as practical—often a management office, facilities desk, posted service number or online work-order system. Give the exact car or bank if the building identifies them, describe the change without guessing at a cause, and say whether anyone was delayed or exposed to a fall, pinch or other immediate hazard. Management can then decide whether to request service, restrict the car or contact emergency responders under the building’s procedures.

2. The doors no longer open or close normally

Door behavior that deserves a report

  • Doors repeatedly fail to open or close completely.
  • A door reverses or behaves inconsistently without an obvious obstruction.
  • The opening or closing cycle is markedly different from the other cars or from the car’s usual behavior.
  • Passengers must repeatedly press controls or wait through several cycles before the doors respond.

Elevator doors interact with detection devices, controls and interlocks. An observer cannot safely determine whether an apparent door problem is caused by alignment, sensing, controls, obstruction or another condition. Do not hold a door with an object, force it open, block a sensor, bypass an interlock or reach into the sill. Those actions can create a hazard and can erase useful information for the technician.

Information that helps the service call

Tell management which landing was involved, whether the problem occurred when entering or leaving, whether the car was moving or stationary, and whether the behavior happened once or on several attempts. If a door remains open, will not close, or repeatedly reopens, follow the building’s instructions for using another car or waiting for assistance. Keep other people from experimenting with the door while the report is being handled.

3. The car does not stop level, or service interruptions keep recurring

Floor-leveling changes

A car that stops above or below the landing creates a trip hazard even if the doors open. Report any noticeable step at the threshold, especially if it is recurring or appears at more than one floor. Do not try to correct the position by jumping, rocking the car or operating controls repeatedly. Leave according to the building’s instructions and notify management with the floor and direction of travel.

Recurring outages and resets

A single interruption may be resolved by the building’s established procedure, but recurring interruptions are a pattern that deserves professional attention. Note how often the car is unavailable, which floors or times are affected, whether the display or indicators show a message, and whether another car remains available. Do not repeatedly reset equipment or enter restricted areas to restore service.

When use should stop

If the building posts an out-of-service notice, places barriers around the landing or tells occupants not to ride, stay out of the elevator until authorized personnel return it to service. Never remove a sign or barrier because the car appears to be working again. A professional assessment—not a successful test ride by a resident or employee—determines whether it can be used.

What to do immediately after noticing a warning sign

  1. Move to a safe position. Do not stand in a doorway that is cycling, enter a car with a visible hazard or reach into a gap. Follow posted instructions if the car stops or the doors do not operate.
  2. Report the observation promptly. Contact building management, the facilities team or the elevator-maintenance provider identified by the building. State what you observed, where and when it occurred, and whether it is recurring.
  3. Preserve useful details. A short written log can include the car number, floors, direction, approximate time, load, sound or motion observed, door behavior and any display message. Avoid recording people’s personal information or interfering with operation to obtain a video.
  4. Follow the building’s decision. Management or the provider may ask occupants to use another car, limit access or wait for inspection. Do not reopen a car that has been taken out of service.
  5. Escalate an immediate danger through the building’s emergency procedure. If someone is trapped, injured or facing an active hazard, use the posted communication method or emergency number rather than attempting a rescue or mechanical intervention.

When an elevator must not be used: the OSHA rule and its limits

For elevators in the marine-terminal workplaces covered by OSHA’s 29 CFR 1917.116, the rule states: No elevator or escalator with a defect which affects safety shall be used. That is a direct prohibition on use when a safety-affecting defect is present. The same section says elevator safety devices must not be overridden or made inoperable.

The scope matters. Section 1917.116 is a marine-terminal workplace regulation; it is not a universal inspection or operating rule for every office, apartment, hotel, school or retail building in the United States. A building outside that scope still has to follow the requirements adopted by its state, city, province or other authority having jurisdiction, along with its equipment instructions and service arrangements. Building managers should identify the rules that apply to their location rather than importing a marine-terminal schedule by assumption.

Inspection intervals are jurisdiction-specific

Under the cited marine-terminal provision, elevators receive a thorough inspection at intervals not exceeding one year, plus additional monthly inspections of satisfactory operation by designated persons. Records of the latest annual elevator inspection must be posted in elevators in that setting. These intervals are useful examples of how a regulation can combine a formal inspection with more frequent operational checks, but they are not an automatic schedule for all buildings.

Elsewhere, the authority having jurisdiction may set different inspection, testing, permitting and record-posting requirements. Equipment type also matters: electric and hydraulic elevators can have different procedures, and special environments or uses can add requirements. A property manager should keep the applicable certificate or inspection record, know the due dates required locally and confirm that the maintenance provider’s visits satisfy the contract and regulatory obligations.

How ASME standards fit into the maintenance decision

A17.1/CSA B44

ASME describes A17.1/CSA B44 as a widely accepted North American safety-code framework covering elevator design, construction, installation, operation, inspection, testing, maintenance, alteration and repair. The edition and provisions that control a particular building depend on what the local jurisdiction has adopted. A reference to the framework does not replace the authority having jurisdiction’s rules or a qualified professional’s evaluation of a reported symptom.

A17.2 (2023)

ASME lists A17.2 (2023) as a guide to recommended inspection and testing procedures for electric and hydraulic elevators, escalators and moving walks. ASME also cautions that the guide may not reflect the latest requirements in current A17.1 and A17.3 codes. It is a technical reference for trained professionals, not a do-it-yourself repair manual or a consumer troubleshooting checklist. Owners who use it for planning should verify the edition and coordinate its use with the applicable adopted code and qualified personnel.

A practical reporting and maintenance workflow for building managers

1. Make the reporting route obvious

Post a current management or service contact where occupants can find it, and ensure staff know the route for routine observations, urgent hazards and trapped-passenger calls. A report that reaches the right person quickly is more useful than a technically detailed report sent to an unattended inbox.

2. Triage without diagnosing

Classify the report by observed risk: unusual operation, door behavior, leveling concern, repeated outage or an immediate injury hazard. That classification helps staff decide whether to restrict use under the building procedure while waiting for the provider. It should not be converted into a claim that a particular motor, cable, sensor or controller has failed.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

3. Give the technician reproducible facts

Provide the car identification, floors, direction, time, frequency and any displayed message. Tell the provider whether the condition is still present and whether the car has been isolated. Do not ask untrained staff to enter a machine room, remove guards or perform a test that the provider has not authorized.

4. Record the outcome

Keep the service call, inspection result, restrictions placed on the car and return-to-service authorization with the building’s maintenance records. If the provider recommends monitoring, write down what occupants should report and who will review subsequent reports. This creates continuity when a problem is intermittent.

Common mistakes that make an elevator concern less safe

  • Waiting for a dramatic failure: A new vibration, door inconsistency or leveling problem is enough to report; occupants do not need to wait for a shutdown.
  • Guessing the cause: A sound or movement can have multiple explanations. Describe the observation and leave diagnosis to the elevator professional.
  • Overriding a safety feature: Never defeat a door detector, interlock, alarm or other safety device to keep service running.
  • Testing an out-of-service car: A posted restriction remains in force until the responsible personnel remove it.
  • Applying the wrong inspection calendar: The annual and monthly intervals in OSHA 29 CFR 1917.116 apply to the marine-terminal context cited above, not automatically to every building.
  • Treating a technical guide as permission to repair: ASME A17.2 supports professional inspection and testing; it does not make an untrained person qualified to work on an elevator.

A concise checklist for occupants and owners

  • Did the ride, sound, vibration or stopping behavior change?
  • Are the doors repeatedly failing to complete their normal cycle?
  • Is there a step between the car floor and landing?
  • Are interruptions recurring, or is the car repeatedly resetting?
  • Was the car posted or announced as out of service?
  • Have you reported the car, floor, time, direction and observed behavior?
  • Are you following the building’s restriction and waiting for qualified personnel?

The useful boundary is simple: notice, report and follow the building’s safety direction. Do not turn a symptom into a diagnosis, and do not use an elevator that has been withdrawn from service.
