Assisted Living Facility vs. Nursing Home: Care, Services, and Costs Compared

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

The practical difference is care intensity, not the building’s label. Nursing homes are designed for people who generally need 24-hour care, including skilled nursing, rehabilitation, or ongoing help with basic activities. Assisted living usually combines housing with help with daily routines, but its clinical services, staffing, admission rules, and prices vary by state and residence. The right choice depends on what the person needs now, how quickly those needs may change, what the setting can safely provide, and how the care will be paid for.

This U.S.-focused guide explains the distinction, compares services, shows the 2025 national median prices, and gives a process for evaluating real facilities.

What is the difference between assisted living and a nursing home?

A nursing home generally serves residents who need continuous supervision or hands-on care, often including licensed nursing, rehabilitation after an illness or operation, or long-term assistance with bathing, dressing, eating, toileting, transferring, and other activities of daily living. Medicare describes nursing homes as facilities for people who usually need 24-hour care. A stay may be short term for recovery or long term for custodial support.

Assisted living is a broader category. It commonly offers an apartment or room, meals, housekeeping, activities, and help with selected daily tasks. However, “assisted living” does not identify one nationwide service package. States use different names and licensing categories, and individual residences set their own admission criteria, staffing model, medication policies, and limits on medical or behavioral needs. One residence may coordinate substantial personal care while another may be intended for people who need only occasional help.

Do not choose from the labels alone. Ask whether a specific residence can safely meet the person’s current needs, what happens overnight, which clinicians are available, and what event would require a move.

When a nursing home is usually the better fit

24-hour supervision or nursing oversight

A nursing home is the setting to investigate when a person needs care around the clock, cannot be left safely alone, or requires frequent assessment by nursing staff. The exact staffing pattern differs by facility and state, so confirm who is present on every shift rather than assuming that a particular license guarantees a ratio or skill mix.

Short-term rehabilitation after hospitalization

Many nursing homes include a skilled nursing facility (SNF) service for temporary recovery after surgery, injury, stroke, serious infection, or another qualifying medical event. Rehabilitation may include physical, occupational, or speech therapy alongside nursing care. A short SNF stay and a permanent nursing-home placement are different decisions, even when they occur in the same building.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Long-term custodial care

Some residents remain in a nursing home because they need continuing help with personal care that cannot be delivered safely in the community. This is custodial long-term care, not the same as a short, insurance-covered rehabilitation episode. The facility should explain how it handles progressive conditions, falls, wounds, incontinence, dementia-related behaviors, and changes in mobility.

When assisted living may fit better

Help with daily routines while preserving more independence

Assisted living can suit someone who is medically stable but needs support with meals, bathing, dressing, medication routines, housekeeping, transportation, or reminders. Residents typically live in a private or shared unit and participate in activities and meals in a communal setting. The arrangement can provide supervision without the more intensive clinical model of a nursing home.

Services must be verified residence by residence

State rules do not establish one universal assisted-living service list in the evidence available for this comparison. Before signing an agreement, ask the residence to describe, in writing:

  • Which activities of daily living staff can assist with and whether help is scheduled or available on demand.
  • How medications are stored, administered, documented, and adjusted when prescriptions change.
  • Whether staff respond overnight, how quickly they respond, and who is on site.
  • How meals, special diets, transportation, and appointments are handled.
  • Whether memory support or secured areas are available and what behavioral needs are excluded.
  • How outside physicians, home-health clinicians, hospice teams, and emergency services are coordinated.
  • What changes in health or behavior would make the resident ineligible to remain.

Verify the residence’s license and complaint or inspection information with the state agency that regulates it. A residence’s marketing description is not a substitute for its license or contract.

What do assisted living and nursing homes cost?

CareScout’s 2025 Cost of Care Survey reported these national median figures. They are benchmarks, not quotes for a particular state, neighborhood, room, or care plan.

Setting and pricing basis 2025 national median How the figure is presented
Assisted living facility care services $74,400 per year CareScout describes this as the monthly fee multiplied by 12 months
Nursing home, semi-private room $114,975 per year Annual room-based cost
Nursing home, private room $129,575 per year Annual room-based cost
Nursing home, semi-private room $315 per day Daily rate reported by CareScout
Nursing home, private room $355 per day Daily rate reported by CareScout

CareScout says it contacted 211,985 long-term-care providers by phone and email and completed approximately 16,000 surveys from July through November 2025. The results were published in 2026. A median is the midpoint of the surveyed prices; it is not an average and does not predict your local quote.

Why these numbers are not perfectly like-for-like

The assisted-living figure is an annualized monthly fee for care services. The nursing-home figures distinguish room type and are reported as both annual and daily amounts. The bundles can include different levels of personal care, nursing, therapy, meals, supplies, and other services. A lower base fee may become more expensive after care-tier charges or add-ons are included.

Request an itemized written fee schedule

Ask every facility for a current price sheet and a sample monthly bill. Identify:

  • The base room or apartment charge and whether it is guaranteed for a stated period.
  • Care-level assessments, tier changes, and hourly or task-based fees.
  • Medication administration, incontinence supplies, wound supplies, and other clinical charges.
  • Memory-support or secured-area charges.
  • Transportation, laundry, cable, internet, guest meals, and activities that cost extra.
  • Admission deposits, community fees, refund rules, and notice requirements.
  • How often prices can rise and how much notice the resident receives.

Medicare’s nursing-home checklist specifically advises requesting the written fee schedule and asking which services are included or extra. The checklist PDF dates from 2019, so use it for comparison questions, not current prices.

How Medicare, Medicaid, and insurance pay

Medicare and short-term skilled nursing facility care

Medicare Part A may cover qualifying short-term SNF care after a qualifying hospital stay, when medical and other conditions are met. The Medicare guidance cited for 2026 lists $0 per day for days 1 through 20 after the applicable Part A deductible, $217 per day for days 21 through 100, and all costs from day 101 onward. Coverage is limited to 100 days per benefit period under the stated conditions. Confirm the person’s benefit-period status, medical qualification, plan rules, and notices before relying on this coverage.

This benefit is not a promise to pay for indefinite residence. Medicare’s plain-language rule is: “Medicare doesn’t cover custodial care if it’s the only care you need.” A person who needs only ongoing help with bathing, dressing, meals, or supervision generally cannot use Medicare to pay the entire long-term custodial bill.

Medicare does not routinely pay assisted-living room and board

Medicare explains that long-term care can occur in an assisted-living residence, nursing home, community setting, or at home, but Medicare and most Medigap policies do not pay for non-medical long-term-care services. Do not assume that Medicare will cover an assisted-living residence’s rent, meals, or continuing custodial fee.

Medicaid and nursing facilities

Medicaid is administered by the states. It may cover nursing-facility care for a person who meets the state’s financial and medical eligibility rules and lives in a Medicaid-certified nursing facility. Federal Medicaid guidance describes nursing-facility services as including skilled or medical care, rehabilitation, and qualifying long-term health-related care unavailable in the community.

Eligibility, asset rules, patient-pay calculations, covered services, and waiting lists differ by state. Most nursing homes participate in Medicaid, but not all do. Ask the facility whether it is certified, whether it accepts new Medicaid residents, how a private-pay resident transitions if funds run low, and whether a specific room remains available under the program.

Long-term-care insurance and private payment

Medicare identifies private long-term-care insurance as another possible funding source. Policies vary: some cover only nursing-home care, while others cover assisted living, home care, or a wider range of services. Review the policy’s benefit triggers, elimination period, daily or lifetime limits, inflation protection, covered settings, exclusions, and documentation requirements.

If no public or insurance benefit applies, the person or family may need to pay privately. Compare the projected monthly total with available income and assets, and ask an elder-law or benefits professional how a move could affect eligibility and estate planning in the person’s state.

A step-by-step way to choose the right setting

  1. Write a current-care profile. Record diagnoses, falls, transfers, bathing and dressing assistance, continence, eating, medication complexity, wandering, nighttime behavior, wounds, therapy needs, and the amount of supervision required.
  2. Separate skilled needs from custodial needs. Ask the treating clinician whether the person needs licensed nursing, daily therapy, wound care, injections, or post-hospital rehabilitation. Also list tasks that require personal assistance but not clinical treatment.
  3. Estimate the next six to 12 months. Consider progressive dementia, repeated falls, worsening mobility, oxygen, swallowing problems, or a caregiver who can no longer provide safe support. A setting that meets today’s needs may be unsuitable soon.
  4. Build a short list by capability. Include only residences that accept the person’s care profile, have the needed license or certification, and fit the likely payment method.
  5. Get complete prices in writing. Request the base rate, care tiers, deposits, add-ons, annual-increase policy, and a sample bill. Ask what happens after a hospital stay or a temporary change in care.
  6. Inspect and interview. Visit during a meal and, if possible, at a less predictable time. Notice response times, cleanliness, odors, food service, resident engagement, call-bell response, and whether staff can explain the care plan.
  7. Check quality information. For Medicare- or Medicaid-certified nursing homes, use Medicare’s Care Compare information and review inspection findings, staffing information, and reported deficiencies. Treat the rating as a starting point, then ask about the issues most relevant to the person.
  8. Confirm the transition plan. Put in writing what happens if the person needs a higher care level, develops behavioral symptoms, exhausts private funds, or returns from a hospital with new equipment or restrictions.
  9. Compare contracts before paying a deposit. Have the resident or representative read discharge, refund, grievance, medication, arbitration, and notice provisions. Seek professional advice for legal or benefits questions.

Questions to ask an assisted-living residence

  • What license category governs this residence, and can I see the current license?
  • How many staff members are present on day, evening, and overnight shifts?
  • Who administers medications, and what happens when a resident needs a new or complex regimen?
  • How are falls, missed medications, dehydration, or sudden confusion handled?
  • Can staff help with transfers, toileting, bathing, eating, and nighttime needs? Are those services included or separately billed?
  • Is memory support available on this campus, and what behaviors or wandering risks require transfer?
  • Can residents receive outside home-health, hospice, therapy, or private-duty services?
  • How is the care plan reassessed, who participates, and how are family members notified?
  • What conditions would cause the residence to issue a move-out notice?

Questions to ask a nursing home

  • Is the facility Medicare- and Medicaid-certified, and are both short-term rehabilitation and long-term care offered?
  • What nursing and therapy services are available on site, and which providers visit?
  • How are hospital discharges coordinated, including medications, equipment, follow-up appointments, and therapy goals?
  • What are the semi-private and private-room rates, and which supplies or services are extra?
  • How does the facility handle residents whose rehabilitation ends but who still need custodial care?
  • What are the rules for family visits, care-plan meetings, complaints, and emergency contact?
  • What happens when a resident’s needs exceed the facility’s staffing or equipment?
  • Can the facility explain recent inspection findings and its plan to correct deficiencies?

Common mistakes that lead to a poor placement

Choosing by appearance or reputation alone

A polished lobby does not establish that staff can manage transfers, medication complexity, dementia-related behaviors, or overnight emergencies. Match the observed services to the person’s care profile.

Comparing only the advertised base price

Base rent can exclude care tiers, medication help, supplies, transportation, and memory support. Compare the estimated total for this resident, not the entry-level rate.

Assuming a nursing home stay is automatically Medicare-paid

Medicare coverage is tied to qualifying short-term skilled care and benefit rules. It does not pay ongoing custodial care simply because the person lives in a nursing home.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Waiting until a crisis forces the decision

Hospital discharge deadlines, a fall, or caregiver burnout can leave little time to check licenses, contracts, Medicaid status, and availability. Begin comparing settings when needs are changing, not only after an emergency.

Ignoring the likely next move

Ask what happens if the person needs two-person transfers, more supervision, skilled treatment, or secured memory support. A lower-intensity setting may be appropriate now but impractical if no higher level is available nearby.

How to make the final decision

Choose assisted living when the person can live safely in a residence that provides the required daily-living assistance, medication support, supervision, and emergency response, with a realistic plan for increasing needs. Choose a nursing home when continuous care, skilled nursing, rehabilitation, or long-term support exceeds what the assisted-living residences under consideration can safely deliver.

Document the decision in a comparison sheet: current needs, expected changes, services confirmed in writing, total monthly cost, payment source, certification, quality information, contract terms, and the trigger for reassessment. Revisit the choice after a hospitalization, major fall, new diagnosis, medication change, or sustained decline in mobility or cognition.
