For adults with chronic nonspecific low back pain, a medium-firm mattress is a reasonable place to start—not a universal prescription. The available clinical evidence compares firmness categories rather than today’s branded mattresses, so it cannot establish that any specific model treats a spinal condition. Here’s how to use that evidence, assess your own fit, and consider the limited product recommendations available.
What the clinical evidence says about mattress firmness
A randomized, double-blind multicenter trial assigned 313 adults with chronic nonspecific low back pain to firm or medium-firm mattresses. Participants had no referred pain and reported backache while lying in bed or on rising. The medium-firm group had greater improvement in pain and disability than the firm group, leading the authors to conclude that medium firmness improved those outcomes in this study. Kovacs et al., The Lancet, 2003.
A 2026 systematic review of studies published from 1999 through 2023 likewise found medium-firm mattresses to be the most consistently supported practical starting point in the available evidence. It emphasizes pressure distribution, neutral spinal alignment, and individualized judgment rather than a single firmness that works for everyone. Journal of Orthopaedics and Traumatology, 2026.
These findings do not establish that a mattress will treat diagnosed spinal disorders, sciatica, acute pain, or every cause of back pain. The trial’s firmness values—Hs 2.3 for firm and Hs 5.6 for medium-firm—are not directly interchangeable with a retailer’s subjective 1-to-10 scale.

Why the right mattress depends on the sleeper
Firmness is only one part of fit. Body habitus, sleep position, age, pressure tolerance, and personal comfort can change how a mattress feels and whether it supports a comfortable, neutral posture. A mattress that feels supportive to one person may create uncomfortable pressure or insufficient support for another.
- Check alignment and pressure together. Consider whether your usual sleeping position feels supported without uncomfortable pressure buildup.
- Use firmness labels cautiously. Brands and reviewers may use different scales, so “medium-firm” is a useful starting description, not a standardized guarantee.
- Don’t rely on “orthopedic” marketing. The reviewed evidence does not validate that label as proof of therapeutic benefit.
- Seek clinical advice when appropriate. A mattress choice is a comfort and support decision, not a substitute for diagnosis or treatment. Consult an appropriate clinician about persistent or concerning symptoms.

How to compare a mattress before buying
- Start with your usual sleep position and comfort needs. Consider where you feel pressure and whether your body feels supported in the position you actually use.
