The practical choice is an adjustable chair that fits the person and workstation, with supportive lumbar positioning, suitable seat dimensions, and armrests that do not interfere with work. Because symptoms and comfort vary, assess the fit in use rather than choosing by a medical-sounding label.
What office chair is best for spondylolisthesis?
Choose by fit and adjustability, not by a claim that a particular model treats spondylolisthesis. The Occupational Safety and Health Administration (OSHA) says, “A chair that is well-designed and appropriately adjusted is an essential element of a safe and productive computer workstation.” Its guidance focuses on lumbar support, backrest adjustment, seat dimensions, height, and armrest fit—not on a chair ranking for this condition. OSHA’s chair guidance recommends trying different chairs before purchase.
Spondylolisthesis is a condition in which one vertebra slips forward. The NHS notes that some people have no symptoms; others may have lower-back pain, pain spreading to the bottom or thighs, tight hamstrings, or pain, numbness, or tingling down one leg. Symptoms can be worse with standing or walking and eased by sitting or bending forward, but this pattern does not apply to everyone. What chair features should you look for?
Backrest and lumbar support
Look for a backrest that supports the lower back and can be positioned or reclined to a comfortable, supported angle. If lumbar support is adjustable, check that it can be set at a height that suits the person using the chair. A feature is useful only if it fits the individual; fixed or pronounced support is not automatically better.

Seat height, depth, and width
Set seat height so the user can sit back against the backrest with feet supported, without the seat edge pressing into the area behind the knees. Seat width and depth should accommodate the user comfortably. OSHA’s workstation evaluation checklist calls for seat dimensions and height suited to the individual, along with lumbar-area support.
Feet, desk, and armrests
The chair must work with the desk, keyboard, and foot support as a system. The user should be able to keep feet flat on the floor or on a stable footrest while sitting at the work surface. Armrests should support the forearms without raising the shoulders, and should not prevent the chair from approaching the desk or make entering and leaving difficult. If adjusting chair height leaves the feet unsupported, a footrest may help preserve support while aligning the user with the work surface.

Adjustability and a real-world trial
Before buying, check whether the person who will use the chair can make its adjustments and whether it fits the actual desk and work tasks. Try more than one chair where possible, as OSHA advises. For a shared chair, test whether it can be reset for each user rather than assuming one adjustment will suit everyone.
How should workers set up and use the chair?
- Adjust the seat. Set the height so feet rest on the floor or a stable footrest. Sit back and check that the front edge is not pressing behind the knees.
- Position the backrest. Set lumbar support, if adjustable, where it supports the lower back, then adjust the backrest to a comfortable supported position.
- Fit the desk and keyboard. Bring the chair close enough to work without reaching, while leaving room for armrests and comfortable arm positioning. Check that desk height does not force the shoulders upward.
- Check the whole setup. Confirm that feet, back, arms, and work surface remain supported in the position the person actually uses. OSHA’s computer workstation positions guidance treats positioning as part of workstation setup.
- Change position and move. Make small chair or backrest adjustments and stand or walk periodically. Mayo Clinic advises against remaining in the same sitting position for hours; see its office ergonomics guide, published May 25, 2023.
Do not force a single posture because it is marketed as ideal. Some people with this condition feel better sitting; others may find particular positions uncomfortable. Use comfort and practical support as guides, and seek clinical advice for persistent or concerning symptoms.

How should employers choose seating for workers with back pain?
Where possible, involve each worker in selecting and fitting a chair. Provide adjustable seating, allow a trial, and evaluate the chair together with desk, keyboard, and foot support rather than treating the chair as the entire solution. In a shared workstation, ensure adjustments are accessible and workers can change them between users. OSHA’s chair and evaluation guidance can help structure those checks, but neither supplies a condition-specific chair recommendation.
- Check that lumbar support and backrest positioning can be adjusted to the user.
- Confirm seat height and dimensions suit the user and the task.
- Make sure feet can be supported and armrests do not obstruct desk access.
- Let workers test chairs at the workstation before committing to a choice.
- Allow position changes and periodic movement rather than requiring prolonged static sitting.
When should someone seek medical advice?
Chair selection should not replace assessment or care. The NHS advises contacting a GP for persistent symptoms and describes symptoms such as pain spreading to the bottom or thighs, tight hamstrings, and pain, numbness, or tingling down one leg. If symptoms persist, worsen, or are otherwise concerning, consult a healthcare professional rather than relying on a seating change to address them.
